We talked about the QI efforts that she took during her fellowship in implementing faster enteral feed advancement and early fortification in a level IV neonatal intensive care unit. She shared the multidisciplinary experience that she developed with this project and also appreciates the support and mentorship she received. Radhika also shared insights on other projects that she got involved in- notably investigating the impact of glycemic variability on treatment-requiring ROP. She shared about her experience being a representative to the Fellowship Recruitment Action Team (FRAT) from the AAP Section on Pediatric Training. Levine Assistant Professor of Women's Health (in Obstetrics and Gynecology) at Columbia University Vagelos College of Physicians & Surgeons. She joined the faculty of Obstetrics and Gynecology at Columbia University in April 2018 to provide routine gynecology care and to further develop a comprehensive well-woman program. She has been a practicing obstetrician gynecologist for more than 20 years, starting in private practice and then joining the faculty at Montefiore Medical Center in Bronx, NY. While at Montefiore, she created, launched, and led the forty-person Division of General Obstetrics and Gynecology. Dr. Rosser received her undergraduate degree at Emory University and a Ph.D. in Endocrinology at the Medical College of Georgia. She attended Wake Forest University School of Medicine and completed her residency at Emory University. She is also a NAMS Certified Menopause Practitioner, able to provide high-quality care for patients at menopause and beyond.Primary care and heart disease in women have always been areas of focus for Dr. Rosser. She conducted basic science research on heart disease during graduate school and was the Chair of the "Women & Heart Disease Physician Education Initiative" for District II of the American College of Obstetrics & Gynecology. Clinical concentration has focused on pediatric dysphagia with medically complex infants with additional efforts in developing specialty programs for adults with acquired neurogenic disorders. She was instrumental in assisting in the development of the Masters of Science Program in Communication Sciences at Yeshiva University and has served as an adjunct professor at both Mercy College and NYU Steinhardt. She did her Internal Medicine residency at Montefiore Medical Center from 2017-2020 and served as chief resident in 2021. Atira Kaplan, MD is a board-certified physiatrist. She received her medical degree from SUNY Downstate Medical Center; and completed residency in Physical Medicine and Rehabilitation, and fellowship in Women's Health Rehabilitation, at Montefiore Medical Center. In 1985, doctors at a methadone clinic in the South Bronx made the harrowing discovery: 50 percent of its patients had HIV. Three years later, in the same neighborhood, a pair of epidemiologists estimated that as many as one in five young men were positive for the disease. Those numbers made the South Bronx one of most critical hotspots for HIV in the country.Joyce Rivera was born and raised in the South Bronx. She watched as heroin flooded into her neighborhood followed by HIV. When Rivera's brother died in 1987, she decided to do something. Working with a heroin dealer and a local priest, she defied the law and set up an illegal needle exchange in an attempt to prevent the transmission of HIV among injection drug users. And she largely succeeded. In our final episode of 2023, the JHLT Digital Media Editors have two manuscripts from the December 2023 issue of The Journal of Heart and Lung Transplantation! Digital Media Editor Erika Lease, MD, transplant pulmonologist at the University of Washington in Seattle, hosts this episode. First, a free-ranging conversation with first author Mark E. Snyder, MD, and senior author John F. McDyer, MD, on their team's study “Impact of age and telomere length on circulating T cells and rejection risk after lung transplantation for idiopathic pulmonary fibrosis.” A subset of patients with idiopathic pulmonary fibrosis (IPF) have a heritable, age-adjusted short telomere length. Mutations in telomere length can manifest as T-cell dysfunction and immunodeficiency. As T-cells are involved in the development of acute cellular rejection (ACR), the authors hypothesized that the combination of age and telomere length would impact the degree of ACR burden in lung transplant recipients—and indeed, the authors found that lung transplant recipients with IPF and short telomere length had premature “aging” of their circulating T-Cells. There was a significant decline in early ACR burden with increasing age, found only in those with short telomere length. How might these findings impact immunosuppression regimens in clinical practice? What follow-up studies to they have planned? In the discussion, Drs. Snyder and McDyer, both of UPMC in Pittsburgh, discuss all these possibilities, as well as the the work of their collaborator, Jonathan K. Alder, PhD, as inspiration for the study. Next, the editors explored “Early optical coherence tomography evaluation of donor-transmitted atherosclerosis and cardiac allograft vasculopathy: insights from a prospective, single-center study,” in a discussion with senior author Snehal R. Patel, MD, of the Montefiore Medical Center in New York. Cardiac allograft vasculopathy (CAV) remains a major cause of death in heart transplant recipients, and donor-transmitted atherosclerosis (defined as a maximal intimal thickness of >/= 0.5mm on baseline intravascular ultrasound (IVUS) early after transplant) is believed to carry a greater risk for the development of CAV. Dr. Patel's team, however, hypothesized that optical coherence tomography (OCT) may have advantages over IVUS as an imaging modality due to its higher resolution. In this prospective, observational study, the authors assessed the prognostic role of OCT, and found that transplant recipients whose OCT imaging showed advanced plaque characteristics had a significantly higher event rate after a mean follow up of 3.3 years. OCT was also an independent predictor of clinic events, while maximal intimal thickness of >/= 0.5mm was not. Dr. Mortimer was a first-year anesthesiology resident at Montefiore Medical Center in New York City. Last year she was highlighted as she was leading the charge on residents forming a union due to conditions that she noted: “The other truth is that with its current conditions, this work is often unnecessarily challenging to our wellness as individual people.” Physician burnout has been discussed a lot on this channel but this is the ultimate example of what can happen when the people we are supposed to be training to be our next physician force can no longer take the burdens and stress placed upon them.
