Despite a mild embolic burden, the cardiovascular response was profound, requiring advanced hemodynamic support, including an Impella RP device (Abiomed, Inc.). Pearls- Clear Vision, Clouded Heart: Ocular Venous Air Embolism with Pulmonary Air Embolism, RV Failure, and Cardiac Arrest Hypoxia, hypotension and tachycardia in a patient following ocular instrumentation are classic findings suggestive of pulmonary embolism from possible air embolism. The diagnosis of RV failure is based on clinical presentation, echocardiographic findings (such as McConnell's sign), and invasive hemodynamic assessment via right heart catheterization. Mechanical circulatory support can be considered as a temporary measure for patients with refractory RV failure. Central Figure: Approach to Pulmonary Embolism with Acute RV Failure Notes - Clear Vision, Clouded Heart: Ocular Venous Air Embolism with Pulmonary Air Embolism, RV Failure, and Cardiac Arrest 1. What is an Ocular Venous Air Embolism (VAE), and how can it be managed in critically ill patients? An Ocular Venous Air Embolism is defined as the entry of air into the systemic venous circulation through the ocular venous circulation, often during vitrectomy procedures. Early diagnosis is key to preventing cardiovascular collapse in cases of Ocular Venous Air Embolism (VAE). The goal is to stop further air entry. This can be done by covering the surgical site with saline-soaked dressings and checking for air entry points. Adjusting the operating table can help, especially with a reverse Trendelenburg position for lower-body procedures. The moment VAE is suspected, discontinue nitrous oxide and switch to 100% oxygen. This helps with oxygenation, speeds up nitrogen elimination, and shrinks air bubbles. Hyperbaric Oxygen Therapy can reduce bubble size and improve oxygenation, especially in cases of cerebral air embolism, when administered within 6 hours of the incident. Though delayed hyperbaric oxygen therapy can still offer benefits, the evidence is mixed. VAE increases right heart strain, so inotropic agents like dobutamine can help boost cardiac output, while norepinephrine supports ventricular function and systemic vascular resistance, but this may also worsen pulmonary resistance. Aspiration of air via multi-orifice or Swan-Ganz catheters has limited success, with success rates ranging from 6% to 16%. In contrast, the Bunegin-Albin catheter has shown more promise, with a 30-60% success rate. Catheterization for acute VAE-induced hemodynamic compromise is controversial, and there's insufficient evidence to support its ...

Break 1: Tom Wages STORY 1: Buford Sophomore Olivia Harris Wins Gold at U-20 World Track and Field Olivia Harris, a 15-year-old sophomore from Buford, made a remarkable debut at the World Athletics Under-20 Championships in Lima, Peru, where she won gold as part of the U.S. women's 1,600-meter relay team. Harris, the youngest athlete at the event, clocked a 400-meter split of 52.05 seconds, surpassing her own Gwinnett County high school record and edging out Georgia's all-time high school mark. Buford track coach Fyrone Davis praised Harris as a fierce competitor with potential to become one of Georgia's top track and field athletes. Harris, who was named Runner of the Year by The Daily Post, comes from a family of track stars, including her older sisters Kimberly and Sydney. STORY 2: Dacula Alum's New Bar Shines Spotlight on Women's Sports Chelsea Fishman, a former Dacula High School soccer player, is opening Jolene Jolene, a bar and restaurant celebrating women's sports, in early 2025. Residing in Decatur and still playing soccer, Fishman has spent the past year planning the venue, inspired by the growing popularity of women's sports and a lack of dedicated spaces in Atlanta. Jolene Jolene will feature food and drinks, with a focus on women's sports, including local teams like the WNBA's Atlanta Dream. The bar's name, inspired by Dolly Parton's song, reflects Fishman's passion for the cause. The menu will include elevated bar food with unique items. STORY 3: Gwinnett County's First Woman Pediatrician Set To Retire Dr. Taru Shah, Gwinnett County's first female pediatrician, is retiring after 44 years of service. She will close her practice, Shah Pediatrics in Lawrenceville, on September 20. Since founding the practice in 1980, Shah has treated an estimated 107,000 patients and seen multiple generations of families. A native of India, she trained at Topiwala National Medical College and Illinois Masonic Medical Center. Shah, who has also taught at Mercer School of Medicine, noted significant changes in pediatric care due to technology and increased focus on mental health. Post-retirement, she plans to spend time with her grandchildren, assist her husband with his business, and engage in community activities. We'll be right back Break 2: Kia MOG STORY 4: Two men arrested in Internet Crimes Against Children sting operations In a recent two-day sting operation, Gwinnett Sheriff's Office deputies arrested two men from Jackson and Dacula, Yash Patel, 28, and Jacob Stipcak, 39, for allegedly sending obscene materials to an undercover investigator posing as a 14-year-old girl. Patel, arrested on August 19, faces charges including Obscene Internet Contact with a Child and Criminal Attempt to Commit a Felony. Stipcak, arrested the following day, faces similar charges. Both men are accused of sending explicit photos and trying to arrange sexual encounters. The Gwinnett Sheriff's Office emphasizes its commitment to protecting children and combating exploitation. STORY 5: Stone Mountain Park Celebrates Labor Day Weekend With Classic Lasershow Stone Mountain Park will celebrate Labor Day Weekend with family-friendly events from August 31 to September 2, and again on September 7. Highlights include a BMX Stunt show featuring professional riders and a free Classic Lasershow with fireworks each night. Despite the recent annexation of the land by the city of Buford, the operation of the maze remains unchanged. The maze will feature the same popular attractions, including a hayride, popcorn jumping pillow, animal viewing area, and a Haunted Forest starting in late September. The 2024 season will embrace a "three pollinators" theme and offer a few new additions. STORY 7: Norcross Grad Diamond DeShields Surpasses 2,000 Career WNBA Points Norcross grad Diamond DeShields achieved a significant milestone by reaching 2,000 career points in the WNBA during a game against the Indiana Fever. Now in her seventh WNBA season, DeShields, who was drafted No. 3 overall in 2018, has played for the Chicago Sky, Phoenix Mercury, and Dallas Wings, before rejoining the Sky this season. She won a WNBA championship with Chicago in 2021 and has been named to the All-Rookie Team, All-Star Team, and All-WNBA Second Team. DeShields also overcame a major health challenge in 2020, following surgery to remove a spine tumor. Previous works on lens detection do not quantify uncertainties in lens parameter estimates or scale to modern surveys. We present a fully amortized Bayesian procedure for lens detection that overcomes these limitations. Unlike traditional variational inference, in which training minimizes the reverse Kullback-Leibler (KL) divergence, our method is trained with an expected forward KL divergence. Yash Patel first started brewing at age 13. While his dad was pretty happy about that, mom...not so much. However, since the brewing bug hit him quickly, he has spent a lot of time brewing beer. So it only made sense for him to take over ownership of Macon Beer company, central Georgia's first craft brewery. We talk to Yash, a native Maconite, about his beers, the growing craft beer scene in Macon, and his ideas behind brewing beer.
