Ina Park, MD, is back again for a focused look at her (second) favorite sexually transmitted infection: syphilis (favorite because it's fascinating). She says, “it's one of the most challenging conditions to diagnose in clinical medicine because it literally can look like anything.” Join Ina and Tammy as they explore how syphilis was almost eliminated around 2000 and then surged again to current rates of infection in the US. Ina spotlights how a rise in congenital syphilis has fueled a mobilization in public health efforts, even while she celebrates declines in rates of the most infectious types of syphilis. As national syphilis screening guidelines are adapted to increase screening, including in emergency departments, Ina is optimistic for a reduction in future syphilis rates. But here's the takeaway for healthcare providers and patients alike: keep syphilis on your radar. Host, Dr. Catherine Cerulli, a professor of Psychiatry at the University of Rochester, is joined by her guest, Nila Bragg, MSN, CPNP, a certified pediatric nurse practitioner and a Medical Consultant for the New York City Administration for Children's Services (ACS). ACS's mission is to protect and promote the safety and well-being of New York City's children. As part of that work, child protective specialists from ACS conduct investigations to determine what is needed to best keep the children safe. For instance, to help keep families together, child protective specialists often connect families to services that can help, including counseling, substance abuse treatment, and access to concrete resources, like food, diapers clothing and even child care vouchers. Throughout this work, it is important that ACS child protective staff understand a variety of medical issues that the family may be coping with, as well as treatments, in order to better identify whether there are any potential safety risks.  As a Medical Consultant for ACS, Nila provides consultation to child protective specialists who have clients with complex medical histories. Nila helps CPS better connect children and their families with services that best support caregivers as they manage their child's medical needs. Nila's key message is that educating families and connecting them to resources is paramount to not only improve children's health, but connections also support the overall well-being of children and their families.

Please subscribe now for as little as 5$ and gain access to a community of over 800 awesome, curious, kind, funny, brilliant, generous souls Check out StandUpwithPete.com to learn more On Today's Show 32 minute News Recap Dr Ina Park begins at 34 mins  From InaPark.net : I was born and raised in the San Francisco Bay Area.  My parents are immigrants from South Korea who had an arranged marriage and ended up actually liking each other.  Being a first-generation Asian kid in the US, the extent of my sex education from my parents was, “don't have sex before you get married or we will kick you out of the house.” (In case you are wondering, I was already sexually active by the time I received this advice) My career in sexual health began as a peer educator at the University of California-Berkeley, where I dressed up as a giant condom and performed a live demo with a prophylactic and a banana on the steps of Sproul Hall.  After that there was no looking back: sexual health, sexually transmitted infections (STIs), and HIV prevention have been a steady presence in my life ever since.  After receiving my medical degree from UCLA, I completed residency in Family Medicine at Kaiser Permanente Los Angeles.  I then followed my husband to the University of Minnesota-School of Public Health for my master's degree. I possess a deep love for Minnesota, but two winters there was enough for me.  I settled back in California, where I completed a fellowship in Sexually Transmitted Diseases at the UC San Francisco School of Medicine (UCSF).  After all this training it was time to get a real job.  I'm now an Associate Professor in the Department of Family and Community Medicine at UCSF.  I also serve as the Medical Director of the California Prevention Training Center and a Medical Consultant for the Division of STD Prevention at the Centers for Disease Control and Prevention.  A few years ago, I decided that my time on this earth would be best spent making people feel better about their sex lives, reducing stigma around STIs, conducting good science and sharing it with the world as best I can.  So I decided to try my hand at writing a book about STIs, Strange Bedfellows, and someone (who is not related to me) thought it was good enough to publish.  Writing a book is similar to pregnancy and childbirth; it's a hell of a lot harder than it looks, and when it's over you need a few years to forget how bad it was before you can think of doing it again.  I live in Berkeley with my husband and two sons.  If I had more time, I would plant vegetables, pickle them, knit and brew bone broth.  I don't do any of those things.  I do practice yoga, and feebly attempt to meditate from time to time, but most of my waking hours are spent parenting and thinking about syphilis.   If you'd like me to come and speak to your group about my book or generally about the topic of sex and STIs or sexual health, please contact me here Buy Strange Bedfellows ------------------------------------------------------------- 1:22 At LOG OFF, we are passionate about lowering social media's impact on mental health while teaching teenage users and their parents about how to navigate the vast inner -workings of life on social media. Today, we are going to have a conversation about sickle cell  and rare diseases with someone whom I believe is uniquely qualified to have this discussion. My special guest today is Dr. Marjorie Dejoie-Brewer, owner of M.A.D.F.I.T MD, a medical consultant, wellness, and health business. She is also a Medical Consultant for the Sickle Cell Disease Association of America, and more recently held the position of Patient Engagement Lead in rare disease sickle cell for Pfizer.  Dr. Dejoie-Brewer is a patient herself with Sickle Cell Disease, and a firm proponent and practitioner of comprehensive and preventative health and wellness planning. Dr. Dejoie-Brewer has combined her expertise as a physician, her work in multiple sector settings, her passion for wellness, and her experience as a sickle cell patient to address the inequalities of sickle cell patients so that they live healthier lives without distrust of medical providers.  As always, Dr. Orsini keeps his promise about two things, that you will feel inspired, and you will have learned valuable lessons to be a better and more compassionate communicator.Marjorie tells us  about her personal journey as an immigrant's daughter to medical school.  She talks about changing career paths when she found out she had Sickle Cell Disease and the support network she had participating in a program called “Bridging the Gaps” which gave her a viewpoint of what medicine could be in real time. She also tells us what attracted her to starting a wellness clinic and focus on rehabbing individuals. Marjorie shares with us her view of chronic and rare diseases and how it is a redefinition of what optimal is every time you get sick.  Marjorie explains how that delivery of information from the doctor is so vital, and how these conversations definitely need more compassion. Dr. Orsini shares how in his workshops he talks about imagine, plan, and adapt when going into that conversation. We learn more about  issues with lower socioeconomic teenagers and minorities with Sickle Cell crisis  and how they are often viewed as drug seekers. She shares advice on what she tells patients and  providers in a crisis situation.   We end with Marjorie telling us the most difficult conversation she's had in her life and how she navigated through that conversation. I interviewed Dr. Ina Park, an associate professor at the University of California San Francisco School of Medicine. Linda Bluestein, M.D. has over 20 years of experience caring for patients and is the founder of Wisconsin Integrative Pain Specialists, a practice devoted to the holistic treatment of those experiencing persistent pain. Dr. Bluestein received her Medical Degree (M.D.) from the University of California, Los Angeles School of Medicine and completed her internship and anesthesiology residency at the prestigious Mayo Graduate School of Medicine in Rochester, Minnesota. Dr. Bluestein specializes in treating patients with connective tissue disorders such as Ehlers-Danlos Syndromes (EDS) and related disorders. She also treats the fatigue, memory dysfunction, sleep disturbances, and mood disorders that commonly co-exist with these painful conditions. Dr. Bluestein places a strong emphasis on the therapeutic relationship between the physician and patient and is certified by the American Board of Anesthesiologists. Let let's say the, the parents argued frequently. Maybe mother perhaps was depressed herself and so on. And so therefore this child grew up in an environment where at any moment at any time something could happen, you know a pot could be thrown across the room. Somebody could have a temper tantrum, a door might slam or, and so on. And so the child then came to view their world as a dangerous place. And as a place that that could change on a dime where they have no control at that they're insignificant and so on. Well, you can see if that's what this child learned, and by the way, given their life at that moment, that's not unrealistic. They have, an accurate view of what it was, what was going on, but what happens you see, as the child develops this life view and then carries it on into their adulthood.   Dr. Larry Waldman (16:28): II've had many individuals who that I work with and so on. And, you know, and often I'll say that, you know, you're doing a good job of depression. you're staying up late, you're sleeping through the days. So you're mixing your days and nights up around, you're eating junk, You're not answering the phone. You're not socializing. When, you go out, you don't smile. You don't even brush your teeth. You know, you wear old clothes and so forth. You know, you look and act like you're depressed. So you're doing depression. I use behavioural psychology, particularly when I work with parents, because it's, it's the easiest, if you will, to comprehend and to implement.   Dr. Manon (23:21): So, in a sense what I'm hearing you say is that it's good for the patient or the client to somehow be informed about the type of therapist that they're going to go to. It's like, you know, You're not going to ask a butcher what the right vegetables are to eat. It's not going to be the same thing. The harm caused by herpes simplex viruses (HSV) 1 & 2 often arises not from the pathology of the viruses themselves but rather from the stigma and shame associated with a positive diagnosis. In this episode, we attempt to lay a clear foundation for understanding not only how these viruses work but also what occurred to change the perception of them from “innocuous infection” to “dreaded disease”. Starting us off with his firsthand account is the incredible Courtney Brame, founder and host of Something Positive for Positive People, a non-profit organization and podcast that aims to provide community support, healing resources, and educational discussions around positive HSV and other STI diagnoses as well as larger issues in sexuality and physical and mental health. A native of New Jersey, Dr. Bonni Goldstein received her undergraduate education at Rutgers College. She pursued her medical degree at Robert Wood Johnson Medical School at the University of Medicine and Dentistry of New Jersey. Her post-doctoral education included internship and residency at Childrens Hospital Los Angeles. Dr. Goldstein also served as Chief Resident at Childrens Hospital Los Angeles. She was a Clinical Instructor in Pediatrics at USC School of Medicine in Los Angeles, Emergency Transport Attending Physician at Childrens Hospital Los Angeles and Emergency Medicine Attending Physician in the Pediatric Emergency Department at Los Angeles County-USC Medical Center. In 2008, Dr. Goldstein developed an interest in the science of medical cannabis after witnessing its beneficial effects in an ill friend. Since then she has been evaluating both adult and pediatric patients for use of medical cannabis. Dr. Goldstein has given numerous lectures to many patient support programs, including the Cancer Support Community, Southern California Prosthetics, and Pediatric Epilepsy. Scientific American recently published the article "", an article that proposes that...   ..."recent research suggests it may be possible to reverse mitochondrial decay with dietary supplements that increase cellular levels of a molecule called NAD (nicotinamide adenine dinucleotide)"...   ...and also that...   ..."the mitochondria in muscles of elderly mice were restored to a youthful state after just a week of injections with NMN (nicotinamide mononucleotide), a molecule that naturally occurs in cells and, like NR, boosts levels of NAD"...   Since that article was published I've received an onslaught of questions about this mysterious molecule called NAD. It just so happens that a friend of mine, Thomas Ingoglia, known as one of the best NAD scientists on the planet and is also in contact with the best NAD clinicians on the planet - both with decades of experience second to none. I consider Thomas himself to be one of the most knowledgeable and frequent users of NAD I've ever met, and one of the few that has been playing around with NAD in combination with cryotherapy, blueberry extract, hyperbaric oxygen and other "biohacks" to completely turn him around from being bed-ridden sick and losing half his family in a car crash to being in the best health of his life, including crushing his first Spartan race with me last year (prior to which took high doses of NAD).   Problem is, most NAD clinical researchers are all underground at the moment. The FDA doesn't look kindly at NAD supplement companies and integrative doctors, they are quite skeptical of naturopathy, and their first impulse is to turn these things and others into patentable drugs because that’s the language the FDA speaks. Plus, NAD can be dangerous. Thomas even knows a guy personally (ironically, a Phd in toxicology that poisoned himself due to his own error) who hospitalized himself experimenting with the substances we’re going to be talking about in this podcast episode. So you need to proceed with caution and with the formal clinical information Thomas has opened my eyes to. Along with Thomas, today's podcast features Dr. Ross Grant, Phd. Dr. Grant is perhaps the most prolific authors in the field of NAD, and he specializes on NAD in the brain. He started researching NAD research back in 1994 while being laughed at, when no one was doing NAD research.   Dr. Ross Grant is Clinical Associate Professor at the University of Sydney Medical School and CEO of the Australasian Research Institute, Sydney Adventist Hospital. A biochemical pharmacologist with a Ph.D. in Neurochemistry/Neuropharmacology, Dr. Grant’s research is focused on NAD - specifically the role of oxidative stress - e.g. emotional stress, diet, and exercise - and NAD metabolism on brain cell death and cellular degeneration. A secondary interest is in the effect of exposure to novel nutritional components, such as polyphenols, on human cellular response to oxidative stress, with a goal of detecting and correcting early degenerative biochemical changes associated with aging-related degenerative disease. Dr. Grant is a member of the Australian Society for Medical Research (ASMR), Australian Neuroscience society (ANS), Australian Society of Clinical & Experimental Pharmacology and Toxicology, Nutrition Society of Australia (NSA).   With forty-eight articles published in peer-reviewed scientific journals, Dr. Grant is perhaps the most prolific author in the field of NAD research. In addition to Dr. Grant, we are joined by Dr. Philip Milgram, MD, from the . Dr. Milgram recovered from his own challenges with addiction and now helps other people in recovery from addiction, specifically by using NAD protocols. He trained in 1991 with Vernon Johnson, the man who coined the term “Intervention”. He was certified as a Prevention Specialist by the Certifying Board of Alcohol and Drug Counselors (CCBADC) in 2001, and has three degrees in counseling from UCSD; in Counseling and Interpersonal Communication, Alcohol and Drug Counselor and Advanced Intervention. In addition to NAD Treatment Center, Dr. Milgram served as the original Medical Director of Confidential Recovery and the Pemarro Detox Center. He has also served as the Medical Consultant for The Soledad House Recovery Home for Women and ABC Recovery since they opened. He is a member of the attending staff at Scripps Memorial Hospital in La Jolla and Board Certified in Obstetrics and Gynecology. Dr. Eric Leuthardt is a neurosurgeon at Barnes-Jewish Hospital and the Siteman Cancer Center in St. Louis and also the Director for the Center for Innovation in Neuroscience and Technology at Washington University in St. Louis. In addition, he is a Senior Inventor with Intellectual Ventures and a Technical and Medical Consultant for Ascension Health Ventures. Eric received his Medical Degree from the University of Pennsylvania School of Medicine, and completed his Chief Residency and Residency at Barnes-Jewish Hospital and the Washington University School of Medicine. He also did a Fellowship in Spinal Surgery and Epilepsy in the Department of Neurosurgery at the University of Washington School of Medicine and Harborview Medical Center in Seattle. Eric is a surgeon, researcher, and innovator. He has received many honors and awards for his work including recently the Google Research Award for Neurosurgery in 2013 and was a winner in the XPrize Visioneering competition in 2013. He has also been presented with the Annual Award of the American Academy of Neurological Surgery in Berlin, one of the highest acknowledgments in his field and been listed among the MIT Technology Review's top 100 innovators. Eric is with us today to tell us about his journey through life and science.
