Over 300 episodes ago (episode 103), I recorded a podcast called The Diet "Rules" that are Actually Worth Following. It was one of my most popular podcasts, and this week I'm doing an updated version. My goal is to simplify nutrition. To throw out the dogmatic cult like approach and start to explore what works for you, while allowing some flexibility so that you can still make progress while enjoying life. There's no doubt, nutrition matters. What you put into your body will affect how you feel, how your body looks and how your body functions. One of the question I get asked ALL the time is “what should I eat?”. If you rely on Dr. Google for your answer, you'll find a spectrum of answers, from Atkins to the Zone Diet (and everything in between), all backed by hundreds of success stories and before and after pictures. They all work… For somebody. And they all work, usually for at least 4-6 weeks. We keep searching for the ‘best diet' or diet hack that will easily help us shed pounds, have more energy, or reach any other goal we desire. But the truth is, there is no “best diet”. There's only the best diet for YOU. And I believe that's one that you can stick to. However, there are a few basic rules that are ACTUALLY worth following. In this episode, I'm excited to welcome Dr. Barry Sears, the renowned creator of the Zone Diet and a pillar in our field. Dr. Sears has not only authored bestsellers but has also made incredibly important contributions to clinical practice which we still rely on today: He is, for instance, the scientist who put the use of the AA:EPA ratio on the map. Dr. Sears provides elegant and accessible explanations of the unassailable importance of diet in addressing inflammation, metabolic health, chronic disease, and longevity, and the role that fatty acids play in that milieu. And while he and I might not agree that polyphenols need to be absorbed intact for benefit - there's plenty of evidence that their postbiotic products are very important players, especially in their interactions with the epigenome - his concerns about the chemical structure of highly concentrated fish oil supplements (with the potential for isomerization, as he argues) has opened my eyes and is important, without doubt. SandiPeiffer (Pie fur) has been happily married to her best friend for 36 yrs. She is a 58yr old mother of 4 (ages 18-31) and grandmother of 2(ages 2 and 1) who lives on the Atlantic coast of Florida. She is a veteran homeschool mom of 26 years, her youngest is graduating this year. She enjoys walking, reading, crafting, cooking and spending time with her family. As an adult Sandi has tried most of the diets, we have all heard of… Weight Watchers, Susan Powter Stop the Insanity, TOPS, The Zone Diet, 40-30-30, Shake diets, Suzanne Somers “Somersize”, South Beach, Atkins, The Rosedale Diet, etc. Her real struggle with weight started when she went on an anti-depressant for anxiety. Then the pounds really started to pack on. 120 lbs by the time it was all said and done. She found intermittent fasting after a diagnosis of Type 2. Diabetes with an A1C of 8.4, and a fasting glucose of 237. Though weight loss has been a nice side benefit, she really has had the focus of reversing her diabetes. Starting IF in Dec. 2018, at 5'4” and 300 lbs. Now after 1565 days of clean fasting, she is currently at 240 lbs, but more importantly to her, her A1C is 6.2 and her fasting glucose stays in the 120's, without diabetes. meds. She started IF by dipping her toe in the water, so to speak. With her previous health issues, especially her recent bout with Adrenal Fatigue, she was determined to be gentle with her body. So she started with 14:10, clean fasting, and didn't change her way of eating at all. After a couple of weeks, went up to 15:9, then did the same to get to 16:8 and 17:7. She actually lost most of her weight at 17:7, even though she went on to 18:6 and eventually 19:5. Doing OMAD, “restaurant style” at 18:6 and 19:5. After 3 yrs of 19:5 OMAD, her body apparently said that was too stressful. Her cholesterol jumped very high, as well as triglycerides and measures of inflammation. So, through working with a functional medicine Dr., she changed her IF schedule to 16:8 and her diet to a modified Paleo plan. That did the trick and the problems have corrected, and she's taken off an additional 10 lbs. We are all an experiment of one, and sometimes we have to “tweak” how we practice IF. Listening to our bodies is so important! Her husband also practices IF and has lost 65 lbs. I intend to convince you that you never need to diet again. Then, if I do my job today, you will understand that you can create a lifestyle in which food is simply a small part and in which eating and losing weight no longer have control over you. Reason #1: We eat too much food. According to recent studies, people underestimate the number of calories consumed by 25 to 50%. Studies show again and again that we humans have a great tendency to underestimate what we have eaten during the day and tend to minimize the calories in the foods that we eat. According to a study at Cornell University, everyone does it. "Normal weight" people underestimate calorie intake by about 20 percent, and overweight people underestimate by about 40 percent. Other sources say it's more like 50 percent. Reason #2: We eat too often. The average person starts eating within 30 minutes to an hour of waking up and doesn't stop until 30 minutes to an hour before going to bed. Based on the most recent data, the average person wakes up a 6am and goes to bed a little before midnight ~ that is 18 hours of being awake and based on the 30/30 ratio mentioned above, that is 17 hours of active feeding time which allows for less than 6 hours of complete fasting per day. The ratio is entirely backward. Reason #3: We eat the wrong combination of foods. This one always shocks people. The first two are rather obvious, but this one requires a little science and understanding of how the body processes food. I promise to keep it simple and easy to understand. Let's use the ZONE Diet as our example ~ but you could replace zone with about 95% of the others being crammed down your throat, and the data would be the same. The Zone Diet prescribes that you consume 40% carbs, 30% protein, and 30% fat. Here's the MAJOR flaw in this diet. They are prescribing two competing FUEL sources in virtually the same ratio. Analogy: The above-mentioned nutritional intake prescription would be equivalent to your mechanic telling you to go to the gas station and fill your car with half regular gasoline and half diesel. Mind-blowing crazy, right!?!? Nobody would do that, and you'd fire your mechanic immediately because that would ruin your engine, and your car wouldn't run. Professor Jacob George is a renowned hepatologist and liver research scientist who studies the causes of and mechanisms for the development of liver disease and liver cancer. Professor George's work has made significant contributions to clinical practice. His team first identified the role of interferon lambda 3 gene polymorphisms for predicting treatment response in chronic hepatitis C, and a second gene polymorphism that interacts with interferon lambda 3. These discoveries are considered major advances in the field and the finest examples of ‘personalized medicine'. Professor George was an author of the groundbreaking study of combination telaprevir, pegylated interferon, and ribavirin therapy for hepatitis C, which changed the landscape of hepatitis C treatments, heralding the era of direct-acting antivirals. His work identifying insulin resistance as the universal underlying pathophysiological abnormality in fatty liver disease underpins the majority of research on this condition and is the cornerstone of current therapy (lifestyle intervention; exercise; insulin-sensitizing agents). He also first described the association between hepatitis C and insulin resistance. Professor George is an advisor to Hepatitis Australia, the Transfusion Related AIDS and Infectious Diseases Service, and at state and national levels, on viral hepatitis. He oversees research for the Asian Pacific Association for the Study of Liver. He is a Fellow of the American Association for the Study of Liver Disease. Do you have a fear of eating too many carbohydrates? Do you think carbohydrates make you gain weight? If you do, you have Carbophobia. Where did this fear of carbs come from? In his book, “The Low-Carb Fraud”, T. Colin Campbell, PhD tells us. Back in 1972, Dr. Robert Atkins wrote a book called, “The Atkins Diet Revolution”. It wasn't very popular, so several years later, in the 1980's when there was a lot of talk about obesity, especially obesity in America, Dr. Atkins decided to re-market his book. In 1988 he revised and republished it with the title, “Dr. Atkins New Diet Revolution”. In this book, he proclaims that obesity is not caused by eating fat, it's caused by eating too many carbs. He advocates severely restricting carbohydrate intake and getting most of your calories from fat and protein. I've never had a fear of eating carbohydrates, but even as a child when I had a hamburger if I didn't want to finish my hamburger my parents told me to eat the meat and leave the bun. Today it is common for people to eat high fat, high protein and restrict and even avoid carbohydrates altogether because they believe carbs will make you fat. Modern versions of what started as the Atkins diet are the South Beach Diet, the Paleo Diet and the Zone Diet. For a person who has been eating a lot of processed carbs as well as a high fat, high protein diet, to severely restrict the amount of processed carbs that they've been eating can result in quick weight-loss. But what's wrong with this approach? 1. Low carb diets are very unhealthy and have long-term health consequences. 2. Low carb diets lumps all carbohydrates into the same category, whether they are whole foods plant-based carbs or processed carbs. By doing so, it confuses people and leads them to believe that all carbs are the same and all are bad and should be avoided. That simply is not true. 3. Low-carb diets emphasizes fat and cholesterol and proclaims them as nutritional heroes that should be increased in a person's diet. In reality, fat and cholesterol are the cause of many of our modern day health problems such as cardiovascular disease, arteriosclerosis, and non-alcoholic fatty liver disease to name a few. 4. A very restrictive diet of any kind, low-carb diet or otherwise is simply not sustainable. Whatever weight is lost from such a restrictive diet is ultimately gained back along with even more weight. In contrast, a whole-foods, plant-based lifestyle isn't restrictive. We don't count, weigh or measure our food and we eat until we're satisfied. Dr. Michael Greger says low carb diets cripple people's arteries. Wow! That's enough of a reason right there to not eat a low-carb diet! By definition, a low-carb diet emphasizes animal based foods, whereas a low fat diet emphasizes a plant-based diet. The healthiest way to fight obesity is to eat a whole foods plant-based diet that emphasizes whole foods that are high in nutrients, high in fiber, low in sodium, low in saturated fat, and very low in cholesterol, while at the same time steering clear of processed foods. Note the word “processed,” because these foods are low in nutrients, low in fiber, high in sodium, high in saturated fat and high in cholesterol. We highly recommend the book, “The Low Carb Fraud” by T. Colin Campbell, PhD and Howard Jacobson, PhD which fully explains the danger of a low carb diet.

It was stated that there has been a slight decline in their net worth in 2020 due to body acceptance and body positivity. Do you know what that really means though??Programs are starting to rebrand.I recently took a copywriting course and one of the keywords she said to be sure NOT to use in your copy is "DIET". Why?Because we are catching on and the diet industry, and people pushing it, know this.Let me clarify, you are not bad or wrong for wanting to lose weight. You don't need to feel ashamed or guilty about this.And truly, it isn't surprising!I mean, seriously, just take a look at any magazine.Watch any show or movie. Look at book covers.There is very clearly an acceptable/right way to look.But back to dieting.....so diet companies, coaches, programs etc. are seeing that women now know "diets don't work" so...they rebrand.They repackage their offer in a way that will seduce you without using the word diet.But dieting isn't just a program or plan.At its core, a diet is a way of thinking about your food.It is when we are emotionally attached to how we eat. There is a right and wrong way. Does a ketogenic diet lead to abnormally high liver function test results? Is my liver overstressed by having to metabolize the extra dietary fat? Hi Jimmy and The Doc, A few years back, I had abnormally high liver function test results and my primary care physician was concerned that my numbers got worse because of keto. My AST and ALT were around 100 and they’re supposed to be no higher than 37. At the time she advised me to start eating more carbs and all my numbers that were bad came within the normal range. I started keto and fasting again in 2016 and lost 64 pounds after my third pregnancy. I’m concerned when I go back to see my doctor next month that my liver tests will be bad again. I’m hypothyroid, but I don’t have any known liver or gallbladder issues. Is it possible my liver was overstressed by having to metabolize the extra fat in my ketogenic diet? Hey Jimmy and Adam, I’ve been doing a ketogenic diet for three months as a physically active 27-year old man with 10% body fat. I’ve noticed that I feel so much better when I’m barely in ketosis at around 0.5 mmol—that’s my sweet spot where I have better mental clarity and energy. Anytime I’ve tried pushing blood ketones up by moderating protein more, my ketones rise to 2.0-5.0 mmol, but I don’t feel quite the same mental benefits as I do in the lower level of ketosis. So my question for you guys is this—should I strive for higher ketones and hope these kick in at some point to give me the benefits I desire or should I listen to my body and return to the lower level where I seem to thrive? By the way, you’ll find it interesting that I seem to tolerate more carbohydrates in my diet than more protein. I’ve been able to eat as much as 50g carbs (10% of my caloric intake) without every impacting my ketosis levels while eating just 2% more protein in my diet will kick me out of ketosis completely. Fascinating! I don’t take any supplements and don’t use any sweeteners at all. I’ve yet to find anyone else dealing with this issue, so I’d love to hear an explanation of what might be going on. Back when they were infants, CrossFit and paleo were the best of friends. Robb Wolf was giving CrossFit nutrition seminars. CF boxes were running paleo challenges and offering meal plans to clients. Tens of thousands of people were introduced to paleo through CrossFit and CrossFit through paleo. Over time, however, they drifted apart. Devon and I were so psyched for this interview with Dr. Bowden about cholesterol and nutrition that we took some time to discuss how to get on track with eating healthy and how to schedule time for yourself to focus on your health. It's Okay to Fall Off the Health Wagon Sometimes Maybe you decided to eat some extra chicken wings, ice cream, or sugar drinks over the Labor Day Weekend (don't worry, so did we!) and you're feeling guilty about it. Don't feel that way about yourself and don't judge yourself. It's okay to enjoy all of our favorite, unhealthy foods and overindulge every once in awhile. Plus it's extremely hard not to once you're around friends and family who are eating the same fatty foods. Just relax and know that being healthy is a journey that we're all on. It's normal to eat food that you normally wouldn't eat. Just get back to focusing on your own personal health goals once that weekend BBQ is over and done. Follow a Health Journey with a Focused Plan So what can you do to get back on the health wagon and start focusing on your health again? Plan and schedule. Now that school is starting, maybe it's extra hard for you parents out there if you have kids to drive to school and to take to all their after school activities. Even if you don't have kids, how do you schedule your life? Stop and think about your own time and your own goals. Time goes by so fast and it's easy to ignore your health needs year after year while you put other people ahead of yourself. Rob and Devon's Action Tips to Create Good Health Habits and Patterns To created a focused plan on improving your health, we have some tips to help you create good health habits and patterns. Your mission for the week: Go ahead and pick as many of our tips that you want to do this week. Try to do at least 1-3 of them each week to focus on building those habits. If you focus on more than 3, then you'll be all over the place by trying to focus on too much at once. Now, take 15-30 minutes and look at your schedule. Think about what 1-3 tasks you're going to do for yourself this week: Drink 80 oz of water. Eat 2 green vegetables in per day. Complete 2-3 workouts a week (even just 10 minutes each time). Do healthy batch cooking and meal prepping once or twice during the week. Have a No category: For example, pick one thing that you're not going to eat this week whether it's alcohol, soda, juice, cookies, or processed foods. Catch up on your sleep by sleeping 7-8 hours consistently during the week. You don't have to complete the tasks perfectly. This exercise just to help you start good health goals. When it comes to completing goals, it can be difficult to do it all on your own. Keep track of being accountable with your goals with our Open Sky Fitness Podcast Facebook group! Cholesterol's Relationship with Our Body There's a lot of feelings of fear when people hear the word, "cholesterol," and we immediately begin thinking about gaining weight and different health problems like heart disease. This fear has been built upon years and years of different sources saying the cholesterol is so bad for us and we've believed in this myth and more for far too long. According to Dr. Jonny Bowden, we've been so wrong about our fear of cholesterol. In fact, there is nothing to fear. Dr. Bowden will explain in detail in the podcast, but all in all, there is no connection of saturated fat with heart disease and our cholesterol levels. If Ketogenic and Mediterrerian diets call for eating healthy fats that don't actually make you fat, then why are we following other diets such as low-carb and low-calories that can make us sick? To truth is, is that the types of diets that focus on consuming a lower amount of carbs and calories don't actually help us in the long run. In reality, we're actually starving our bodies from the nutrients and healthy fats that it needs to feel full and be energized. How to Analyze Cholesterol Levels Cholesterol is so much more complex than just knowing a single number and analyzing the good cholesterol (High-Density Lipoprotein) and the bad cholesterol (Low-Density Lipoprotein). That was the past but today, we know that there are so many different types of HDL and LDL that do not all behave the same. When you talk to your doctor about your cholesterol, have a particle test done to truly know where your cholesterol levels are at. A doctor should not be prescribing a medication such as Statins without taking a closer look at all of the different factors in someone's cholesterol. A medication shouldn't be prescribed by just looking at the good HDL and the bad LDL. There's more to cholesterol than just HDL and LDL. But are there any lessons we can learn from that experience to use in the promotion of the science behind low-carb diets? DEBBIE ASKS:In Canada, . It seems after decades of nutritional decay, government health leaders there are finally seeing the light on the science. Do you think a similar announcement for adults will be forthcoming in Canada? How close are we in the United States seeing something similar to this happen here? BOB ASKS:It seems to me that the main reason low-carb diets are not an accepted part of the mainstream is that organizations such as the USDA and the NIH produce documents such as the Food Pyramid and MyPlate that push for carbohydrates to be a major part of a healthy diet. In light of the evidence that supports the health benefits of a high-fat, low-carb diet, why has there not been a lawsuit challenging this erroneous advice? Wouldn't someone who has meticulously followed this advice and then developed worse symptoms with their diabetes, for example, have a legitimate case and be able to set the precedent that high-fat, low-carb is a valid alternative? Are you aware of anything like this in the works? NANCY ASKS:People like Dr. Phil and Dr. Oz seem to have a lot of influence with average people when it comes to healthy living because of their prominence on national television. I personally do not watch them because I like to do my own research and come up with my own informed decisions on how to live a healthy life. But everyday I hear people around me asking, 'Oh did you see what Dr. X said about BLANK?' Is there anyone out there who you see breaking into mainstream television like Dr. Phil and Dr. Oz have to promote the scientific principles of low-carb lifestyle? What would it take to make something like this happen in terms of sponsorship, profitability and keeping the messages communicated focused on what the science is showing? RICHELLE ASKS:I would like to suggest that efforts be directed at the universities and scientific publications – especially textbooks. This year I purchased the “Guyton and Hall Textbook of Medical Physiology 12th edition” and I was mortified by the ignorant comments in the textbook about cholesterol, nutrition, heart disease and basic metabolic processes like lipid metabolism and ketosis which was actually confused with ketoacidosis. This is a current textbook teaching emerging medical professionals and it's spouting information that has long since been dismissed and corrected. Food and pharmaceutical companies pay for research to be conducted in the universities that have the experts that write these textbooks and so it is obvious why it is difficult to change their minds, perhaps unless the government insists. We need to convince our government health departments of the fact that unhealthy populations cost the government and the community a boatload of money. Does Dr. Feinman have any ideas about where and how to start making inroads on this front? PETRO ASKS:Given the remarkable results of treating both metabolic syndrome and Type 2 diabetes with a low-carb diet, is Dr. Feinman aware of any research on the combination of treating either of the two conditions with both a low-carb diet and metformin? Previous research suggests the combination of metformin and exercise may be more complicated than expected. If Type 2 diabetes is the health crisis of the future, it seems that careful research into the combination of treatments typically used is indicated and ought to be ripe for funding by any responsible health organization. DAN ASKS:What research has been done or is planned for the effectiveness of low-carb in controlling diabetes and especially reducing cardiovascular risk for diabetics? Studies like ACCORD or ADVANCE claim to show no or worse cardiovascular benefit to lowering blood sugar with drugs. What about those who maintain excellent blood sugar control with low-carb? Doctors are quick to prescribe statins to diabetics, though I don't see any benefit in the study data for diabetics who haven't already had a heart attack. It would be good have data that shows diabetics who control blood sugar with low-carb have no higher cardiovascular risk than non-diabetics.

The quality was top notch. The Fat Guy did get a little overwhelmed when the huge box of portion controlled meals and snacks arrived on his doorstep. it took tremenous will power on his opart not to eat the entire box worth of food in one sitting. But overall he was very impressed with the whole operation. The Fat Guy's kid loved the mac and cheese and most of the healthy snacks that were in there, especially the soy bbq chips. The Fat Guy tried to exercise last week and got depressed. We also talked a little about September 11 and how the Doctor, because of his intense fear of flying, initially thought the planes were crashing into the Twin Towers because of pilot error.
