I see you a lot on social media and reached out because I know that not everyone has an advanced directive.Discussing advanced directives can be uncomfortable, but it's crucial when making decisions for someone else. Family members often feel obligated to pursue aggressive care, which might not align with the patient's wishes. An advanced directive can guide family members or friends, ensuring decisions reflect the patient's desires.Dr. Tanno, how did you get into this field? Your experience highlights the importance of advanced directives, especially in hospitals where critical decisions are made. Please share your journey and the background of Advanced Direct MD.Thank you for having me. During my time in the ICU, I noticed that very few patients had discussed care planning with their families. Only 36% of people have an advanced directive, often skewed towards older individuals. This lack of planning leads to family members making uninformed decisions, often opting for more aggressive and sometimes futile care. I realized that advanced directives should be created with physicians, not just attorneys. This led me to teach people how to create comprehensive advanced directives through courses and lectures, providing real-life examples to clarify their importance.Let's discuss some tips for our listeners on creating an advanced directive. Firstly, you don't need an attorney to make an advanced directive. You can download your state's form online. However, it helps to create it with someone knowledgeable to ensure it's legally sound and up to date.Tip one is to name your medical decision-maker. Without one, your spouse or adult children might be the default, which can lead to conflicts. Name someone explicitly to avoid these issues. Tip two is to have a detailed conversation with your decision-maker about your medical wishes. Teach them the answer to the question: "I would want to be kept alive as long as I am still able to ___." Today is D-day, aka Design Day, and John is finally going to see the design of his yacht. For the big reveal, Tanno invites him for a cruise on a small yacht from the Feadship Heritage Fleet: the Ammerland. As they glide through the beautiful lake district, Tanno reveals the process he and his team went through to translate John's mid-century desert bungalow dreams into the blueprint of a yacht. He shares his visual explorations of geometric structures, interesting rooflines, ornamental sunscreens and shady breezeways. Then comes the big reveal: an A0-sized blueprint with detailed renderings of four decks, each space carefully crafted to John's hobbies and wishes. It is a minimalist yacht whose most distinctive feature is an extended roof that resembles Eero Saarinen's iconic 1962 TWA Flight Centre at JFK airport. It is a yacht that works with nature, not against it. Underscoring this is Giedo, who reassures John his yacht is indeed the most sustainable on the planet. In this episode, John shares a moment only the richest 1% of the richest 1% get to enjoy: the reveal of a superyacht entirely customized around his lifestyle. If there's one industry under fire for the size of its ecological footprint, it's the superyacht industry. Without a radical shift towards more responsible material sourcing and non-fossil fuel propulsion, superyachts run of the risk of becoming Noah's Arks. With owners willing to spend nine figures on a boat, a few more zeroes for sustainability shouldn't be a problem, right? Inspired by the hydrogen-powered yacht the TU Delft students showed him in the previous episode, John wants to design the most sustainable superyacht on the planet. What fuel-reducing technologies are available to him? Are there viable alternatives to teak? And is it true that biofuels like ‘Hydrotreated Vegetable Oil' (HVO) can already reduce a yacht's carbon footprint by 95%? And if so, why aren't owners doing it? In this first episode of Feadship Uncovered , John gets his first taste of pure custom creation at Feadship's ‘closed' shipyard on the small Dutch island of Kaag. Strictly off-limits to anyone but billionaire buyers and master craftsmen, John has come to meet with Tanno Weeda, head of design, and Arjen van Elk, sales director. It's their jobs to turn his ideas into reality and, more concretely, transform his yacht dreams into a blueprint of a real yacht. The Feadship journey begins for every owner the same exact way: with a blank sheet of paper. John has a vision for 1950s modernist-inspired floating bungalow that can carry him to the world's best surf breaks. With a suitcase full of inspiration, he challenges Tanno and Arjen with impossible demands like a hydraulic ping-pong table and a bowling alley, a cryogenic chamber and on-board wave pool. No matter what he asks for, however, the answer is always: Yes. Everything is possible…at a price. In this episode John tries to get his head around the thousands of decisions that go into designing a superyacht from scratch. Eleanor Tanno, MD is a Family Physician, Speaker, and Advance Directive Consultant. She completed her medical degree at the U. of Maryland School of Medicine, and her Family Medicine Residency at the Virginia Commonwealth University. She is a full-time primary care family physician and partner in her private practice. She started her side venture, Advance Directive MD, to help people create meaningful advance directives. She does this by providing Workshops, Webinars, Public Speaking Events, and Medical Lectures. This interest developed after meeting many families in the ICU who had never had end-of-life conversations with their family members. And she discovered that most patients who completed advance directives were being shown how to do so by attorneys. However, attorneys and their staffs were not in the best position to explain the options from a medical perspective. So, Dr. Tanno created a business to educate the public about advance directives, and teach individual clients how to properly describe their wishes in these critical documents. Listening to podcasts while driving does not affect knowledge retention (Gottleib et al. 2021) Treatment of Hot flashes: Avoid paroxetine, fluoxetine, and sertraline in breast cancers survivors taking tamoxifen. Instead use venlafaxine (or another SNRI), citalopram, or escitalopram. Gabapentin given at bedtime is also an option. (Clinical Update 2019: Tamoxifen and Antidepressants) Distress in patients with cancer can be measured using the NCCN screening tool [NCCN Distress Thermometer] Don't forget to ask for a Cancer Survivorship Plan from oncology [CDC Cancer Survivorship Care Plans] Ask about sexual dysfunction in cancer survivors. How will climate change impact our patient's health? NIH framework; CH2OPD2 environmental screening tool [CH2OPD2 ]; and the 15 steps of climate communication. Use the AM-PAC score cutoff of >18 to avoid a PT consult in hospitalized patients (Martinez 2021). Early antibiotics after an aspiration event do not improve outcomes (Aga 2021). Biased language is more common in notes about Black patients (Beach 2021). Be thoughtful about what you write and role model good behavior. While plant-based meats are more eco-friendly than animal-sourced meats, they are heavily processed foods (van Vliet 2021) and may have high sodium. Silicone breast implant rupture can be insidious. Guidelines recommend screening with MRI after 6 years, then every 2-3 years (FDA 2020, More FDA info) (Note: FDA's most recent guidance also recommends ultrasound as an alternative screening method, so always check with the patient's insurance about which modality will be covered) Document a trial of NSAIDs, physical therapy, attempted weight loss, and supportive bra usage before breast reduction surgery for macromastia. Doxycycline is now first line for chlamydia rather than azithromycin [STI Treatment Guidelines Update], BUT the most effective treatment is the one your patient will take! Metronidazole does not cause a disulfiram-like reaction and is no longer recommended by the guidelines [STI Treatment Guidelines Update] More clinician-educator pearls from Era: Don't be afraid to brand yourself! Network with like-minded individuals (e.g. interest groups).
