Is That Normal? What To Expect From A Newborn with Dr. Tracey Agnese - A Revisit

Newborn babies have some peculiar quirks, habits, and bodily functions. If you're entering parenthood unfamiliar with what's normal and what isn't you might be scared or deeply concerned by things that are pretty routine for new babies.  To have this conversation today with me on Yoga| Birth| Babies I have Dr. Tracey Agnese. Tracey received her undergraduate degree from the University of Delaware.  She graduated from SUNY Downstate medical school and completed her pediatrics residency at NYU. As a Pediatrician and a mom, Dr. Agnese knows how difficult it is to have and care for a newborn baby. She also knows that new moms often neglect taking care of themselves and wants to help change that. Dr. Agnese sees patients in her private practice on the UWS and is also an International Board Certified Lactation Consultant (IBCLC).  In this episode, Dr. Agnese goes over many things from oddly colored baby poop, to irregular breathing patterns, to signs that something is really wrong and you should consult your doctor. My hope is that new parents can be put at ease by learning about what is normal to expect from their little one!   On this episode of Unsupervised Learning Razib talks to Tade Souaiaia, a statistical geneticist at SUNY Downstate about his new preprint, Striking Departures from Polygenic Architecture in the Tails of Complex Traits. Souaiaia trained as a computational biologist at USC, but also has a background as a division I track and field athlete. Razib and Souaiaia discuss what “genetic architecture” means, and consider what we're finding when we look at extreme trait values in characteristics along a normal distribution. Though traits like height or risk for type II diabetes can be thought of as represented by an idealized Gaussian distribution, real molecular and cellular processes still underlie their phenotypic expression. Souaiaia talks about how genomics has resulted in an influx of data and allowed statistical geneticists with a theoretical bent to actually test some of the models that underpin our understanding of traits and examine how models like mutation-selection balance might differ from what we've long expected. After wading through the depths of genetic abstraction and how it intersects with the new age of big data, Razib and Souaiaia talk about race and sports, and whether there might be differences between groups in athletic ability. Souaiaia argues that the underlying historical track record is too variable to draw firm conclusions, while Razib argues that there are theoretical reasons that one should expect differences between groups at the tails and even around the memes.

Our guest this week is Dr. Robert Karp. Dr. Karp is Emeritus Professor of Pediatrics at SUNY Downstate Medical Center in Brooklyn. A native Philadelphian, he is a graduate of Central High School, Muhlenberg College and Thomas Jefferson University Medical College, did his residency in pediatrics and fellowship in nutrition at New York Hospital/ Cornell Medical Center and completed training as Chief Resident at St Christopher's Hospital for Children in Philadelphia. He stayed on the St Christopher's staff in a War on Poverty School Health and Nutrition project. His 14 years in Philadelphia are summarized in a 1993 text, "Malnourished Children in The United States: Caught in the Cycle of Poverty." The remainder of his active career was in Brooklyn at SUNY Downstate where he was director of residency training and service clinics at Kings County and SUNY Downstate Hospitals. While at Kings County he read a study from 1962 by Harold Jacobziner and Harry Raybin describing the epidemiology of lead poisoning in New York City. Many of the children attending Kings County's lead poisoning clinic were from three `lead belt' neighborhoods in north Brooklyn described – Fort Greene, Bedford Stuyvesant and Crowne Heights. More recently, with publication of the FHA maps of 1934, he recognized the same neighborhoods as being “redlined.” His commentary on this connection “Redlining and lead Poisoning: Causes and Consequences” followed, and was recently published in the Journal of Health Care for the Poor and Underserved and is available Open Access.

In this episode of BackTable Urology, Dr. Aditya Bagrodia discusses benefits and advice for effective mentorship with Dr. Andrew Winer, Chief of Urology at Kings County Hospital Center and assistant professor at SUNY Downstate. --- SHOW NOTES First, the doctors define the purpose of mentorship as a relationship that serves to connect a mentor, with a certain skill set and knowledge, with a mentee, who wants to obtain the same skill set and knowledge. Dr. Winer considers mentorship a two-way street, since he has been able to learn a lot from his past mentees. He draws a distinction between coaching and mentorship; coaches give small pieces of advice sometimes, while a true mentor invests lots of time in the relationship. Additionally, he encourages mentees, especially medical students, to get rid of the fear factor of reaching out, as all mentors have been in their shoes before. He also emphasizes the importance of mentees showing up prepared for meetings out of respect for the mentor's time Next, the doctors discuss the requirements of being a mentor. Although both of them agree that there is no formal training requirement to be a mentor, mentors should possess certain qualities, like selflessness. They should not follow their own agenda–instead, they should focus on what their mentee's interests are. Additionally, taking credit for mentee's work is unacceptable. Next, mentors should be accessible to their mentees and let them know that they can remind their mentors about deadlines. Finally, honesty is very important. Mentors should be able to give their mentees constructive criticism. Then, they give practical advice for mentors who want to guide medical students and residents. Establishing a goal first and compartmentalizing their mentees' journeys is always helpful. Mentors should serve as advocates, but have to be honest when writing recommendation letters for students and residents. Additionally, they explain that residents become organic leaders early on, as many of them take on the responsibility of teaching medical students. Attending leadership most commonly trickles down to resident leadership. They end the episode by discussing benefits of mentoring a mentee who is different from themselves, in terms of generation gap, gender, ethnicity, and life experiences.

Homeschool, Adversity and Education with Dr. Carline Crevecoeur Dr. Carline Crevecoeur is a Haitian American board-certified obstetrician and gynecologist, a wife, and a mother of five adult children.  She was raised in NY and attended St. John's University as an undergraduate, earning a bachelor's degree in chemistry. She did her medical school and residency training at SUNY Downstate.  After several years of medical service in New York and Pennsylvania, Carline gave up her career to raise and homeschool her five African American children. Her homeschool journey is told in her new memoir: Pressure Makes Diamonds: From Homeschooling to the Ivy League – A Parenting Story.  She has volunteered on education boards such as WPSU, a local public radio, and NAAHP (National Asso. for the Advancement of Haitian Professionals) and sponsored many public health programs.    Dr. Crevecoeur has recently won her school board election race, making her the first Black person in her school district to do so.  In This Episode "Education is about life. And life happens." In this episode Dr. Carline Crevecoeur and I explored our thoughts on parenting, learning, homeschooling, education and school. I know you will enjoy the warmth and heart that Carline brings to our conversations. In this episode you get to learn more about her work, homeschool life and the joy, adversity, grief and acceptance that came along with their experiences. This episode is not only for homeschool parents, but any parents that are looking to do better for their learning community. Listen in as we talk about honouring the individual child and why sometimes the best we can do for their learning is to get out of their way. We also explore the power of family and community and being a role model within those spaces. For some, it may not seem like we belong but our voices are actually powerful and important. We also talk about Carline's wonderful book, Pressure Makes Diamonds- From Homeschooling To The Ivy League: A Parenting Story Homeschool, Adversity and Education I love talking with Carline and here's why I know you will love listening to our conversation; Born in Haiti, Carline immigrated to New York and then raised her family (one a very few black families) in Pennsylvania. Through these experiences she brings diverse perspective. Dr. Carline Crevecoeur is a doctor that decided to leave her practice to homeschool her 5 children because that decision reflected their core values of family, connection and self confidence best. I know it was not an easy decision! But for me, her careful thought and personal reflection on that experience offers insight and a confidence that I appreciate. All of her 5 homeschooled children attended Ivy League schools. You'll hear from Carline what made their homeschool successful. (It was not the schools but the connection and personal confidence that her children built) Carline homeschooled through tough times. She was diagnosed with stage 3 cancer at 49 years of age. Her bravery and outlook on life is encouraging to every one of us. And the perspective she gives as a homeschool mom through fear, pain, grief, struggle and then acceptance is invaluable. She now serves her greater community as a publicly elected school board trustee. All of her previous life and homeschool experiences are now blended with her current role in the public service. To me this seems to only enhance her dedication and heart. Resources Thank you Dr. Crevecoeur. It is always a please to chat with you about life, your experiences and beliefs on homeschool, the adversity that you face and overcome and the importance of education. CardioNerds (Amit Goyal and Daniel Ambinder) and guest host, Dr. Priya Kothapalli (UT Austin fellow and CardioNerds Ambassador), join SUNY Downstate cardiology fellows, Dr. Eric Kupferstein and Dr. Gautham Upadhya to discuss a case about a patient who had coronary artery bypass grafting that was complicated by a LIMA grafted to the great cardiac vein. Dr. Alan Feit (Professor of Medicine, SUNY Downstate) provides the E-CPR for this episode. Dr. Moritz Wyler von Ballmoos (Director, robotic cardiac and vascular surgery for Houston Methodist Cardiovascular Surgery Associates) provides a special perspective regarding coronary artery bypass grafting as it relates to this case. Episode introduction with CardioNerds Clinical Trialist Dr. Jana Lovell (Johns Hopkins). Left Internal Mammary Artery (LIMA) to Left Anterior Descending (LAD) artery anastomosis is the cornerstone of Coronary Artery Bypass Graft (CABG) surgery. Anastomosis of the LIMA to the Great Cardiac Vein (GCV) is a known but rare complication of the surgery. Currently there are no clear guidelines in regard to further management. We report a case of a LIMA to GCV anastomosis managed with a drug eluting stent (DES) to the mid LAD after ruling out a significant left to right heart shunt. The patient determines when the angina is no longer stable angina. The placebo effect of our interventions should not be discounted.LIMA to GCV anastomosis creates a left to right cardiac shunt. A Qp:Qs greater than 1.5 signifies a significant shunt.Increasing the pressure in the coronary sinus may actually be beneficial to the patient.LIMA-LAD is remains the most efficacious and long lasting graft but why not other arterial grafts? Notes - Coronary Artery Bypass Grafting: An Iatrogenic Left to Right Cardiac Shunt Iatrogenic anastomosis of the LIMA to the GCV is a rare but noted complication of CABG surgery. Review of the literature has reported under 40 such cases of arteriovenous fistula formation in the coronary system. Detection of the anastomosis generally stems from recurrent angina which can be attributed to unresolved ischemia or coronary steal syndrome but also can be detected with new heart failure (namely right sided heart failure due to left to right shunting). Diagnosis is usually made with coronary angiography, but CT coronary angiography has also been reported. Due to the rarity of this complication, no clear guidelines are in place directing the management leaving it to the discretion of the various Heart Teams. Evaluating for signs of heart failure and/or ischemia, and measuring the Qp:Qs have been the most common signs directing management. Various options are available for closing the fistula and include coil or balloon embolization, vascular plugs, venous ligation or a covered stent. Redoing the surgery is also an option. Spontaneous closure of the fistula has also been reported. Lastly, if redo surgery is not performed then regardless of fistula closure, coronary intervention for the native diseased artery may be pursued to relieve symptoms. Back when I was growing up, there was no one to turn to when I couldn't eat my chicken if it had touched the rice on my plate, when I was afraid of getting wet, when I didn't feel comfortable in my own skin. I attended Brooklyn College while I was working as a commodity trader on Wall Street. That existence was net positive for my mind and my finances, but that wasn't enough. I knew that to net positive in life, I would need something else. So I went to the OT program at SUNY Downstate. After I graduated I started working with children with fine motor delays. Eventually I noticed that many children with physical issues or delays essentially suffered from integration issues. While the report listed the external manifestations of “poor handwriting” or “low tone” or “visual processing delays,” when I started to work with the child, I could see their challenges through their eyes and relate to their experience. I became my own OT client. I worked on myself as soon as I had the knowledge - allowing me to be free and lead a happy, accomplished, satisfying life helping others overcome their limitations as well. You see, I haven't forgotten what it feels like to be stuck. That memory fuels my drive for research, for training other professionals, and for making changes in the educational system. And it drives me to be here at Hands on OT, to help your child get unstuck, to work through her integration issues, so she too can net positive in life. https://www.instagram.com/handsonapproaches/   Dini Weinberg - Dini is the founder and creative director of Dini Wigs. She maintains a private psychotherapy practice, seeing individuals, couples, and families. Along with her practice, Elena spent 25 years in medical education administration, first at SUNY Downstate, then as Clinical Associate Professor of Psychiatry and Associate Dean for Student Affairs at the NYU School of Medicine. Elena's primary goal is to assist those struggling with relationship issues like poor communication, drifting apart, disagreements about in-laws, friends, or raising children, infidelity, serious illness, separation, divorce, and dynamics between adult children & their parents. Her background in career counseling is also handy for clients with work-related problems, dual career stress, or achieving better work-life balance.  Elena's approach to sensitive and uncomfortable topics is her signature, using a humane and non-judgmental style to better uncover people's strengths, unrecognized needs, new or underutilized talents, and problem-solving abilities. Those who have crossed paths with her often cite her sense of humor and “patience of Job” as two of the sharpest tools in her skill box. Elena's non-traditional, winding career path has a common thread that allows her to speak with authority about all kinds of relationships and relationship rifts: family, friendship, colleague, work, mentorship, and compatriot. In several books, she reaches a plethora of audiences with titles like Marrying Well, Not on Speaking Terms, and, written during the pandemic, Estranged. What We Discuss In This Episode: Although we talk about some general issues around relationships, as a Licensed Marriage & Family Therapist, Elena shares her wisdom about how we can better our relationships all-around, but also with our significant other.  In the midlife phase of life, we often find that as the children leave home and we're left as “empty-nesters,” our relationships have been so embedded into the lives of our children, we have to rebuild those connections with our spouse or significant other to bring back the love and closeness we once enjoyed. Laurie has worked for JFK Rehabilitation, a variety of school systems, Early Intervention and private practice. Susie received her bachelor degree in Physical Therapy from Thomas Jefferson University, in Philadelphia, in 1986. Initially specializing in adult rehabilitation with a focus on spinal cord injury and traumatic brain injury, she quickly fell in love with pediatrics and specialized in incorporating hippotherapy treatment strategies into her practice. Dr. Tyndall assumed the role of the Interim Dean of the University of Florida College of Medicine on July 31st 2018. He is a Professor of Emergency Medicine and has been the Chair of the Department of Emergency Medicine at the University of Florida College of Medicine since 2008. He is a graduate of the University of Maryland School of Medicine and the Emergency Medicine Residency at the University of Maryland. In addition, he received his Master's Degree in Health Services Management and Health Policy from Columbia University, NY. Prior to his recruitment to UF, Dr. Tyndall held academic appointments at Cornell and SUNY Downstate and served as a residency program director for emergency medicine. At UF, he has served on the board of directors of UF Health Shands at the University of Florida since 2010. He was the Chair of the Dean's council for diversity, inclusion and equity at the University of Florida from 2010-2018. He was also the senior medical school representative to the Council for Faculty and Societies at the Association of American Medical Colleges from 2013-2018. He was also named a 2018-2019 fellow of the Council of Deans of the AAMC. He is a member of the Board of Directors and Immediate Past President of the Florida College of Emergency Physicians and is a member of the Board of Trustees of the Society for Academic Emergency Medicine Foundation. He has been inducted in both the Alpha Omega Alpha as well as the Gold Humanism honor societies. His research interests focus on brain injury and health services research. He is also an editor for the upcoming 10th edition of the emergency medicine text book - Rosen's Emergency Medicine - Concepts and Clinical Practice. He was most recently appointed to the role of Associate Vice President for Strategic and Academic Affairs at UF Health. As mentees, according to Dr. Adrian Tyndall, we must be bold in pursuing mentors. Today, Dr. Tyndall explains how mentorship is a bi-directional relationship: The mentee must actively show interest in the mentor and their areas of research, and in turn, this will activate the intrinsic motivation in that mentor to invest their time in the mentee. Simply put, it's all about give and take. On top of that, we'll learn how mentorship is helpful in self-reflection: All of us have blind spots, and we're able to achieve a “360 degree view” of our trajectory when we ask for guidance from mentors and peers. And Dr. Tyndall reminds us that if we look around and we don't like what we see, we have to have the courage to adjust our forward trajectory. Pearls of Wisdom: 1. Mentorship is a bi-directional relationship. As the mentee, we need to show our interest in the mentor and their work. In turn, that will activate a mentor's intrinsic motivation in the mentee. 2. As physicians, we need to know how to refuel our energy tank. Self-reflection is key, and seeking guidance from those around us will give us a 360 degree view of our journey. 3. There are four core values to live by in medicine (especially as a student): Self-reflection, balance, confidence, humility.

She went on to complete a Faculty Development Fellowship at the University of North Carolina—Chapel Hill. Dr. O'Rorke has won numerous teaching awards including Southern Society of General Internal Medicine Clinician Educator-of-the-Year Award and the University of Texas Presidential Teaching Excellence Award. When one of Dr. Jane O'Rorke's students doesn't know something, she encourages them to celebrate! As she explains, not knowing something is an opportunity to fill in knowledge gaps—and that is a good thing. Today, we learn how Dr. O'Rorke approaches learning (both for herself, and her students). She takes a personal interest in the people around her—because it makes her more invested in the work she does. And finally, she shares her personal development and how she has been proactive in working on developing certain skills even if they weren't natural talents for her, emphasizing that if we put our mind to something we care about, we have the power to improve at anything. Pearls of Wisdom: 1. Don't accept that you either have talent, or you don't. Recognize that skills are something we can develop like muscles, and there is nothing we can't improve on. 2. The key to being a great teacher is as simple as taking an interest in your learners. 3. Embrace vulnerability. It's difficult, but it's the way to tap into wisdom, and avoid the pitfalls that people have experienced before us.
