Prime Minister Benjamin Netanyahu has repeatedly challenged the assertion that starvation is rampant in Gaza, most recently when he slammed a UN report on famine as “blood libel.” On the Haaretz Podcast, Yarden Michaeli takes listeners behind the scenes of his in-depth reporting with Nir Hasson on the scale of severe acute malnutrition in Gaza. As Israel continues to bar journalists from entering the Strip, Michaeli and Hasson virtually “toured” clinics and hospitals in Gaza – including the pediatric ward of Nasser Hospital in Khan Yunis. An explosive expose by Haaretz featured testimonies from IDF soldiers and officers that they were ordered to use live fire to disperse thousands of unarmed Palestinians in Gaza who had crowded the handful of stations set up to distribute humanitarian aid. The story grabbed international attention, and sparked fury among top Israeli officials, including Prime Minister Benjamin Netanyahu and Defense Minister Israel Katz, who both called the article a “blood libel.” On the Haaretz Podcast, one of the three journalists who wrote the story, Nir Hasson, takes listeners behind the scenes of his reporting, explaining the reasons soldiers decided to speak to him and his fellow journalists. The soldiers’ motivations, he said, were two-fold. First the “moral issue” of being put in the position to use deadly force to stop “hungry people trying to get some food for their family” bothered them. “The second thing they spoke about was the fact that this was not the IDF that they used to know. These were not values of the army that these reservists used to serve in,” Hasson added. “They told me, this is not the way a professional army deals with a civil population. They were very angry at their commanders for telling them to use this kind of tool to control a crowd,” and even refused to employ non-lethal methods like tear gas. Israel’s new controversial aid initiative in Gaza and its support for the Abu Shabab criminal gang rivaling Hamas share the common goal of helping Prime Minister Benjamin Netanyahu prolong the war, journalist Nir Hasson said on the Haaretz Podcast. “Netanyahu must preserve the radical right-wing fantasy of ethnic cleansing in Gaza for political survival. For this, he needs the war to continue,” said Hasson, who covers the humanitarian toll of the war for Haaretz. Hasson said that until “we have any other proof” of who is behind the shadowy Gaza Humanitarian Foundation, he regards it as “a proxy of the State of Israel.” Therefore, he said, Israel’s leaders are responsible for the “humiliating” and “dangerous” scenes at GHF aid distribution sites. In his conversation with host Allison Kaplan Sommer, Hasson also discussed his Haaretz investigation into the failure of Israel’s evacuation warnings to protect civilians in Gaza. “In Gaza, there is nowhere to run. Even the IDF safe zones are not safe,” he said. “Israel has really pushed the civilian population of Gaza to the edge.” The unprecedented level of destruction and human suffering there, Hasson said, has reached the point where “I can’t find the words anymore to describe the way I feel about what we’re doing in Gaza. Haaretz journalist Nir Hasson has been covering the war in Gaza for months. He has seen so many horrifying acts of war go unpunished and uninvestigated, that he was hardly surprised by the killing of 15 aid workers at the end of March that shocked the world. The IDF first said the paramedics who were killed were suspicious, and claimed the vehicles they were in did not have their emergency lights on. Then a video of the incident was exposed to the world in the New York Times, showing clearly marked ambulances. “There is no accountability when it comes to commanders and soldiers killing innocents or even medical personnel,” Hasson said on the Haaretz Podcast. In his coverage of the army’s operations in Gaza and their effect on the Palestinian population, he regularly sends the IDF spokesperson questions about all kinds of incidents in Gaza. “I ask them: ‘You killed a family; You bombarded a school - what happened there?’ And I have received thousands of replies saying that it will be looked into in an internal investigation unit. But there are no results of any investigations. I don't know of any trial of any soldier who paid any price for killing innocents in Gaza.” Alongside these disturbing military procedures regarding civilian killings, Hasson said, a “really terrifying humanitarian disaster” is brewing. “I sometimes find it hard to get to sleep because I'm thinking about the families and the kids in Gaza and the despair,” he told podcast host Allison Kaplan Sommer, in a conversation about his coverage of Gaza as well as the West Bank, where, he says, a new level of cooperation and coordination between the Israeli military and violent settlers is a “severe and frightening” development. You get called to see a consult in the middle of the night. It is a middle-aged woman with a bariatric history, and she says her stomach is smaller but doesn't know the name of the operation. She developed worsening abdominal pain after dinner and it's been getting worse. She's not peritonitic, but she's clearly in discomfort. Is it cholecystitis, diverticulitis, pancreatitis, marginal ulcer, or an internal hernia? What do you do? Join Drs. Matthew Martin, Adrian Dan, and Paul Wisniowski on a discussion about initial evaluation and management of bariatric patients with internal hernias.  Show Hosts: Matthew Martin Adrian Dan Paul Wisniowski Show Notes 1.     Initial Evaluation a.     Focused history and physical, labs, and imaging                                       i.     Presenting symptoms may vary and include: nausea, emesis, and abdominal pain ranging from vague to severe.                                        ii.     A basic lab panel can aid in developing the diagnosis and guide resuscitation.                                     iii.     CT of the abdomen and pelvis with IV and oral contrast can assist in identifying intra-abdominal pathology                                     iv.     Reviewing the previous operative report is beneficial to have a framework of the anatomy, i.e. type of bariatric surgery, and configuration of small bowel limbs (ante- vs retro-gastric and ante- vs retro-colic). 1.     According to a 2019 study, 40-60% of closed defects had reopened at time of re-exploration                                       v.     If the patient is peritonitic with abdominal pain, they should be treated similarly to any patient with an acute abdomen with emergent exploration. b.     CT Imaging                                        i.     A mesenteric swirl sign with twisting of the soft tissue and mesenteric vessels with surrounding fat attenuation has been shown to have a sensitivity of 78-100% and specificity of 80-90%. Other findings include: a Bird's beak, dilation of roux or biliopancreatic limbs, SMV narrowing, and displacement of JJ limb to the RUQ and can be used to support the diagnosis of internal hernia                                      ii.     An experienced radiologist familiar with bariatric anatomy has been shown to have a positive predictive value to 81% and negative predictive value to 96% at radiologically diagnosing internal hernia.                                      iii.     A CT scan can provide insight for a suspected diagnosis but it cannot rule out internal hernia c.      Nasogastric/Esophageal Tube                                       i.     Use judiciously based on patient's presenting symptoms                                      ii.     Placement should be done by the surgical team                                      iii.     This may mitigate the risk of aspiration during intubation. 2.     Operative Management a.     Entry should be dependent on the comfort of the operating surgeon.                                        i.     Veress entry into the abdomen with dilated bowels may lead to increased injuries.                                       ii.     Optiview allows for direct visualization of each layer of the abdominal wall. Focusing on twisting the trochar and limiting perpendicular pressure.                                      iii.     Hasson entry also allows for direct visualization but may be limiting in bariatric patients with thick abdominal walls b.     Exploration – a systematic approach                                       i.     Start with evaluation of the gastric pouch and run the roux limb to the jejunojejunostomy, and examine Petersen's and mesojejunal defects.                                       ii.     Follow the biliopancreatic limb to the ligament of Treitz                                     iii.     Lastly, identify the terminal ileum at the sail of Treves and run backwards to the jejunojejunostomy                                     iv.     This will allow for examination of all possible defect and possible intussusception at the jejunostomy c.      Defect Management                                       i.     All defects should be closed, with studies demonstrating reduced rates of internal hernia when defects are closed with a running suture. There is no strong evidence to support the use of a specific suture material. 1.     The use of suture is superior to other methods of closure such as metallic clips, fibrin glue, mesh, or abrasive pads. 2.     A barbed suture can be considered. d.     In a patient with unfavorable anatomy or those unable to tolerate pneumoperitoneum surgeons should consider early conversion to open exploration  3.     Postoperative Care a.     Patients are started on ERAS protocol with limited narcotic use, same day mobilization, early oral nutrition with advancement, and no nasogastric tubes or foley catheters b.     Patients with bowel resection and those with suspected postoperative ileus may benefit from judicious advancement of diet. 4.     Pregnancy a.     Pregnant patients with history of anastomotic bariatric surgery are at increased risk of internal hernia especially in 3rd trimester due to loss of intra-abdominal space b.     Evaluation of a pregnant patient should include abdominal imaging.                                        i.     In a non-acute setting, an MRI abd/pelvis can be considered.                                       ii.     Patients with abdominal pain presenting to the Emergency Department should undergo CT imaging.                                     iii.     The risk of radiation to a fetus, especially beyond the 1st trimester, is limited. Based on the CDC guidelines, a human embryo and fetus are sensitive to ionizing radiation at doses greater than 0.1Gray.
