In the May episode of Chattering with International Cat Care, host Yaiza Gómez-Mejías introduces two expert-led conversations that bring the internal workings of the cat into sharp focus - from complications at birth to techniques for examining the feline fundus.First, Dr Petra Černá and Professor Danièlle Gunn-Moore discuss their JFMS article on dystocia and kitten mortality in pedigree cats. They describe the most common types of dystocia, reflect on breed-related risk factors, and share practical considerations from both clinical and breeding perspectives. Then, Dr Natalia Giannakopoulou and Dr Ben Blacklock offer hands-on advice from their recent Clinical Spotlight article on fundoscopy in cats. They cover how to build confidence with the ophthalmoscope, recommend suitable lenses for general practice, and explain how to approach the feline fundus methodically to avoid common diagnostic pitfalls. For further reading material please visit:Feline dystocia and kitten mortality up to 12 weeks in pedigree catsFundoscopy in cats: a practical guide and common findingsFor iCatCare Veterinary Members, full recordings of each episode of the podcast are available for you to listen to at portal.icatcare.org. This episode features Prathamesh Prabhudasai, co-founder and CEO of SafeBVM, discussing the critical issue of ineffective manual ventilation during resuscitation. We uncover the alarming statistic that up to 50% of patients experience hypoxia due to improper bag-valve mask (BVM) use. SafeBVM addresses this problem with a novel device that works with any commercially available BVM, providing a flow-limiting valve to ensure safer and more effective breaths. Prath also emphasizes the challenges faced by healthcare providers due to infrequent BVM use and high staff turnover, and how SafeBVM's technology helps maintain competency and improve patient outcomes.SafeBVM's approach offers a comprehensive solution that includes training coupled with their optimized manual bag for ventilation. Prath touches on the importance of federal funding in supporting clinical trials and supporting their development journey. Thanks to this foundation, the company has already seen significant success in cities like Colorado Springs and Saint Charles, with positive clinical outcomes and growing adoption. The company is now focusing on scaling, while also preparing to release exciting clinical trial data, be sure to tune in so you can be informed and inspired.

Father Peter Armenio joins Patrick to discuss Marriage (3:20) Is there a sacredness to marriage? How does the trinity work with marriage? (19:52) Break 1 Barbara - Would like to talk about Trinitarian part of marriage. Married 50 years. (31:26) Karen - Never give up and keep trying. Married 61 years. Had plenty of struggles and made it. 3 retreats. Marriage encounter and Retrouvaille. Amazing marriage now. Off to Fatima at the end of month. (35:03) Break 2 11:45 Marie - I think a lot of couples miss the point...the sacrament of marriage. It's very important. Didn't get married in church right away and went through many problems compared to those who went through it the right way. Could have saved us a lot of problems. Get it done right the first time. Also, attend retreats. (40:19) what does it mean to be open to life? Mary - Married 62 years. Parents were divorced. When I was getting married, made a novena that ended on my wedding day that BVM would protect our marriage. I attributed it to Mary. 4 children...1 miscarriage.
The principles of preoxygenation and passive oxygenation should be familiar to any experienced intubator. Best practice recommends the use of a nasal cannula in conjunction with either a non-rebreather mask or bag-valve mask (BVM) to prevent hypoxemia during the apneic phase of Rapid Sequence Intubation (RSI). In the PREOXI Trial, the authors pushed this concept further, questioning whether non-invasive positive pressure ventilation (NIPPV) might be more effective than these formally mentioned adjuncts. In this podcast, we dive into the strengths and weaknesses of the trial and explore whether its findings are truly practice-changing. We might even disagree along the way… (We also provide a rebuttal to some of the feedback we received on our previous podcast episode.) Survival from out of hospital cardiac arrest continues to be a benchmark for EMS Agencies across the globe. Focuses on chest compression quality and early defibrillation have led to substantial improvements in return of spontaneous circulation and thousands more cardiac arrest survivors being able to leave the hospital. Join this month's PCRF Journal Club to discuss a paper that evaluates the size of BVM used and its impact on cardiac arrest survival.

The tongue is the most common airway obstruction in an unconscious patient. For patients with a decreased level of consciousness that can't control their airway, yet have an intact gag reflex, the nasopharyngeal airway (NPA) should be used as an alternative to the oropharyngeal airway (OPA).Examples of when a NPA should be considered. Contraindications and considerations for nasal airway insertion. Measuring a nasal airway for appropriate length and diameter.Insertion of a nasopharyngeal airway. Patients with a NPA in place can receive supplemental O2, be ventilated with a BVM, have ETCO2 monitored, and have their upper airway suctioned as needed. Hypoxia is a state of low oxygen levels in the blood. Determining hypoxia using a pulse oximeter or arterial blood gasses (ABGs). A goal of ACLS is to recognize signs of hypoxia and provide timely treatment to prevent an arrest. Examples of some things that might lead us to think of hypoxia as a cause of cardiac arrest. Why we should not rely on pulse ox to give accurate readings during CPR. Delivering ventilations with near 100% oxygen concentration using a BVM attached to supplemental O2 and a reservoir. Using end tidal waveform capnography to assess the quality of CPR. Changes to ventilation rates, tidal volume, and O2 concentration affects a patient's oxygen, carbon dioxide, and pH. The danger of excessive ventilation of a patient in cardiac arrest. The JournalFeed podcast for the week of Dec 18-22, 2023.These are summaries from just 2 of the 5 article we cover every week! For access to more, please visit JournalFeed.org for details about becoming a member.Monday Spoon Feed:For the majority of out-of-hospital cardiac arrest (OHCA) patients undergoing standard (30:2 compressions:breaths) cardiopulmonary resuscitation (CPR) prior to definitive airway placement, bag-valve mask (BVM) ventilations did not achieve adequate lung volumes most of the time. Receiving adequate BVM ventilations >50% of the time was associated with significantly higher rates of return of spontaneous circulation (ROSC) and survival.Friday Spoon Feed:This was a stratified analysis of patients according to vulnerability to complications from COVID-19; it showed clinical benefit in most extremely vulnerable groups, but less benefit for healthier patients, even over age 70.
Contributor: Dr. Taylor Lynch Educational Pearls: Time of arrival until intubation was 26 minutes but nobody tried anterior neck access like a cricothyrotomy until his dad arrived Traditional ACLS protocol is not enough for anaphylactic respiratory arrest Circulating O2 from compressions alone is not enough to sustain the brain Patients need a definitive airway and endotracheal tube is the best method BVM ventilation is not enough to get patients the oxygen they need Time to anoxic brain injury during a respiratory arrest is 4 minutes Definition of anaphylactic shock: Acute laryngeal involvement with bronchospasms after known exposure to an allergen Do not need to have skin symptoms like the classic wheal and flare Must also have either hypotension (from vasodilation or end-organ hypoperfusion) or severe GI symptoms (crampy abdominal pain or repetitive vomiting) Treatment of anaphylactic shock: Push-dose IV epinephrine is better than IM epinephrine because IM epinephrine takes 4 minutes to circulate and get to the lungs Ketamine has broncho-dilating properties so it can be used as an induction agent for intubation Albuterol and ipratropium as continuous bronchodilators Magnesium and IV steroids AMAX4 acronym Adrenaline, Muscle relaxant, Airway, Xtra (bronchodilators, ventilation, vasopressors, and consideration of pneumothorax), 4 minutes to anoxic brain injury References Commins SP. When we show devotion to the Blessed Virgin Mary, we fulfill her prophecy that “all generations will call me blessed.” The Catechism clearly shows us how such devotion does not compete with devotion to God, but in fact fosters adoration of God. Fr. Mike declares that the better we know Mary, the better we know Jesus, and the more we love Mary, the more we love Jesus. Today's readings are Catechism paragraphs 971-975. Earlier in the Catechism, we unpacked Mary as the Mother of Jesus. Now, we look at what it means for Mary to be the Mother of the Church. Fr. Mike makes it clear that, when he entrusts his disciples to Mary, his Mother, from the Cross, he's entrusting us to her as well. Today's readings are from Catechism paragraphs 963-970.
