Sometimes when you're the one with boots on the ground, seeing the devastation wrought by a pathogen, you become the person who needs to find a new path. Dr. Jeff Cohen found himself facing a spate of MRSA cases while he was still working in clinical medicine, and rather than continuing to follow outdated CDC guidelines, he used positive deviance to discover inside experts. What does this mean? The experts at the CDC weren't seeing the same data or results that Jeff and his team were. So he began to sort through what each team member and group knew to come up with better solutions for reducing MRSA transmission. They put together a series of questions to determine which underlying practices each team member was using to keep from transmitting MRSA. In fact, they surveyed the specific people who were most likely to transmit the disease, but who hadn't. It was using this method that they developed entirely new protocols regarding MRSA and other highly transmissible hospital pathogens. Listen as Jeff shares more about this practice and his story. During our discussion group exploring TVOL's Third Way Series, What is Positive Deviance? There's a small chance that you know all about it and a larger chance that you've never heard of it at all. That's because successful cultural change methods have a way of emerging at a particular time and place, spreading to a degree on the basis of their success, but then coming up against boundaries, beyond which they remain unknown. So it is with Positive Deviance, which has been used to prevent child malnourishment in Vietnam, female circumcision in Egypt, and even improve an American pharmaceutical company's outreach to doctors. Monique Sternin is the co-founder of the Positive Deviance Approach & Initiative, and Adjunct Associate Professor, at Tufts University Friedman School of Nutrition Science and Policy. Over the past 25 years, Monique has been involved in developing and promoting the Positive Deviance Approach, an asset-based method to solve complex problems requiring behaviour and social change which she pioneered with her husband Jerry Sternin in Viet Nam. She has developed the PD methodology in a variety of international public health issues including childhood malnutrition, maternal &newborn mortality & morbidity, anemia prevention & eradication in adolescent girls, polio eradication, advocacy against female genital cutting, condom negotiation and usage by commercial sex workers. In the US, Monique has been involved in pioneering the use of the PD approach to eradicate and prevent hospital acquired infections (specifically MRSA) in 6 US hospitals in collaboration with the Plexus Institute under a Robert Wood Johnson grant. Clinical managing editor Karen Roush speaks with Dr. Jodie Gary, assistant professor at Texas ANM Health Science Center College, and author of the article “Exploring the Concept and Use of Positive Deviance in Nursing.” She describes the difficulty nurses encounter providing patient-centered care while remaining within strict policies in a complex environment. Nurses who extend care beyond these policies know that they’re deviating, but are doing so in a responsible way with honorable intentions that benefit the patient. She suggests increased flexibility through guidelines rather than rules under circumstances that require the adaptation of care to suit the patient and their immediate needs.

Jerry Sternin argued that traditional expert-driven models for individual, social and organisational change often don’t work. The Positive Deviance approach builds on successful but ‘deviant’ (different) practices and strategies that are identified from within the community or institution. Positive Deviance is based on the belief that in every community, organisation, business or group, there are individuals or entities whose uncommon, but demonstrably successful behaviours or strategies enable them to find better solutions to problems than their neighbours or colleagues who have access to exactly the same resources. How does this happen? What can we learn from it? Could it work in Glasgow?
