On this episode of JHLT: The Podcast, the JHLT Digital Media Editors explore two studies from the June issue of The Journal of Heart and Lung Transplantation. Digital Media Editor Van-Khue Ton, MD, a transplant cardiologist from Massachusetts General Hospital, hosts this episode.   First, Dr. Ton and Digital Media Editor Marty Tam, MD, interview their first guests, first author Matthew Carey, MD, MBA, and senior author Justin Fried, MD, both of the Columbia University Irving Medical Center in New York City, on their study “Aortic Root Thrombosis in patients with HeartMate 3 left ventricular assist device support.”   This retrospective study of all patients receiving a HeartMate 3 LVAD at a single center between November 2014 and August 2020. The study evaluated findings related to patients with aortic root thrombosis, classified as having at least 1 echocardiogram or contrast-enhanced CT scan with thrombus. In the population of 197 patients, 19 had aortic root thrombus, which was ultimately associated with an increased risk of developing significant aortic regurgitation during the study period.   Drs. Carey and Fried discuss whether aortic valve opening is associated with increased risk of aortic root thrombus, how to balance the bleeding-thrombosis scale in patients, and how the study fits in the context of prior generations of LVAD.   Next, Dr. Ton joins and Digital Media Editor Erika Lease, MD, FCCP, to interview, Jacqueline DesJardin, MD, a Fellow in the department of medicine at the University of California San Francisco. Dr. DesJardin is first author on the study “Investigating the “sex paradox” in pulmonary arterial hypertension: Results from the Pulmonary Hypertension Association Registry (PHAR).”   PHAR is a multicenter US-based registry of patients with PAH, and this study analyzed 1,891 patients from the registry, 1,425 (75%) of whom were female. At baseline, compared to men, women had worse functional status and worse hemodynamics. Women were more likely to be on triple therapy or parenteral prostacyclin therapies at baseline. Interestingly, women had better survival than men, even after adjusting for numerous variables.   In the discussion, Dr. DesJardin explains what collider stratification bias is, and how it may illuminate the complex epidemiological system that creates this disparity. She shares the three potential causal models posed in the study, and considers how the study might be followed up.   Follow along at www.jhltonline.org/current, or, if you're an ISHLT member, access your Journal membership at www.ishlt.org/jhlt.   Don't already get the Journal and want to read along? Illuminating Fungi: the Science of Fungal Bioluminescence with Brian Perry PhD Fungal bioluminescence was first described by Aristotle (384-322 B.C.E.), and continues to fascinate and puzzle scientists today. While over 100 species of fungi are known to produce luminescent mushrooms or mycelium, the chemical and genetic basis of the light-producing reaction was only recently discovered, and the reason these fungi glow remains somewhat of a mystery. Come learn about our current understanding of the evolution, ecology and biochemistry of this phenomenon. And yes, there will be lots of pictures of glowing mushrooms! In this work, which blends sports and history together, Desjardin looks at the historic and turbulent events of September 1918 that affected Boston. The Red Sox won their fourth World Series in seven years, but the specter of World War I had shortened the baseball season. Players, disgruntled by the idea that they would be shortchanged in the regular season and the World Series, went on a brief strike before Game 4. A young Babe Ruth was a masterful pitcher but coming into his own as a hitter, but during the World Series, manager Ed Barrow was reluctant to use his left-handed star in the lineup when he was not pitching. Overseas, a Massachusetts volunteer unit led the first unified U.S. fighting unit into battle in France, while teenager David Putnam established himself as America’s flying ace during his short life. At home, an outbreak of Spanish influenza erupted in Boston and its suburbs, causing death at an alarming rate. Desjardin also touches on the women’s suffrage movement, spearheaded by Maud Park, who gained an audience with President Woodrow Wilson; the actions of Massachusetts Lt. Gov. Calvin Coolidge; and baseball players’ exemption from the “work or fight” order until after the World Series. Bob D’Angelo earned his master’s degree in history from Southern New Hampshire University in May 2018. “Stand Firm Ye Boys From Maine” With a new preface and updated maps and illustrations, Stand Firm Ye Boys of Maine offers a compelling account of one of the most crucial small engagements of the Civil War. This powerfully narrated history tells the story of the 20th Maine Regiment, the soldiers who won the battle of Little Round Top. The culmination of years of detailed research, the book uses more than seventy first-hand accounts to bring the personal experiences of the soldiers to life, relating the story from both sides. Thomas Desjardin was born and raised in Maine and did his doctoral work in American History at the University of Maine. He has worked as an interpreter, giving programs on the Gettysburg battlefield for the National Park Service. The author of These Honored Dead: How the Story of Gettysburg Shaped American Memory and Through A Howling Wilderness: Benedict Arnold's March to Quebec, 1775, he is working on a TV documentary on Joshua Chamberlain.

Part 1 - Living in Gettysburg, historian Thomas Desjardin discovered that many of the most well-known stories of the battle are badly distorted versions of the original events. In These Honored Dead: How the Story of Gettysburg Shaped American Memory Desjardin describes how the postwar efforts of John Bachelder, Dan Sickles, and others helped to create the modern view of what happened at the battle. He raises the disturbing question: is it possible ever to know what really happened in the past?
