一、思想問題：身為神的僕人，有什麼是被神限制的？神設立限制的目的何在？二、鑰節： 16:5 耶和華如此說：「不要進入喪家，不要去哀哭，也不要為他們悲傷，因我已將我的平安、慈愛、憐憫從這百姓奪去了。這是耶和華說的。16:8 你不可進入宴樂的家，與他們同坐吃喝。神禁止耶利米前往喪家、婚宴之家，來表達即將臨到的刑罰，人根本來不及辦喪禮，根本不可能結婚，先知用自己的生活來表達神的信息與心意。今日聖徒當然要與歡樂的人同樂、與哀哭的人同哭，應當要前往喪家慰問、婚宴祝福，但是更要知道這一切都是地上短暫的關係，最重要還是要讓所有人明白永恆的死亡與永恆的婚約。三、經文結構：→→C16章-17:4 不可娶妻或進入哀傷或宴樂之家：深刻的罪。 16:1-9 神吩咐先知三不：1-4 不可娶妻，5-7 進喪家，8-9 進宴樂處。 16:10-17:4 百姓受刑罰： 10-13 受罰之因：超越祖宗的墮落。 14-18 刑罰必臨：14-15 超越出埃及的拯救；16-18 將要來的刑罰。 19-21 外邦得救：超越種族的救恩。 17:4 刑罰必臨。四、結構亮光： Q：神禁止耶利米做的3件事是什麼？為什麼？ A：三不vv.1-9： 1.不可娶妻：vv.1-4先知的婚姻要被神所限制，看似犧牲，實際上是保護！因為刑罰將臨，不是娶妻的時候，而是悔改的時刻，並且這地的兒女也將要承受極大的刑罰與死亡。挪亞的日子人們嫁娶，人子來的日子也是如此，求主使我們能夠分辨日子將到，並警醒禱告！ 2.不可進喪家：v.5-7全面的死亡，人來不及埋葬、舉哀。從大到小、不分地位高低、貧富差距，都要經歷全面的審判。 vv.6-7提到4個表示哀慟的禮儀：劃身、禿頭、擘餅、喝酒。前者早為神所禁止，可能與異教習俗有關，但在以色列民間似乎仍然流行；餅、酒則是送給居喪者的食物，以示安慰。 3.不可進入宴樂之家：vv.8-9在有生之年，必看見神止息一切婚禮。 Q：vv.10-21所提到的犯罪與刑罰，有何特別之處？ A：強調「超越性」： 1）超越祖宗的墮落：vv.10-13提到了他們的罪惡，先指出祖宗所犯的罪，v.11離棄神去事奉別神已是大罪，v.12但是這群人所犯的罪「更甚」，堅持頑梗地行惡，完全掩耳不聽神的話，所以要承受比祖宗更甚的刑罰。 2）超越埃及的拯救：vv.14-15這一方面是個應許，將來神要把他們從被擄之地帶回來，而人要記念神；二方面也可見被擄所承受的痛苦，遠遠大過在埃及為奴時所承受的苦難。因為v.16敵人如同漁夫、獵人來獵取他們的性命；v.18因為地上充滿了偶像、罪惡。 v.14神的恩典憐憫常常不「足夠」使人悔改，人得在在管教中才能學習敬畏神。 3）超越種族的救恩：vv.19-21過去救恩只在以色列人中，但是現在神要v.19使外邦人可以認識神，v.21使他們知道神的能力與神的名，就是神自己一切的屬性，並使人看見偶像的虛無、虛空，看見神的大能與拯救。以色列人的墮落，反倒使萬國可以認識神，這是神的智慧！五、反省問題：我是否能分辨在對的時間做正確的事？什麼是神現在吩咐我要做的事，或禁止我去做的事？我是否認識我裡面的重大罪惡？我是否經歷了神更大的拯救與憐憫？－－－－－－－－－－－－－－－－講員：貴格會合一堂徐坤靖牧師聖經之鑰-各卷書播放清單： https://thfc.pse.is/3epsdf 【聖經之鑰相關資源】YouTube： https://thfc.pse.is/3cfams電子書： https://thfc.pse.is/3ccluu Powered by Firstory Hosting

一、思想問題：什麼是倚靠神的人生？有什麼樣的記號？二、鑰節： 17:7 倚靠耶和華、以耶和華為可靠的，那人有福了！耶利米身處在一個背叛神的時代，無論神如何大聲疾呼，百姓就是拒絕聆聽，在這樣的環境中，耶利米仍然堅定選擇倚靠神，拒絕把生命的根基放在世人自以為的安全裡，而是深深地扎根在神的裡面，v.8即便有環境炎熱的挑戰，仍然可以在神裡面得著極大的滋潤與喜樂。倚靠神的具體行動就是：謹守安息日；呼求主使我們戰兢地尊主日為聖日，不做私事、不說私話、不存私意，堅定走跟隨神之路。三、經文結構： →→C16章-17:4 不可娶妻或進入哀傷或宴樂之家：深刻的罪。 →→→D17:5-18 中心點：個人的審判信息：兩種人生。 5-8 兩種倚靠：5-6 倚靠人；7-8 倚靠神。 9-11 兩種心：9,11 充滿詭詐；10 接受神的鑒察。 12-16 兩種對神態度：13,15 離棄神；12,14,16 讚美跟隨神。 17-18 兩種結局：17 不要驚恐；18 敵人驚惶。 →→C'17:19-27 安息日的信息：不要在安息日從家中擔出擔子去。 a19-22 重申律法。 →b23 人拒絕。 a24-26 守安息日的祝福。 →b27 違背安息日的遭禍。四、結構亮光： Q：vv.1-4提醒我們罪惡的可怕，它如何深度地影響著我們？ A：形容猶大根深蒂固的罪。 v.1我們應當把神的話語深刻在心上、在壇角上；但猶大卻是把罪惡放在心上，深深地刻在壇角上。 1.心：生命的中樞。 2.壇角：敬拜的中樞。 3.後代：記念偶像，不記念神，反倒記念這些謊言。從內在到外在，從個人到教會，從教會到社會，都充滿了墮落的風氣，充滿了罪惡。罪惡有很大的延續性，甚至會影響到後代，求主使我們戰戰兢兢地尋求主，懇求神的救恩臨到。 Q：vv.5-18提醒我們兩種人生，各有哪些相對應的選擇與結局？ A：這段類似智慧文學的教訓，是先知個人的宣告：兩種倚靠vv.5-8：地上人生～ v.5倚靠人的膀臂，離棄神。v.6沙漠、曠野、鹼地代表荒涼。天上人生～ v.7倚靠耶和華，有福。 v.8樹在溪水邊，不懼怕乾旱，因為根深深扎在活水裡。兩種人心vv.9-11：地上人生～ v.9充滿詭詐，壞到極處，無人能了解。 v.11不按著正道追求財富，這財寶到最後也不是他的，被證實是愚人。天上人生～ v.10接受神的鑒察、試驗，在神的光照中，接受神的賞罰。兩種對神的態度vv.12-16：地上人生 v.13離棄神，名字在塵土中，是虛空、短暫，被踐踏的，並離開了活水泉源。 v.15藐視神，說神的話在哪裡呢？這不過都是空談。天上人生～ v.14神是我所讚美的，是我的醫治、拯救。 v.16敬重神： 1）使命：沒有離開牧養的職分；2）心態：沒有渴望刑罰日臨到；3）言語：傳講討神喜悅的話語。兩種結局vv.17-18：地上人生～ v.18蒙羞、驚惶，災禍臨到。天上人生～ v.17有避難所不驚惶、不羞辱。我們每一天都要做選擇，呼求主使我們勇敢做有智慧的選擇，選擇擁有豐盛的生命，選擇倚靠主，選擇接受神的光照與引導。 Q：vv.19-27為什麼特別提到「守安息日」？ A：「守安息日」是律法中最具體的表現，主日時能否停下自己的忙碌，而單單來到神面前是敬畏神的重要測試。這一段採abab架構，a是守安息日的祝福；b是百姓拒絕。 a的部分提到守節的重要與祝福。v.21不要擔擔子，要停下世界的工作，意思就是要停下世界的工作，選擇安靜下來。安息日最重要的一個任務：聽命勝於獻祭；v.20,23,24,27連續出現了6次「聽」，神期待我們用心聆聽神的話、明白神的旨意、以安息日為聖日。神期待我們有安靜的心來到他的面前，聆聽神對我們的話語。敬拜要領受的福氣是v.25國家平安、城市長存，大衛的子孫繼續為王；v.26人們從四面八方湧來加入敬拜，真正的敬拜將會帶出全地的敬拜。用心敬拜神的人，對身邊的人有正面、積極的影響力，使人看見敬拜的重要。相反的，若是不肯聽從，神要v.27興起刑罰，有戰火要在四圍興起，而且無法熄滅。五、反省問題：我是否謹守主日，安靜心來聆聽神的話？還是仍然掛心、費心我的擔子？當如何改變？－－－－－－－－－－－－－－－－講員：貴格會合一堂徐坤靖牧師聖經之鑰-各卷書播放清單： https://thfc.pse.is/3epsdf 【聖經之鑰相關資源】YouTube： https://thfc.pse.is/3cfams電子書： https://thfc.pse.is/3ccluu Powered by Firstory Hosting

The Whole View, Episode 461: Oats-M-G, Are These Actually Healthy? Welcome back to episode 461! (0:28) Stacy has mentioned in the past how great she feels when she includes some oats in her diet. She clarified that it's not something she loads up on daily. Finding high-quality, gluten-free, and non-glyphosate was a game-changer for her as well. For years, Stacy and Sarah have talked about how grains were problematic for digestion, proper absorption of nutrients, and gut health. However, it's important to revisit how we look at food with a scientific approach as often as possible. Both Stacy and Sarah's health journeys took a path through the Paleo Diet to a point where they are now very open to experimenting with foods they were not maligned to for a long time. For Sarah, that means revisiting the science of these foods to get away from the idea of grouping foods together and instead look at the pros and cons of individual foods and how they can fit into an overall healthy diet to empower others to experiment. Sarah is a self-described Thrive Market junkie, and many of the (non-sponsored) products she and Stacy will be discussing can be found on their site. This question in particular resonated the most: First off, I ADORE you both. I feel like you are my best friends and just don't know it yet :) I want to eat raw heads of lettuce (I've done this for years!) and get angry about poorly conducted research with Sarah, and go lift heavy weights while discussing clean beauty with Stacy. I can't get enough of you guys! So, my question is related to oats. Because my diet has been restrictive for so long while I've tried to heal my digestive system after being diagnosed with celiac three years ago, I have now developed anaphylactic responses to a lot of paleo foods. I was really happy this month when I was successfully able to reintroduce rice and oats, finding them to actually feel calming on my stomach. Because my list of paleo foods that I am reacting to keeps growing longer the more I rely on them, I am finding myself turning to these other foods like legumes, rice, and oats, on a daily basis in order to simply get enough food in. While I seem to tolerate it, it makes me nervous that I may be doing long term damage. I know you have discussed briefly how rice and some beans are actually not bad for the gut, but that was still always with the caveat of them being eaten "occasionally." And I don't think I've heard you discuss oats much at all. Could you perhaps address the effect of GF oats on the microbiome, as well as the overarching effects of a diet that includes a lot of these "non paleo" foods, but in the context of an otherwise veggie and fruit rich diet with only healthy fats, high quality meat and organ meats, and no refined sugars? Thank you! I love you both and am so thankful for all you do! - Anna Dogma of Paleo and Other Rigid Diets Stacy shares how much this question resonates with her. She first started on this health journey from guilt and feeding into the dogma of paleo that glorifies some foods and demonizes others. (8:00) In fact, what qualifies as a "paleo" food will vary from person to person. It's okay to lean into foods that make you feel good even if they don't fit a diet "framework" of what good and bad for you. For example, it was easy for Sarah to lump oats in with wheat and other grains and not critically evaluate it independently. Paleo has a very rigid set of rules, and it's important to Sarah to give listeners the tools and confidence needed to find what works best for them as individuals. Even if you stray from the framework of a particular diet but still identify with the term Paleo, there's nothing wrong with that. What works today might not work two years from now. It's okay to change to something better suited for what we need in a moment. Stacy stresses that they are not here to tell you what good, bad, and qualifies under a certain label. Health Benefits of Oats Oats have been very well studied for at least 30 years and have some well-established health benefits. (16:27) Whole grain oat products have shone to positively impact coronary heart disease, diabetes, satiety/weight management, low glycemic index, and blood pressure. These characteristics are mainly attributed to the high content of oat-specific beta-glucans, which are soluble food fibers. However, some people don't do well with oats. This is likely due to gluten cross-contamination. Gluten is a prolactin that is inflammatory that increases intestinal permeability, and is linked to adiposity signals. But avenin, prolamin type lectin, may not be as bad. Oats are often grouped in with wheat and barley as problematic, but not because oats themselves are a problem. Avenins are present at a lower concentration (10%–15% of total protein content) in oat as compared to gluten in wheat (80%–85%). The avenins in the genus Avena are free of the known CD immunogenic epitopes from wheat, barley, and rye. T cells that recognize avenin-specific epitopes have been found very rarely in CD patients. In fact, avenins are highly digestible and don't come out half-digested like gluten often does. Unlike gliadin peptides, oat peptides are also sensitive to digestion by pepsin, trypsin, and chymotrypsin in the gastrointestinal tract. They are unlikely to have any clinical relevance. Some cultivars of oats are gluten cross-reactors, meaning that if your body forms antibodies against gluten. But this appears to be a fairly low-frequency cross-reactivity and even not a problem for most people with celiac. High Likelihood of Cross Contamination However, oats are often processed on the same wheat, making the potential contamination pretty high. For example, the U.S. specification for No. 1 oats allows the presence of up to 2% foreign material, which could be all wheat and barley! So yes, the biggest con of oats is that it's frequently contaminated with gluten. As we discussed on a recent show, upwards of 55% of people, have at least one gluten sensitivity gene. Stacy references Oat O's and Thrive Market (not sponsored on this show!) for reduced risk of cross-contamination and what she personally leans on. (26:50) A 100-gram (2/3 cup) serving of oats has a moderate glycemic load (37) but serves a fairly high (389) in calories. That serving of oats does provide an impressive 246% DV for manganese and 40-50% DV for vitamin B1, magnesium, and phosphorous. It also provides 20-30% DV of iron, copper, zinc. Not too shabby... but not crazy impressive either. Sarah has been calculating Nutrivore Scores, and oats are the third-highest grain after corn and buckwheat. She adds that this is neither a pro nor a con for oats but shouldn't really be considered a "foundational" part of your diet, but rather fit in for added benefits. Revisit 29:30 to figure out what Stacy is talking about with more context. Oats and the Microbiome That same 100-gram serving contains a whopping 10.6 grams of fiber, including being particularly rich in a very special soluble fiber: mixed linkage beta-glucan. (34:31) This is a highly fermentable fiber that feeds anaerobic microbes in the gut and significantly increases the production of the short-chain fatty acids butyric and propionic acids. Oats contain 3-7% beta-glucan! Studies show over and over again that beta-glucans from oats significantly increase the growth of very important probiotic strains of Bifidobacterium as well as the Lactobacillus-Enterococcus group. And oat beta-glucans seem to be better than beta-glucans from other grains. One study showed that an oat-based diet resulted in higher Bifidobacteria levels than either a barley-based or wheat-based diet supplemented with equivalent amounts of beta-glucan. In addition, these high levels of beta-glucan appear to be the "magic ingredient" behind oat's most famous properties, including their cholesterol-lowering effects. This occurs because beta-glucan increases the viscosity of the chyme in the upper GI tract, leading to increased binding and secretion of bile acids. In turn, plasma cholesterol becomes a substrate for newly formed bile, causing blood levels to decrease. Beta-glucans contribute to the maintenance of normal blood cholesterol levels. Consumption of beta-glucans from oats and barley as part of a meal (4 g/30 g carbohydrates) contributes to reducing the blood glucose rise after that meal (EU 432/2012). Oat grain fiber contributes to an increase in fecal bulk (EU 432/2012). Reducing saturated fat consumption contributes to the maintenance of normal blood cholesterol levels (EU 432/2012). Oat beta-glucan (3 g/day) has been shown to actively lower/reduce blood cholesterol. High cholesterol is a risk factor in the development of coronary heart disease (EU 1160/2011). In addition, oat fibers increase the fecal bulk, which contributes to a normal stool and positively impacts the functioning of the microbiome. Oats in particular also have high bioactive potential and relatively high lipid content relative to other grains. Their unique composition contributes some special benefits to gut health compared to other whole grains. Surprise! Oats Are Super Digestible Protein content in the oat groat is relatively high (15%–20% by weight), as is the digestibility of oat protein (90%), and the majority (85%–90%) of oat protein consists of globulins. (43:15) This is in contrast to wheat, in which the vast majority consists of gluten. Starch (about 60% of the total dry weight) is a major component of the oat grain. The amylopectin: amylose ratio is about 3, and the digestibility of oat starch is about 100%. Oat starch digests slowly, partly due to high amounts of fiber and the high oil content in whole grain oat, which retards stomach emptying and improves digestion. This results in a gradual supply of glucose to the intestine, which maintains a long feeling of satiety. As a result, whole grain oat foods have a low glycemic index (GI), which is advantageous in cases of diabetes and obesity. Compared to other cereals, oat grains have a relatively high oil content of on average 7% oil, but some varieties can have up to 18%: The three most abundant fatty acids are palmitic (C16:0; 20%), oleic (C18:1; 35%), and linoleic (C18:2; 35%) acids, which account for about 90%–95% of the total fatty acids. The largest part is thus unsaturated, but there is more omega-6 than omega-3. Alpha-linolenic acid (18:3, omega-3) is notably present in the oat germ. The high lipid content can also hurt the sensory quality of oat products because of lipid oxidation, which produces fatty acid hydroperoxides and volatile aldehydes, causing rancidity. Therefore, before further processing, oats are kilned—a high-temperature treatment to inactivate especially lipases. So, Which Oats Are Best? Sprouted is best! Sprouting happens the seed is trying to turn itself into a plant. However, they are tough to sprout at home unless you buy unkilned oat groats. (48:35) The nutritional and bioactive properties and key enzymatic activities were studied in sprouted oat powder and compared with those of oat grain powder (control). Sprouted oat powder was an excellent source of protein (10.7%), β-glucan (2.1%), thiamine (687.1 μg/100 g), riboflavin (218.4 μg/100 g), and minerals (P, K, Mg, and Ca). This presented better amino acid and fatty acid compositions and levels of γ-aminobutyric acid (54.9 mg/100 g), free phenolics (507.4 mg GA/100 g), and antioxidant capacity (1744.3 mg TE/100 g) than control. Enhanced protease and α-amylase and reduced lipase activities were observed in sprouted oat powder. Promising features to improve its nutritional, sensorial, and health-promoting properties! Since 2009 (EC 41/2009) and 2013 (FDA), oat products may be sold as gluten-free in several countries provided a gluten contamination level below 20 ppm. Note: 20ppm may still be too high for celiac or gluten allergy! Types of Oats Interestingly, when it comes to oats, the size of the flakes appears to make a difference in how the gut microbiota are affected. One study found that thick oat flakes, but not thin oat flakes, increased Bifidobacterium levels during fermentation while also producing 2.5 times more butyrate than the thin oat flakes. The authors of this study suggested the resistant starch content of the thick oat flakes may explain these differences. Steel-cut oats have a little more resistant starch, slightly higher beta-glucans, and higher total fiber than old-fashioned rolled oats. Remember, old-fashioned rolled oats are better than quick oats. One small study showed slightly higher butyrate production with steel-cut oats than old-fashioned, but it wasn't statistically significant. Sarah recommends One Degree brands for the best-sprouted oats (not sponsored! Just a Sarah-approved favorite). Personally, Stacy and Sarah have both Old Fashion and Steel Cut oats currently in their pantries. Sarah recommends using an instant pot as well! Sarah's ideal recipe is 2 cups steel cut oats, 5-6 Cups of water (depending on how thick you like your porridge), and 1/2 a teaspoon of salt (scale up or down as desires). In Instapot, cook for 4 minutes on high pressure and then let the pressure naturally decline. Add flavoring as needed, such as maple syrup to slightly sweeten. Stacy also recommends trying this pumpkin oatmeal recipe, which has always been a crowd-pleaser. How to Experiment Sarah recommends starting by utilizing a food journal to see if you're reaching your nutrient targets for the day. If you are, first cut out the food from your diet for at least two weeks, even including trace amounts. This ensures you're testing your actual reaction and not the reaction you're currently having while already consuming the food. This helps you isolate and identify. Next, pick a day to challenge when you're feeling good, slept well, not stressed, and not eating anything else out of the ordinary. If you're worried about an allergy response, make sure you go slow since they can get really big very fast. If you check for sensitivity, like mood, stomach, or skin issues, it's okay to go more all-in. If you're not sure if something was a reaction (mild reactions can be tough), give yourself a few days to reset, and then try again to see if you have the same experience. If you're thinking about experimenting with oats, Sarah recommends buying sprouted oats (which reduces anti-nutrient content) that are certified gluten-free. Be sure to follow a methodical reintroduction protocol! Also, oats are way less likely to be tolerated by people with celiac disease (and potentially other autoimmune diseases) or anyone who has tested positive for wheat/gluten or oats on a food allergy/intolerance screen in the past. Final Thoughts Sarah views oats as a "sometimes food" due to the importance of quality. (1:01:30) Stacy's philosophy is all about variety and introducing foods to develop a palate. And the more science we look at, the more we can emphasize foods we know are beneficial to our gut and health overall. It's important not to emphasize "yes" and "no" in terms of food, but instead look at each food as an individual in terms of a healthy diet overall. Identifying your triggers is very important, and knowing that not every food will work for every person. For the first time, men were beginning to shave themselves instead of visiting the barber, and a whole new market emerged to cater for rising demand in all sorts of shaving products - soaps, pastes and powders. But the way these were promoted suggests there was confusion over exactly what the ideal man should be. On the one hand, razor makers appealed to masculine characteristics like hardness, control and temper in their advertisements whilst perfumers and other manufacturers of shaving soaps, stressed softness, ease and luxury. So enter the world of Georgian personal grooming to discover the 18th century's inner man. Alun Withey lectures in the Centre for Medical History at the University of Exeter and is a Wellcome Research Fellow and a BBC/AHRC New Generation Thinker. He has edited an essay collection on the history of facial hair (Palgrave), curated a photographic exhibition of Victorian beards in the Florence Nightingale Museum in London and has written for BBC History Magazine and History Today. Dr Carolyn Lam: Welcome to Circulation on the Run, your weekly podcast summary and backstage pass to the journal and its editors. I'm Dr Carolyn Lam, associate editor from the National Heart Center and Duke National University of Singapore. Dr Greg Hundley: And I'm Greg Hundley, associate editor as well at Circulation, and director of the Poly Heart Center in Richmond, Virginia, at VCU Health. Well, I'm going to talk about anti-hyperglycemic agents and look at a very important meta-analysis. Dr Carolyn Lam: Those are the rage: GLP-1 receptor agonists and SGLT-2 inhibitors. But first, let's talk about psychosocial stress and cardiovascular health. So what is the joint impact of multiple stressors on racial or ethnic disparities in cardiovascular health? Well, this question was tackled by Dr Albert, from University of California San Francisco Center for the Study of Adversity and Cardiovascular Disease and her colleagues. They basically studied more than 25,000 women participating in the women's health study follow-up cohort, and examined the relationship between cumulative psychosocial stress and ideal cardiovascular health as defined by the American Heart Association Strategic 2020 Goals. As a reminder, this health metric includes smoking, BMI, physical activity, diet, blood pressure, total cholesterol and glucose, and higher levels indicate more ideal cardiovascular health and less cardiovascular risk. So, they found that both cumulative psychosocial stress and ideal cardiovascular health varied by race or ethnicity. Mean cumulative psychosocial stress scores were higher in Hispanic, Black, and Asian women compared to white women, even after adjusting for age, socioeconomic status and psychological status such as depression and anxiety. The mean ideal cardiovascular health scores remained worse in blacks and better in Asians compared to whites, despite taking into account socioeconomic factors and cumulative psychosocial stress. Dr Greg Hundley: So Carolyn, how should clinicians incorporate this information in what we do every day? Dr Carolyn Lam: Although the cumulative psychological stress and socioeconomic status did not fully explain the racial or ethnic differences in ideal cardiovascular health that we saw, clinicians should be informed by these data that psychosocial stressors are social determinants of health that have different prevalence according to race and ethnicity. I think that's what we need to learn. And of course these data support the need for additional work that addresses the joint impact of multiple social determinants of health on cardiovascular disease and in diverse populations. Dr Greg Hundley: Very good, Carolyn. That was really an interesting article. Well, I'm going to switch gears and talk about the role of red blood cells in promoting vascular calcification. My article is from Dimitrios Tziakas from the Department of Cardiology in Thrace, Alexandropoulos, in Greece. Now, the presence of extravasated erythrocytes in human atherosclerotic lesions was described several years ago, but little is known about a possible active role of red blood cells during these cardiovascular disease processes. Clinical studies suggest that intraplaque hemorrhage may be associated with the progression of coronary, carotid, and atherosclerotic lesions and degenerative calcific aortic valve stenosis. So, in the present study, the authors examined the contribution of erythrocytes to vascular and valvular lesion progression, focusing on the effects of red blood cells on the osteoblastic transdifferentiation of smooth muscle cells in calcification. Dr Carolyn Lam: Interesting. So, what did they find? Dr Greg Hundley: So, lysed erythrocytes, and in particular their membrane faction, enhanced human and murine arterial smooth muscle cell mineralization and vascular aortic ring calcification. Red blood cell membranes injected in the vascular regions of atherosclerotic-prone mice also promoted calcification and red blood cells were found to co-localize with osteoblast like cells in human atherosclerotic plaques, stenotic aortic valves, and abdominal aortic aneurysms. And so, the study demonstrated that intra plaque hemorrhage promotes atherosclerotic and valvular lesion calcification and membranes of extravasated lysed red blood cells appeared to play an important role in the process. The investigators also showed a mechanism, that nitric oxide derived from erythrocyte endothelial nitric oxides synthase is involved, at least in part, in mediating the effects of red blood cells on vascular calcification. Dr Carolyn Lam: Thanks, Greg. Now back to another, well, clinical paper with the next one asking, do mid-life biomarkers of heart and kidney damage associate with the level of and decline in physical capability with aging? Dr Kuh and colleagues from MRC Unit of Lifelong Health and Aging at University College London used data on 1,736 men and women from the oldest British birth cohort study. And, looked at their walking speed, chair rise speed, balance time, and grip strength. Assessed at ages 60 to 64 and 69 years. They tested the associations between Cystatin C, NT-proBNP, interleukin-6, and E-selectin all at ages 60 to 64 years with their performance at 69 years. And what they found was the lower levels of NT-proBNP in interleukin-6 in middle aged adults were independently associated with better physical capability up to nine years later. And all these associations were stronger than those observed for conventional risk factors: including lipids, blood pressure, and glycemia, and were not explained by the onset of cardio vascular and kidney disease or diabetes. Dr Greg Hundley: Carolyn, is this saying we should now measure these biomarkers in mid-life? Dr Carolyn Lam: Ah, before considering the use of NT-proBNP and IL-6 or interleukin-6 for risk stratification, we really do need further research to untangle whether these associations exist because the biomarkers are an integrated measure of accumulated exposures to stressors. Or, whether they are really capturing early an organ damage. Or, whether they are marking additional risk pathways. So, this and more is discussed in a great accompanying editorial entitled "Putting the Measurement of Physical Capacity in Older Adults in its Place". And that's by Dr Kritchevsky from Wake Forest School of Medicine. Dr Greg Hundley: That's a favorite of my heart, Caroline. The old institution Wake Forest. But, I'm going to switch now and tell you a little bit about plasma ceramides and this is an article from Wei Zhao from the Department of Epidemiology in Population Health at Albert Einstein College of Medicine in Bronx, New York. The study evaluates the role of ceramides and what are those? Well, they're a class of bio-active lipids composed of sphingosines and fatty acids. And are involved in the development of cardiovascular disease. Elevated circulating levels of ceramides have been shown to be associated with increased risk of cardiovascular events, cardiovascular death, and even so, after adjusting for other cardiovascular disease risk factors. Now, interestingly, ceramide metabolism has long been noted to be closely related to HIV infection. But, the relationship has not been fully understood. HIV infected cells may cause enhancement of sphingomyelin volume breakdown and accumulation of intercellular ceramides, whereas intercellular accumulation is associated with enhanced replication of HIV. So, what did this study do? They evaluated circulating levels of four ceramides species which have been investigated in previous studies of non-HIV populations. And were measured in 737 women and men, 520 HIV infected and 217 HIV uninfected from the Women's Intra-Agency HIV Study and the Multi-Center Aids Cohort Study. And they compared the relationships with the progression of carotid artery disease assessed by B-mode ultrasound over a seven year period. Dr Carolyn Lam: Interesting approach. So, what did they find? Dr Greg Hundley: Elevated ceramide levels were associated with anti-retroviral therapy use. Particularly, protease inhibitor use HIV infected individuals. All four ceramides were highly correlated with each other and significantly correlated with total cholesterol and LDL cholesterol. And of note, remember they were measuring four, but C16:0 and C24:1 ceramides rather that C22:0 and C24:0 ceramides were more closely correlated with specific modified activation and inflammation markers and, surface markers of CD4 t-cell activation. Elevated plasma levels of C16:0 and C24:1 ceramides were also associated with progression of carotid artery atherosclerosis. So, in summary, the results of this study provide new information on biological mechanisms that may involve the specific mono-site activation and inflammation beyond cardiovascular disease traditional risk factors like cholesterol levels. For the association between ceramides and CVD, particularly among individuals living with HIV infection. Dr Carolyn Lam: Fascinating. Thanks, Greg. Now that sets us up for beautifully for our featured discussion. Dr Greg Hundley: Welcome everyone, to our podcast. My name is Greg Hundley and we've got a very exciting paper for the second part of our program today. With us we have Dr Thomas Zelniker from Brigham and Women's Hospital. And then, also, a guest editor, Dr John McMurray from Glasgow, Scotland. We're going to be discussing a meta-analysis in type 2 diabetic patients. Thomas, can you tell us a little bit about the study population, your design, and what where the outcomes that you saw in this study. Dr Thomas Zelniker: As you know, the last half decade, members of two drug classes, GLP1 receptor agonists and SGLT2 inhibitors, so our goal was to provide clear context by comparing or contrasting the benefit of these two drug classes, and in particular to investigate the potential heterogeneity in the treatment site between patients with and without atherosclerotic cardiovascular disease. For that reason, we performed meta-analysis of all randomized partially controlled cardiovascular outcome trials of GLP-1 receptor antagonists and SGLT-2 inhibitors. We included data from more than 77,000 patients, nearly 43,000 patients coming from the five GLP-1 receptor antagonist trials and approximately 34,000 patients coming from the three SGLT-2 inhibitor trials. We tried to compare patients with those with known established atherosclerotic cardiovascular disease with those that have multiple risk factors for but no evident ASCVD. And as you can see, our interests included MACE, or major atherosclerotic cardiovascular events, and its individual components, MI, stroke and cardiovascular death. And then we looked at hospitalization for heart failure and progression of kidney disease. The progression of kidney disease was defined as one of the broad composites consisting of new onset of macroalbuminuria, worsening of eGFR, end-stage kidney disease, or death due to renal cause. And then we also had a more narrow kidney outcome which excluded macroalbuminuria. Dr Greg Hundley: Thomas, did you observe differences in the types of events between the two agents, as they would have impacted hospitalization for heart failure or the progression of renal disease? Dr Thomas Zelniker: Right. So foremost both trial analyses reduced the risk of MACE, major atherosclerotic events, but the reduction of MACE was actually confined to those patients with atherosclerotic cardiovascular disease. We saw a 40% reduction in patients with known ASCVD, where neither of these groups reduced the risk of MACE in patients with only multiple risk factors but without ASCVD. Now, in terms of the individual components of MACE, both trial analyses reduced the risk of myocardial infarction cardiovascular death but only GLP-1 receptor agonist reduced the risk of stroke. In contrast, as SGLT-2 inhibitors vastly reduced the risk of hospitalization with heart failure by more than 30%, where there was only a non-significant 7% relative risk reduction with GLP-1 receptor antagonist. GLP-1 receptor antagonists also reduced the broad kidney composite outcome. However, this effect was mainly driven by reduction macroalbuminuria. When excluding macroalbuminuria we found a non-significant relative risk reduction by 8% and this stands in contrast to a very robust relative risk reduction with SGLT-2 inhibitors of more than 45%. Dr Greg Hundley: Thomas, you mentioned there was a difference in benefit for those with existing cardiovascular disease versus no-known cardiovascular disease upfront. What do you think the reason for that might be, and then did you have the same number of patients in the non-cardiovascular disease group? Did you have enough events in that group? And finally, do you think we might need to follow that patient population a little bit longer in time, to see those events as they didn't have pre-existing cardiovascular disease? Dr Thomas Zelniker: These are fantastic points. I personally think it's biologically plausible that both drugs and receptors have the same benefit in both patient population to treatment effect may just require more time to become evident in patients with lower risk. You also mentioned a very good point, we had substantially more events in the patient cohort with ASCVD. Dr Greg Hundley: Very good. So John, we want to turn to you now. Can you help us put those results of this study in perspective? Can you put this into context for us with other published reports using these particular ages? Dr John McMurray: Certainly Greg, and I'd like to congratulate Thomas on what very important and very timely meta-analysis because, of course, what Thomas and his colleagues have done Greg, is to put all these studies together, to give us what meta-analysis does, which is much more power to look, for example, at components of composite outcomes, and we will in that way compare and contrast the differences and similarities between these two treatments. And as Thomas has mentioned, so interesting differences stand out but there are also some similarities that perhaps were not clear from the individual trials and I suppose the one that would perhaps stand out to me and might not have been realized by many of our readers, is myocardial infarction, that seems to be reduced to pretty much a similar extent by both GLP1 receptor antagonists and SGLT-2 inhibitors. I think there had perhaps been a view out there from the individual trials, that maybe GLP-1 receptor antagonists have more effect on atherosclerotic events and SGLT-2 inhibitors more effect on heart failure and renal events and to some extent that's true, both agents seem to reduce myocardial infarction to approximately the same extent. Which in itself is interesting, perhaps raises some mechanistic questions. I mean, the differences that stood out is stroke is reduced by GLP-1 receptor antagonists but not by SGLT-2 inhibitors and conversely heart failure which is the opposite, which is by SGLT-2 inhibitors, but not by GLP-1 receptor antagonists in this meta-analysis. So, I suppose, in summary what this tells us is that these drugs have complementary, perhaps additive cardiovascular benefits. Together, they potentially reduce the whole spectrum of the adverse cardiovascular events that occur in our patients with Type 2 diabetes, especially those who've got established cardiovascular disease. Dr Greg Hundley: And so, just a last question here, for both Thomas and John, if you're considering in your practice, you have a diabetic patient that's not on these, one of these agents, and they have existing cardiovascular disease, how do you go about considering the addition or the switch to this type of medicine, and what practices do you use to effect that change? Dr Thomas Zelniker: I guess, looking at patients, so we know that both drug classes have great benefits from MACE, right, but to people on antagonists having also reductions in stroke. So probably the associated risk is in the focus, I would probably rather go with the GLP-1 receptor antagonist. While looking at it from the heart failure perspective, or from the renal perspective, we see obviously bigger advantages attributed to inhibitors. Dr Greg Hundley: And John, how about you? Dr John McMurray: I would agree with Thomas' perspective, although I might add just a little caveat which is, of course, that the prevention of heart failure which is what, I think, the clear benefit of SGLT-2 inhibitors is, prevention of heart failure is different to the treatment of heart failure. So, patients at risk of heart failure sadly, an SGLT-2 inhibitor would make sense, but when it comes to patients with established heart failure event, of course we will get that answer because one of the great things about this recent incredible development of new therapies for diabetes, is that now there are now more studies underway including remarkable five trials in patients with different heart failure phenotypes, patients hospitalized, patients in the community, so we will learn a lot more about the use of these drugs, in particular cardiovascular populations. Dr Greg Hundley: Excellent. I want to thank both Thomas Zelniker from Brigham and Women's Hospital and John McMurray, guest editor from Glasgow, Scotland for helping us work through this just fantastic meta-analysis study pointing us in a new direction for utilizing medications to treat diabetes and those that we see every day, with cardiovascular disease. Objective:The aim of the study was to compare plasma carnitine profiles in fortified human milk (HM)-fed preterm infants or formula-fed preterm infants.Methods:Plasma acylcarnitine concentrations were determined in 20 formula-fed and 18 HM-fed preterm infants (birth weights between 1000 and 2200 g) by isotope dilution ESI MS/MS technique on study days 0, 14, and 28.Results:Concentrations of free carnitine (FC) and different acylcarnitines did not change during the 4 weeks of the study in infants fed HM. In contrast, in infants fed formula FC increased markedly (day 0: 29.989 16.646] mol/L, median interquartile range], day 14: 43.972 8.455], P

pizsamas, fec Hatalmas melegben totál lelassulva vettük fel az adást. Volt itt szó gamékről, egerekről meg nagy melegről illetve mindenről, ami eszünkbe jutott. Ketten voltunk Fecóval, mert Dani nyaral. Sokszor megkapjuk, hogy gyenge a minőséghez azért van, mert Fecónak kissé gázos a fülese. Ezen dolgozni fogunk mindenképp hamarosan lecseréli a srác és akkor más lesz a dolog. Türelmet és kitartást kérünk mindenkitől.
