CardioNerds Critical Care Cardiology Council members Dr. Gurleen Kaur and Dr. Katie Vanchiere meet with Dr. Yash Patel, Dr. Akanksha, and Dr. Mohammed El Nayir from Trinity Health Ann Arbor. They discuss a case of pulmonary air embolism, RV failure, and cardiac arrest secondary to an ocular venous air embolism. Expert insights provided by Dr. Tanmay Swadia. A 36-year-old man with a history of multiple ocular surgeries, including a complex retinal detachment repair, suffered a post-vitrectomy collapse at home. He was found hypoxic, tachycardic, and hypotensive, later diagnosed with a pulmonary embolism from ocular venous air embolism leading to severe right heart failure. Despite a mild embolic burden, the cardiovascular response was profound, requiring advanced hemodynamic support, including an Impella RP device (Abiomed, Inc.). Multidisciplinary management, including fluid optimization, vasopressors and mechanical support to facilitate recovery. This case underscores the need for early recognition and individualized intervention in cases of ocular venous air embolism. US Cardiology Review is now the official journal of CardioNerds! Pearls- Clear Vision, Clouded Heart: Ocular Venous Air Embolism with Pulmonary Air Embolism, RV Failure, and Cardiac Arrest Hypoxia, hypotension and tachycardia in a patient following ocular instrumentation are classic findings suggestive of pulmonary embolism from possible air embolism. The diagnosis of RV failure is based on clinical presentation, echocardiographic findings (such as McConnell's sign), and invasive hemodynamic assessment via right heart catheterization. Mechanical circulatory support can be considered as a temporary measure for patients with refractory RV failure. Central Figure: Approach to Pulmonary Embolism with Acute RV Failure Notes - Clear Vision, Clouded Heart: Ocular Venous Air Embolism with Pulmonary Air Embolism, RV Failure, and Cardiac Arrest 1. What is an Ocular Venous Air Embolism (VAE), and how can it be managed in critically ill patients? An Ocular Venous Air Embolism is defined as the entry of air into the systemic venous circulation through the ocular venous circulation, often during vitrectomy procedures. Early diagnosis is key to preventing cardiovascular collapse in cases of Ocular Venous Air Embolism (VAE).  The goal is to stop further air entry. This can be done by covering the surgical site with saline-soaked dressings and checking for air entry points. Adjusting the operating table can help, especially with a reverse Trendelenburg position for lower-body procedures. The moment VAE is suspected, discontinue nitrous oxide and switch to 100% oxygen. This helps with oxygenation, speeds up nitrogen elimination, and shrinks air bubbles. Hyperbaric Oxygen Therapy can reduce bubble size and improve oxygenation, especially in cases of cerebral air embolism, when administered within 6 hours of the incident. Though delayed hyperbaric oxygen therapy can still offer benefits, the evidence is mixed. VAE increases right heart strain, so inotropic agents like dobutamine can help boost cardiac output, while norepinephrine supports ventricular function and systemic vascular resistance, but this may also worsen pulmonary resistance.  Aspiration of air via multi-orifice or Swan-Ganz catheters has limited success, with success rates ranging from 6% to 16%. In contrast, the Bunegin-Albin catheter has shown more promise, with a 30-60% success rate. Catheterization for acute VAE-induced hemodynamic compromise is controversial, and there's insufficient evidence to support its ...

Dan Drake, President and CEO of Trinity Health PACE, discussed how a PACE comprehensive community-based approach can allow older adults continue to live at home as long as possible. Trinity Health PACE, a member of Trinity Health, provides comprehensive medical, health, recreational, and social services to qualified elderly individuals, allowing them to remain at home while receiving essential care. Dan describes PACE, Program for All-Inclusive Care for the Elderly, as inside-out nursing home care, it delivers nursing home-level medical services in PACE participants’ homes or at a PACE center with proven results in reducing admissions to hospital settings. To enroll, an individual must qualify for nursing home care with low income; he notes that most qualify for both Medicare and Medicaid, with little or no co-pays. Once enrolled PACE operates as both insurer and care provider. For the protection of the care providers and the resident they perform an in-home review, making sure the home is livable and safe; they will even help with decluttering of a residence. Once in PACE care they have access to social and recreational opportunities, occupational and physical therapist, dieticians, transportation to medical appointments, as well as a dedicated team for primary care and registered nurse home care. Here are 3 big things you need to know—   One — President Trump says a call with Russian President Vladimir Putin was highly productive.  The President says both men agreed that the Russia-Ukraine war needs to come to an end.  Trump says the two will work closely together on negotiations including visiting each other's nations at some point.    Two ---- The Trump administration is rescinding the Biden Adminisration's rules related to N-I-L revenue-sharing payments to male and female college athletes. Just before Biden left office, the Department of Education's Office of Civil Rights said schools could be in violation of Title Nine if they didn't hand out revenue-sharing payments proportionately to men's and women's sports.  Now schools can distribute money to the sports they believe have the most value.    And number three —   Trinity Health Ann Arbor says an anonymous donor has given the hospital ten-million-dollars for a neurosciences institute. Construction on an expansion will begin this year and should be completed in the spring of 2026. The expansion will include new outpatient dementia and Alzheimer's care.

Delaware is facing a critical shortage of medical providers; everything from primary care doctors to psychiatrists and dentists.To help bridge the gap and incentivize providers to both work and live in Delaware, the state Department of Health and Social Services launched the Health Care Provider Loan Repayment Program, offering clinicians up to $50,000 a year in loan repayment. Since 2022, more than $1 million has been doled out.This week, Delaware Public Media's Joe Irizarry is joined by Dr. Nancy Fan – Chair of the Delaware Healthcare Commission and OB/GYN with Trinity Health – and Certified Nurse-Midwife Angela Madariaga to learn more about the program and how it's addressing the need for medical providers in the First State.

She'll say, No, we don't" without even thinking. Aetna Medicare Advantage plans are struggling with last minute negotiations to keep Trinity Health (with 92 hospitals) in their networks. Just months after graduating with a Master's in Health Administration, Alana found herself in the role of interim CEO at Manning Regional Healthcare Center, a critical access hospital close to her hometown in Iowa.  In our conversation, she discusses how she rapidly transitioned her leadership skills, adapting to the role thanks to a lot of trust from her team. Her story is a testament to the potential for young leaders to make significant contributions to healthcare in rural settings. “If you were ever facing a challenge or responsibility that might seem daunting to you, just do it because your future will thank you for the growth” -Alana Monson Alana Monson is an administrative fellow with Trinity Health – MercyOne in Des Moines, Iowa and recently served as Interim Chief Executive Officer for Manning Regional Healthcare Center in Manning, Iowa. Alana grew up in rural Western Iowa and graduated from the University of Iowa with her Bachelor of Business Administration in Accounting in 2020 and Master of Health Administration in 2023.  While pursuing her graduate degree, she worked with classmates to form the Student Association for Rural Health. In addition to her experience as CEO at a critical access hospital, Alana also has experience working for a Medicare administrative contractor, an academic medical center, a health system, and serving on the Iowa Rural Health Association board.
