Chris and Nada take a deep dive into gender equity in medicine, the impact of the pandemic and the possible solutions. First, they discuss the difference between gender equity and equality. Equality is the act of treating everyone the same. Whereas equity, focuses on levelling the playing field so that there is more representation and participation. This includes asking questions such as why inequity exists? And why aren't women progressing? Evidently, women experience gender inequity at different times of their career. It may be after they've had children, or it may be when they want to step up and take on leadership roles. What is most obvious however, is that the impact of gender inequity in medicine extends far past the individual. Gender inequity impacts how we look after female patients, what kinds of questions we ask in research and how we perform and apply that research in the context of women's healthcare. Chris and Nada discuss what needs to happen to make the system more accommodating. This includes implicit bias training, intersectionality training and leadership training. Investment in leadership skills and training is crucial. We can have all of the right policies in place, however if leadership doesn't set the standard to encourage uptake, inequity will always exist. In this second episode of the podcast series Women and Girls Bleed Too, Haematologist Dr. Roseline d’Oiron (Reference Centre for Haemophilia and RBD, Bicêtre Hospital AP-HP and University of Paris-Saclay in France) and Obstetrician/Gynaecologist Prof. Rezan Abdul-Kadir (The Royal Free Foundation Hospital in London, UK) discuss how to identify women and girls who are likely to have a bleeding disorder. Prof. Abdul-Kadir addresses the steps she takes when she suspects a potential bleeding disorder in one of her patients. Dr. d’Oiron confirms it is key to get a clear understanding of the personal bleeding history and family history. There are several screening tools available to help assess the patient’s signs and symptoms. The experts discuss when a patient should be referred to a Haematologist, it is important to know that Haematologists are easily accessible for referral when in doubt. Finally, Dr. d’Oiron and Prof. Abdul-Kadir address the types of testing that should and should not be done in first line. It turns out there are many misconceptions about laboratory testing for bleeding disorders. For example, contrary to common belief a normal aPTT and PT does not exclude a bleeding disorder. Furthermore, referring to a Haematologist for testing does not mean the patient should remain untreated. Starting basic treatment is crucial to limit the bleeding and address any iron deficiency as quickly as possible.

Nwokwu made the call in an interview with News Agency of Nigeria (NAN) in Abuja to commemorate the World Blood Donor Day (WBDD). Following the introduction of an Electronic Health Record System (EHRS) at UCLH, data is no longer technically difficult to collect, and the research potential of an organisation can rise dramatically. This throws up new challenges for clinicians consenting patients, ethics committees and information governance. We discuss the lessons learnt from the controversial collaboration between the Royal Free Hospital and Google's Deep Mind in 2016, and how to stay patient focused when meeting a patient and using EHRS at the same time. Will EHRS help in the earlier detection of patient deterioration , and what other interesting directions will clinical research take now that randomised trials can be designed and run within a completely electronic system?

A new handheld device for migraine is being pioneered at Guys and St Thomas's Hospital in London. Using single pulses of transcranial magnetic stimulation the device is helping prevent and treat migraines in people who haven't responded well to other treatments. Dr Anna Andreou, director of headache research, and nurse specialist, Bethany Hill talk Mark through how it works. Some people, particular of North European and Irish ancestry have the faulty genes that mean they are unable to get rid of excess iron in the body. This can lead to symptoms ranging from tiredness, joint pain, and diabetes to skin discolouring and liver disease. New research has shown the condition is far more common than has been previously thought and is often missed as a diagnosis. Haematologist at Gartnavel Hospital in Glasgow, Ted Fitzsimons and epidemiologist, David Melzer of the University of Exeter, talk testing and treatment for iron overload, or haemochromatosis. Cancer is an umbrella term which covers a spectrum of disease. Some cancers, like lung cancer grow and spread rapidly. But others like some forms of breast, thyroid and prostate cancer have a less than 5% chance of progressing over twenty years. So should we redefine low risk cancers? GP Margaret McCartney and consultant histopathologist, Murali Varma of University Hospital of Wales in Cardiff discuss this question.

Shireen is a Consultant Haematologist and Honorary Senior Lecturer at King's College Hospital, London with a specialist interest in the treatment of patients with lymphoma (cancer of the lymphatic system).
