Contributor: Ricky Dhaliwal, MD Educational Pearls: What can you do to control bleeding in a penetrating wound? Apply direct pinpoint pressure on the wound as well as proximal to the wound. Build a compression dressing. How do you build a compression dressing? Think about building an upside-down pyramid with the gauze. Consider coagulation agents such as an absorbent gelatin sponge material, microporous polysaccharide hemispheres, oxidized cellulose, fibrin sealants, topical thrombin, or tranexamic acid. What are the indications to use a tourniquet? The Stop The Bleed campaign recommends looking for the following features of “life-threatening” bleeding. Pulsatile bleeding. Blood is pooling on the ground. The overlying clothes are soaked. Bandages are ineffective. Partial or full amputation. And if the patient is in shock. How do you put on a tourniquet? If using a Combat Application Tourniquet (C-A-T) tourniquet, apply it proximal to the wound, then rotate the plastic rod until the bleeding stops. Then secure the plastic rod with a clip and make sure the Velcro is in place. Mark the time - generally, there is a spot on the tourniquet to write. Have a plan for the next steps. Does the patient need emergent surgery? Do they need to be transfered? How long can you leave a tourniquet on? Less than 90 minutes. What are the risks? Nerve injury. Ischemia. In today's episode I speak with trauma surgeons Drs. Morad Hameed and Elliott Haut. With the announcement of President Biden's first-ever federal Office of Gun Violence Prevention, today's conversation is on point. We chat gun violence in the USA and Canada and discuss the Stop The Bleed campaign. Morad and Elliott speak on what they do to prevent burnout in themselves given that they are exposed to so much trauma themselves. The STOP THE BLEED® campaign was initiated by a federal interagency workgroup convened by the National Security Council Staff, The White House. The purpose of the campaign is to build national resilience by better preparing the public to save lives by raising awareness of basic actions to stop life threatening bleeding following everyday emergencies and man-made and natural disasters.  Morad Hameed is a trauma surgeon and intensivist at the Vancouver General Hospital (VGH) and an Associate Professor of Surgery at the University of British Columbia (UBC). He completed medical school and surgical residency at the University of Alberta, graduate studies in public health at Harvard University, and fellowships in Trauma Surgery and Surgical Critical Care at the University of Miami.  Elliott Haut MD PhD is the Vice Chair of Quality, Safety, & Service, Department of Surgery, Professor of Surgery at Johns Hopkins Medicine and Editor in Chief at Trauma Surgery & Acute Care Open.

On November 14, 2019, my daughter's high school experienced a mass shooting. Children died. Don't think that "it cannot happen to you." Because of my family experience and the current culture crisis, I just went through training with "Stop the Bleed" to get certified and become an instructor. This is valuable training for how to manage large bleeds in an emergency trauma situation whether it be a mass shooting or car accident, etc. The training is FREE! You can watch the training video online and take the quiz for free--it takes about 25 minutes. If you want the actual certification, you need to take the hands-on course which takes about 60-90 minutes, and the in-person course is also FREE! If you don't have a trauma kit, they show you how to stop bleeds with a T-shirt, so why not go through free, concise, and such useful training? Who should do the training? Anyone that is old enough to understand it. In terms of youth and watching the online video, I think it would be helpful for some elementary students by 4th-5th grade if the parents are okay with the uncomfortable topic of trauma management.There is a national movement to put bleed kits at every AED station across the country--some schools and venues already have them in place. Spina Bifida is a spinal development disease that can be identified before birth, with a new surgical pre-delivery technique to try to correct it. And, Spina Bifida Occulta is a milder form that affects spinal alignment. Dr. Shyam Kishan, Board-Certified Orthopedic Surgeon at Medical City Children's Hospital stops by to tell us all about Spina Bifida. There's a program that was developed after the Boston Marathon bombing that trains citizens on how to administer emergency trauma assistance to literally help stop someone from bleeding to death in the event of an emergency. This can range from automobile accidents to some of the all-too-familiar tragedies we see nearly every day somewhere in our country. Crystal Perry, Trauma Program Director at Medical City Denton tells us about the program and how you can participate. Finally, Katherine Yoder, VP of Governmental Relations, Legislative Affairs at Parkland Health talks with Steve about the upcoming legislative agenda, and particularly how it might affect healthcare in North Texas. As a group of Rural Volunteer Firefighters, our Fire Department has built our firefighter training from both old and new tactics in the fire service and adapted them to fit the realities of equipment, manpower, and costs around what our community requires. May is National Trauma Awareness Month. May 19 is National Stop the Bleed Day. The number one cause of preventable death after injury is bleeding. Stop the Bleed is an initiative that was launched by the White House in 2015 as a national public awareness campaign to encourage bystanders to get trained, equipped and empowered to help in a bleeding emergency before medical professionals arrive. In this episode, you'll hear the story of 13-year-old Brett Baxter who was injured while playing football at school. But thanks to a campus police officer trained on how to Stop the Bleed, Brett is alive and recovering well. Show NotesJuly 7, 2021Please join me for this 20-minute program with Pamela Finnie, RN, Trauma Program Manager at Regional One Health, Memphis, TN. We discussed the role of a regional trauma center and an important public health program, "Stop the Bleed."Pamela explains what trauma is and the need for immediate specialized care. As a Level 1 Trauma Center, Regional One Health accepts patients from a 150-mile radius. These include motor vehicle accidents, gunshot wounds, falls, burns, and other bodily injuries. Ground and air ambulances arrive at the hospital regularly. The trauma team treats as many as 40 patients each day, every day of the year.Pamela is a local ambassador for the "Stop the Bleed" campaign, which teaches first aid for gunshot wounds and other injuries that result in bleeding. She explained that the American College of Surgeons spearheaded "Stop the Bleed" after studies of mass shootings revealed that bystanders could have saved lives if only they knew how. For example, law enforcement officers prioritize neutralizing the threat in a mass shooting rather than assisting injured victims. An uninjured bystander's simple application of pressure or tourniquet could decrease bleeding sufficiently until emergency medical services arrive.The "Stop the Bleed" program recommends three steps:1.     Before helping others, first make sure you are safe.2.     Call 911, 3.     Apply pressure to the wound or a tourniquet "Stop the Bleed" educational programs are free of charge.
