Dr. Christy Sutton is a chiropractor who focuses on finding and correcting the root cause of their health problem, preventing future health problems, and giving her patients the tools they need to have the healthiest life possible. She uses many tools, including applied kinesiology, nutrition, chiropractic adjustments, exercise programs, lab, and genetic testing, to give her patients a precise and powerful treatment plan that goes beyond what they could find anywhere else. In addition to providing her patients with the highest level of care, she is an author and teacher of epigenetics and using genetic testing with environmental changes to take your health beyond your genes and prevent health problems. Dr. Sutton earned her Bachelor of Science in Microbiology, Anatomy, and Health and Wellness from Texas State University and Parker University. She earned her Doctorate of Chiropractic from Parker University. She was drawn to health care because of her extensive personal health struggles, which included being diagnosed with Crohn's disease at the age of 16.   In the second part of our discussion about iron overload and hemochromatosis, Christy and I discuss strategies for obtaining sufficient protein while maintaining caution about red meat intake, the connection between high iron and low copper, supplements that will reduce iron, and antioxidant recommendations. Today I am fortunate to have Dr. Sutton. She earned a Bachelor of Science in Microbiology, Anatomy, and Health and Wellness in Texas State University and Parker University .  She received her Doctorate of Chiropractic from Parker University. Her serious personal health issues, such as being diagnosed with Crohn's disease at the age of 16, drove her to pursue a career in health care. Her passions include supporting her patients in avoiding health problems and becoming as healthy and pain-free as possible without the use of medications or surgery. She is more interested with identifying and treating the underlying issue than with masking the symptoms. She now lives in Dallas, Texas, with her beloved husband and daughter. The two types of Hemochromatosis gene are C282Y and H63D. There is a medical misinformation about you cannot develop hereditary hemochromatosis with only one gene, and having H63D will not put you at risk. He is convinced that most of us have too much iron in our bodies. Virtually every disease of aging is linked to excess iron accumulation: depression, anxiety, dementia, skin problems, liver failure and more. Iron Overload [27:14] Sarah says: Hi Robb and Nicki, Let me start by saying that I truly love your podcast and never get tired of listening to you two! My question is in regards to iron overload, sometimes referred to as iron dysfunction. Have you heard or read the work of Morley M. Robbins? It is based on the premise that so called iron deficiency anemia is in fact not a deficiency at all, but in fact iron dysfunction which results in an overload of iron in the tissue, subsequently causing all the problems that are often diagnosed as anemia. One of the important points he makes is that we suffer from iron dysfunction because of a lack of bio-available copper, as well as a lack of retinol. I have myself had lower blood iron levels for quite some time, even after incorporating grassfed beef and organs into my diet on a regular basis. I have been eating a (now dairy-inclusive) paleo diet for many years, and recently switched to a more animal-based type diet, and saw massive improvements in energy, but still not optimal, particularly on the first week of my cycle. Dr Shivan Pancham and Dr Farrukh (Farrah) Shah both present a podcast on the Guidelines for the monitoring and management of iron overload in patients with haemoglobinopathies and rare anaemias.   Both Dr Pancham and Dr Shah discuss the guideline in three main parts: 1) Diagnose and complications of iron overload (Dr Shah) 2) Medications use to treat iron overload (Dr Pancham and Dr Shah) 3) Initialization of iron chelation therapy and maintenance treatment and monitoring for complications of therapy (Dr Pancham and Dr Shah) Iron overload (IOL), resulting from regular or intermittent blood transfusions or from increasing dietary iron absorption can cause serious and life-threatening complications. Patients at risk of IOL include those with inherited anaemias such as transfusion-dependent thalassaemia (TDT) and non-transfusion-dependent thalassaemia (NTDT), transfused sickle cell disease (SCD) and rarer anaemias such as congenital sideroblastic anaemia (CSA), congenital dyserythropoietic anaemia (CDA), Diamond-Blackfan anaemia (DBA) as well as red cell enzymopathies, membrane disorders and defects in haem synthesis pathways. The United Kingdom has approximately 15 000 patients with these disorders and diagnosis and management of IOL is important in minimising morbidity and mortality. Other disorders that are associated with IOL such as hereditary haemochromatosis or acquired anaemias such as the myelodysplastic syndromes are not covered by this guideline. The extent and severity of IOL is affected by both the underlying disorder and the intensity and duration of transfusion. Patients on regular top-up transfusions are at most risk whilst those on intermittent transfusions develop IOL more slowly. In the absence of blood transfusion, sickle cell disorders tend not to accumulate excess iron: however, manual and automated exchange transfusion may result in mild degrees of IOL or even iron deficiency.1-3 Dr Shivan Pancham is a  Consultant Haematologist with Sandwell and West Birmingham NHS Trust. She is one of two lead clinicians for the West Midlands Sickle HCC. She has been a writing group member for the national sickle and thalassaemia guidelines. Dr Farrukh (Farrah) Shah is a Consultant Haematologist at both the Whittington hospital in London and NHS Blood and Transplant (Colindale). Dr Shah has a specialist interest in the management of transfusional iron overload and haemoglobinopathies. She is actively involved in clinical research in iron chelation and is a local investigator and national investigator for a number of clinical trials.

Treatment for iron overload involves the removal of blood to reduce excess iron. A new handheld device for migraine is being pioneered at Guys and St Thomas's Hospital in London. Using single pulses of transcranial magnetic stimulation the device is helping prevent and treat migraines in people who haven't responded well to other treatments. Dr Anna Andreou, director of headache research, and nurse specialist, Bethany Hill talk Mark through how it works. Some people, particular of North European and Irish ancestry have the faulty genes that mean they are unable to get rid of excess iron in the body. This can lead to symptoms ranging from tiredness, joint pain, and diabetes to skin discolouring and liver disease. New research has shown the condition is far more common than has been previously thought and is often missed as a diagnosis. Haematologist at Gartnavel Hospital in Glasgow, Ted Fitzsimons and epidemiologist, David Melzer of the University of Exeter, talk testing and treatment for iron overload, or haemochromatosis. Cancer is an umbrella term which covers a spectrum of disease. Some cancers, like lung cancer grow and spread rapidly. But others like some forms of breast, thyroid and prostate cancer have a less than 5% chance of progressing over twenty years. So should we redefine low risk cancers? GP Margaret McCartney and consultant histopathologist, Murali Varma of University Hospital of Wales in Cardiff discuss this question.

Dr. Gordon Lithgow of the Buck Institute for Research on Aging tells us about worms! This unassuming scientific model has a lot of important advantages for science: they can be frozen and subsequently thawed and retain viability, they are extremely well understood down to the precise number of cells in their body and the wiring of their nervous system, known as the connectome. Additionally, they have a short lifespan and are cheap to work with. Why would that be advantageous, you may ask? This is where Dr. Lithgow's work on the Caenorhabditis Intervention Testing Program comes in. Short-lived organisms give Dr. Lithgow and his colleagues the opportunity to see how their biology responds to compounds in different contexts and to do so cheaply and rapidly. Think a vitamin, pharmaceutical or one of any number of other compounds may have a broad effect on longevity? Try it on Caenorhabditis first! Taking this approach allows the broad screening of compounds that might not otherwise get its chance in the limelight if science were limited to only working with rodents, for example. In this episode, you'll discover: Nematodes... especially C. elegans, of course! The synergistic way in which research in lower organisms (C. elegans in this case) works together with rodent research to better understand the biology of aging and identify potential therapeutics. The story behind the Caenorhabditis Intervention Testing Program, a multi-institutional effort using the advantages of C. elegans short lifespan to screen for potential compounds that may increase lifespan. We also discuss the more expensive-but-closer-to-humans version of this program known just the Intervention Testing Program. The various qualities of C. elegans, especially as related to science's deep knowledge of the organism, that make it a perfect fit for this type of research. Some of the early experiences that actually lead to my early interest in the aging field and how it actually holds a bit in common with Gordon's story today. The role of protein aggregation as a possible fundamental mechanism of aging... even beyond its more established role in neurodegenerative diseases. Some of the fascinating research surrounding mild heat stress as a means to increase lifespan in lower organisms. Some of Gordon's research into the effects of metal accumulation, particularly iron, on aging in lower organisms and how, in humans, it may be important to approach the intake of these minerals with care. The interplay between genes that influence iron-binding and the development of Alzheimer's disease. Some of Gordon's research into the (perhaps surprising) effect of vitamin D on aging in C. elegans and, particularly, on the solubility of proteins... the loss of which is a feature of aging of particular interest. The critical involvement of the Nrf2 stress-response pathway in conferring benefits of vitamin D in worms... a pathway many of you may know about from previous discussions of sulforaphane, a robust activator of Nrf2 in humans. Some of the challenges encountered with ensuring standardization of protocols across all of the participating labs in the Caenorhabditis Intervention Testing Program. The amazing genetic diversity represented in soil-dwelling nematodes and how this is also an advantage in longevity research. Iron levels and biochemistry have impacted my personal performance in a dramatic way. Recently I've been racing cyclocross, something for which I lack talent. For those of you who don't know cyclocross, it's a lot like short track mountain bike racing but most of the bikes look a road bike with drop handlebars. The start is really important, and the course has hundreds of corners and so the sport doesn't suit slow-twitch athletes like me. This year I’ve been enjoying some dramatic gains in my performance. These gains are nothing to do with training, in fact, I've been training less than ever before. The key to performance is health, and that's why I've got Dr. Tommy Wood on again today to talk about some of the biochemical tweaks I've been making and also the serious issue of iron overload (haemochromatosis). First let’s talk about anaemia. Some people think of anaemia as synonymous with iron deficiency, but iron deficiency is just one of many things that can cause anaemia. Diagnosing anaemic tendencies requires the consideration of the production, destruction and loss of red blood cells. Other nutrients such as zinc, B6, B12 and folate are as important as iron for the production of red blood cells. Sometimes we see results that show both iron overload and anaemia. Why is iron overload a problem? Most micronutrients cause little harm when consumed in excess. Vitamin C is a good example--consume too much and you'll simply pee it out. Excess iron is different in that there's no easy way to excrete it. The main way we lose iron is through bleeding, and for men and postmenopausal women this doesn't happen very often. Excess iron becomes bound to proteins and acts as a pro-oxidant, literally causing your tissues to rust. If left untreated, the excess iron can cause diabetes, cirrhosis of the liver and a host of other chronic diseases. The day after we recorded this interview four of my six follow-up appointments were with masters athletes with iron overload. Please don't guess, do the test! How should your diet change if you are diagnosed with iron overload? Research shows diet can have an impact of the absorption of iron. Things that block iron absorption: Calcium rich foods like dairy, leafy vegetables and sardines (whole with bone). Phytates from leafy vegetables and nuts. Tannins in red wine and coffee. Anthocyanins in any colour berry (the darker the colour of the berry the better). Things that increase iron absorption: Vitamin C. Added fructose, table sugar. Blood testing for iron levels is not that difficult to do. In fact, my team can do it for you. Iron overload can be ruled out as part of our O2 Boost programme. She eats very little and gets nauseous easily. Do you have any ideas of what she could try eating that would not make her nauseous and would be easy to digest. In the interview he said that waking up you still have your muscle glycogen full and you are almost ready to race. (and, if racing, you only need 100-300 cals, which sounds very different to my belief that we are in a low glycogen state). Do I understand correctly? Q2 - he says on a long event you can get 2/3 of energy from fat. My understanding was that 20-40% is as high as you can expect, is this simply because the intensity is super low for ultra endurance? I used to use the extra virgin but start to get nauseous so I been using light olive oil on my salads but did not know if this stuff was healthy. It is my hypothesis that of all the environmental stressors that we are exposed too, food leads the decline in mental health over the last 30 years. What are your thoughts on this? Also I work with the very poor and I am wondering what you would have to offer as advice to me to offer my clients who cannot make the greatest food choices due to limited income. After listening to the recent podcast, I am assuming it would be better to consume the shake at a different time of day and eat more carbs after weight training. Is this correct? Also, am I "wasting" the L-glutamine by taking it on weight training days and not running days. James says: The dreaded cramping question! I'm a competitive age group triathlete and I've been having cramping issues in races. I know the usual answer is your not training at the same intensities as I race at and/or you're not taking in enough nutrition/electrolytes. In my case, neither are the issue (I think). I train 15-18 hrs/week which includes a lot of intensity and I am spot on with my diet, training nutrition and race day nutrition. I take all of it very seriously, so when it comes to cramps I am stumped. The cramps can come on as early as the swim. Often I get them near the end of the bike though and almost guaranteed in the run. I never get them in 10k's or 5k's, just the longer stuff half marathons, olympic triathlons and especially half ironmans. Do I need to just load up on the salt tabs? Tweak in the diet? The Effect of Iron Overload on the Long-term Toxicological Effects of Fumonisin B1 in Rat Liver

The long-term toxicological effects of hepatic iron overload on the cancer initiating and promoting properties of fumonisin B1 (FB1) were investigated in male Fisher 344 rats. An initial pilot (dose response) study over 15 weeks was performed to determine a level of dietary iron that achieves a high hepatic iron concentration in the absence of significant side effects, to be used in the subsequent long-term carcinogenesis study with FB1. Doses of 1%, 1.5% and 2% dietary carbonyl iron (Fe) were used for 10 weeks, followed by a level of 0.5% Fe for another 5 weeks. After 10 weeks of feeding 1% Fe, the hepatic iron level was 30-fold that of controls. Following the reduction in dietary iron to 0.5%, the hepatic iron level was maintained and side effects were minimal. Irrespective the dose of iron, the iron deposition was initially confined to zone 1 (periportal region), but with increased iron loading, extended to zone 2 (mid-zonal region) and zone 3 (perivenular region). Iron overload was shown to increase hepatocellular proliferation as seen in labelling of cells with 5-bromo-2’-deoxy-uridine (BrdU). Lipid peroxidation, measured as MDA, was also significantly enhanced by excess iron and levels correlated the iron dosage. In the long-term study, half of the rats were submitted to an iron-loading regimen with 1% Fe for 10 weeks, while the other half received the powdered AIN-93M diet only. The iron was then reduced to 0.5%; the rats were divided into four treatment groups: FB1/Fe (n=15); Fe (n=14); FB1/AIN (n=15); and control group (n=15), respectively. FB1 was fed at 250 mg/kg AIN-93M diet for 5 weeks, followed by 100 mg/kg for 20 weeks. The effect of iron on FB1-induced initiation and promotion was assessed in 5 rats from each group at week 35. FB1 was removed from the diet of the remaining rats but iron supplementation continued for 25 weeks. The rats were terminated and the effect of dietary iron on the progression phase of FB1-induced carcinogenesis was assessed. The feed intake of rats from the Fe group (weeks 4-10), and FB1/Fe and FB1/AIN groups (weeks 10-60) was measured daily and the other groups were given averaged feeding accordingly; thus, the total body weight gain (tBWG) gain did not differ significantly between groups. Excess iron significantly increased the liver to body weight (LW/BW) ratio; however, iron could not counteract the mitoinhibitory effect of FB1 on hepatocyte proliferation and the LW/BW ratio of the FB1/Fe group was significantly lower than the Fe group. During the 25-week FB1-treatment period, more foci and nodules of dysplastic cells developed in the FB1/Fe group than in the FB1/AIN group. After removal of FB1, the number of nodules remained constant in the FB1/Fe group, while increasing in the FB1/AIN group. Initially, the pattern of iron deposition was the same as in the pilot study. Hepatocyte necrosis caused by FB1 then resulted in a shift of iron from hepatocytes to Kupffer cells and portal tract macrophages. FB1/Fe resulted in increased hepatocellular proliferation as demonstrated by increased BrdU labelling at 35 weeks. The withdrawal of FB1 reversed this effect and the index of BrdU labelling decreased to even lower levels than the Fe and FB1/AIN groups. Hepatic iron was almost 9-fold that of controls at 10 weeks; a significant increase in the hepatic iron levels in the FB1/Fe group was noticed as compared to all other groups. However, after removing FB1, the hepatic iron decreased to the same level as seen in the Fe group. During the FB1 treatment period, MDA increased in the FB1/Fe group to levels significantly higher than in the FB1/AIN and control groups. The present study suggests a dual role of iron overload in cancer development during FB1-induced carcinogenesis. During the cancer initiation/promotion phase, iron overload in combination with FB1 enhanced the susceptibility of the liver to the formation of foci and nodules; but after removal of FB1, continued supplementation of iron impaired the progression of pre-neoplastic hepatic lesions.
