Dr. McGuff's Body By Science is a transformational book about strength training that advances the extremely compelling and scientifically validated premise that it takes very little time to build pure muscle strength, and that many workouts are not only ineffective, but quite likely counterproductive. If you care about fitness, becoming stronger, aging gracefully, and avoiding breakdown, burnout, illness and injury, listen carefully to this show and maintain an open mind about implementing a different strategy in the gym. The book was published in 2009 and I can't believe I'm only recently bringing this to the forefront of my consciousness! As I record the session nursing another high jump injury to add to one that plagued me in 2020, I am more reflective than ever on ways that we screw up fitness goals with a haphazard approach. Dr. McGuff calls his method a “purified” approach to fitness! The essence of Dr. McGuff's training protocol is to perform the “Big 5” exercises once a week, for a single set to muscular failure. These are basic, safe, and functional exercises (leg press, chest press, overhead press, pulldown, seated row) that allow you to focus on pure strength without being overly complex. Doing a single set to failure allows you to most safely train the complete spectrum of muscle fibers, from slow twitch to explosive non-oxidative fast twitch. Dr. McGuff draws an important distinction between building pure strength and skill development for your chosen sport of fitness endeavor. These two should be on separate and distinct tracks—a single, very short, super difficult strength session per week for strength gains, and then doing skill development when you feel sharp, focused, and explosive. You never want to pursue skill development in a fatigued state or become fatigued during workouts, or you will compromise your ability to hit the golf bar, clear the high jump bar, or sink the basket. This show will be a real eye opener that will compel you to rethink the “maintenance” sessions you do in the gym that don't stimulate fitness adaptation, and the ill advised sessions where you traumatize muscles and joints and invite injury risk. [01:15] A lot of us who are really enthusiastic about fitness might be overdoing it and performing workouts that are minimally effective and/or counterproductive. [04:41] If you're doing something where the mechanical load is too severe, the muscles will get injured instead of strong! [09:32] How did Dr. McGuff come up with the premise of 12 minutes a week for exercise. [11:44] A lot of good pieces of equipment have some sort of mechanism to vary the resistance. [16:20] Whenever you're doing any sort of work with muscle, it goes through a very step-wise process of how it's been recruited to do work. [19:43] Often a person will get cramps in the first part of the race rather than near the finish line. [22:47] There's a window between your selected weight and your force capability that as you're training and recruiting and fatiguing those motor units, the window is closing. [27:25] Strength and conditioning are general applications that apply to any sport. [32:30] The higher you get in any given sport, the worst the training seems to become. [39:49] Is stretching beneficial or counterproductive? [41:59] What do we see in the big city gyms that is really helping? [48:42] When you develop the skill of your particular sport, this skill makes the best use of your physical conditioning. Dr. Doug McGuff is an expert in High Intensity Training, emergency room physician, and bestselling author of Body by Science. Dr. McGuff sheds light on the aerobic benefits of resistance training and how to get a complete workout in 20 minutes once a week. “You cannot go into this process with an idea of someone that you admire. Doug McGuff, MD became interested in exercise at the age of 15 when he first read Arthur Jones’ Nautilus Training Bulletin No. 2. His interest in exercise and biology led him to a career in medicine. In 1989, he graduated from the University of Texas Medical School at San Antonio and went on to train in Emergency Medicine at the University of Arkansas for Medical Sciences at Little Rock where he served as Chief Resident. From there, Dr. McGuff served as Faculty in the Wright State University Emergency Medicine Residency and was a staff Emergency Physician at Wright-Patterson AFB Hospital. In 1995, Dr. McGuff moved to Seneca, South Carolina where he joined Blue Ridge Emergency Physicians with whom he continues to practice full-time emergency medicine. Throughout his career, Dr. McGuff maintained his interest in high-intensity exercise. Doug realized a lifelong dream when he opened Ultimate Exercise in November 1997, where he and his instructors continue to explore the limits of exercise through their personal training of clients. One could say that Dr. Doug McGuff is one of the pioneers of BMX motocross bike racing in Texas. He built the state’s first race track, having gotten hooked on the sport as a teenager in the 1970s. The sport also triggered a deeper interest in fitness. As McGuff tried strengthen his core for bike racing, he discovered Arthur Jones’ Nautilus training technique and bartered janitorial services for a Nautilus gym membership. McGuff’s interest and aptitude for studying the body led him to pursue medicine at the University of Texas in San Antonio. He specialized in emergency medicine, was chief resident of emergency medicine at the University of Arkansas in Little Rock, and a staff physician at Wright-Patterson Air Force Base Hospital in Ohio. McGuff is currently an ER physician with Blue Ridge Emergency Physicians in Seneca, South Carolina. The other side of McGuff’s career is dedicated to fitness, or as he says—helping people never have to go to the ER. Realizing a lifetime dream, he opened up his own fitness facility in 1997 called Ultimate Exercise. The gym is dedicated to the type of high-intensity fitness training using the Super Slow protocol. In this episode of STEM-Talk, McGuff talks about why this type of exercise is better for the body, safer, and able to prevent age-related conditions such as sarcopenia. To say that book changed the course of my life would be a massive understatement.” 8:13: During the summer of 1994, McGuff met Arthur Jones, who greatly influenced his thoughts on exercise resistance training. 12:00: McGuff went into ER medicine because “It was rare to find something that I felt that I had intrinsic talent in. I felt like I functioned very well in that environment.” His career has focused on two things: taking care of people who fall down and get hurt; and trying to prevent it from happening in the first place. 13:00: McGuff talks about being a pioneer of BMX in Texas, as he built the first track there and went back to racing in the late 90s and won the state championship. He also trained some world champion level BMX racers. 14:30: Now he characterizes himself as “a practicing physician so busy with the chronically sick and massively debilitated; the chasm between day to day life and actually thinking about prevention is such a wide chasm that it’s hard to imagine.” 15:00: “I would love to see the day where the commercial says, ‘Ask your doctor if diet and exercise are right for you….

In a culture that worships impulse, self-control has all the appeal of soggy blanket. We see discipline as an imposition and chafe against the curtailment of our will. Cultural messaging and social belonging often hinge on following blind custom or our most unhealthy momentary inclinations. We exercise our autonomy or “choice” through (often market influenced) poor decisions. Freedom is conflated with whim. Any attempt to rein in stupidity is more than just the voice of a killjoy but an act of aggression. We've become such a precious, entitled bunch that the mere suggestion we temper our instinctive response feels like an insult. Where does that leave our health?
