In this week's episode we'll learn about asciminib plus dasatinib in Philadelphia chromosome-positive acute leukemia. In a phase 1 study, combining two tyrosine kinase inhibitors with distinct mechanisms of action had encouraging activity in patients with de novo disease. After that: new research demonstrates that the iron exporter ferroportin contributes to the iron-sensing properties of liver endothelial cells. But is it the primary mediator of systemic iron homeostasis? Finally: anemia in pregnant patients with beta-thalassemia minor. In the third trimester, about one-third of patients have hemoglobin levels of 9 grams per deciliter or lower. This report may help to guide the appropriate diagnosis of anemia while limiting unnecessary testing and interventions. This book has been quite a learning journey. From the discovery of Insulin to how the body is designed to maintain homeostasis and what has happened over the past 25 years, I can see how becoming a Type 2 Diabetic was almost inevitable.Homeostasis depends on active regulation, with dynamic adjustments that keep the environment of your cells and tissues relatively constant. The brain is part of many homeostatic systems, providing signals that coordinate your body's internal clocks and regulating hormone secretion by the endocrine system. These functions often involve a region of the forebrain called the hypothalamus, a command centre that regulates the activities of internal organs, among many other functions.Today, we often eat five or six times a day. The quality is just not there. The food is highly processed, refined carbohydrates, and sugar. Both forms convert quickly into glucose in the blood. The combination of stress released glucose and glucose from multiple meals of poor-quality results in the cells being constantly abused by Insulin trying to unlock them to absorb the energy source. They become insulin Resistant, which is a hall mark of PreDiabetes and Type 2 Diabetes.I can now see how over the past 25 years stress and nutrition have contributed to my becoming a Type 2 Diabetic. It is clear how even small things, which seemed a good idea at the time, have contributed to where I am now.

BUFFALO, NY- January 7, 2025 – A new #research perspective was #published in Aging (listed by MEDLINE/PubMed as "Aging (Albany NY)" and "Aging-US" by Web of Science) Volume 16, Issue 22 on December 9, 2024, entitled “Nuclear lipid droplets: a novel regulator of nuclear homeostasis and ageing.” In this article, Dr. Konstantinos Palikaras from the National and Kapodistrian University of Athens and Dr. Nektarios Tavernarakis from the University of Crete explore how tiny fat droplets, known as nuclear lipid droplets (nLDs), accumulate in the nucleus of cells as we age. Unlike regular lipid droplets, which store energy in the cytoplasm, these nuclear droplets may weaken the nucleus by disrupting critical cellular processes. The authors suggest that excessive buildup of nLDs could lead to nuclear instability and may be linked to metabolic conditions such as fatty liver disease, obesity-related disorders, and premature aging. This highlights the potential role of nLDs in cellular aging and age-related diseases. Previous research by the authors using Caenorhabditis elegans (C. elegans), a model organism commonly used to study aging, revealed the role of ATGL-1, an enzyme that regulates fat storage in the nucleus. When functioning properly, ATGL-1 helps maintain a healthy lipid balance. However, when it becomes inactive or overwhelmed, fat droplets build up around the nuclear lamina, making it more difficult for the cell to function properly. “These findings establish excessive nuclear lipid deposition as a key hallmark of aging, with profound implications for nuclear processes such as chromatin organization, DNA repair, and gene regulation.” The authors emphasize that lifestyle interventions, such as caloric restriction and better insulin regulation, can significantly reduce harmful nLD buildup, reinforcing the role of healthy metabolism in slowing cellular aging. They also call for further studies to understand how nLDs behave in human experimental models, particularly in patients with conditions like metabolic syndrome and progeria. A deeper understanding of these processes could lead to treatments designed to preserve cell health and delay age-related diseases. In summary, by presenting nuclear lipid droplets as potential therapeutic targets, this research perspective presents a new path for exploring treatments aimed at combating age-related diseases driven by lipid dysregulation. The reason we have questions is that we don't see the bigger picture. As children, we don't recognize the trauma we experience, but as we get older the sense that something is not right in life may lead us to the spiritual path. What happened to us informs how we respond to life in the present, but we live out of the programming of the past. As long as we're reacting from childish programming that was designed to protect us then we're unable to accept what is in the present. Self-observation and writing are ways to practically work with our thoughts, emotions, and patterns. When we have reactivity, we can come back to the body, be as present as possible, and be kind to ourselves. We can learn to be with what is going on for us instead of thinking that we should not feel the way we do. Homeostasis is when the body brings itself back into a stable and balanced state. Our habitual reactions keep the world intact and keep us identified with the person we believe ourselves to be. Loosening the intensity of our reactions makes it possible for us to see who we are underneath that. There is always more, always a bigger picture. We don't fear the unknown; we fear the known ending. The prison we find ourselves in is the known. We may say we want to get beyond our known reactions, but it can be scary and our inner work will bring up resistance. Who are we if we don't have our stories about ourselves? We can see how affirming, denying, and reconciling forces operate in our lives. There is benefit to working with people in a group who can remind us of our practice. We can hold to and nurture the experience that we sometimes have of who we are beyond our reactivity and definition of ourselves based in the past.
