I’ve been eating this way for five years and incorporate regular periods of intermittent fasting of 16-20 daily. I even ran in the Boston marathon with just water. So surely I’m fat-adapted. I recently switched over to keto for cancer and dementia prevention but now I have a problem—I’m CONSTANTLY hungry. I consume well over my usual 2500 calories which includes 200g healthy fats. I’m doing yoga and keep my net carbs under 20g daily. I’m 36 years old and have a normal BMI. But this hunger is excruciating and nothing seems to satisfy it. Will this eventually normalize? Can you diagnose a low thyroid function on a TSH number alone? How will keto help to fix the issues that concern my doctor?   Hi Jimmy and Dr. Cole,   I’m a big fan of all your books and podcasts, Jimmy, and I've been a Keto Talk listener since the very beginning. I’m excited to hear what Dr. Cole has to add to an already amazing show. I went to see a new doctor last week for a physical and she has all her new patients do some bloodwork. When the results came in (see them below), her office called me to inform me that my total cholesterol and LDL-C are way too high (230 and 153 respectively). I asked why these numbers mattered when my HDL and triglycerides are 62 and 73 respectively thanks to eating a ketogenic diet and I was told my "bad" cholesterol was too high. I wasn’t worried about it because I’d read Cholesterol Clarity and knew what really mattered.   When I went to see my doctor in the office, she wrote me a prescription for Levothyroxine because she says my thyroid level is too low and that it's driving up my total cholesterol and liver enzymes. When I asked whether a higher insulin level could cause this, she insisted that I couldn’t possibly have high insulin with my normal HbA1c. She wouldn’t run the fasting insulin nor the particle size cholesterol test that I requested. And I also noticed she only tested TSH, which does seem high. But I don't know if that means my thyroid is low or not. So my question for you guys is this: Is it possible that the thyroid function is low simply based on a high TSH level? When I consulted a medical weight loss clinic, they put me on a 1200 calorie, 120g carb diet and I gained weight in the two weeks I did it. Do you have any clinical experience or know about any research that looks at the use of keto for former lap banders looking for a way to keep their weight under control? I had my hormones tested and my testosterone came back at zero. I suspect having the lap band decreased my T levels from the stress it put my body through. So if the lap band changed my body’s hormone levels, what else has it changed? Is that why I’m struggling to lose weight like everyone else can? However, I’ve noticed I’m dealing with a lot more joint pain now than I did before eating ketogenic, especially in my knees. I thought things would improve now that I’ve taken off some extra weight. I am definitely in ketosis when I test for them and I’m trying to avoid what my mother just went through with a double knee replacement. I feel pain in the joints but don’t have any noticeable inflammation. Is being in ketosis causing this to happen? Thank you so much for your help. Does a ketogenic diet lead to abnormally high liver function test results? Is my liver overstressed by having to metabolize the extra dietary fat? Hi Jimmy and The Doc, A few years back, I had abnormally high liver function test results and my primary care physician was concerned that my numbers got worse because of keto. My AST and ALT were around 100 and they’re supposed to be no higher than 37. At the time she advised me to start eating more carbs and all my numbers that were bad came within the normal range. I started keto and fasting again in 2016 and lost 64 pounds after my third pregnancy. I’m concerned when I go back to see my doctor next month that my liver tests will be bad again. I’m hypothyroid, but I don’t have any known liver or gallbladder issues. Is it possible my liver was overstressed by having to metabolize the extra fat in my ketogenic diet? If so, then does that mean keto isn’t right for me? I sure hope not because I feel so fantastic in ketosis. Thanks for any insight about this and thank you for all you both do! Kelly KETO TALK MAILBOX – Should I strive to have higher levels of blood ketones if I seem to be thriving in my mental clarity and energy when I am barely in ketosis? Hey Jimmy and Adam, I’ve been doing a ketogenic diet for three months as a physically active 27-year old man with 10% body fat. I’ve noticed that I feel so much better when I’m barely in ketosis at around 0.5 mmol—that’s my sweet spot where I have better mental clarity and energy. Anytime I’ve tried pushing blood ketones up by moderating protein more, my ketones rise to 2.0-5.0 mmol, but I don’t feel quite the same mental benefits as I do in the lower level of ketosis. So my question for you guys is this—should I strive for higher ketones and hope these kick in at some point to give me the benefits I desire or should I listen to my body and return to the lower level where I seem to thrive? I’ve been able to eat as much as 50g carbs (10% of my caloric intake) without every impacting my ketosis levels while eating just 2% more protein in my diet will kick me out of ketosis completely. Fascinating! I don’t take any supplements and don’t use any sweeteners at all. I’ve yet to find anyone else dealing with this issue, so I’d love to hear an explanation of what might be going on.
