This episode promises profound insights into the complexities of trauma from those at the forefront.  About our Host Maureen O'Connor, Ph.D., J.D. is the president of Palo Alto University (PAU) which specializes in Psychology and Counseling. Prior to joining PAU, Dr. O'Connor held multiple roles at the City University of New York (CUNY) in New York City over a period of 18 years. Dr. O'Connor completed a Ph.D. and J.D. in a dual degree program in psychology, law, and policy at the University of Arizona and clerked for the late Honorable Patricia Wald, then Chief Judge of the D.C. Circuit Court of Appeals, prior to launching her academic career. An American Psychological Association (APA) Fellow, she has held numerous governance roles in APA, including as Chair of its Task Force on Human Rights. She has been active in the Society for the Psychological Study of Social Issues (SPSSI/APA Division 9) for many years, including serving as its President. Recent work focused on evidence-based pedagogy development for doctoral students and on innovative mentoring strategies. Her 2019 book, Teaching Psychology: An Evidence-Based Approach (2019), with co-authors Drs. Jill Grose-Fifer and Patricia Brooks, were published by Wiley Press. She is a member of the 39th class of the American Leadership Fellows program in Silicon Valley and serves on the Executive Committee of the Association of Independent California Colleges and Universities.    About our Guests Lisa M. Brown, Ph.D., ABPP, is a Professor and the Director of the Trauma Program at Palo Alto University and an Adjunct Clinical Professor, at Stanford University School of Medicine who is a licensed clinical psychologist and boarded in geropsychology. She graduated from the Pacific Graduate School of Psychology in 2002. Prior faculty positions have been in the School of Aging Studies, the University of South Florida, and the James A. Haley Veterans Hospital, Tampa, Florida. She is the former President of Division 20, Adult Development and Aging, American Psychological Association (2020-2021), and the founder of the Disasters and Older Adults Special Interest Group, Gerontological Society of America. Jessica Walsh, Ph.D., is a clinical psychologist at the Michael E. DeBakey Veterans Affairs Medical Center. She serves as the Technology Innovations Coordinator, a role which involves supporting the VA, both clinically and administratively, in the design, implementation, evaluation, and sustainability of telemental health innovations implementation, operations, and planning. Clinically, she specializes in PTSD and couples therapy. Flora White-Cooper, Ph.D., strives to combine her education, training, work experiences, and interests to create a career that reflects "never growing tired." She is an Army Veteran and retired Correctional Officer. She co-created and operated an in-house rehabilitation program for nonviolent inmates. She graduated from PAU in Clinical Psychology with a certificate in Neuropsychology. She obtained a 2-year postdoctorate certificate in Neuropsychology. Flora White-Cooper spent most of her career working with offenders with mental illnesses. Today's episode of Ask Dr Jessica features Dr Cathy Shin, a pediatric surgeon at Children's Hospital of Los Angeles.  In this episode, we talk about the most common reasons why children visit a pediatric surgeon--including skin bumps (6:58), lymph nodes (9:25),  appendicitis (13:28) and hernias (22:58).  She gives great advice on what to look for--what are reassuring signs and what are the signs to visit a doctor.  Dr Shin is also the director of the trauma program at Children's Hospital of Los Angeles---so we discuss ways that parents can help prevent common pediatric trauma--such as car safety (29:57),  swim safety (31:33), gun safety (33:23) and the importance of helmets (34:53).  Dr Shin is a very accomplished surgeon and it was an honor to have her speak on Ask Dr Jessica! This episode explains the care and process available when serious pediatric trauma occurs. Dr. John Draus is the Chief of Pediatric Surgery at Nemours Children's Health and he also serves as the Trauma Medical Director for the Trauma Program at Wolfson Children's Hospital. Dr. Draus came to Wolfson in July of 2022 after having served as the Trauma Medical Director at the University of Kentucky's level 1 pediatric trauma center from 2015-2021 and chief medical officer from 2021-2022. Lisa Nichols is the Trauma Program Manager at Wolfson Children's Hospital. Lisa came to Wolfson in November of 2019 to manage the program at Wolfson. She has over 15 years of pediatric trauma program management experience and over 34 years of pediatric nursing expertise. In his role as a trauma program manager, Jamin oversees clinical, operational, and performance improvement processes for a newly verified trauma center in north Louisiana. Apart from nursing, Jamin enjoys spending time with his family as well as hunting, fishing, camping, and golfing. Dr. Ross explains how he defines dissociation, and talks about flashbacks caused by Post-Traumatic Stress Disorder (PTSD). Francesca brings up how hard it is for people to let go of trauma, and they touch on how the mental health field can cause mental health problems. “I always say that a flashback flashes back. If it doesn't have the quality of flashing back, it's just with you, it doesn't intrude. So, in order to have the quality of a flashback, it has to be dissociated, disconnected, stuffed inside somewhere.” – Dr. Colin Ross The Locus-of-Control Shift (24:40) Francesca asks Dr. Ross about how he sees shame versus guilt. Dr. Ross brings in his concept of the locus-of-control shift, and gives examples from his work with combat veterans who have shifted the blame about loss of life onto themselves as a way to retain some form of control. He talks about why it might be better to label PTSD as Pre-Traumatic Stress Disorder. “So the locus-of-control shift is the self-blame, which can lead to both guilt and shame, self-hatred, [and] self-punishment.” – Dr. Colin Ross A Memorial to Trauma (48:55) Dr. Ross explores a case where he was treating a woman who was traumatized by sexual abuse. He and Francesca discuss the power of memorials such as the one in Auschwitz, but lament that most trauma survivors don't have a place like that to put their sorrow and grief. Grab your fave beverage because my guest Courtney Wells, PhD, and I are about to spill the kombucha on taking down the patriarchy.  Courtney is many things, including a licensed clinical psychologist and executive director at Pivot Psychological Services in Chicago, and tamer of two Pekinese. Additionally, they're simply an amazing human with profound insights into dismantling the systemic oppression that leads to self-doubt and wayward searches for validation. Courtney's path towards genuine self-expression informs their approach to trauma work offers a model for societal healing as well as the creation of non-hierarchical, inclusive systems in therapy and beyond.  Courtney ascribes to Acceptance and Commitment Therapy, or ACT. “Destroying the patriarchy is part of ACT therapy where you take on the patriarchy, and then you destroy it,” Courtney laughs. Tongue-in-cheek aside, they credit ACT with undermining establishment principles like grind culture or the “right way” of doing and being. The right way of white supremacy, they point out, “is generally where somebody else benefits from all of your work. You get a little bit, but somebody else benefits greatly from all that you do.”  Courtney says that patriarchal forces crush our will under fear of retribution and ostracization, pushing us to favor avoidance at all costs to keep the gears running full steam ahead. ACT can be complicated and requires commitment (as its name implies). The model is “about staying in it with someone long enough to also see what the feeling is that's there or what the internal experience is that's there.” Regardless of model, healing is an evolutionary process, and the more expansive our collective recovery, the stronger our resistance to patriarchal influences. GUEST BIO Dr. Courtney Wells received their BS in Psychology at John Carroll University, their MA in Community Counseling at John Carroll University, and their PhD in Counseling Psychology at Texas Woman's University. They completed their predoctoral internship at the Dallas Veteran's Affairs and their postdoctoral fellowship at the Jesse Brown Veteran's Affairs, specializing in Posttraumatic Stress Disorder, chronic pain, and substance use. Courtney was also the creator and Director of the Trauma Program at one of the largest Partial Hospitalization and Outpatient Programs in Chicago.
