In this episode, we're taking a closer look at ketosis and how the ketogenic diet has evolved beyond just a weight-loss tool. I'm joined by Dr. Dominic D'Agostino, who unpacks the science behind Ketone Metabolic Therapy (KMT) and its growing role in treating conditions like epilepsy, neurological issues, and metabolic disorders. His research sheds light on the therapeutic potential of ketones, which are not only vital for energy but also key players in reducing inflammation and enhancing brain health. We also clear up some common misconceptions about keto, like battling the keto flu, and talk about practical ways to adopt and optimize the diet for better overall health. Whether you're curious about using keto for disease management or just want to understand how it can support mental and physical wellness, this episode has something for you. It's all about understanding the science and making the keto lifestyle work for your body! Key Takeaways: The ketogenic diet historically originated as a treatment for epilepsy and has expanded to other therapeutic areas, such as metabolic syndrome and neurodegenerative diseases. Ketone bodies, especially beta-hydroxybutyrate, perform dual roles as potent energy sources and cell signaling molecules that offer anti-inflammatory benefits. Keto metabolic therapy (KMT) encompasses multiple approaches, including diet, fasting, and supplements, to maintain therapeutic ketosis. Adoption of a ketogenic lifestyle may require supplement support, particularly with electrolytes, to avoid side effects like keto flu. Variations in genetic and gender responses can influence the effectiveness and experience of ketosis, necessitating personalized dietary adjustments. More of Dr. Dominic D'Agostino: Dr. Dominic D'Agostino is a tenured Associate Professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida (USF) Morsani College of Medicine and serves as a Visiting Senior Research Scientist at the Institute for Human and Machine Cognition (IHMC). He earned his B.S. in Nutritional Science and Biological Sciences from Rutgers University in 1998, followed by a Predoctoral Fellowship in Neuroscience and Physiology at Rutgers and the University of Medicine and Dentistry of New Jersey (UMDNJ) in 1999. In 2004, he completed a postdoctoral fellowship in Neuroscience at the Boonshoft School of Medicine at Wright State University in Ohio before joining USF in 2006. In addition to his teaching responsibilities, Dr. D'Agostino leads a lab focused on developing and testing nutritional strategies and metabolic-based therapies for neurological disorders, cancer, and metabolic optimization. His research includes both basic science studies and human clinical trials. He has a personal interest in environmental medicine and developing methods to enhance the safety and physiological resilience of military personnel and astronauts in extreme environments. Dr. D'Agostino's research in metabolic control and neuroscience is supported by the Office of Naval Research (ONR), Department of Defense (DoD), private organizations, and nonprofit foundations. In this episode, we chat with Louis DiGiovine PT, who is a clinically talented orthopedic manual therapist with 41 years of experience. Lou graduated from the very first class at The University of Medicine and Dentistry of New Jersey (UMDNJ) in June 1980 giving him the distinction of being one of the first eighteen physical therapists graduating in the State of New Jersey. He continued that “pioneering” spirit when he took his first position as a staff therapist at the busy and reputable Christ Hospital in Jersey City, NJ. Working and training in an urban General Hospital provided him with a strong background in medical and surgical knowledge. In 1983 he became a co-founder of Hudson Heights Physical Therapy (one of the first few private practices owned by physical therapists in New Jersey). Through that organization, he performed contract-hospital work at St. Mary's Hospital in Hoboken, Palisades General Hospital in North Bergen, Meadowlands Hospital in Secaucus, Essex County Geriatric Facility in Belleville, The Industrial Medicine Center in Secaucus, Union City Visiting Nurse Association, and Christ Hospital Homecare Agency for more than five years. During those early years, he attended many extensive training courses, workshops, seminars, in-services, study groups, and conferences throughout the country. Lou learned all of this extensive training to become the best physical therapist that he could be and provide his patients with the best care possible. Throughout his early professional years, his primary area of interest and goals was to become an orthopedic manual therapist with a specialty in spine care. In time, Lou became known as a “spine therapist” and as a result received many more referrals from spine care physicians, also helping him to accelerate his knowledge base in the treatment of spinal injuries, conditions, and surgeries. It was through the discovery process of learning spine care and treating many patients that Lou became acutely aware of the relevance and close interrelationship of back and neck problems, TMJ dysfunction, headaches, upper quarter dysfunction, shoulder problems, posture and pain, orthotics, body mechanics, ergonomics and lifestyle management. All of which he has received specific training and education. He has lectured on these topics to study groups, in-services, seminars, physical therapy students, industrial work sites, corporations, athletic organizations, and varied community groups. Throughout the past twenty-nine years, Lou has participated in various treatment programs and clinics including sports medicine clinics, amputee clinics, prosthetics and orthotics clinics, arthritis clinics, work hardening, worksite ergonomic assessments, pre-employment screens, and pre-season athletic screenings. In 1996 Lou founded Excellent Physical Therapy Back and Neck Care an orthopedic physical therapy private practice dedicated to the treatment of spine care. Within the practice, he developed and implemented administrative and treatment standards of care through policies, procedures, and protocols. The interweaving of professional knowledge and experience with tested business practices is what made Excellent Physical Therapy a very unique facility where dedication to excellence was evident. Lou DiGiovine PT continued his training and education by establishing a working relationship with two recognized spine surgeons and performing rounds with the physicians and their patients six hours per week for eight years. This once-in-a-lifetime experience allowed him to gain an understanding of the complete spectrum of spine care treatment including medicine, pain control methods, epidural injections, spinal diagnostics, indications, and contraindications of spine surgery, types of surgeries alternative care, and spine surgery rehab. Lou is an active participating clinician of the New Jersey Spine Institute, North American Spine Society, and The American Physical Therapy Association. He is licensed by the New Jersey State Board of Physical Therapy. He has inspired others to follow his lead and with the addition of experienced and qualified staff over the years, he has expanded the scope of Excellent Physical Therapy to include all orthopedic musculoskeletal conditions including the treatment of knees, shoulders, fractures, joint replacement, pediatric and geriatrics. (Since recording this interview, Lou's practice has been acquired by Jag-One Physical Therapy. Lou continues to practice here and is also the Clinical Director.) Highlights of this podcast include: Upper quarter dysfunction Postural syndromes Orofacial pain TMJ Trigeminal neuralgia Spine-health Back pain and rehabilitation Progressing through therapy Trigger Points (network) Soft tissue release Posture Throat/Neck pain Pterygoid muscles Tinnitus Subclavius muscle Heat vs Ice And So Much More! In 1987, Mark Hollis, a practice management consultant in New York City, began to research practice management software for several clients and friends, including a dentist and a family practitioner. Mark determined that, thanks to their ease of use, Macintosh computers would be best for his clients. He bought a Macintosh II and began looking for suitable software for his friends. A year later, after attending numerous trade shows and seeing many demos, Mark discovered HealthCare Communications, Inc. (HCC). Mark knew that their software was the one that met his criteria, and soon MediMac, DentalMac, and ChiroMac became an integral part of his practice management consultancy. Mark Hollis had continuous experience with MediMac, DentalMac, and ChiroMac marketing, sales, training and support for the next 15 years. He and his organization supported and consulted with over 600 clients in New York, New Jersey, and Pennsylvania. 20-30% of all dentists and physicians who used MediMac, DentalMac, or ChiroMac nationwide were Mark's clients, due in great measure to the superior ongoing local support he and his staff provided. During the past decades, Mark was responsible for designing and implementing a number of marketing and training initiatives for Apple Computer. For example, he authored a business plan for the Apple SBSS (Small Business Selling System) initiative creating a duplicateable reseller/developer model relationship. This model partnership leveraged the marketing and personnel strengths of each to advertise and produce over 200 Apple seminars, which Mark presented for HealthCare Communications. Mark delivered the very first vertical market seminar in the nation in an Apple Market Center. This model was followed by other HCC sales representatives throughout the US and significantly contributed to the success of HCC. At Apple's request, he developed and implemented a national training program for Apple MicroAge resellers interested in the healthcare market. He worked with an Apple Evangelist to the healthcare market to create and deliver a seminar program across the US at Apple Market Centers and SBA offices. Interested doctors who attended these activities were put in contact with local representatives. Many of Mark's sales and marketing initiatives provided the model for numerous HCC marketing programs. He was given numerous awards by HCC for his contributions and for being the top sales representative in the US. The University of Medicine and Dentistry of New Jersey (UMDNJ) requested that Mark construct and deliver a practice management course to dental students at their school, which he did for several years. Mark has also delivered several presentations at various dental and medical meetings. Mark continued to work with DentalMac customers, providing training, support, and service. He maintained a cooperative relationship with the MacHealth™ division of WebMD, and worked together with the Lincoln Nebraska staff to maintain customer satisfaction. He, along with his wife and business partner Mickey, have been steadfast in their commitment to their clients, to MediMac, DentalMac, and ChiroMac, and to the Macintosh platform. Mark found many ways to enhance what his clients were able to accomplish with MacHealth software. Mark was an Apple VAR for 12 years and participated in Apple's Science and Technology initiative. He was an Apple Product Professional, Apple Solutions Expert (ASE), a member of Apple Consultant Network (ACN), and a Macintosh Trainer for 10 years. By 2004 Mark felt the need to advance the software and support to a new level to create the next generation of Practice Management. His thinking was shared by the original programmer of MediMac, DentalMac and ChiroMac, Patrick Clyne. Together they founded MacPractice, Inc. in May 2004. https://www.macpractice.com/
Dr. V.A. Shiva Ayyadurai, the inventor of email and polymath, holds four degrees from MIT, is a world-renowned systems scientist, inventor and entrepreneur. He is a Fulbright Scholar, Lemelson-MIT Awards Finalist, India’s First Outstanding Scientist and Technologist of Indian Origin, Westinghouse Science Talent Honors Award recipient, and a nominee for the U.S. National Medal of Technology and Innovation. His love of medicine and complex systems began in India when he became intrigued with medicine at the age of five as he observed his grandmother, a farmer and healer in the small village of Muhavur in South India, apply Siddha, India’s oldest system traditional medicine, to heal and support local villagers. These early experiences inspired him to pursue the study of modern systems science, information technology and eastern and traditional systems of medicine to develop an integrative framework linking eastern and western systems of medicine. In 1978, as a precocious 14-year-old, after completing a special program in computer science at the Courant Institute of Mathematical Science at NYU, Ayyadurai was recruited by the University of Medicine and Dentistry of New Jersey (UMDNJ) as a Research Fellow, where he developed the first electronic emulation of the entire interoffice mail system (Inbox, Outbox, Folders, Address Book, Memo, etc.), which he named “EMAIL,” to invent the world’s first email system, resulting in him being awarded the first United States Copyright for Email, Computer Program for Electronic Mail System, at a time when Copyright was the only protection for software inventions.
