The Ottawa Hospital, one of Canada's most extensive medical facilities, suffered a devastating electrical fire that disrupted the main power supply and knocked out the hospital's emergency power system. Firefighters' efforts to extinguish the flames with water further compromised the electrical infrastructure. Over the next 48 hours, essential services—including 120 beds—were taken offline, and 96 patients had to be relocated, including neonatal ICU infants who were hand-carried down eight flights of stairs.“Although the emergency power system immediately kicked in, it too was soon compromised by both the fire and the enormous amount of water firefighters used to douse it.”Despite rigorous planning and an immediate emergency response, the failure of the backup system transformed a power disruption into a full-blown crisis.AuroraGenerators.com

But last week, the Canadian Task Force on Preventive Health Care released a draft recommendation that said it's not going to lower the recommended screening age to 40 for women with average risk – and that decision has been met with widespread disapproval. Our AMA guest this week was Dr. Jean Seely. She's the head of the breast imaging section at the Ottawa Hospital and professor of medicine in the radiology department at the University of Ottawa. Dr. Seely answered your questions about screening for breast cancer.

In Season 2, Episode 2 of CLOT Conversations from Thrombosis Canada, Dr. Tzu-Fei Wang, an associate professor and hematologist, discusses a recent study titled "The prevalence of relevant drug-drug interactions and associated clinical outcomes in patients with cancer-associated thrombosis on concurrent anticoagulation and anticancer or supportive care therapies." The study aimed to investigate drug-drug interactions (DDIs) between anticoagulants and other medications in cancer patients and their impact on clinical outcomes.High Risk of DDIs: The study found that approximately 41% of cancer patients on anticoagulation therapy had relevant drug-drug interactions, particularly with antidepressants and certain other medications.Overall, the study highlights the importance of assessing and managing drug-drug interactions in cancer patients receiving anticoagulation therapy, while also suggesting that not all DDIs necessarily lead to adverse clinical outcomes. His clinical interests include cross-sectional abdominal imaging, Abdominal/GU oncologic imaging in MRI and CT modalities, with research interests in the appropriate utilization of medical imaging and AI-assisted augmented radiology. He serves as the Chair of the Canadian Association of Radiology Standing Committee on Artificial Intelligence and Ad-Hoc Member of the Health Canada Scientific Advisory Committee on Digital Health Technologies. Dr. Kumanan Wilson is a board member at AMS.  He is the Interim CEO and Chief Scientific Officer, Bruyère Research Institute and VP Research and Academic Affairs, Bruyère Continuing Care. He had previously served as an innovation advisor for Bruyère. Dr. Wilson is also a specialist in General Internal Medicine at The Ottawa Hospital and a Full Professor and Faculty of Medicine Clinical Research Chair in Digital Health Innovation at the University of Ottawa. He is the co-founder and Chief Scientific Officer of CANImmunize, a science-based technology company specializing in immunization software. Dr. Wilson's research focuses on digital health, health data, immunization, pandemic preparedness and public health policy and innovation. He has received support from multiple organizations including the Canadian Institutes of Health Research (CIHR), the World Health Organization (WHO), The Bill and Melinda Gates Foundation and Canada's Immunity Task Force.

Dr. Kwadwo Kyeremanteng is the department head of critical care at The Ottawa Hospital. He dedicates his time to care for the sickest of the sick patients in the intensive care unit (ICU). As a researcher, he is interested in using ICU resources more efficiently and improving access to palliative care in the ICU. To help do this, he founded the Resource Optimization Network, a multidisciplinary research group working to reduce health spending in this area without compromising care. In September 2019, Dr. Kyeremanteng launched his ever-growing podcast Solving Healthcare with Kwadwo Kyeremanteng” These podcasts feature interviews and discussions on improving healthcare delivery in Canada. Underpinned by the values of cost-effectiveness, dignity, and justice, these podcasts will challenge the status quo, leaving no stone unturned as we explore gaps, assumptions, and different perspectives to solve problems in Canada's healthcare system. During the COVID-19 Pandemic, Dr. Kyeremanteng created ‘Solving Wellness,' a virtual health & wellness platform for healthcare professionals. ‘Solving Wellness' has been helping address healthcare burnout and providing health, fitness and self-care for its members. In this interview, Gregory Hundemer, MD, an associate scientist with the Ottawa Hospital Research Institute and Assistant Professor at the University of Ottawa, provides an overview of eating disorders and discusses new research he was involved with that could help with early diagnosis and intervention. About the Expert   Gregory Hundemer, MD, is an associate scientist in the Clinical Epidemiology Program at the Ottawa Hospital Research Institute, an assistant professor of medicine at the University of Ottawa, and a staff nephrologist at the Ottawa Hospital. He received his medical doctorate from Vanderbilt University and Master of Public Health degree from Johns Hopkins University. After medical school, he served 4 years as a flight surgeon in the United States Air Force. He then completed his internal medicine residency at Massachusetts General Hospital and nephrology fellowship at the combined Massachusetts General Hospital and Brigham & Women's Hospital Joint Nephrology Fellowship Program.

The applications of large multiplex panels for detection of pathogens have greatly expanded in the past several years. Initial tests were for detection of respiratory viruses, and the first such test was labor intensive and vulnerable to frequent contamination. Since then, additional sample types have been added, such as cerebrospinal fluid and positive blood culture broths, and tests are easier to perform and reasonably reliable. We discuss a research use only multiplex PCR assay for detection of pathogens in joint infections, and learning how it compares to targeted metagenomic sequencing and culture for detection of pathogens in periprosthetic joint infections. Taurine doses ranged from 500 mg daily to 1,000 mg three times per day.Participants who received taurine had lower fasting blood glucose, hemoglobin A1c (HbA1c, a marker of long-term glucose control), and insulin resistance, compared to those given a placebo.“Taurine emerges as a new option for the management of patients with diabetes,” the scientists asserted. (next) Vitamin D Deficiency Linked to Breast Cancer Ankara Numune Research Oncology Clinic (Turkey), November 1, 2022 Most women with breast cancer were found to be deficient in vitamin D, according to the results of a study published in the journal Progress in Nutrition.Researchers analyzed the vitamin D status of 561 women, average age 55, with non-metastatic breast cancer.In this group, 81% of the women were deficient in vitamin D, and 11% had insufficient levels of the vitamin.Median 25-hydroxyvitamin D blood levels were only 13.91 ng/mL in this group of Turkish women.Optimal ranges by U.S. standards are 30-80 ng/mL. Life Extension's minimal target for 25-hydroxyvitamin D is around 50 ng/mL.The researchers concluded that vitamin D levels should be measured in breast cancer patients and low levels should be corrected whenever diagnosed. (next) Marijuana smokers show higher rates of emphysema, airway diseases than tobacco smokers University of Ottawa (Ontario), November 15, 2022 Researchers from the University of Ottawa and The Ottawa Hospital have found marijuana smokers have a higher rate of emphysema and airway diseases compared to cigarette smokers.The findings, published in Radiology, examined the chest CT examinations of 56 marijuana smokers, 57 non-smokers and 33 tobacco-only smokers between 2005 and 2020. They determined higher rates of paraseptal emphysema and airway inflammatory changes, such as bronchiectasis, bronchial wall thickening, and mucoid impaction, in the marijuana smokers.”What's unique about this study is that there hasn't been anything comparing the imaging findings in tobacco smokers to marijuana smokers before. In fact, there is a lack of imaging research in marijuana, probably because it's still illegal in many parts of the world, and in many U.S. states, which is why I think we were the first to do a project like this.” Despite the small sample size, Revah's findings suggest that marijuana smokers saw additional effects on the lungs above tobacco alone, including more instances of large and small airways diseases. (next) Morning physical activity is associated with the lowest risk of heart disease and stroke Leiden University Medical Centre (Netherlands), November 14, 2022 Morning physical activity is associated with the lowest risk of heart disease and stroke, according to a study in more than 85,000 individuals published today in the European Journal of Preventive Cardiology. The findings were consistent regardless of the total amount of daily activity.The study included 86,657 adults aged 42 to 78 years who were free of cardiovascular disease at baseline. The average age was 62 years and 58% were women. Participants wore an activity tracker on their wrist for seven consecutive days. During six to eight years of follow up, 2,911 participants developed coronary artery disease and 796 had a stroke. Comparing peak activity times across a 24 hour period, being most active between 8 am and 11 am was linked with the lowest risks of both heart disease and stroke. In a second analysis, the investigators divided participants into four groups based on the peak time of physical activity: 1) midday; 2) early morning (~8 am); 3) late morning (~10 am); and 4) evening (~7 pm). The categories were selected according to peak times of activity in the study population, rather than being pre-determined before the study began.Participants who were most active in the early morning or late morning had 11% and 16% lower risks of incident coronary artery disease, respectively, compared to the reference group. In addition, those who were most active in the late morning had a 17% decreased risk of incident stroke compared with the reference group.When the results were analyzed separately according to sex, the investigators found that the results were particularly prominent in women but no longer significant in men. Women who were most active in the early morning or late morning had 22% and 24% lower risks of incident coronary artery disease, respectively, compared to the reference group. In addition, women who were most active in the late morning had a 35% decreased risk of incident stroke compared with the reference group. (next) New study shows astaxanthin is effective against daily mental and physical fatigue AstaReal Research (Japan), November 7, 2022 Japanese nutraceutical firm AstaReal says its astaxanthin has been found to be simultaneously effective against both mental and physical fatigue in a new published study. The research, published in the Journal of Clinical Therapeutics & Medicines, states that individuals were treated with astaxanthin for eight weeks in a double-blind, placebo-controlled study. The study was designed to induce fatigue and stress, similar to that encountered in daily life. There was a mental challenge, where individuals were subjected to a number of timed calculations (the Uchida-Kraepelin test), and a physical test with a bicycle ergometer. Metrics of fatigue were assessed both before and after the stressor tests. The study states: “Thirty-nine healthy subjects reporting awareness of fatigue were divided onto two groups. The subjects in one group (Astaxanthin group) ingested 12mg of astaxanthin and 20mg of tocotrienols for eight weeks. The other group (control group) ingested 20mg of tocotrienols for eight weeks.” Analysis showed that astaxanthin significantly reduced perceived symptoms of mental and physical fatigue compared to the placebo. These included improvements in clarity of thinking, concentration, motivation, and mood. Irritation and feeling of body heaviness were reduced. In the calculation test, an increase in errors observed in the placebo during the second half of the test was almost eliminated in the astaxanthin group. Supplementation with astaxanthin also significantly reduced salivary cortisol, a biomarker for stress. The study concludes: “The results suggest that astaxanthin supplementation has beneficial effects on fatigue encountered in daily life.” (next) High-fat diet can provoke pain sensitivity without obesity or diabetes University of Texas at Dallas, November 14, 2022 A new study in mice from researchers at The University of Texas at Dallas suggests that a short-term exposure to a high-fat diet may be linked to pain sensations even in the absence of a prior injury or a pre-existing condition like obesity or diabetes. The study, published in the journal Scientific Reports, compared the effects of eight weeks of different diets on two cohorts of mice. One group received normal chow, while the other was fed a high-fat diet in a way that did not precipitate the development of obesity or high blood sugar, both of which are conditions that can result in diabetic neuropathy and other types of pain. The researchers found that the high-fat diet induced hyperalgesic priming—a neurological change that represents the transition from acute to chronic pain—and allodynia, which is pain resulting from stimuli that do not normally provoke pain.

David Stewart, MD; Head, Division of Medical Oncology, University of Ottawa; Author, A Short Primer on Why Cancer Still SucksDr. Stewart received his MD degree from Queen's University, Kingston, followed by training in internal medicine at McGill University and in medical oncology in the Department of Developmental Therapeutics at the UT MD Anderson Cancer Center. He first moved from MD Anderson to the University of Ottawa in 1980, and served as Chief of Medical Oncology at the Ottawa Civic Hospital from 1989 to 1999. He returned to the Department of Thoracic/Head and Neck Medical Oncology at MD Anderson Cancer Center from 2003 to 2011 where he served as Chief of the Section of Experimental Therapeutics (2003-2005), Chair Ad Interim (2005), Deputy Chair (2006-2009), and Director of Translational Research (2009-2011). He was also the Principle Investigator of MD Anderson' phase II N01 contract with the National Cancer Institute, and was the clinical leader of a number of other federally-funded translational research projects. I spoke with Tuvik Beker, CEO of Pangea Biomed, an Israeli-based company tackling oncology drug development and treatment recommendation by not only looking at the single mutations in tumor cells, which the Pharmaceutical industry has already found targeted therapies for. Cancer treatments are evolving very rapidly, but precision and targeted therapies are still only effective in roughly 10% of cancer patients.
