In this episode of the Society of Critical Care Medicine (SCCM) Podcast, Diane C. McLaughlin, DNP, AGACNP-BC, CCRN, FNCS, FCCM, is joined by Aarti Sarwal, MD, FAAN, FNCS, RPNI, FCCM, and Brian L. Erstad, PharmD, FCCP, FASHP, MCCM, to discuss the 2026 guidelines for neuromuscular blockade in adult patients with acute respiratory distress syndrome. The guidelines, “Society of Critical Care Medicine Guidelines for the Administration of Neuromuscular Blockade in Adults With Acute Respiratory Distress Syndrome,” were published in the March issue of Critical Care Medicine. Drs. Sarwal and Erstad discuss how the evidence in two key trials, ACURASYS and PETAL-ROSE, has helped shape the recommendations provided in the SCCM guidelines. Despite how influential these trials were in shaping the recommendations, only conditional recommendations were made due to low or very low quality of evidence. The lack of evidence proved to be a driving factor in including a call to action in the guidelines. Future research priorities largely revolve around precision medicine and finding more patient-specific interventions to improve patient outcomes. Aarti Sarwal, MD, FAAN, FNCS, RPNI, FCCM, is a professor of neurology and the division chair of neurocritical care at Virginia Commonwealth University (VCU) School of Medicine in Richmond, Virginia, USA. She is also an associate editor of Critical Care Medicine, secretary of the American Society of Neuroimaging, and director of VCU-Wake Forest neuro-ultrasound courses. Brian L. Erstad, PharmD, FCCP, FASHP, MCCM, is a tenured professor and interim dean at the University of Arizona R. Ken Coit College of Pharmacy in Tucson, Arizona, USA. He is also a center investigator for the Center for Health Outcomes, a member of the BIO5 Institute and Comprehensive Center for Pain & Addiction and Pharmacoeconomics Research Center, and a codirector for the Arizona Clinical and Translational Research Graduate Certificate Program. The perspectives and experiences she shares are her own and do not represent her employer or any organization she's affiliated with.   In this episode, we're discussing neuroaffirming care, "dispensing" practical tips to help you better serve patients on the autism spectrum, and providing context to help you understand the broader ecosystem that patients on the spectrum and their caregivers navigate every day. Once you hear about neuroaffirming care, it just might change the way you approach all neurodiverse patients and possibly improve your relationships with them, too.   At the very end of this episode, I give an update about my son Kraig, who is now 23 and has autism.   Take-aways from this episode If you've met one person with autism, you've met one person with autism. Every person with autism is different. Curiosity is a clinical skill. When you're curious, you get the information you need to have positive patient interactions. Presume competence, but verify understanding. Use tools like the teach-back method, especially when a caregiver is involved. Meet patients where they are. Tailor your patient counseling to meet your patient's needs. That might mean slowing down your rate of speech during patient counseling, adjusting your communication style, including a parent or caregiver in the conversation, or being mindful of sensory needs like noise, light, taste, texture, or smell. Social stories are a useful tool. Use pictures paired with statements to help patients on the spectrum learn about a shot or a medication before they experience it. Language matters. People with autism are "people first." Don't objectify or dehumanize them with language - intentionally or accidentally.  Pharmacists are consistent touch points in our fragmented US healthcare system. We may see neurodiverse patients more often than their primary care or other providers see them. You are a constant in someone's life. Think ability first. Think about what you are able to do as that constant in your patient's life. The next time you interact with a patient who is neurodiverse, try doing just one thing differently. Wait a little longer for your patient to respond, ask one more question, use their name, etc. Those small moments may even save you time later. They build trust, prevent confusion, reduce callbacks, and help patients get it right the first time.    These calls to action will make your care more effective, not add additional burdens.   If this episode resonated with you, I'd love to hear from you. Today we are bringing you a conversation on the evolving landscape for idiopathic pulmonary fibrosis (IPF) and progressive pulmonary fibrosis (PPF) treatment. Ryan Haumschild, PharmD, MS, MBA, vice president of ambulatory pharmacy at Emory Healthcare and Winship Cancer Institute, spoke with Marilyn Glassberg, MD, the John W. Clarke Professor and Chair of medicine at Loyola University Chicago Stritch School of Medicine; Ayodeji Adegunsoye, MD, MSc, PhD, FACP, FCCP, assistant professor at the University of Chicago; and Janet Pope, MD, MPH, professor of medicine and division member of rheumatology at the University of Western Ontario in Canada. IPF and PPF are increasingly prevalent conditions that pose a growing burden on both patients and health care systems. Despite the availability of FDA-approved antifibrotic therapies, the median survival for patients with IPF and PPF remains less than 5 years after diagnosis—underscoring the critical unmet needs that persist in this field. The panelists discussed the impact these conditions have on patients' lives, evaluated emerging agents, and addressed economic considerations that affect treatment decisions and health care resource allocation.

He completed his residency in internal medicine and fellowships infectious diseases and critical care medicine at Naval Medical Center San Diego. During his military service, he served as the flight surgeon for Carrier Air Wing SEVENTEEN embarked onboard the USS George Washington (CVN-73), at the Naval Medical Research Unit No. 6 in Lima, Peru, conducting preclinical and clinical studies in antimicrobial drug resistance and vaccine development, as director of medical services at the NATO Role 3 Multinational Medical Unit at Kandahar Airfield, Afghanistan, and as ID division chief and fellowship director in San Diego. He retired from active duty in 2021 and joined the faculty at Wake Forest. He is the chair of the ABIM Critical Care Medicine Examination Board, co-chair of the SCCM Congress Program Committee, and Chair-Elect of the Chest Infections and Disaster Response Network in CHEST, as well as deputy editor for outreach for the journal CHEST and contributing editor for Critical Care Explorations. He is an author of over 150 scientific manuscripts, 15 textbook chapters, and 100 conference abstracts and invited lectures.
