Dr. Ade Adamson speaks to why he has dedicated his career to studying this phenomenon, some of his initial feelings before the article was published, and what motivates him to continue to shed light on this problem. Dr. Adewole Adamson is a dermatologist, researcher, and advocate. He is currently an Assistant Professor of Internal Medicine (Division of Dermatology) and Director of the Pigmented Lesion Clinic at Dell Medical School at the University of Texas at Austin. He attended Morehouse College and then earned his MD with honors at Harvard Medical School, where he also participated in the Health Sciences and Technology Program with the Massachusetts Institute of Technology. He actively studies skin cancer, evidence-based medicine, and health policy. He is passionate about how effectively and efficiently care is delivered to patients with skin cancer. This week's 7-minute feel-good short story is about the NHL Hockey Coach, Brian Hamilton who was working on the bench of the Vancouver Canucks and Seattle Kraken's match when I determined young medical student named Nadia Popovici, who was sitting right behind the bench, noticed a peculiar mole on the back of his neck that she thought might be cancerous. So, she decided to type him an urgent message on her phone saying "The mole on the back of your neck is possibly cancerous.  Please go see a doctor" and she held her phone up to the plexiglass while pounding to get his attention.  Well, she certainly got his attention.  Enough so, that he decided to go get it checked and sure enough, the doctor diagnosed him with stage 2 malignant melanoma, and told him "You definitely have an angel looking out for you."Excerpt from the podcast: "What I love about this story is it completely stems from one human's concern for another…a complete stranger nonetheless.  We are all truly in this together.  Many of us would have said 'who am I to intervene in his life regarding a private matter like this?' and never said a word.  But when Nadia's concern for Brian's well-being, was stronger than her desire to 'stay out of it,' or just 'not get involved,' both her world and Brian's world suddenly shined just a little bit brighter. In this episode of the Epigenetics Podcast, we caught up with Emily Bernstein from Icahn Schoon of Medicine at Mount Sinai to talk about her work on MacroH2A function and the role of Polycomb proteins in its epigenetic regulation, and how this affects in stem cell development and disease. The Bernstein Lab focuses on histone variants, in particular the variants of macroH2A. Chromatin architecture is influenced by the composition of the nucleosome and, hence, exchanging the core histones for histone variants can have a major impact on chromatin structure. MacroH2A is the histone with the most variants, due to a 30kDa non-histone domain (macro domain) at their C-termini. This variation leads to many macroH2A variants, which have been found to have regulatory roles in the cell. I asked him about to major changes that have effected his life recently.The first was a trip to Hawaii where by chance he met with Archie Kalepa and trained for big waves with him. This led to a real moment of change in Dom's surfing career and equipped him with the knowledge to take on some of the biggest waves the UK has to offer. Change number to comes in the form of devastating news when Dom discovered he had skin cancer earlier this year. He's currently classed as a Stage 3 Malignant Melanoma, which is about as far down the cancer road as you would ever want to go. Thanks to some good learning and knowledge as well as the support from his oncologist Dom has managed to become a test case for a new immunotherapy drug. It's a fascinating story and a must listen episode for fans of this podcast!

Miliary pattern of brain metastases - a case report of a hyperacute onset in a patient with malignant melanoma documented by magnetic resonance imaging

Background Miliary brain metastases are a rare condition but associated with an exceedingly poor prognosis. We present the case of a patient suffering from malignant melanoma with an acute progressively worsening of neurological symptoms up to the loss of consciousness. The magnetic resonance imaging (MRI) demonstrated a new onset of disseminated, miliary spread of central nervous system metastases from a malignant melanoma within 4 days. Case presentation We report on a 57-year-old woman suffering from metastatic malignant melanoma positive for BRAF-V600E mutation who developed an acute onset of neurological symptoms. The patient received vemurafenib and dacarbacin as chemotherapeutic regime for treatment of malignant melanoma. After admission to our hospital due to progressive disturbance of memory and speech difficulty a magnetic resonance tomography (MRI) was performed. This showed no evidence of cerebral tumour manifestation. The symptoms progressed until a loss of consciousness occurred on day five after admission and the patient was admitted to our intensive care unit for orotracheal intubation. No evidence for infectious, metabolic or autoimmune cerebral disorders was found. Due to the inexplicable acute worsening of the neurological symptoms a second MRI was performed on day five. This revealed a new onset of innumerable contrast-enhancing miliary lesions, especially in the grey matter which was proven as metastases from malignant melanoma on histopathology. Conclusion This case describes an unique hyperacute onset of tumour progression correlating with an acute deterioration of neurological symptoms in a patient suffering from miliary brain metastasis from BRAF positive malignant melanoma.

Born and raised in Philadelphia, Dr. McClay served as a staff sergeant in the US Air Force. He received both his B.A and M.D. degree from Temple University, graduating in 1980. He completed a Residency in Internal Medicine and a Fellowship in Medical Oncology at Thomas Jefferson University, where he was subsequently appointed to the faculty as an Assistant Clinical Professor. Dr. McClay has held several positions in academic medicine, including Associate Professor and Medical Director at the Medical University of South Carolina, Hollings Cancer Center, and Professor of Medicine and Director of Clinical Affairs at the University of California, San Diego (UCSD) Cancer Center.Dr. McClay continues his special interests in malignant melanoma and brain tumors. He co-authored the book 100 Questions and Answers about Malignant Melanoma and other Skin Cancers with his wife, Mary-Eileen. Background: The usefulness of S100 as a prognostic marker and aid in follow-up care in patients with malignant melanoma as well as in individuals with various neurological pathologies is well known. The aim of this study was to investigate its release and clinical relevance in benign and malignant disorders beyond these indications to elucidate tumor and organ specificity of S100. Methods: S100 levels were studied in serum samples of 1856 untreated patients, among them 59 healthy individuals, 358 patients with benign disorders, and 1439 patients with malignant tumors. Results: Healthy individuals had low S100 levels reaching a median of 0.041 ng/mL and 95th and 100th percentiles of 0.096 ng/mL and 0.144 ng/mL, respectively. The medians of patient groups with benign diseases ranged from 0.030 to 0.057 ng/mL, patients with malignant diseases from 0.020 to 0.059 ng/mL, and thus were comparable to healthy individuals. Only 2% of patients with benign diseases, mainly suffering from infectious, autoimmune, or benign gastrointestinal diseases and 1% of patients with malignant diseases showed slightly higher values than healthy individuals, in most cases up to 0.5 ng/mL. Conclusions: In contrast to many other oncological biomarkers, S100 is only rarely released in elevated levels from most benign and malignant diseases apart from malignant melanoma and neurological diseases, resulting in superior organ and tumor specificity. As potentially influencing factors, severe infectious diseases have to be considered.
