I am so excited to bring you my friend and colleague Paul Robinson in this podcast episode to discuss the many problems with the current ways we treat and test for low thyroid or hypothyroidism. Paul also shared some of his story with us and how being undertreated for hypothyroidism basically destroyed his life. We also got into the specifics of T4, T3 and Reverse T3 to hopefully help you understand and take more control of your thyroid and health issues. And what do you do if your doctor won't help you. Please let us know if you have any questions or feedback in the comments. Paul Robinson became ill with hypothyroidism in his late twenties. Over the next thirty-plus years, Paul accumulated a wealth of knowledge on thyroid and adrenal dysfunction. His scientific background enabled him to fight through the morass of misinformation and recover his health, and help many many others. At first, Paul was put on the most common thyroid treatment - Levothyroxine (also known as Synthroid or T4). However, the majority of his symptoms remained, much like they do for many on T4-therapy. Over time he became even more ill, with both low cortisol and low thyroid symptoms. This then spurred Paul to put everything he learned on paper - writing his 3 books. Paul wrote 'The CT3M Handbook' as a companion book to 'Recovering with T3' to explain more about raising cortisol levels if needed. And Paul wrote his third book ‘The Thyroid Patient's Manual'. This book covers all types of thyroid medications (T4, T4/T3, NDT, and T3). We tend to eat a lot of them in the evening with dinner (or after dinner), is that a bad time to eat a lot of olives?How many would you recommend, considering we do put olive oil or pumpkin seed oil on our plate as well? In one of your handouts they're in the PROTEIN section, so they shouldn't be combined with fats and carbs..? Thank you!!   Diane: Hi Dr. Cabral I’ve been listening to your podcast since 2018. It has definitely been a positive influence on my life. I have a question about TSH test. I’m 66 years old female. Both test were taking while fasting. I did a finger-stick at 4:30 AM and then I did a blood draw at 8:30 AM, both were done in about a three week span. The fingerstick said my TSH was five the blood drawn said my TSH was 2.42. I did not get my T4 T3. Can you explain why the huge difference. With the blood drawn be more accurate thanks. Diane   Justin: I have struggled with candida since I was 25 I'm not 41 I have been checked by a doctor and she just put me on dyfucan it killed them like your said in podcast but they came back my sugar cravings are mainly at night. I got sleep apnea at 36 I know have type 2 diabetes from I believe my diet and my stress. This video outlines how to optimize your thyroid medication so that you can get back to feeling 100%. There's no question that many thyroid patients continue to suffer with low thyroid symptoms despite taking their thyroid medication faithfully. As a result, this has prompted some thyroid patients to believe that perhaps the issue is related to the treatments for hypothyroidism. I've been getting a lot questions asking me when will new types of treatments for thyroid disease become available. Unfortunately, this type of thinking is not really the right way to look at the problem. Yes, there are a lot of thyroid patients suffering but it's not necessarily of available treatments and more an issue of how they are using those treatments. When you look at thyroid medications you will find that every single one is bio-identical to your thyroid. In other words, that means that all thyroid medications look identical to the thyroid hormone that your body produces naturally. In other words, the treatments are pretty good. The issue stems from how you take it. If you aren't feeling well then you can consider making these changes to optimize your thyroid medication: #1. Change the type of thyroid medication that you are taking. Many thyroid patients are only aware of levothyroxine and synthyroid but there are many subcategories of thyroid medication including: T4 only thyroid medications. T4 + T3 combinations of thyroid medications. NDT formulations. And T3 only medications. #2. Change how you take your thyroid medication. There are 3 different ways that you can take your thyroid medication including: Orally (swallowing your pill) Sublingually (dissolving your medication under your tongue) Injection form (taking your thyroid medication via injection) #3. Change the time of day that you take your thyroid medication. You can take your thyroid medication at different times of the day including: In the morning In the evening Splitting your dose and taking it multiple times throughout the day Alternate daily total doses of your medication #4. Change your dose of thyroid medication. Lastly, you can adjust your thyroid medication either up or down. If you do this make sure you wait at least 4-6 weeks to determine if it has worked for you! Is it possible to naturally increase free T3 levels? The answer is yes! There are actually two ways to do it. The first way I call the easy way. This way is easy because it involves the use of thyroid medications like Cytomel and liothyronine. If you take T3 thyroid medication then you will increase your free t3 levels. Unfortunately, it's not easy to get those medications from your doctor which is why we need to talk about the second way or the not-so-easy way! Fortunately, these things ARE within your control and are things that you can do. Why should you care? Because T3 is the most biologically active thyroid hormone and is hundreds of times more powerful than T4. Here are 6 things that you can do to increase your free T3 levels: #1. Optimize key nutrients - zinc, selenium, Guggul, anti-oxidants, and anti-inflammatory nutrients. These nutrients all help the T4 to T3 conversion process in various ways. #2. Improve your gut health Up to 20% of thyroid conversion occurs in the gut. Even minor gut issues may impair conversion! #3. Reduce reverse T3 levels. High reverse T3 levels may automatically reduce free T3 levels. If you can lower your reverse T3 then you will naturally increase free T3 levels. #4. Check your ferritin Low iron and low ferritin result in abnormal T4:T3 ratios. #5. Her symptoms include proptosis in one eye, very low TSH and normal T4 & T3. Although all of her antibodies have shown very low or zero, even after repeat testing. Is there a specific imbalances or deficiencies you commonly see paired with low TSH ? What could be causing the proptosis if she doesn't have any antibodies? Could Candida be at play? What are your top recommendations for Grave's/Thyroid Eye Disease? So grateful for your support. Thank you! Anonymous: If someone is sensitive or allergic to wormwood and/black walnut, what product or ingredients would you recommend instead to be able to do both a parasite protocol and a CBO protocol? It seems like omitting the microbial balance and parapro would not achieve the best results, but I can't tolerate either of these. Even using the smallest doses and titration, I get severe stomach pain when using these supplements. I have learned so much from your podcast. Thank you for all you do! Michelle: I'm always on a quest to source the best possible animal proteins I can find. Recently I went on a search for turkey, specifically turkey slices like deli meat. I know better than to buy any grocery store deli meats so I tried searching for local pasture raised turkeys but couldn't find anything except a whole bird. Then I referenced The Cornucopia Institutes Organic Poultry Scorecard which gave me one option I could find in the stores with a 4-bird rating but on further research of that company I found they only have a special Holiday bird that qualifies for the GAP step 5 rating and all their other birds are only GAP step 3, which means outdoor access, not pasture raised. Very frustrating and I've come to the conclusion that I'll probably need to buy a whole bird and cook it myself. What I am wondering though is even with the local pasture raised farms it's normal to supplement with an organic / non-gmo feed that is a grain product. I do not eat grains due to my autoimmune history. If I'm eating animal protein that had some grain feed during it's life span does that affect me? To quote Michael Pollan "You are what you eat eats.” Melania: Yeast showed up in the stool test but not in the OAT. How is this even possible? Tina: I am a 52 year old female who loves doing bodybuilding shows every couple of years. I eat a very healthy macro-based diet with Whole Foods play on for the most part I am very healthy. I just ordered the big five and I'm excited to test my hormones. Curious how I can put on more muscle without taking steroids? How can I increase my testosterone levels naturally? Is there something I could be eating or taking to help me put on more muscle since I do strength train five days a week moderately heavy but I feel like I'm not putting any new muscle on. Time for another round of thyroid questions and answers! If you have questions, leave them below! Have questions that you want answered? Please leave them in the comments below! I am going to try and answer your questions at least a couple of times per month. Questions in this video come from a post I left on youtube and I will draw from future questions here for additional videos. Questions answered in this video include: Q - Does Hypothyroidism cause swelling/edema? And how do you fix it? A - YES! Low thyroid function can absolutely lead to edema and swelling. The key to fixing this problem is to fix your thyroid. Not ALL cases of edema/swelling are caused by low thyroid, however. Q - How do I know if I am hyperthyroid and whether or not it's due to my medication or Hashimoto's? A - You have to be careful about how you define hyperthyroid. Taking thyroid medication may drop your TSH but that doesn't necessarily mean that you are hyperthyroid. Many patients can be low thyroid with a low TSH even though their lab tests look hyperthyroid. Q - How do I know if I have just Hashimoto's or an under active thyroid? How do I diagnose Hashimoto's? A - The key is that you must have BOTH low thyroid symptoms AND elevated thyroid antibodies. Some people have what's known as seronegative Hashimoto's but that is not common. When in doubt it's statistically in your favor to assume you have Hashimoto's. Q - Can you still have symptoms even if your thyroid lab tests are in the normal range? My doctor says no. A - YES! You can definitely have symptoms even with "normal" or "optimal" thyroid lab tests. Each person is different and thyroid lab tests should not be used without also evaluating your symptoms. Q - Can you comment on the connection between Hashimoto's and high testosterone? Is this normal? A - Most women with Hashimoto's tend to have low testosterone but some women with Hashimoto's do have high testosterone, it's just far less common. Sometimes treating your thyroid can completely improve your testosterone levels. Q - How do know what the right dose of Nature-throid is? It's hard to pinpoint what my dose should be. A - When trying to adjust your dose of thyroid medication you may find that compounded T4/T3 thyroid medication allows for finer tuning of your dose. Sometimes taking a small dose one day and then a larger dose another day can solve the problem as well. What happened to NP Thyroid? Why it was recalled

NP thyroid was recently recalled at the end of April due to concerns over the potency of this medication. NP thyroid was one of the remaining few NDT brands left after Nature-throid and WP thyroid were pulled off of the market for reasons last year. NP thyroid was recalled due to issues in dosing where it was found to be sub potent. Subpotency means that in certain lots NP thyroid was falling below the 10% therapeutic index set for this medication. What this is that some lots of NP thyroid contained much LESS thyroid medication and thyroid hormone than advertised. To prevent issues, these lots were voluntarily recalled by the manufacturer. The only remaining NDT brand of thyroid medication on the market right now is Armour thyroid. Is this your only option if you were previously using NP thyroid with success? Not at all. There are still several options available to you. Option #1: You can simply switch to Armour thyroid. Armour thyroid has issues, though, so it may not be the best option for everyone, especially if you've previously used it without success. Option #2: You can try to get a compounded desiccated thyroid from a compounding pharmacy. The issue with this option is that many pharmacies are struggling to keep stock right now so this option may not be available to everyone. Option #3: You can switch to a combination of synthetic T4 and T3 to replace the combination T4 + T3 in NP thyroid. This would look like a combination of Tirosint + Cytomel/liothyronine/SR T3. Between these 3 options you should find something that works for you! When will NP thyroid and the other thyroid medications be back? At this time we don't know but I will update you if any further information comes out. Hypothyroidism has been rampant for decades,  and it's estimated that 90% of hypothyroidism in the United States is caused by Hashimoto's Autoimmune Thyroiditis. Hashimoto's is the single most common autoimmune disease. This podcast discusses everything about the thyroid - not just Hashimoto's. It's importance within the body, it's metabolism, the labs, the ranges, the patterns - a little bit about everything thyroid. Every cell in the body has receptors for thyroid hormone, so low thyroid can impact everything - the neurological system, digestive system, immune, endocrine, and any other cell, tissue, organ, or system in the body. The thyroid is also sensitive to changes in each of these systems, and very sensitive to environmental triggers such as endocrine-disrupting toxins. It is also the #1 target for autoimmunity. I call the thyroid  a 'puzzle' because thyroid lab testing has at least 9 important puzzle pieces we can measure, and I don't know how most primary care doctors and endocrinologists expect to solve the puzzle with only 2 or 3 of the pieces? Their goal isn't to solve the puzzle though, it's to give you the proper medication, which is often necessary and appropriate, but often the puzzle remains unsolved. In this episode I go over the basic mechanisms of thyroid function (the pituitary tells the thyroid to produce T4, T4 gets converted to T3, etc). Question: Guidance on what time of day it is best to take T4 and/or T3? Carrie: It actually depends if you're taking immediate release T4 or T3 especially or sustained release because T4 has a much longer half-life which is why we traditionally say to take it in the morning since it helps with energy and metabolism and all those things. Although I do know some people choose to take their T4 at night before bed. But T3 has a very short half-life, and so what I'm finding is some practitioners are now doing what's called a sustained release T3. They take their T3 and it helps sustain longer throughout the day, or they will take their T3 twice. They'll take it in the morning and then they'll sort of take it again in the mid-afternoon. Now, if you're taking a combination T4/T3 such as Armour or Nature-Throid, you can't get the sustained part. I do know some people who will take their Armour or their Nature-Throid in the morning, and then they will take in additional dose of T3 in the early afternoon like an extra, whatever it is, 2.5 or 5 micrograms of T3. The answer is yes! Synthroid (and levothyroxine) still have a lot of value even though many patients don't necessarily do well on them. Before you call me a pharmaceutical shill, realize that we need to be even-handed in our discussion of thyroid medications. There ARE some people who do well on Synthroid (even if they are the minority). Why don’t people like Synthroid? #1. Their dose is probably not right (they are taking too little) You can push up free T3 and total T3 by increasing your dose. But if you dose based on your TSH you will almost always be underdosed. #2. It’s one of the most widely prescribed medications in the world People have had bad experiences on it #3. Doctors are often dismissive of concerns over Synthroid Do you have to change thyroid medications to feel better? Not necessarily. In fact, there are minor changes you can make to feel better very quickly. Some patients do VERY well on T4 only (these patients include those who are super converters). Some people have no problem taking T4 and turning it directly into T3. What should you do if you are on levo/Synthroid and not feeling better? #1. Dose based upon your T4/T3 and reverse T3 level (NOT your TSH) #2. Switch to a different brand of T4. #3. If you are Synthroid try levothyroxine. #4. If you are on levothyroxine try Synthroid. #4. If you’ve tried both then switch to Tirosint or Tirosint-sol. #5. Try the 50mcg tablet of Synthroid/levo and dose based on that Clean up your gut and diet. Elle Russ chats with Paul Robinson about his latest book The Thyroid Patient’s Manual.  Paul is a thyroid patient who became ill with hypothyroidism in his late 20s. He was eventually able to recover using T3-only replacement therapy. He has written 2 other books: Recovering with T3 and The CT3M Handbook.  At first, Paul, was put on the most common thyroid treatment - Levothyroxine (also known as Synthroid or T4-only). However, the majority of his symptoms remained, much like they do for many on T4-therapy. Over time he became even more ill, with both low cortisol and low thyroid symptoms. Paul saw many endocrinologists and doctors as he got more desperate and worried. During this time, he was told, "You will have to live with it”, “You have Chronic Fatigue Syndrome", "You cannot have a thyroid issue, as your thyroid results are within range".  Even though  these statements were nonsense, Paul was written-off by the medical profession. Eventually, he realized that he had to figure out what was wrong himself. Natural dessicated thyroid (NDT) did not help him. T4/T3 combinations did not help him. The struggle over many years cost him his career and damaged his relationships. He had lost all of his thirties and some of his forties on this struggle. Paul got well by using T3 on its own - with no T4 at all. He calls this T3-Only treatment. He developed  a safe and effective protocol for using T3 (Liothyronine) and developed a way of raising cortisol levels if needed. Paul has now been taking T3-Only (T3-monotherapy) for over 20 years and has worked with thousands of hypoththyroid and low cortisol patients. He knows that every individual thyroid patient needs a solution that suits him or her. With good clinical judgment, and all the available thyroid treatments, there is no reason for anyone to stay unwell for a long time on poor thyroid treatment. When this happens it can seriously affect someones's life. Paul wrote his third book The Thyroid Patient’s Manual to help any patient, whether they merely suspect thyroid disease, or are already under treatment that may not be going well. It covers all types of thyroid medications (not just T3). It is also consistent with recent research findings, regarding thyroid diagnosis and treatment and will help patients and doctors alike. This is my breakdown of the verywell health thyroid survey which went out to over 800+ thyroid patients. This survey is full of very interesting and relevant results which can potentially help guide thyroid treatment in the future. This survey was NOT done by me but this video is my commentary and opinion on the survey. Regarding thyroid treatment with thyroid medication, only 6 percent of people felt completely satisfied with only 1 in 5 people (or 20%) feeling completely unsatisfied in their treatment. #2. No matter how many years they have been getting treatment, only 35% of thyroid patients claim that they have gotten better. #3. 45% of people in this survey have been receiving thyroid treatment for over 10 years. The results of this survey clearly show that the current hypothyroid treatment paradigm is failing patients, but why is that? I think one of the main reason has to do with how doctors (and even patients) look at hypothyroid treatment. The majority of this treatment focuses on T4 thyroid medication (which over 90% of patients in this study are taking) and reliance upon TSH testing. My own personal experience suggests that about 90%+ of patients feel great when they are taking combination T4 + T3 thyroid medication. 5 Conditions Which Make Your TSH Less Accurate

The TSH is probably the most standard way to test for thyroid dysfunction but it's certainly not the most accurate way. There are at least 5 different conditions which, if present, can alter how reliable the TSH test really is. These conditions include: #1. Low T3 syndrome. - The first condition is low T3 syndrome. People with this condition have low levels of free T3 or total T3 but their free T4 and TSH may stay relatively normal. Free T3 should take precedence because it is the active thyroid hormone. #2. high TSH but normal T4. - Many people look at the TSH reference range and think that if they fall anywhere on that range that they are "normal". The problem is that this reference range includes people who aren't 100% healthy. Instead of using the standard reference range look at a tighter range which is less than 2.0. #3. Partial thyroidectomy with falsely normal T4/T3. - If part of your thyroid has been removed or radiated with radioactive iodine then the remaining portion must enlarge to try and satisfy the demand for thyroid function. This process causes changes to cellular sensitivity as your body attempts to produce enough thyroid hormone. People in this situation can have a normal TSH even though they are clearly hypothyroid. #4. Elevated TPO antibodies. - Those with Hashimoto's have elevated antibodies to their thyroid which causes inflammation and damage to the thyroid gland. These patients can have a normal TSH for a long time even though they are clearly hypothyroid. #5. D2 genetic mutations. - Certain genetic mutations can reduce how effectively you convert T4 into the active thyroid hormone T3. Up to 15-20% of people have this genetic mutation which can cause symptoms if you develop hypothyroidism. Host Elle Russ sits down with longtime friend and Paleo Thyroid Solution Cara Haun.
