Hi Everyone, This week we are joined by Dr Anastazia Keegan an obstetric haematologist and the head of Haematology at our Women's hospital here in Western Australia. Join us as Anastazia educates us about congenital bleeding disorders in pregnancy - the common ones, Von Willebrands disease and haemophilia - and how to approach a woman with one of the many uncommon ones (which working in a tertiary referral centre are more common than you would expect). Thanks for all the great advice and insights - we look forward to having Anastazia back in the near future for the lowdown on some more really important haematological conditions we encounter in women's health. The social determinants of health (SDH) are the non-medical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life. With Natalie Philbert, Manager of Women's Programs and Services at Hemophilia Ontario, and Dr. Meghan Pike, pediatrician and clinical fellow in Pediatric-Hematology Oncology at Dalhousie University/IWK Health Centre. We don't always have the language to speak about our periods – let alone menstrual bleeding disorders. What are the signs and symptoms? What are the gaps in care and diagnosis? And how does it connect with gendered inequities? May 28 was International Day of Action for Women's Health, so for the next few episodes, we're focusing on gender and health matters we may know bits and pieces of but probably need to learn more about. Today, we focus on menstrual bleeding disorders. Natalie Philbert is a PhD candidate focusing on delay in diagnosis for menstruators with bleeding disorders. Natalie brings a professional and personal passion to the bleeding disorder community given her own diagnosis of Von Willebrand disease Type 1. She co-created a website called heroixx.ca, specifically designed for menstruators with bleeding disorders. Dr. Meghan Pike launched the WeThrive App that can identify adolescents with heavy menstrual bleeding. She is an advocate for free access to menstrual products. Her research interests include patient-reported outcome measures, impacts of cancer treatment on reproductive and menstrual health, and advocacy for menstruators. This talk goes over the most common causes of inhirted bleeding disorders. There are two major types of hemophilia – deficiency in factor VIII and IX – which predominant occur in men. Hemophilia A is deficiency of factor VIII and is the most common form of hemophilia treated with factor concentrate. Hemophilia B is less common but treatment principles are the same. There are multiple types of von Willebrand Disease as this is a complex molecular that both carries factor VIII and forms large multimers essential for platelet function. Mild forms of vWD can be treated by desmopressin but von Willebrand factor is needed for more severe types.
According to the Centers for Disease Control, pain is one of the most common reasons people seek medical care. However, for persons affected by hemophilia A and B, the experience of pain has provided the subtext for many issues related to health and wellbeing across the lifespan, and irrespective of gender, age, and, potentially, hemophilia severity levels. Although less-well characterized in children, pain is already embedded in the life experience of a PWH, and a substantial contributor to poor health-related quality of life in adolescence and young adulthood, further increasing in negative impact throughout adult life. In this episode, we explore pain's prevalence, mechanisms, and more. Prophylaxis in hemophilia has led to extraordinary progress, however, recent studies suggest that although the overall musculoskeletal health of persons with hemophilia has significantly improved, the goal of attaining a level of musculoskeletal health comparable to that of the general population without hemophilia has still not been achieved. Furthermore, the impact of prophylaxis has been uneven across hemophilia populations and the promise of greater physical activity free of hemorrhage has not been entirely fulfilled. This episode explores the history, evolution, and current state of prophylaxis, and highlights the current and future areas of further investigation.
