Hyperbaric oxygen chambers have long been used for acute issues such as strokes and brain injuries—but can they be used for chronic conditions as well? This week, I'm joined by Dr. Scott Sherr, an internal medicine physician certified in hyperbaric oxygen therapy. We sat down to discuss what makes this chamber "a wound healer from the inside out" as well as the proper protocols one should follow before even considering oxygen therapy for chronic issues. In this episode, you'll discover: The Benefits of Hyperbaric Oxygen Therapy (HBOT): As soon as you step into the hyperbaric chamber, your body works to reoxygenate the cells in low-oxygen tissue. In the long term, not only does the body rebuild damaged blood vessels, but you can also expect to see a decrease in inflammation and swelling, an enhancement of the immune system, and lymphatic and detoxification optimization. Having a Healthy Foundation Is Key: "You want to get some of that foundational stuff working before you start stressing the system with oxygen," says Dr. Sherr. This includes having the right vitamins, minerals, and nutrients required to detoxify and create energy or ATP effectively. "The challenge in the hyperbaric world is that we don't think about these things as much," he adds. HBOT's Role In Cancer Treatment: Dr. Sherr uses hyperbaric oxygen therapy in conjunction with other interventions, such as vitamin C IVs and a ketogenic diet, as a way to support and sensitize people for chemotherapy and radiation. What Methylene Blue Does: In terms of supplemental support, Dr. Sherr turns to methylene blue, which helps optimize the mitochondria—the parts of the cells that produce ATP—while promoting detoxification at the same time, which the internal medicine physician calls "very unusual." For quite some time now, ever since entering the world of red light therapy — but particularly when getting into the realm of methylene blue — I've wondered what nitric oxide's true impact on our body and wellness is. For the vast majority of my life, I was under the belief that all nitric oxide = good. That was ingrained at an early age with the countless pre-workout supplements I was taking during my heavy lifting days during high school through college. But it didn't stop there. The message of nitric oxide = good and that "more is better" has been perpetuated in the wellness, and even longevity, industries by countless experts and influencers.But how is that possible if one of methylene blue's main actions is nitric oxide inhibition? How is that possible if nitric oxide is a free radical and an environmental pollutant and, thus, is effectively an "electron stealer"?For whatever reason, I have never been able to get a straight answer from anyone on this topic. But serendipitously, I was recently reading a book that provided the most clearcut and sensible explanation that would allow for nitric oxide to be both "good" and "bad". The book is Methylene Blue: Magic Bullet: The Ultimate Mitochondrial Support, by Dr. John Lieurance. As I learned from the book, it turns out that nitric oxide is quite similar to how inflammation can be both a positive and a negative for our physiology; but with some additional nuances through in there. Today's episode is about nitric oxide and the roles it plays in our wellness and what the best strategies are for best harnessing the "good" nitric oxide while downregulating the "bad" nitric oxide. Dr. Mohammed Elamir, lead physician at Aviv Clinics, offers diagnostics and treatments for brain injuries such as PTSD, stroke, and traumatic brain injury and to address cognitive decline. After comprehensive physical and mental assessment, including brain scans, patients receive hyperbaric oxygen therapy and other therapies like psychotherapy and neurocognitive training. Individualized plans are designed to get to the root causes of neurological and cognitive issues and stimulate the growth of new cells and blood flow, even for patients with long-standing conditions. Dr. Mo explains, "Our pre-assessment testing is a lot more rigorous than your kind of annual physical. We do three to five days of testing that can involve medical evaluations with the medical doctors and advanced blood work for different biomarkers for injury or inflammation. Two very specific brain scans that I think separate our assessment from most, and one is an MRI with DTI analysis. That is a microstructure analysis of the brain. So we can look at the nerve tracks running through the brain, how well-connected, and how dense they are." "If somebody's approved for the treatment, they would undergo up to a 12-week treatment regimen involving hyperbaric oxygen therapy. And we can go into the details of that oxygen therapy, but in addition to that, they'll have other appointments, whether it's PT, psychological counseling, or neurocognitive training. And by the end of the 12 weeks, they should be feeling better, but objectively, we want to see why.
