[01:45] The Reason You Should Stop Taking Melatonin Every Single Night Jimmy has been working on his sleep for about ten years. If you're going to be healthy, you need to get your sleep dialed in. Health isn't just about food; it's also about sleep. When people have sleep issues, they start taking melatonin. Melatonin helps you get to sleep, and you do feel heavy with melatonin. Over time, Jimmy realized that he kept needing to have more and more melatonin. Eventually, Jimmy started to train his circadian rhythm by getting sun in his eyes. Also, ice baths before bedtime are helpful for sleep. [10:40] Cortisol Isn't The Enemy; However, You Need To Manage Stress Still Check out the Oura Ring: https://ouraring.com/ You want a high level for HRV. You can get that through meditation and stress management. However, people live in a stressful state, never managing it. You can be on keto forever, but that stress will negatively affect your health if you don't have it under control. Most people don't have high cortisol. In fact, they have super low cortisol. Well, you need cortisol production throughout the day to be healthy. If you have unmanaged stress, it can really affect your hormones. [31:00] Start A New Tradition: Going To Sleep As A Family Do the best you can when it comes to your sleep. If you can't afford the best meat possible, just do the best you can. Grain-fed beef will be better than cereal. So, talk with the people in your family about improving your sleep. Start a tradition where you and your family all go to sleep at the same time. [34:30] Listen To Your Body Because The Oura Ring Is Just A Tool An Oura Ring is a tool for your health journey. It will not be 100% accurate. You need to take everything with a grain of salt. Don't get too upset if one night on your Oura Ring doesn't seem right. Listen to your body because you know when you slept well. [39:50] Adding Fruit and Honey To The Carnivore Diet Jimmy is disappointed in the persistence of redefining terminology. For instance, there has been a massive trend of adding fruit and honey to your carnivore diet. If fruit and honey make you feel good on the carnivore diet, then do it! However, when you call it carnivore, you are changing what the term means. Jimmy says that it's disappointing they are changing the meaning behind standard health terms. [43:55] Keto Isn't A Weight-Loss Tool; Keto Is A State of Metabolism You are doing a disservice to your clients if you're a coach, and you promote keto solely as a weight loss diet. Keto is a state of metabolism, not a diet. If you tell people to eat keto, then it doesn't make sense. You get into a state of metabolism to improve your health. Don't promote keto as a weight-loss tool. Of course, keto is a powerful tool for weight loss; however, it's a weight-loss tool for everyone. Keto puts your body in a better position to be optimally healthy. [47:00] The Mental Benefits of Keto Will Change Your Life Forever The psychological component of being in ketosis is the best part of keto. When you're not thinking about food 24/7, that's life-changing. On keto, you'll be satiated and have hunger control. When you nourish your body well, your body will reward you. The psychological benefits are priceless from keto. He decided to take back control of his own health thanks to the Atkins diet. Jimmy lost 100 pounds in the first 100 days. He was normalizing metabolism for the first time in his entire life. At the end of the year, Jimmy lost 180 pounds. Atkins featured Jimmy’s story on their website. Eventually, Jimmy started a blog and a podcast about his health journey. [09:55] Get Clear On Your ‘Why’ For Jimmy, he is excited about health and low carb, especially because it can change people’s lives. Put your focus on something. If you have kids, then they are your “why.” If you care about yourself, that can be your “why.” Jimmy wants to pay it forward for the gift that he is given. He loves to share education, encouragement, and motivation for others. [12:50] Issues With Keto There are people who are taking advantage of the keto trend. Make sure that the people you listen to do not have an agenda. Jimmy doesn’t make money from his rants. He isn’t trying to make money; he’s trying to spread information. Look for the people that want to educate people genuinely. There are so many shysters in the keto world. Keto isn’t about profitability; it’s about educating. [17:45] How To Find The Truth About Keto If you listen to a show that has conflicting information, evaluate it within your context. For instance, the big debate is more protein or more fat for keto? In this situation, test both and see what works best for you. Nutritional health is not black and white. You have to test and see what works. If you’re going to test high fat, do it for a month. Pay attention to how you feel. Are you finding that your brain health is more astute? Then, you can switch to a month of high protein. [29:05] Tests To Request From Your Doctor Jimmy’s doctor stopped testing his fasting insulin. Also, Jimmy’s doctor was not testing for inflammation. These are some of the reasons why Jimmy stopped seeing that doctor. You want to test for blood sugar. If you’re eating keto, it will stay under 100 all of the time. Most of the time, keto will keep it under control. Request these tests: Blood sugar Fasting insulin HSCRP A full thyroid panel Most doctors are not running these numbers. You may need to order your own lab work. [37:55] Cholesterol Clarity Statin medications can have terrible side effects. Now, statins are not enough, and there is a new drug to be taken in conjunction with their statins. There is no such disease as high cholesterol. [41:00] It’s Going To Be Okay During COVID You have to be true to who you are. In 2020, people have felt that they need to conform. Even if you disagree, you have to follow along. Jimmy is more concerned about the mental fallout from COVID than the actual deaths from COVID. There will be tens of millions of people who need to deal with their mental health issues. If you want to wear a mask, it’s all good. If someone chooses not to wear a mask, then let them make that choice. There is no reason to blame or shame people because of their choices. [43:45] The One Things You Can Do To Up-Level Your Health Eat real food. Ditch the mainstream food industry. You have to go to the outskirts of the grocery store to see somewhat real food. How many people are still loading their carts with junk food? Jimmy would get rid of all the junk food and have people eat real and whole foods. It would make a dramatic change in our society. People get too caught up with what they are familiar with. AND MUCH MORE! I buy pork only from humanely-raised brands. That’s really the only feasible healthy meat choice for me to consume. I buy organic whenever I can. I like beef liver, beef heart, and chicken liver, but they don't ever come from grass fed or even organic sources. What would you advise to NOT eat at all amongst these non-grass-fed/organic choices for meat? What should I be eating on my keto plan that is very healthy that maybe I’m not thinking about? How do I stay on budget in my finances while pursuing optimal health through nutritional ketosis? Why did I experience reactive hypoglycemia after eating a ketogenic meal? Hey Jimmy and Dr. Cole, I’m a big fan of you guys and four days into eating keto. I feel like I’m starting to get the hang of this and wanted to run something by you. I ate the following food a couple of days ago: one medium avocado, two eggs over easy, two Italian sausages, 3 tablespoons of sour cream, and 3 tablespoons of Kerrygold grass-fed butter. After consuming this food, I almost immediately felt flush, dizzy, and off. I started researching Dr. Google and it looks like I had reactive hypoglycemia. Is this normal on a ketogenic diet or what did I do wrong? Interestingly, I had a fatty coffee and pastrami with cream cheese today and didn’t have any of those same symptoms from he other day. Could it be the extra fat in my meal today compared to the other day? Thanks for the advice on this one. Matthew 5. Are there concerns that medicinal herbs and adaptogens can play a negative role in a patient’s health much in the same way as a pharmaceutical drug? Hey Jimmy and Will, I listen to you guys all the time and Will I hear you talk about various herbs and adaptogens that you’ve found useful in your clinical practice for your patients basically every week. You also talk about the side effects that come from prescription medications. As a fellow medical practitioner, I see both herbs and pharmaceutical drugs as basically extra chemicals substances that you are adding to a patient’s body in the hopes of eliciting a helpful or healing response. All these extra chemicals and compounds, whether food/plant based OR pharmaceutical based, potentially can interact with each other in very bad ways. In the hospital where I work, I’ve seen examples of herbs causing liver and bleeding problems and interacting with other medications. The word “natural” does not particularly reassure me; cocaine, toxic mushrooms, poison ivy, and castor beans are all natural and they are also all noxious. A lot of the pharmaceuticals we use (penicillin, digitalis, the list goes on and on) are basically purified natural products. Pharmaceuticals, at least in theory, have to go through a rigorous testing process including ruling out the very powerful placebo effect. My problem is that herbs and such don’t have to go through that same scrutiny before being made available to the general public. We really have no idea what the dosage of the active ingredient(s) might be from bottle to bottle, brand to brand, of all these herbs, do we? I’ve been low-carb, high-fat, ketogenic for the past five years, was a featured success story in Keto Clarity, have been on Jimmy’s podcast, and teach at an Osteopathic medical school now. Hey Jimmy and Adam, Craig and Maria Emmerich say in their book KETO that taking exogenous ketones should be avoided because the body can’t store them. On the oxidative priority list alcohol is number one followed by exogenous ketones, carbs, protein, and then fat. Since alcohol and exogenous ketones can’t be stored in the body, it immediately begins working to get rid of it as a matter of priority. They say that fat comes in last on the list because it is the body’s preferred fuel source. According to them, when these exogenous ketones are ingested, lipolysis (fat burning) stops. At least that’s how I understood the explanation. This is so frustrating for me to try to understand because exogenous ketones are pushed so strongly in the ketogenic community. I want my body burning stored fat, not exogenous ketones. I’m so very confused on this issue. Hey Jimmy & Adam, Last night, my fiancé and I went out to eat at an Italian restaurant for a friend’s birthday dinner and I decided to cheat. I had a salad, squid ink pasta w/lobster, and for dessert…a big slice of cake. Needless to say, this was a very high-carb, low-moderate protein, and low-fat dinner. I’ve been doing keto on and off for almost 2 years now, and this morning my fasting blood sugar showed 83 and blood ketone was 2.3. I’ve never experienced anything like this before. Is it possible that I’m just so well keto-adapted now that my body can go in and out and back into ketosis much more quickly? I must also mention that because this high-carb meal was premeditated, I prepped for it by drinking a green drink w/apple cider vinegar and some kimchi before we left for the restaurant. Do you think this helped? I’m also in fairly good shape, and workout quite often so that might have also played a factor. I’ve also heard that eating carbs separately from protein and fats helps with digestion and vice versa. There’s one question I haven’t heard anyone provide a good answer for yet and I’m hoping you guys are up for the challenge of answering it. It has to do with the concept of gluconeogenesis. I’ve heard you and Dr. Nally state that it can play a significant role in kicking you out of ketosis and raising blood sugar levels which is why you need to moderate protein intake. You also discuss eating a high-fat diet to satiety and that this within the context of a low-carb nutritional intake does not lead to weight gain. You’ve noted on several occasions that if fat is consumed in excess of what is needed, then the body excretes the extra fat out of the breath, urine, and feces. However, there seems to be two very opposing views amongst the greater keto world and I’d like to know which side is right and why you think the other side is wrong. One the one side, you’ve got people who say that gluconeogenesis is not something to worry about and that eating a higher protein intake will not affect ketosis (or fat adaptation) and that dietary fat should be moderated for those needing to lose weight so your body can tap into those fat stores instead of using dietary fat for energy. On the other side, you have people like you guys who say the body is burning both stored body fat and the dietary fat that you eat simultaneously so you do not gain weight when you eat more fat. This side also thinks you should moderate down on the protein to prevent the insulin and blood sugar effects that can come from consuming this macronutrient in excess of what the body needs. So who’s right? Is there some middle ground between these two positions? My main question is whether moderating protein or moderating fat is the best way to shed pounds which is my current goal. It seems that many people listening to this and other leto-focused podcasts are interested in weight loss, so we need to be crystal clear about what will work to make that happen. I understand everybody needs to find the version of keto that works for them but this is difficult for the common person to figure out. Hello Jimmy and Dr. Cole, I am a 38-year old male with Type 2 diabetes. I am 6'3" and currently weigh 307 pounds. I started keto at the beginning of the year and have lost 25 pounds so far. I stopped taking all my diabetes medications on am only on blood pressure medication at the moment. My blood glucose numbers run between 155-200 no matter what time of day I check it. I have been implementing intermittent fasting with keto for the past month. So my question for you guys is this: Did I stop taking my medications for controlling Type 2 diabetes too soon? I was taking 1000mg metformin twice a day, 20mg glipizide twice a day, 10 units of lantis per day. I feel so amazing on keto and never want to go back to eating any other way. Do I need to go back on the medications for a short period of time until I heal a little more or will my body heal with a ketogenic diet and lifestyle changes over time? Most shocking to me when when my doctor told me I’d be on medication for the rest of my life and that diet and lifestyle don’t make a difference with Type 2 diabetes. In fact, one of his nurses told me that I was not insulin resistant. How the heck can a Type 2 diabetic NOT be insulin resistant? Here’s a little bit about me and my current routine: Info: weightlifter, sprinter, 33 years, 5' 11" male, 70-120 g protein/day, fat to satiety, 1/2 lb raw liver/wk, 2 cloves raw garlic/day, 1.5 g fish oil/day, 600 mg transdermal magnesium/day, 400 mg iodine/day (kelp), 1 g vit C/day (acerola cherry powder), 10 g glycine/day and an occasional scoop of collagen powder. SALT: If I supplement more than about 2 g of sodium a day (usually in 2 L water, Redmond Real Salt) I get nasal congestion and have NO IDEA why (vasodilation or constriction? blood pressure?). Yet in the keto community it is recommended to eat 4 g sodium/day or more. Are the amounts and ratio of sodium and potassium just very understudied and based on the shit standard diet? NOBODY seems to have a good grasp on this. POTASSIUM: Ideal potassium falls in the 2 - 4 g/day range. The fact that sodium helps retain potassium complicates this further. Also, I can't supplement with NoSalt (potassium chloride) because it's said to be dangerous (I take 500mg anyways and haven't noticed anything bad such as heart palps.) But, if I were to take 2 - 4 g would it be dangerous? I can't get this from vegetables because I'm zero carb, and how on earth did our ancestors get it? It makes zero sense to me that you would need to eat inordinate amounts of spinach and chard just to reach this questionable potassium goal. I feel like I really have this diet bolted down, but the lack of specificity with electrolytes in the community is majorly frustrating. Please drop a knowledge bomb on me. Is it possible to feed my healthy microbiome with only 10g vegetable carbs per day? Hey guys, I have questions about how to best maintain my healthy microbiome while on the keto diet. I have researched this topic extensively and keep doing the math. My hunger is minimized and blood sugar is better controlled by counting TOTAL carbohydrates aiming for around 20g daily, including my vegetable intake. I never get more than 10g vegetable carbs per day. Is this enough for me to feed my good gut flora? I take probiotics and have heard that without fiber the bad bacteria grow and will even start snacking on the stomach lining. Any thoughts? Thanks! Pat 2. I recently noticed a big flare up of my Keratosis pilaris (KP) and it’s spread to my thighs and calves when it has remained mostly on my arms since I was diagnosed. A clean diet is supposed to help improve KP, but this seems to have gotten worse than ever before. Is this because of my autoimmune disease or could it somehow be related to keto? Previously on my ketogenic diet, I’d see closer to 0.5 blood ketones and blood glucose levels in the 90’s. Hi Jimmy and Will, I just had some blood work done and my lipid panel looks worse, although I understand that this isn't uncommon when you’re losing weight following a ketogenic diet. Maria Emmerich stated in one of her books that all the excess fat from the fat cells gets released into the blood when you go keto. So, that part makes some sense to me. However, my fasting insulin has also increased (it was 10 in June 2016 and is now 13) and my C-Reactive Protein reading is also dramatically higher (1.6 in June, now 7.7). I'm mostly just shocked about the fasting insulin since I'm not eating hardly any carbs at all and no sugar. I'd be interested to hear any ideas you might have. I eat chicken or beef (homemade) bone broth regularly, Sauerkarut (no sugar) regularly, avocados almost daily, red leaf lettuce salad with black olives, some onion (white or green) with Primal Kitchen Ranch dressing almost daily, free range eggs, grass fed meats whenever possible, Wellshire sugar-free bacon, grass-fed ghee, coconut oil, avocado oil, Chai tea sweetened with Stevia with whipping cream (grass-fed), Lakanto monkfruit chocolate, cheeses from grass-fed cows, some beets, Miracle Noodles from time to time, and ketogenic wine usually only once per week. That's pretty much the extent. No fruit, no starchy vegetables. I usually don't eat until about noon every day and try not to eat after 6:30 pm. Is there any scientific research that connects ketosis with treating vertigo? Dear Jimmy and Dr. Cole, I am a HUGE fan of Keto talk and I have been eating keto for over four months. I'm a very healthy 38-year old woman who suffered from vertigo for about six months everyday that forced me to stop doing yoga since I couldn’t keep my balance. Within the first six weeks of eating keto, my vertigo completely vanished and I was shocked to say the least. On New Year’s Eve, I went on a bit of a sugar binge cheat and came out of ketosis and my vertigo returned with a vengeance much worse than it had ever been before a couple of days later. It thankfully only lasted about a week and hasn’t been back since. Seeing how powerful keto was in managing my vertigo, I definitely don’t want to ever get out of ketosis again. Hi Jimmy and Will, I wanted to ask you guys about the safety of nursing while in ketosis. I am a physician who was reamed in an online medical mom-focused weight loss forum because I shared that I was nursing my baby while in ketosis and people were saying that ketones are harmful to babies and can cause developmental delays. I knew this was completely whacked since we know ketosis is a natural state for babies and humans in general. I’m eating 60+g carbs daily, so I’m probably only mildly keto anyway. The studies they cited had to do with epileptic children eating ketogenic diets and having neurological delays. My baby eats the same keto foods I eat and I add in a few berries for a sweet treat. She’s completely healthy and has never been sick. I was just stunned to hear so many of my fellow medical colleagues react so harshly to what I see as being an extremely healthy way to eat. Hi Jimmy and Dr. Limansky, Thank you for the Keto Talk podcast as I’ve found the discussions very helpful. I first went low-carb (via Atkins) in 2008 or so and I bought the pee strips to test for ketosis. While I lost the weight I wanted to lose—about 20 pounds—I allowed carb-creep to set in! Menopause also came along about that time and whammo blammo, I put on nearly 30 pound in the next year. I decided to start eating ketogenic in January and lost weight and inches off my waist with a concerted effort to stay in ketosis for longer periods. But I’ve noticed I am often VERY warm. At first I thought it was hot flashes again which had pretty much stopped for years, but my temperature goes up and stays up. All I found online is that it is possible that fat burning increases body temperature. Do you know if this is true for people who are in a constant state of ketosis? Is this a good or a bad thing? I have lost 50 pounds since last May using keto and intermittent fasting, implementing much of your advice, and I am now very close to my weight goal. I hit the occasional plateau along the way and at those times I noticed I would need to shift my focus from losing weight to understanding the big picture of my overall health. In that regard, my Mirena IUD (which releases synthetic progesterone teaching my body to make less of its own progesterone) began to seem relevant. I had it implanted in November 2012 and replaced in November 2017. When I had it replaced, I was weighing daily so it was easy to notice a six pound weight jump overnight for no apparent reason.This happened three days after I got the new device and it led me to do some research. I concluded that I really need to get that thing removed which I did in mid-December 2017 over the pushback from my doctor. I got my period back almost immediately and it lasted about a week, but since then (40 days) I have not had my period again. More disturbingly, I am now experiencing other symptoms like itchy skin, shakiness, dizziness, fatigue, brain fog, mood swings, and weakness. I considered electrolyte balance and treated for that with extra salt and magnesium (which always works for me during fasting)—but no effect. I tested my blood sugar when feeling extra shaky and weak—it really felt like hypoglycemia—and it was 83 preprandial (my A1C in November was 4.7). Based on my research, my symptoms suggest low progesterone; if that is the case, how should I proceed? Should I tough it out and wait for my body to ramp its own progesterone production back up? Should I take bioidentical progesterone, or would that throw off my overall hormone balance? Should I look instead to adaptogens like Maca? Is there anything I can focus on nutritionally to encourage a return to optimal hormone balance? Dear Jimmy and Dr. Cole, People sometimes talk about burning fat and burning ketones in the same breath as though they were more or less the same thing, but don't think that's quite the case, is it? Let me describe what I think is roughly going on and then please correct me where I am wrong: Assuming blood insulin levels are low enough, fat can be released from the adipose cells. It was in the adipose cells in the form of triglycerides and after release, it is converted into free fatty acids and then the glycerol part is converted into glucose. The free fatty acids go via the blood and can be used in the muscles directly as fuel, for example. Some of the free fatty acids make it to the liver and can be converted to ketones, some of which are used to fuel the brain, and probably can also fuel the muscles and other parts of the body. Now, in some descriptions, I see ketones being described as byproducts of fat metabolism which I assume means that first the fatty acids are burned for fuel and then ketones result as a byproduct of this metabolism. Is that correct, and if so, can those ketones also be used as fuel? If so, it implies that fat is indeed a super fuel because it can be used twice so to speak. So then what is the biochemical result of burning ketones? Keep in mind that I am seeking, if possible, a reply that is understandable to a layperson like myself but also one which does not oversimplify the science if that’s humanly possible. Hey guys, My daughter is a big believer in keto and she really wants to do the diet for her health. But she has a terrible time with eating fat, even the healthy fats, because of a bad bile reflux condition where she produces too much bile. Foods like guacamole, sausage, and even eggs make her queasy and nauseated. The standard medical advice for this condition is to go on a low-fat diet--something she knows from her research and experience is not a good thing. This condition left me crying often and feeling absolutely awful. Catalina 3. Why are my fasting blood glucose levels rising since giving up wine when I went keto and will they normalize over time? Hi Jimmy and Dr. Cole, I've been following a low-carb diet for years and recently shifted over to keto. I love drinking wine every single day and I realized I probably was drinking too much of it. BOO! So I decided to give it up altogether to see if it would boost my ketogenic efforts to lose weight and improve my health markers. I lost 35 pounds a couple of years ago eating low-carb, but I haven’t lost anything more since. One thing I’ve always hung my hat on despite the lack of weight loss is my impeccable blood sugar control as well as other health markers I regularly track. I take a medication called Atacand Plus for hypertension but no other medications. I’m sure dropping the remaining 35 pounds of weight from my body will enable me to come off that medication for good. So here’s my issue: since going keto and coming off of my beloved wine, my fasting blood glucose levels have risen. I thought coming off the wine would be a good thing, but it seems my blood sugar has gone in the opposite direction. What’s causing this to happen and will it eventually normalize over time? I use straight glycine powder (about 4g under my tongue and hold it there for at least 2 minutes) about 45 minutes after every meal to help my digestion. From what I understand, glycine is an amino acid that helps to release insulin to lower glucose and causes glycogen to be released leading to the release of fat cells. My blood ketones range from .9-1.9 mmol and my blood glucose is usually 4.8 to 5.4 (86-97 mg/dL). Am I in ketosis or ketoacidosis? But it occurred to me as I reviewed my micronutrient intake on keto that the RDA levels from our government are based on a carbohydrate-eating population. Why did my body suddenly stop thriving despite still being in a state of nutritional ketosis? Hi Jimmy and Maria, I started my keto diet a couple of weeks ago and it was typical—the first 3 days were miserable, but on day 4 I felt a drastically better as the ketones kicked in. I had five days of incredible energy, suppressed appetite, and superb cognitive benefits as my blood ketones soared into the 2-3 mmol range. It was great! But then things took a change for the worse as I suddenly started feeling cranky, sluggish, and dealing with brain fog. From my understanding, you multiply the number you get on the BAC meter by 4-5 to get the equivalent blood ketone reading. So a 0.4 reading on the BAC would equate to 1.6-2.0 mmol in blood ketones. Is this a valid way to test for ketosis? I know it’s probably inaccurate, but how far off is it? Using this equation to determine my level of ketones with my keto and fasting efforts, I got a 1.3 reading on the BAC which would equate to 5.2-6.5 mmol on the blood ketone meter. I usually have a resting heart rate around 55-60 bpm, but when I started eating keto it jumped up to around 75 bpm. I've been doing a lot of research on this and most of what I'm finding deals with electrolyte imbalances. I have been salting my foods with pink Himalayan sea salt, using lite salt for potassium, and magnesium. How long does it usually take to rebalance all the electrolytes when all of these things are being dumped at the beginning of keto so my heart rate will return to normal? Can you explain in everyday language what the connection might be between this condition and IR and if there is reason to have hope that this might play a part in my insulin resistance improving more now that I've had the bad gland removed? Is it an issue that I only have one well-formed and easy-to-pass bowel movement every other day since shifting my diet to keto? Hey Jimmy and Will, Thanks so much for talking so openly about poop! When you said that all we need is two snakes a day, all I could think of was no way! After shifting my diet to keto, I noticed the amount of food I was consuming decreased with more nutrient dense foods satisfying me. My theory is my body was actually able to use most of the food I was consuming without much waste which explains why my bowel movements went down. I simply generate less waste now, right? My estimation is that I average a poop every other day but it is well formed and passes easily so it doesn’t appear I am constipated. Should I be worried that it is so infrequent and what should I alter in my diet if my bowel habits sound problematic? In August, I began struggling with regular bouts of diarrhea on a daily basis. I can't remember the last solid stool that I had. I no longer track my macros since I'm at my goal weight. I typically have a Bulletproof coffee with MCT oil powder and butter in the morning and eat leftovers for lunch, an afternoon snack of a cheese stick or almond butter, and then dinner with meat and veggies. I've been playing around with my carbs, including adding back in healthy carbs like sweet potato and checking my blood ketones to confirm I'm always in ketosis. I drink lots of water and I'm good about getting in my electrolytes. I've even cut back on magnesium to see if that would help. It has not. My stomach isn't upset and overall I feel a lot better than I did before keto. Any clues on what I can do to decrease the diarrhea? Thank you for your help! What is causing my poop to be watery and yellow when I’m eating keto and extended fasting? Hello Jimmy and Dr. Cole, I’ve been eating keto with intermittent fasting since mid-April and have been slowly healing my gut from a lot of issues I was experiencing. But here is the issue: my poop is watery and yellowish in color (my liver levels were tested and it came back as optimal on the lab results). I especially see this yellowish loose stool when I fast for longer than 16 hours. My MCT oil consumption is 1 Tbs in my coffee and I did that without issues at the beginning of keto. It kicked in at the three- month mark. My food intake hasn’t changed much since going keto, but I have noticed that when I eat off plan even a little I get the runs hard and furious. One other issue that might be connected to this is I’m dealing with hair loss from thyroid issues as well as high estrogen levels. Can you help me figure this out? Thanks so much for your podcast! Hey Jimmy and Will, I’ve been doing well on less than 20g carbs/day consumed in a single midday meal and working out vigorously with treadmill at an incline and six-mile bike rides. All was going well and I never felt the need to test for ketones. Unfortunately, I was suddenly diagnosed with a horrible inflammatory disorder called Polymyalgia Rheumatica and the only treatment for it is the steroid prednisone which causes an insulin response that can eventually lead to Type 2 diabetes. Sadly, I have to take this steroid for the time being and I’m concerned pairing it with my low-carb, high fat diet will lead to massive weight gain in the belly due to the extra fat since my body will have a more difficult time breaking down fat stores. I’ve only been on the prednisone for a few days, but I’m trying to deal with this now before it becomes a problem. Do you have any ideas about whether my concerns are justified or not? Hi Jimmy and Dr. Cole, I recently spent two weeks with my mom who is a stubborn 76 year old Type 2 diabetic and she is determined to eat sweets now and then because she just “has to have them.” Sadly, she covers these sugary dalliances with insulin. She’s lost 15 pounds this year from simply cutting calories, but she’s still eating things like Saltines every single night. While her A1c has gone down, it’s still at 8.0. I made her listen to the Keto Clarity audiobook on a long car ride and we got through the first 10 chapters with her hanging on every word. Thanks to your book, Jimmy, she wanted to try to make a change in her diet for the first time ever. It is VERY unusual for her to be interested in anything new, so needless to say I was excited. My question for you guys is about her advanced age. Does she need to be more careful than a younger person incorporating a low-carb, high-fat, ketogenic approach into her healthy lifestyle? I have struggled staying in ketosis, but have lost over 20 pounds so far. But my biggest problem is I’ve been having bouts of diverticulitis since making this dietary change forcing me to take Metamucil to raise my fiber intake. My doctor is recommending that I discontinue the ketogenic approach even though he was the one that encouraged me to pursue it in the first place. My C-Reactive Protein is elevated and I am now getting concerned. I’ve done my homework on a ketogenic diet reading all the books and websites and am fully convinced it’s the way to go. I hold a PhD in Biomedical Science from a US medical school and even though all I have read runs counter to what I learned in graduate school, I am fully converted and convinced of keto. What impact on ketosis will there be with the complete removal of the thyroid? Hi Jimmy and Dr. Cole, I have been eating keto since May 2016 and I feel great! I regularly check my blood ketones and am usually in the 0.7-2.0 range. I was diagnosed with Hashimoto's in 2006 and have been on 100 mcg Synthroid ever since. My thyroid levels were all in range last June but my thyroid kept getting larger. During the summer, my doctor found a nodule that ended up showing Hurthle cells. This concerned my doctor who said it’s a cancer risk and he recommended a total thyroidectomy. My surgery is schedule for next week and I’m wondering what impact this will have on my pursuit of nutritional ketosis. Will this make it more difficult for me to stay in ketosis if I no longer have a thyroid? I just want to know what I’m getting myself into so I can respond accordingly after the surgery. What can I do to my keto approach to remedy this? Hey guys, I’m a big fan of Keto Talk and I’ve been eating this way for about a year with great results for the most part. But I noticed a couple of months after starting a ketogenic diet, my sex hormone binding globulin rose from 50 to 70 and my testosterone tanked. I’ve since run these tests several more times and they all confirm the same thing. From what I’ve read, this means my body’s ability to use any available testosterone has been hampered and many sites online blame a low-carb diet as the culprit. If so, should I be worried about this at all? Dear Jimmy and Dr. Will, Yesterday, I went to see my General Practitioner to review my latest bloodwork. She knows I’ve been eating a ketogenic diet for the past nine months and was nothing but pleased with all my markers—except for one. My urine pH was 5.5 and she noted that indicates my body is a bit acidic. She said I shouldn’t worry about this since all the other numbers are perfectly fine, but she made the following statement that stopped me in my tracks: “I am not sure, but maybe it’s the ketones you are excreting that makes your urine more acidic.” What?! Is it true that being in ketosis makes your urine acidic? Should I be worried at all about this or is this of no concern at all. Thank you for your help!
