Dr. John Sweetenham shares highlights from Day 5 of the 2025 ASCO Annual Meeting, including data from large trials in advanced malignant melanoma and mCSPC plus a new approach to first-line treatment for patients with multiple myeloma who are not transplant eligible. Transcript Hello, I'm Dr. John Sweetenham, the host of the ASCO Daily News Podcast, with my takeaways on selected abstracts from Day 5 of the 2025 ASCO Annual Meeting. My disclosures are available in the transcript of this episode. The selected abstracts from this final day of ASCO25 include important new data from large, randomized trials in patients with advanced malignant melanoma and patients with metastatic castration-sensitive prostate cancer, as well as a new approach to the first-line treatment of patients with multiple myeloma who are not transplant eligible. Starting with LBA9500, this study was conducted in patients with completely resected stage III or IV malignant melanoma and compared the combination of relatlimab plus nivolumab versus nivolumab alone in this population. The study, named the RELATIVITY-098 trial, was presented by Dr. Georgina Long from the University of Sydney, Australia. In her introduction to the study, Dr. Long explained that the current standard of care for adjuvant therapy of resected stage III/IV melanoma is with PD-1 monotherapy with nivolumab, but that about 50% of patients will suffer from a subsequent relapse. In the first-line setting in patients with advanced or unresectable melanoma, the combination of nivolumab with the LAG-3 inhibitor, relatlimab, has been previously shown to improve progression-free survival in the RELATIVITY-047 trial. The current study evaluated this same combination in the adjuvant setting. More than 1,000 patients from 24 countries were randomized to receive either nivolumab alone (546 patients) or the combination of nivolumab with relatlimab (547 patients). Both treatments were given for a maximum of 1 year or until progression of disease, unacceptable toxicity, withdrawal, or death. Various biomarker studies were also undertaken including LAG-3 and PD-1 expression on CD8-positive T cells. The primary endpoint of the study was relapse-free survival, and Dr. Long reported that this was the same in both arms of the study. For example, at 24 months, the relapse-free survival was 64% in the monotherapy arm compared with 62% in the combination arm. The hazard ratio was 1.01 and the P value was 0.928. Metastasis-free survival was also identical in both arms. No benefit was observed for the combination in any of the prespecified subgroups. No new toxicity signals emerged compared with the RELATIVITY-047 trial. Interestingly, the baseline surface expression of LAG-3 and co-expression of LAG-3 and PD-1 on CD8 T cells in the 098 adjuvant trial were lower than in the 047 advanced disease trial, perhaps explaining why the combination did not confer benefit over nivo alone in the adjuvant setting. This is an important result, demonstrating that results from one clinical setting cannot always be extrapolated to another. Although the combination has gained some use in the adjuvant setting, this study clearly demonstrates that more drug in this situation is no better and that monotherapy remains the current standard of care. Results from the AMPLITUDE trial for patients with metastatic castration-sensitive prostate cancer with alterations in homologous recombination repair (HRR) genes, in LBA5006, were presented today by Dr. Gerhardt Attard from University College London, UK. This international, multicenter study evaluated the combination of the selective PARP inhibitor, niraparib, in combination with abiraterone acetate and prednisone. The same combination has been previously shown to improve outcomes in castration-resistant metastatic prostate cancer harboring BRCA mutations in the MAGNITUDE study. The current trial included patients with castration-sensitive disease with HRR mutations including BRCA1/2. Six hundred and ninety-six patients were randomized between niraparib, abiraterone, and prednisone plus androgen deprivation therapy, or the same combination with placebo instead of niraparib. Permitted prior therapies included no more than 6 months of prior androgen deprivation therapy and the use of docetaxel, or prior palliative radiation therapy. The primary endpoint of the study was radiographic relapse-free survival. Dr. Attard reported that the risk for radiographic progression-free survival in the whole population was significantly reduced by 37% with niraparib and abiraterone acetate plus prednisone compared with the placebo arm. The radiographic progression-free survival risk reduction with niraparib in the prespecified BRCA1/2 subgroup was 48% and reached statistical significance compared with the placebo arm. The secondary endpoint of time to symptomatic progression was also improved with niraparib in the HRR population and the BRCA1/2 subgroup. There was a trend for overall survival favoring the niraparib combination. However, the overall survival data were immature at this first interim analysis and did not yet reach statistical significance. No new safety concerns emerged with the toxicity data consistent with the MAGNITUDE study. Less than 5% more of the patients on the experimental arm discontinued treatment in comparison to the control arm. The authors conclude that the AMPLITUDE study results support the use of niraparib, abiraterone, and prednisone as a new treatment option for patients with metastatic castration- sensitive prostate cancer and BRCA and homologous recombination repair gene alterations. The results certainly support this conclusion and are potentially practice-changing. Turning to hematologic malignancies, my final selection from today's presentations is Abstract 7504, presented by Dr. Hang Quach from St Vincent's Hospital, Melbourne, Australia, and describes a novel combination of elranatamab, daratumumab, and lenalidomide in patients with newly diagnosed multiple myeloma who are not transplant-eligible – the so-called MagnetisMM-6 trial part 1. Elranatamab is a novel bispecific T-cell engaging antibody directed against BCMA and CD3, which has previously been approved for certain patients with relapsed and refractory multiple myeloma. In the present study, this was combined with lenalidomide and daratumumab in newly diagnosed patients. The report today describes the dose-finding phase of this study, which was part 1, specifically addressing so-called dose level ‘G', comprising elranatamab 76mg subcutaneously every 4 weeks plus daratumumab 1800mg subcutaneously and lenalidomide 25mg given orally. Thirty-seven patients were entered at this dose level, of whom 32 were on treatment at the time of analysis. Early response data show an overall response rate of 97.3%. With median follow up of 7.9 months, the current CR rate is 27% with a VGPR rate of almost 68%. The most frequent toxicities were hematologic, with neutropenia observed in 75%. Some cytokine release syndrome was observed in about 60% of patients, but none was greater than grade 2. The authors conclude that this combination is active in untreated multiple myeloma, with manageable toxicity and evidence of responses which appear to deepen over time. The dose-finding component of this trial is continuing and will subsequently progress into a phase 3 trial based on the data from the current study. This will compare daratumumab plus lenalidomide with the same combination plus elranatamab in previously untreated patients. That concludes our special coverage from the 2025 ASCO Annual Meeting. Now the radio propagation report, compiled by G0KYA, G3YLA and G4BAO on Thursday the 29th of May 2025 Last week's high-speed solar wind, from a massive coronal hole on the Sun, took its toll on the HF bands. However, this week has been little better. Another coronal hole running diagonally across the Sun has been emitting particles that have pushed the Kp index up to a maximum of 6.67 w,hich puts us into visible aurora territory. Nevertheless, some DX has been worked, including CP4BT in Bolivia, ZP5KP in Paraguay and V44KAI in St Kitts and Nevis. These were reported by Karl, G0SKW on the CDXC Slack alerting system. BA100IARU, a special event station celebrating the 100th anniversary of the International Amateur Radio Union, has been active from Beijing, China and has been worked on 20m CW. The station's QRZ.com page provides real-time updates on which band the operators are working on. Most of the DX has been worked on 21, 18 and 14 MHz, with the 10m band mostly closed due to the summer doldrums. There have not been too many Sporadic-E openings on the 10m band. Instead, most of the Sporadic-E reports have been on the 6m band. The solar flux index has increased, standing at 144 on Thursday, the 29th of May, which is always a good sign. The solar disk is looking quite healthy at the moment with spots in both hemispheres. As of last Thursday, the 29th of May, there had been 43 C-class flares over the previous five days, as well as six more potent M-class events and one X-class flare. So, the Sun is still very active. Next week, NOAA predicts that the solar flux index will reduce again, perhaps going as low as 110 by Wednesday, the 4th of June. It does look like we are in for a rough ride with unsettled geomagnetic conditions forecast for the coming week. Looking further ahead, the 13th and 14th of June seem to be the worst days, when a Kp index of five is forecast. As usual, a high Kp index can result in lowered maximum usable frequencies and aurora-like conditions, with a slow recovery. And now the VHF and up propagation news from G3YLA and G4BAO The weather continues to be unsettled into the coming week, especially for northern areas, but with some drier and warmer spells in the south. There do not appear to be strong signs for any Tropo success, but there should be a chance of occasional rain scatter on the GHz bands. Some models suggest we are on the edge of high pressure over the North Sea. Others drive a deep low right across the country. This will resolve itself next week, but for now, it's not at all clear which forecast will win through. The prospects for meteor scatter are mostly random meteors, but towards the end of this wee,k the daytime Arietids will peak to enhance chances further. The solar auroral alerts keep coming, so it's reasonable to expect the chance of some radio response, such as watery-sounding signals on HF, even if we don't go to the full effects on VHF. It's certainly worth watching for those increasing Kp values. The Sporadic-E prospects often burst into life about now. So, after a few reasonable 6m band openings, like the one into southeast Europe on Wednesday the 28th, make sure you are ready for the first week in June – one of the prime weeks for Sporadic-E opportunities. This is partly due to meteor input and the fact that the tidal winds in the E region are taking on their summer pattern. The long polar days mean that the low-pressure weather patterns are also beginning to affect the northern polar routes to China and Japan across northern Russia, with their possible weather triggers, like jet streams. EME path losses are increasing again. Moon windows shorten, and peak elevation is falling as the Moon's declination goes negative again on Wednesday, the 4th of June. 144MHz sky noise is low and remains low as the week progresses. And that's all from the propagation team this week.

A notre micro, Stéphane raconte ses premières années dans la Sarthe, l'équitation à haut niveau, puis la découverte du skate et des vagues qui allaient complètement chambouler son plan de carrière. La découverte du snowboard dans les Alpes, puis celle d'Hossegor au début des années 90, ont fini par le convaincre que c'était autour de ces univers que sa vie tournerait désormais. ENGLISH Guest: Stephane Marteau (MC Box Events) If you've been to any major surfing events in France over the past 15 years, chances are you have seen a yellow background with the big black letters « MC BOX » on pick-up trucks driving along the beach or on the back of staff T-shirts. These letters represent the company, that Stéphane Marteau founded in 2008 and which has become a benchmark in cultural and sporting events management. They also embody his successful path through the initially emergent and eventually blazing surfing industry, some fifteen years earlier. Stéphane sat down with us to talk about his formative years in the Sarthe. He talks about elite horseback riding but also how he first got into skateboarding and waves, which would completely upend his world. After discovering snowboarding in the Alps and later Hossegor in the early 90's, he eventually came to understand that was where he wanted his life to go from that point on. Over a 30-year career, Stéphane has had a variety of jobs which would all, unbeknownst to him, be stepping stones towards his own success with MX BOX Events…they would also make for many great stories. But events are not Stéphane's only playing field. He will also tell us about his successful experience in the restaurant business and his ambitions in business tourism. Today he continues to pursue these other projects, while keeping a sharp eye on the various key events he's involved in, from the Paris-Dakar, to Hell Fest, including the Roland Garros French Tennis open, and of course surfing events that intertwine with action sports culture, its industry and athletes. This episode throws a rare light on a key player in our industry, who usually stays out of the spot light.

Teen-Turn and PayPal recently joined forces to host a coding and app development boot camp at PayPal's Dublin campus. This boot camp saw approximately 50 participants from schools and colleges across Ireland working with mentors and volunteers from PayPal for the global Technovation competition. Teen-Turn, a volunteer-led organisation and registered charity, aims to support course decision-making processes, advise on education and career options, and promote the accessibility of STEM career environments for girls from underserved and underrepresented communities. Meanwhile, Technovation is a tech education nonprofit that offers interactive learning programs in which young people and adults learn how to use technology to solve real-world problems. Whilst many of the attendees are taking part in this year's Technovation competition, some of the attendees on the day were from previous years of the competition and presented the apps they have created and the social enterprises they have launched through the Teen-Turn Incubator and Accelerator Program - including HerdSync and UNdump. In addition to coding and app development, boot camp participants worked with mentors and volunteers on business plans to support the rollout of their innovations. The attendees came from a range of areas and schools including Archbishop McHale College (Galway), Belmayne Educate Together Secondary School (Dublin), Coláiste Bríde (Dublin), Coláiste Nano Nagle (Limerick), DominicanCollege (Galway), Dunmore Community School (Galway), Galway Educate Together Secondary School, Glenamaddy Community School (Galway), Kylemore College (Dublin), Millstreet Community School (Cork), Mount Carmel Secondary School (Dublin), Mount St Michael Secondary School (Mayo), Mungret Community College (Limerick), Presentation Secondary School Warrenmount (Dublin), Salerno Secondary School (Galway), St Jarlath's College (Galway), St Patrick's Comprehensive School (Clare), and St Vincent's (Cork). Speaking about the event, Jacquie Murphy, Munster Co-ordinator with Teen-Turn, said: "We were blown away by the number of mentors in attendance to work with the girls on the day! The range of expertise was second to none, as was the warm welcome and friendly atmosphere. The girls were given expert advice, help and encouragement to make significant progress on their apps. "Working with mentors in the STEM industry, including leaders such as PayPal, gives the girls an opportunity to visualise themselves in those roles in the future. Visiting the campus is an invaluable experience that many girls would not get to do as a second-level student in Ireland. We are incredibly grateful to the team at PayPal for their work on this boot camp visit." He is the Head of the Heart, Exercise and Research Trials Lab at St Vincent's Institute and the Victor Chang Cardiovascular Research Institute, as well as being a Cardiologist and Head of Cardiovascular Research at St Vincent's Hospital Melbourne. He completed a PhD at St Vincent's and University of Melbourne and 4 years of post-doctoral research at the University Hospital of Leuven in Belgium. His research and clinical work focuses on the effect of exercise on the human heart. Dr. DeHeer serves as a Trustee on the APMA Board of Trustees. He practices in Central Indiana as part of the national supergroup Upperline Health. Dr. DeHeer serves as the residency director of the podiatric surgical residency program at Ascension St. Vincent Hospital in Indianapolis. In addition, he served as the team podiatrist for the Indiana Pacers for thirty years, from 1992 to 2022. Dr. DeHeer, DPM, is a member of the American Podiatric Medical Association (APMA) and a fellow of the American Society of Podiatric Surgeons (ASPS), and American College of Foot and Ankle Surgeons (ACFAS). He is also a member of the Royal College of Physicians and Surgeons of Glasgow. Dr. DeHeer is a Diplomat of the American Board of Foot & Ankle Surgery certified in foot surgery and reconstructive foot and ankle surgery. He is the inventor of The Equinus Brace™ and a partner in IQ Med. He is the President and Founder of Step-By-Step Haiti and Foot Aid. Dr. DeHeer's passion for teaching and providing lower extremity healthcare in developing countries culminated in recognition by the 2014 International Federation of Podiatrists Humanitarian Award and the 2011 APMA Humanitarian of the Year Award. Annie Clark, known as St. Vincent, launched her career as a guitar virtuoso—a real shredder—in indie rock, playing alongside artists like Sufjan Stevens. As a bandleader, she's moved away from the explosive solos, telling David Remnick, “There's a certain amount of guitar playing that is about pride, that isn't about the song. . . . I'm not that interested in guitar being a means of poorly covered-up pride.” Her songs are dense, challenging, and not always easy, but catchy and seductive. Remnick caught up with Clark before the launch of her new album, “MASSEDUCTION.” They talked about the clarity of purpose she needed in order to “clear a path” to write the “glamorously sad songs” she's become known for.This segment originally aired on October 13, 2017.
