Our guest today is Dr. Jason Fung, a Toronto-based nephrologist, and the best-selling author of “The Obesity Code,” “The Diabetes Code,” and “The Cancer Code.” Jason is best known for his success in combining a low-carb diet with intermittent fasting to help thousands of overweight patients reverse their type 2 diabetes, lose weight, and improve their metabolic health. Jason is the author of the blog “The Fasting Method” and the co-founder of the Intensive Dietary Management program, an initiative that provides low-carb dietary guidance and counseling on various fasting regimes. Jason is also the co-author with Jimmy Moore of “The Complete Guide to Fasting,” which looks at the history and culture of fasting and how it helps people improve their metabolic health. In today's episode, Ken is joined by Visiting IHMC research scientist Dr. Tommy Wood and together, they and Jason discuss: How in the beginning of his practice, Jason prescribed insulin for type 2 diabetes patients. How a series of landmark studies starting in 2008 changed Jason's mind about using glucose-lowering medication for type 2 diabetes. Jason's realization that type 2 diabetes is largely a dietary disease and therefore requires a dietary solution rather than a pharmaceutical one. The origins of Jason's Dietary Management program, which counsels overweight and obese patients to follow a low-carbohydrate, high-fat diet to reduce insulin. A critique of the “eat less, move more” strategy acclaimed by many obesity experts. Mark Mattson's research into the powerful impact of intermittent fasting on metabolic health and a recent paper that questioned the effectiveness of time-restricted eating when compared to daily calorie restriction. Recent research and evidence that fasting during chemotherapy may reduce the side effects of the treatment. Show notes: [00:02:25] Tommy opens the interview mentioning that Jason was born and raised in Toronto and asks what drew Jason to science as a kid. [00:03:43] Tommy mentions the irony that Jason has written several best-selling books, yet Jason was not fond of English or writing when he was in school. [00:04:53] Ken mentions that after graduating from high school, Jason stayed close to home and attended the University of Toronto, entering into medical school just after turning 19 to study internal medicine, eventually specializing in nephrology. Ken asks Jason what intrigued him about becoming a kidney specialist. [00:06:36] Tommy asks Jason what led him to go to UCLA after medical school for his specialty training in kidney disease at Cedars-Sinai Medical Center. [00:07:37] Tommy mentions that Jason has been practicing clinical nephrology in Toronto since 2001, and that in those early years of his practice, Jason saw patients with type 2 diabetes and prescribed medications to keep their blood glucose low. When that didn't work, he would prescribe insulin, which is the standard medical practice. Tommy asks what Jason observed during these early years of his practice. [00:09:28] Ken mentions that in 2008, two landmark studies were published, the ACCORD study and the ADVANCE study (a summary of ADVANCE). These studies were followed by two more studies, the ORIGIN and VADT studies, all four of which demonstrated that using blood glucose-lowering medication for type 2 diabetes didn't necessarily have the expected benefits. Ken asks Jason to talk about how these studies were eye-opening for him and confirmed his own experience in treating patients. [00:16:27] Tommy explains that in 1972 Robert Atkins published the “Dr. Atkins' Diet Revolution,” which shared his findings on the effectiveness of low-carb, high-fat dieting. Tommy goes on to mention that in the late 1990s, a string of Atkins-styled diet books surged in popularity, with most physicians being opposed due to the conventional wisdom that these high-fat diets would cause heart disease. You know, we've already seen a lot of those investors start to sell, they'll continue to sell their inventory, even with what institutional investors right now in the market, which is probably 4% or 5% now, and we're talking about the big Blackstone's market home sprintt, just plenty of opportunity. [00:00:35] Sam Wilson Paul Bettinelli is an experienced real estate developer and asset manager. After years as a corporate director and executive growing the business, he decided to launch his own company at the age of 39. Paul, welcome to the show. [00:00:48] Paul Pettinelli Thank you for having me. [00:00:49] Sam Wilson The pleasure is mine. Paul, there are three questions I ask every guest who comes on the show in 90 seconds or less. Can you tell me where did you start? Where are you now? And how did you get there? [00:00:57] Paul Pettinelli Started off in Hotel Management right out of college and worked in hotel operations for several years, first of all, when I was 22, so when I left the hotel industry, I decided to do it full-time. So I started a company, ran it for three years, joined a company called Parallel Real Estate, which led into working as a director for a startup SFR institutional fund. And then that segued into me running Nautic. So basically, I've been very much in SFR, but I have also worked in multifamily and commercial. And, yep, so really, the hunger that I learned in hotels, and watching the corporate guys talk about CAPEX improvements, really launched my interest in that next level of real estate. [00:01:45] Sam Wilson Got it? And so today you guys are focused on the single-family residence build-for-rent communities, like you're developing entire BFR communities. Is that right? [00:01:57] Paul Pettinelli Well, that's a part of it. SFR and multifamily, we talk we will do consulting in both, but the investment strategy is primarily single family and built for rent. Camellia houses founded by Jimmy Moore and Jim Isaminger, I'm a partner in that company. And we're building in the Birmingham area. And we're also with Nautic, which is a partner in that we're doing the infill products property flips, and then also doing buying holds. [00:02:28] Sam Wilson That's okay, that's a lot, so you got buy and hold, you got infill, you got property flips, and then… [00:02:34] Paul Pettinelli It's all single-family though, single-family investments. [00:02:37] Sam Wilson Right? Tell me I guess, you know, with all these, you know, various pieces of the puzzle, where do you fit into this? [00:02:45] Paul Pettinelli Sure, I mean, my core competency has always been on the asset management strategic investment side, there's a lot better operations manager than I am, I'm very good at looking at a plan the financials of it, assessing risk. Looking at the macro economy, I consider myself fairly well-read in investments in what's going on in the world. And I've used that knowledge to set the course on where, where that investment portfolio will be in real estate. And I think, you know, it's, it's been successful, it's been me focusing on my core competencies always made me an attribute to the companies. [00:03:26] Sam Wilson And it sounds like you see some good things, obviously, in the future for what it is you guys are building and investing in today? What's the opportunity that you see maybe that other people aren't seeing? [00:03:40] Paul Pettinelli Well, like anything, just manage around risk and you manage around the obstacles, but the good thing is that, you know, the heightening that increasing rates, slowed down the economy, which in a way there, it was good, the advantages will be that prices will be more affordable for investments that prices will come down. The interest for in single-family, interest rates are going to soften the market and there'll be more inventory available, there will be 10 people fighting for 10 investors fighting for one home, and then there also be a demand in the thick of demand for single-family matters. We already saw that demand with the new consumer coming into the single-family rental space. This will just exacerbate it. [00:04:27] Sam Wilson How do you guys select a market to do a build-for-rent community? [00:04:33] Paul Pettinelli There's got to be a good amount of land there's been there's a few things the land has got to be there. And but there if you can find enough information to see how much of that has already been entitled, how much you know what is the average time from entitlement to getting the right zoning permitting. Luckily, you can see that information through companies that are already building not for rent but building just regular can Unity's and then you know, really the influx of people moving into that market. So a lot of the primary and secondary markets are a little bit tougher, because you have to really expand outside of that city center. But if there's good growth, then that expansion outside of that, that main city, those surrounding areas will if you build the communities, there'll be plenty of demand. But sailboats hassles. So Kansas City is a good example, using that as a Midwest market, because Midwest is a great market and built for it. Kansas City has a lot of opportunity on those outskirts as well as Birmingham. So variant has that perimeter, that is expanding out to see build those communities. The house was available in Birmingham, maybe over prides not seeing it as she's known. But as long as the demand keeps coming in the average income of that tenant household keeps coming into that market, you know, that can be absorbed before. [00:06:00] Sam Wilson Right, right now, that makes a lot of sense. And you'd mentioned that there's some tenant demographics, that you guys are seeing that you said, Hey, this, this is going to be here for a while, if that tenant pool isn't is not gonna be there for a while, but also going to grow? What are some of those? What are those some of those demographics that you guys are seeing, he say, Hey, this is what we're looking at. And this is why we think that this, you know, will continue to be a needed product. [00:06:23] Paul Pettinelli It's basically household formation, household formation, and the household income. So, you know, that's a stat that you can look at, but then you can look at the millennial generation as a sub the average age right now of the household formations. And, and households that are purchasing homes, getting mortgages or getting mortgages, those same, that same age range is the same tenant demographic that wants to rent, the house may not want to be in the apartment, because we want the lawn, that separated walls. So it's anywhere from 35 to 40. And then again, later on in life. So we see that a lot. It's kind of it's kind of like that barbell, maybe hope I know, my parents are an example. They sold their house, and they now rent, because they looked at the opportunity of reinvesting that cash into a house, as opposed to putting it in the markets, you know, whatever their investment strategy is, and, you know, the return, the opportunity cost wasn't worth it. So we find a very educated tenant that ends up once they educated, maybe not paper, but they understand the cost of owning the home. Right? And, you know, on and on was a great thing. But there's, you have to look at it differently than it being an investment because it can be and it can't be, just depends. [00:07:47] Sam Wilson Right? Right. Yeah. And there's a lot of different ways that you could slice and dice that, that comment and have a whole conversation about whether or not owning a home is an investment, which we won't get into here today. But do tell me this, like what's the total total business plan for you guys? So you're gonna do a build for rent community? You bring in tenants, how long do you hold it? What are the risks? What are the upsides? I mean? What's the whole business plan look like for you? [00:08:16] Paul Pettinelli Sure, well, as we scale the focus on the Lake Street fund, because that is basically what's absorbing everything, even the build for it. So we have a, you know, about a seven-year window, maybe 10 years to raise, deploy, and stabilize and then exit. Well, there is a lot of housing right now, that is owned by Mom and Pop landlords so to speak. You know, we've already seen a lot of those investors start to sell, they'll continue to sell their inventory, even with what institutional investors right now owe in the market, which is probably four or 5%. Now, and we're talking about the big Blackstone's American homes grant, just plenty of opportunity to purchase that inventory. And Bill as well, because there's a housing shortage. So the housing shortage, along with the opportunity to be able to take those renters in that want that home is betting and the risk is jumping into the space honestly, and not understanding the investment strategy. That's the risk. That's the housing new stuff. So now if you look back years, you have enough data to know how housing will go. You know, twould say it's the safest IRR at that level that you'll get because even multifamily is a great investment over wrong but you can't diversify your risk so you can be in many markets. And that's a great investment strategy because you can be in an area of town you can have 100 units over several zip codes, and have your assets protected. Even if it's 50 basis points of a drop in value. Now the first 10 years, we had to figure out how to operate that, now that there's best practices on how to operate single-family and scale. We're able to capitalize on that. [00:10:06] Sam Wilson What do you feel like some of those best practices are some lessons, maybe you guys have learned the hard way? [00:10:12] Paul Pettinelli It's really keeping a good product, maintaining it. I work for a company that when I was overseeing asset management and the management section of it, a lot of the fault landlords were the primary business. So they had just come out of 2008, 2009. And they were very cost-conscious, essentially, every single maintenance item, they have decide on, you know, we have our budgets, we know the mouth operate the whole, but the biggest cost in operating a home is a cost of losing a tenant. So if we maintain a home Well, every turnover we're putting are making sure their useful life on the carpet, the paint, everything is crisp, and new, because that's the only thing that sets us aside from not every mom and pop landlord, we just walk great small operators out there. Let's say mom and pop shop, anyone under 100 homes. So yeah, just making sure that every full American home is right is a great example and invitation home, they have really good products that they roll over. And they don't short, the quality of those houses, they move into a house freshly painted new carpet, maybe not hardwood the lbpd floor to feel like you're moving into something fresh, a lot of times it's nicer than homey. So that is the key point is tenant moves in that experience is what's it's going to be that first impression. If you can keep those retention rates up, you're gonna have retention rates up, occupancy cost down, the costs, you'll spend a lot more maintenance even more than you budgeted? You'll end up better off a long. [00:11:53] Sam Wilson I like what you said there that the biggest expense is tenant turnover, occupancy, you know. You lose it, you lose a tenant for a month or two. And that's an enormous expense. [00:12:03] Paul Pettinelli But mitigate rent control, too. We don't want to increase rents too much. You want to price out your target market. And so if your costs are high, because you're not offering a portfolio, and you make and let's say every operator made up for it by driving rents, well, that's not a good thing. Becauyou don't want to drive the rent pass a certain threshold affordability, if you manage when read, natural rent increases are needed. But if you operate the retention piece, you can keep their rents reasonably lower, or direct raise reasonably lower, and you don't have to price at your market. [00:12:46] Sam Wilson What's the average tenancy period? [00:12:50] Paul Pettinelli It's approaching three years. Yeah, I'm gonna say it's about a 70% retention rate. So I'm looking at a five-year window. So let's say your average hold time on that asset is five years, you if you're operating it reasonably well should keep a tenant two and a half or two years. [00:13:09] Sam Wilson Yeah, that makes a lot of sense. And I think that's longer than probably than what the average would be, say, in an apartment, you know, and also, again, it goes back to the fact that people want to live in a home these aren't people that want to live there in an apartment, the end of that five year period. Are you guys selling off those properties? Are you, what's the disposition strategy? [00:13:29] Paul Pettinelli That's I mean, essentially, you know, you spend the first two years raising and deploying the capital, right, leverage it up. And yeah, five years is generally the point at which you, you want to exit out of that, that special purpose vehicle, meaning the best strategy is to sell that portfolio at a lower cap rate to another institutional investor. But we can extend you know, if you're offering memorandums are written, well, you can have that extension because maybe five years, maybe you want to extend it out a couple of years. There's 10 years fund out there, there's evergreen funds, but five years tends to be the sweet spot. [00:14:07] Sam Wilson That's the sweet spot for you guys. That's really, really cool. And I guess the other thing you probably get on brand new build for rent is decreased maintenance costs overall. [00:14:18] Paul Pettinelli That's definitely a plus. You know, I would say rental home is about 58 to 60%. And the y margin didn't get up as high for the first two year 70%. Maybe higher. They got warranties. So yes, absolutely. [00:14:35] Sam Wilson That really, really cool. And I guess maybe the last thing I guess as it pertains to maintenance when you have entire families moving into a home, you know how Are you preventing those from just getting, you know, just torn to pieces? I mean, it gets somewhat, you'd have an apartment complex, but I would think a home might even take more wear and tear than an apartment. I don't know. Is that… [00:14:54] Paul Pettinelli Yeah. Well, when you're, you know, when you're managing a large portfolio, yeah, you have to have the best practices in place, which is screening. I mean, you have a lot of times, I mean, there's a lot of technologies out there now, which is good for screening where it automates it, you know, it was companies will grant me, for example, has an application as the lockbox company that has the screening on the back end. If you keep that screening to a level and you're and you're, you're pretty stern on abiding by that you'll have a bit tan, not just credit of that about, you know, landlord referrals, or if you can't catch everything, but for the most part, if they've been a good tenant, or they've been a, you know, they're they don't have a huge blemish on your record. It's usually a good sign. I mean, if you're just getting heads in beds, and you're recruiting everyone, that's when your turnover becomes an issue. And I'm not saying that, I'm not saying that they're going to get the homeless. I'm not saying that. I'm just saying as a rule of thumb, you know, people who have looked after their trade line and their credit will look after your own. [00:15:59] Sam Wilson Right, no, that's a fair baseline analysis there. So that's really really cool. I love what you guys are doing certainly the build-for-rent industry is exploding I don't see it going anywhere, anytime soon. You've shed a lot of light on you know, everything from maintenance down to finding tenants to you know, just the way you guys are finding opportunity here in the country, how you're, how you're picking your markets and things like that. Episode 101: Fasting Precautions. By Danish Khalid, MS4; and Sapna Patel, MS4. Ross University School of Medicine. Comments by Valerie Civelli, MD; and Hector Arreaza, MD. Fasting is a healthy lifestyle that may impact your health but fasting is not for everyone. Sapna, Danish, Dr. Civelli, and Dr. Arreaza explain some precautions to be taken in certain populations. We've talked about intermittent fasting, but we need to add a very big caveat: fasting isn't for everyone. It carries certain risks. Some people who should absolutely not attempt fasting include those severely malnourished or underweight, children under eighteen years of age, pregnant women, and breastfeeding women. And the concern for these individuals involves providing adequate nutrition for normal growth or development. We also have to be cautious in patients with chronic heart problems, renal issues, eating disorders, fragile diabetics, or recently hospitalized patients. Underfeeding during this period may result in stunted growth, which may be irreversible. D: Or in pregnant women, the developing fetus requires adequate nutrients for optimal growth, and nutritional deficiency may cause irreversible harm during this critical period. It's for this reason many women take specialty formulated pregnancy multivitamins. S: The same concept applies to breastfeeding mothers. Developing babies receive all their nutrients from the mother. So, if the mom becomes deficient in vitamins and minerals, then the baby may also be deficient. Which again would result in irreversible growth retardation. D: Others that should take caution when fasting but don't necessarily need to avoid it include those who have gout, diabetes, gastroesophageal reflux disease, or are taking medications. For these individuals, it is wise to seek medical advice from a healthcare professional. A: Chances are that you may not find a physician who is pro-fast, but after reading about it and trying it myself, I think it is a safe way to lose weight or maintain a healthy weight. S: Gout is an inflammatory arthritis caused by excess uric acid in the joints. It can be either due to decreased uric acid excretion through urine or increased production of uric acid through breakdown of nucleic acid. Fasting decreases the elimination of uric acid through urine. Thus, theoretically worsening gout. Now although most patients with a history of gout tolerate fasting without any exacerbation, knowing the potential risk is important. D: If you have type 1 or type 2 diabetes, it's essential to be particularly careful while fasting or even just changing dietary patterns. This is especially true if you are taking medications. If you continue to use the same dose of medication but reduce food intake, you run the risk of your blood sugar getting low - a situation called hypoglycemia. Symptoms include shaking, sweating, irritability or nervousness, feeling faint, confusion, delirium, seizures, and if left untreated may even lead to death. What is even more worrisome, these symptoms may appear very rapidly, so understanding your body and the cues it provides is essential. Thus, you must consult with your physician to adjust the doses of diabetic medication before starting any dietary program to avoid having any hypoglycemic episodes as they can be potentially life-threatening. A: The risk of hypoglycemia is high in patients with diabetes who are taking medications, but it's less likely to happen in patients with obesity without diabetes. The body fat (stores) acts as the fuel for your body functions. Patients will not die if they stop eating. S: If you have GERD (heartburn) this is oftentimes due to increased pressure on the stomach which forces food and stomach acid back up. This can be made worse during fasting because there is nothing in the stomach to absorb the stomach acid. Sometimes, fasting can improve symptoms because food stimulates the production of stomach acid, so fasting reduces it. A: My GERD improves with fasting. D: Patients who are taking regular medication for any condition need close follow-up as certain medications are best taken with meals. The most common medications that cause problems during fasting include aspirin, metformin, iron, and magnesium supplements. S: Myth: Women shouldn't fast. One area of specific concern with women is that fasting could affect reproductive hormones, LH and FSH, similar to that seen in anorexia. This can lead to amenorrhea and difficulty conceiving. However, these problems only arise when body fat percentages fall too low. And women with excessively low body fat should not be fasting in the first place. As mentioned earlier, these individuals are severely malnourished and should stop immediately. V: Women: 12-13 hrs. fasting, increase to 1 hr. up to 16 hrs. 8hr feeding window. 6-8 wks. for full benefit.A: Exercise is allowed during fasting. Conclusion: Now we conclude episode 101, “Fasting precautions.” Fasting is safe for most patients but be cautious in certain patients such as pregnant women and diabetics. Make sure you take the necessary steps to avoid side effects or complications during fasting. Even without trying, every night you go to bed being a little wiser. She brought up a topic that has recently gained public interest. Intermittent fasting. So, if you're listening, Hina Asad, this one's for you! Let's jump in! V: 2/3 women are overweight and obese. 1.5 pounds gained/yr on avg age 50-60's.S: So like we said earlier, intermittent fasting has recently gained much public interest as a weight loss approach. Or should I say, revitalized itself, as it has been around for years. It describes an eating pattern in which you alternate between periods of eating and fasting (or not eating). The length of each fast can vary in duration. A: There are feasting and fasting periods, or fed states and fasting states. What is more effective: Intermittent restriction of calories or continuous restriction of calories? D: Before we dive in, let's go back. We know that calorie reduction has been consistently found to produce reduction in body weight and improve overall health. We talked about how to calculate our basal metabolic rate and subtracting calories from our daily caloric intake to result in weight loss. However, this can be difficult to sustain over a long period. Additionally, it requires that you adjust your caloric needs every so often as you lose weight, which can further make it difficult. So how is intermittent fasting different from this? S: Well, in contrast to calorie reduction, intermittent fasting focuses on when calories are consumed and the total quantity consumed. Intermittent fasting works through an altered liver metabolism, referred to as the “metabolic switch.” It's where the body periodically switches from liver-derived glucose to adipose-derived ketones. In doing so, it stimulates an adaptive response including improved glucose regulation, improved insulin sensitivity, and increased stress resistance via conditioning. V: When you eat is more important than what you eat. Benefits: reducing cancer, Alzheimer's, DM risk, better sleep, less hangry(*find evidence). D: What happens when we fast? In our previous podcast we mentioned ketosis, but let's talk about the physiology behind fasting.Feeding: blood sugar levels rise as we absorb food and insulin levels rise in response to move glucose into the cell. Excess glucose is stored as glycogen in the liver to convert it to fat.S: Postabsorptive phase (6-24hrs after beginning fasting): Blood glucose and insulin start to drop. To supply energy ,the liver starts to breakdown glycogen, releasing glucose. Glycogen stores last 24-36hrs. V: Insulin levels are low, and fat stores are available and improves mental clarityD: Gluconeogenesis (24hrs - 2 days after beginning fasting): Glycogen stores run out. The liver manufactures new glucose from amino acids called “gluconeogenesis” ( literally “making new glucose) S: Ketosis (2- 3 days after beginning fasting). A: Autophagy: “Auto” means self and “phagy” means eat. So the literal meaning of autophagy is “self-eating.”S: The protein conservation phase (5 days after beginning fasting): High levels of growth hormone maintain muscle mass and lean tissues. The energy for basic metabolism is mostly supplied by fatty acids and ketones. Blood glucose levels are maintained by gluconeogenesis using glycerol. Increased adrenaline levels prevent any decrease in metabolic rate. There is a normal amount of protein turnover, but it is not being used for energy. V: How long should we fast for? D: Fasts can range from 12 hours to three month or more. We can categorize them as short (24 hours). However, shorter regimens are generally used by those mostly interested in weight loss. The short daily fasting regimens can be divided into the length of fasting - 12 hours fasts, 16 hours fasts, and 20 hours fasts. S: Daily 12 hour fasting introduces a period of very low insulin levels during the day with 3 equally spaced meals throughout the day. This prevents the development of insulin resistance, making the 12 hour fast effective against obesity. Although a great preventative strategy, it is not the most effective at reversing weight gain. D: Fun Fact: In years past, the 12 hour fasting period was considered a normal eating pattern. This probably explains why prior to the 1970s, there was much less obesity. It wasn't until the 1970s when the USDAs made dietary changes making a higher-carb and lower-fat diet a staple. That's when obesity started to rise. S: On the other hand, during the 16 hour fasts most people skip the morning meal to account for the extra hours. In this regimen, you have an 8-hour eating window period, this is why it's also called time-restricted eating. Although you can still eat 3 meals most people tend to stick to 2 meals. The 16 hour fast certainly has more power than the 12 hour fast, but it should be combined with low-carb diets to allow for a slow and steady weight loss. A: Feasting periods should not be so liberal, and over time it becomes easier to control hunger. V: Feeding hours: healthy fats, proteins, fish, avocados, grass fed butter, unprocessed carbs (especially Low glycemic berries, squash, quinoa, vegetables, Low sugar, low alcohol intake… eating healthy basically. D: Fun Fact: A Swedish bodybuilder named Martin Berkha popularized this regimen, which is why you will also hear it being called the LeanGains method. V: Skipping breakfast reduces caloric intake by 20-40%, addresses visceral fat.S: Lastly, the 20-hour fasting regimen, also known as “the Warrior diet.” Ancient warrior tribes such as Spartans and Romans devised a “warrior diet” in which all meals are eaten in the evening during a 4 hour window. This results in a 20-hour fasting period each day. This diet also emphasizes natural, unprocessed foods and high-intensity interval training. A: Summary: 12-hour, 16-hour, 20-hour. Dr. Jason Fung also recommends 24-hour fasting. It is basically skipping breakfast every day and skipping lunch 3 times a week. “Hunger is your friend”. D: Before we move forward, I just want to add that not all fasts are the same. For instance, I'm a Muslim, and there's a month where we fast for religious purposes, called Ramadan. During this time we fast from sunrise to sunset, or dawn to dusk. In contrast to traditional fasting, this fasting differs in that we don't eat or drink anything. Even water. Whereas in intermittent fasting it's different. Now, there have been studies done where they studied individuals during this time to see if there was any weight loss during this period. It was found that people typically lost about 1-2 pounds of weight. However, I do want to clarify this weight loss could be fat loss or muscle loss. A: Another group of people who fast are Mormons. They traditionally fast once a month, the first Sunday of every month. It's a complete abstinence of food and water for 24 hours, skipping 2 meals. Fasting periods are linked to improve your spiritual well-being as well. S: Certain Hindu festivals and holy days require devotees to observe fasting as part of their worship. For example, Navarātrī, the nine-night celebration that occurs yearly. Some people take only water during these nine days, while some eat fruit while some eat one meal a day. Hindus will observe fasts of varying strictness depending on individual beliefs or practices. Here are some examples of common fasts observed by Hindus:not partaking any food or water for a set number of days.limiting oneself to one specific vegetarian meal during the day.eating or drinking only certain food types for a set number of days.Avoiding eating certain food types for a set number of days.' S: So what can I consume when I fast? Do I have to completely stop eating and drinking for those hours? D: Only certain fluids can be consumed during fasting periods: water, tea and coffee ( iced or hot) and homemade bone broth. It's important for you to drink water frequently throughout the day. You can enjoy flat, mineral or carbonated water. V: While Fasting: ok to have coffee, tea and water. Fasting creates a state of alertness. S: What can you add to your water? Limes, lemons, sliced fruit (do not eat the fruit itself), vinegar, Himalayan salt, chia, and ground flaxseeds ( 1 tbsp in 1 cup water). Do not add sweetened powders even if it's sugar-free. D: You can consume up to 6 cups of caffeinated or decaffeinated coffee on a fasting day. Black coffee is preferred, but you can add up to 1 tbsp of certain fats in your coffee. These include: coconut oil, medium chain triglyceride oil (MCT oil), butter, ghee, heavy whipping cream (35% fat), half and half, whole milk, ground cinnamon for flavor. V: Ghee butter is clarified butter with no lactose. A: You can curve appetite by drinking water, eating grains of salt, and drinking pickle juice (use a straw to avoid dental problems) S: You can consume unlimited herbal tea during your fasting period. I know Danish and I both are Tea Connoisseurs. Right Danish? Teas can suppress your appetite, lower your blood sugar levels and are otherwise beneficial (positivi-tea). Bitter melon tea, black tea, cinnamon chai tea and oolong tea, help lower blood sugar levels. Peppermint tea and green tea help suppress appetite. Peppermint is good for GI discomfort such as gas and bloating. A: Peppermint oil is good for IBS. D: It's not uncommon to experience some lightheadedness during your first few days of fasting periods. This is often caused by dehydration and decreased levels of electrolytes. An easy remedy is a good homemade broth. Both vegetable and meat or bone broth will work. Things you can include in your broth: any vegetable that grows above the ground, leafy greens, carrots, onions, bitter melon, animal meat and bones (mostly bones, any animal), Himalayan salt, any herbs or spices, ground flaxseeds. Avoid vegetable puree, potatoes, yams, beets or turnips and store bought broths. (Dr. Fung). A: This is the end of this part on “How to fast”. Some people think fasting includes being hungry the whole day, but the “hungry” feeling goes away after 1 hour, and you learn to recognize the cues from your body about hunger and satiety. ___________________________________________________________________________ Now we conclude our episode number 99 “Intermittent Fasting 99.” This is not a complete guide to fasting, it's only a brief overview. Fasting has become a new nutritional trend with proven benefits. Remind your patients that one of the secrets of fasting is “delay, don't deny”, meaning they can delay eating a few hours and then enjoy what they like the most. [01:45] The Reason You Should Stop Taking Melatonin Every Single Night Jimmy has been working on his sleep for about ten years. If you're going to be healthy, you need to get your sleep dialed in. Health isn't just about food; it's also about sleep. When people have sleep issues, they start taking melatonin. Melatonin helps you get to sleep, and you do feel heavy with melatonin. Over time, Jimmy realized that he kept needing to have more and more melatonin. Eventually, Jimmy started to train his circadian rhythm by getting sun in his eyes. Also, ice baths before bedtime are helpful for sleep. [10:40] Cortisol Isn't The Enemy; However, You Need To Manage Stress Still Check out the Oura Ring: https://ouraring.com/ You want a high level for HRV. You can get that through meditation and stress management. However, people live in a stressful state, never managing it. You can be on keto forever, but that stress will negatively affect your health if you don't have it under control. Most people don't have high cortisol. In fact, they have super low cortisol. Well, you need cortisol production throughout the day to be healthy. If you have unmanaged stress, it can really affect your hormones. [31:00] Start A New Tradition: Going To Sleep As A Family Do the best you can when it comes to your sleep. If you can't afford the best meat possible, just do the best you can. Grain-fed beef will be better than cereal. So, talk with the people in your family about improving your sleep. Start a tradition where you and your family all go to sleep at the same time. [34:30] Listen To Your Body Because The Oura Ring Is Just A Tool An Oura Ring is a tool for your health journey. It will not be 100% accurate. You need to take everything with a grain of salt. Don't get too upset if one night on your Oura Ring doesn't seem right. Listen to your body because you know when you slept well. [39:50] Adding Fruit and Honey To The Carnivore Diet Jimmy is disappointed in the persistence of redefining terminology. For instance, there has been a massive trend of adding fruit and honey to your carnivore diet. If fruit and honey make you feel good on the carnivore diet, then do it! However, when you call it carnivore, you are changing what the term means. Jimmy says that it's disappointing they are changing the meaning behind standard health terms. [43:55] Keto Isn't A Weight-Loss Tool; Keto Is A State of Metabolism You are doing a disservice to your clients if you're a coach, and you promote keto solely as a weight loss diet. Keto is a state of metabolism, not a diet. If you tell people to eat keto, then it doesn't make sense. You get into a state of metabolism to improve your health. Don't promote keto as a weight-loss tool. Of course, keto is a powerful tool for weight loss; however, it's a weight-loss tool for everyone. Keto puts your body in a better position to be optimally healthy. [47:00] The Mental Benefits of Keto Will Change Your Life Forever The psychological component of being in ketosis is the best part of keto. When you're not thinking about food 24/7, that's life-changing. On keto, you'll be satiated and have hunger control. When you nourish your body well, your body will reward you. Could you share with people a little bit about that and kind of how you got to where you are now? Eating the Standard American and Low-Fat Diets Jimmy Moore: Oh yeah. I was eating a low-fat diet before finding low carbs. And I think that we all do that. We all kind of default, when you need to get healthy, when you need to lose weight, what you did, you cut back, right? We all heard that message our whole lives. And if that works for you, you're doing that and over and over and over again. I think what draws people back to it is the reinforcement, when you do that and then you do lose some weight. You feel somewhat better. I would argue because you get rid of the junk in you feel a little better. For me, I had tried all those things and I had been successful up down, up, down, up, down, up down with low-fat diets. And in fact, in 1999, you mentioned family health problems. My brother had a series of heart attacks and one week, so four years older than me, my brother, Kevin. In 1999, he had a heart attack, he was in the hospital for some routine checkup and had a heart attack while it was in the hospital. Thank God, because it probably would have died had he not been there. Then later, he'd have another heart attack, on and on and on. He died at the age of 41, but when he had the heart attacks and again, that was a wake-up call for me and I lost a bunch of weight. And at the end of about nine months,

About 20 years ago, after the birth of her youngest daughter, Nurse Kate O'Connor began to gain weight on her small 5'2” frame and noticed that if she ate any carbohydrates during the day it would make her especially sleepy. Diet-induced sleepiness was a difficult challenge for a full-time working mother of three! This caused Kate to restrict her carbs and as the extra weight disappeared, she also noticed that she had much more energy. Kate maintained that weight loss and felt better for quite some time, but gradually, as her children left the house and she was able to have a slightly more relaxed lifestyle she moved towards eating more carbohydrates again.Fast forward to a few years ago Nurse Kate was 52 years old, Peri-menopausal with hot flashes, having difficulty sleeping, and taking two different medications for Hashimoto's thyroiditis as well as another for high blood pressure. Kate happened upon the podcast Keto Talk with Jimmy Moore and Dr. Adam Nally. She binge-listened to all of those episodes because the science made sense to her and she was eager to change and to learn. A light-bulb went on for Kate from that period, and she gradually started to change and adapt to the keto lifestyle. Rather quickly Kate lost the weight she had gained, started having better sleep and best of all experienced far less brain fog and found she had her energy back for the first time in years.Today Kate takes less than half the thyroid medication that she did three years ago and has not had a hot flash since she became fat-adapted. Her quality of life is much higher.Always a lover of science Kate completed a BS in Biology from Worcester State University. Kate's study in biology lead to an appreciation of physiology and the amazing abilities of the human body, which resulted in her pursuit and completion of a Nursing degree. Kate spent about 10 years working in the float pool of UMass Medical Center, which means that she worked on every floor of a bustling hospital, enabling her to learn constantly about the many different aspects of patient care. Kate spent another 5 years working in urgent care at UMass, where she was again in a position to see a broad spectrum of health concerns and treatment.
