Think core training is all about crunches and six-packs? What if the real key to a strong, pain-free core is the muscles you can't see? Hosts Amy Hudson and Dr. James Fisher dive deep into the science of core strength, bust common myths, and reveal how personal training can help you target the low back and pelvic muscles that truly support your spine, posture, and overall function. Today's episode features hosts Amy Hudson and Dr. James Fisher addressing a listener's question about the core. Dr. Fisher starts his contribution by discussing the origin of the core, as well as where it's located and why its role is crucial. Amy points out that, for many people, “core” is synonymous with abdominals - when, in reality, other muscles, including the back, low back, and pelvic floor, are all part of the core. Wondering whether you have weak muscles in your core? If you've experienced some degree of low back pain or low back stiffness, then those may be symptoms of a core that needs to be strengthened… “One of the most common reasons for non-specific mechanical low back pain, which accounts for about 80% of back pain, is simply weak lumbar muscles,” says Dr. Fisher. Furthermore, it's been shown that strengthening the lumbar muscles can reduce back pain. Dr. Fisher explains why the plank isn't an exercise that can serve as a good assessment of our trunk - or core - muscles. Having a weaker core and the challenge of training low back muscles link back to the so-called deconditioning hypothesis - Dr. Fisher unpacks this further. Paraphrasing Dr. Fisher, Amy stresses how “It's not very easy to effectively activate and train and strengthen those muscles in the low back with everyday activities.” Dr. Fisher highlights how low back muscles are predominantly type-1 muscle fiber, so they're not high-force muscles, but rather high-fatigue resistance muscles. Dr. Fisher and Amy go through some workout routines that can help strengthen low back muscles (especially the ones you don't see). Working in their laboratories with people with low back pain, Dr. Fisher and his colleagues have seen a 200% strength increase over a 10-week training period. In this episode of the Postural Respiration podcast I am joined by Cody Gillis, DPT, PRC, CSCS. Cody grew up in Boston, MA and earned his DPT from Northeastern University in 2017. It was during his time at Northeastern that he was introduced to PRI by the Strength and Performance staff their who had utilized various positional activities with their athletes. During this explorative track as a 3rd year DPT student Cody was pursuing his Strength & Conditioning certification and spending time with the performance team there. Cody admits to being skeptical at first regarding what he saw but wanted to learn more. During his two required 6 months clinicals Cody gained experience in a local orthopedic setting, but it was during his second rotation that he truly immersed himself. Moving to Hawaii and spending time learning from Randy Fukuji and the clinical cases at Fukuji & Lum in Honolulu produced a desire to dig even deeper. Cody recalls a specific patient that impacted him early on, who had tried everything and been everywhere, but was coming to the clinic solely to work through the Right Sidelying - Left Adductor Pullback technique because that one technique did more for the patient than anything else had. Once Cody heard this he knew it was time to take his first course.Almost immediately Cody was on a flight from Hawaii to New York for his first course, Postural Respiration. Over the following years Cody completed his additional coursework and became certified as part of the 2021 PRC Class. Out of all the coursework he has completed, Cody credits Impingement & Instability and the concepts therein, for providing him the link to a more neurological mindset which has shaped his practice ever since. After spending many years learning and treating at Fukuji & Lum in Hawaii, Cody's desire to spend more one on one time with individual clients, led him to create Respire, initially as a personal training and coaching opportunity in his spare time. After getting married and moving back to the mainland Cody opened his Respire Physiotherapy doors, offering all encompassing care in Santa Barbara and Monterey California.Cody has a strong interest in interdisciplinary care and has attended numerous conferences with other disciplines including joining the PRI community at past AAPMD meetings among others. Most recently Cody's desire to understand and communicate with other disciplines, led him to undergo a PRI Mini Residency at the Hruska Clinic. We discuss his time here and what offering an integrative treatment plan looks like for him as he navigates building relationships with other professionals and offering patients holistic care via those relationships. Cody has a passion for being on, in, or near the water and he looks forward to working with more individuals in the surfing community as well as pursuing more knowledge and the autonomic regulation required of the sport of Free Diving and those who do it.
I sat down with Marcus Filly to talk about what happens when training stops being about beating records and starts being about feeling better - mentally, emotionally and physically. We explored how Marcus shifted from elite CrossFit competition to founding Functional Bodybuilding, a method that puts longevity and mental clarity at the centre of fitness. A steep rise in gym-related ACC claims for injuries could be down to strength training's rising popularity. Accident Compensation Corporation data shows it accepted about 50,000 gym-injury claims last year - worth about $50 million dollars. It notes training and gym activities now account for the most claims. Body Torque Personal Training founder, Alex Flint, says training with weights can result in shoulder and lower-back injuries. What does aerobic fitness really mean — and how can personal training help you improve it?? Amy Hudson and Dr. James Fisher break down VO₂ max, heart rate, and the science behind combining strength training with interval workouts. From boosting cardiovascular health to reducing fatigue and disease risk, they explain how targeted personal training can transform your conditioning and overall well-being. Amy Hudson and Dr. James Fisher look at the benefits of having increased aerobic capacity, how to increase it and the role of both strength training and brief interval training in aerobic capacity. One of the most common terms used in scientific literature is VO₂ max which represents the maximum volume of oxygen our body can take up and use. Dr. Fisher explains that whenever we talk about cardiovascular fitness or cardiorespiratory fitness, it's about how well oxygen can move around our body. Dr. Fisher and Amy talk about why VO₂ max and CO2 (carbon dioxide) are important in terms of their fitness. Did you know that your conditioning level, to some extent, can determine what exercise level or exercise intensity you can work at to maintain working aerobically? Not only daily fatigue but also your sleep, risk of depression and anxiety, risk of coronary artery disease and hypertension, and the risk of diabetes are all positively impacted by an improved aerobic conditioning. Dr. Fisher approaches the audience question: Does knowing one's resting heart rate indicate anything to that person about their current state of cardiovascular health or aerobic capacity? As you improve your fitness, you improve what's called cardiac output and stroke volume. If you're curious about your maximum heart rate, you can calculate it by taking the number 220 and deducting your age from it… Amy brings interval training and glycogen into the conversation. Dr. Fisher explains that the importance of having a strength training workout and then doing concentrated cardio at the end is actually great because it serves as this glycogen dump. Dr. Fisher goes into EPOC – Excess-Post Exercise Oxygen Consumption – and the specificity of an action and, specifically, a sport itself.
