Today, I am blessed to have here with me Dr. Yoshi Rahm. He is a board-certified osteopathic family physician with a separate board certification in Integrative and Holistic Medicine. Dr. Yoshi grew up in a small town in Northern California. His simple upbringing, which included living without electricity, TV, or phone, gave him an early appreciation for healthy living.   He attended U.C. San Diego, where he majored in Animal Physiology and Neuroscience and minored in Human Development. He ran Track and Cross-Country for UCSD before co-founding a running club. After graduating, Dr. Yoshi interned at a neuroscience laboratory in Germany and traveled throughout Europe before working at a nonprofit Biotech laboratory in San Diego. Dr. Yoshi attended Western University of Health Sciences, College of Osteopathic Medicine. As a medical student, he participated in many volunteer activities, including medical missions to Mexico and Ecuador. After medical school, he completed his Internship and Residency in Family Medicine at Glendale Adventist Medical Center. During his time there, he was elected Chief Resident. Upon graduation, he received the two highest distinctions: “Resident Teacher Award” as presented by the faculty and “Resident of the Year Award” as recognized by the staff. Dr. Yoshi then became an Attending Physician teaching at the same residency program. In this episode, Dr. Yoshi speaks about the inspiration behind his career in brain health. Today we have Dr. Benjamin Bikman on the Keto Kamp Podcast. Dr. Bikman is a biomedical scientist, professor, and expert on insulin resistance. Dr. Benjamin Bikman studies diabetes and obesity, and much of what he focuses on is a seemingly obscure condition called insulin resistance. But it's, in fact, not very obscure at all. Half of all adults in the U.S. have insulin resistance. That means you could have it too, and you don't know it. What is insulin resistance? Why did Dr. Benjamin Bikman devote his career to studying something like this? Well, insulin resistance is essentially two things. First of all, some cells in your body aren't responding very well to the hormone insulin. Second, you have more insulin than you did before, a condition called hyperinsulinemia. Insulin resistance is at the root of virtually every chronic disease. If we go through the chronic diseases that are killing us today, things like Type 2 Diabetes, heart disease, Alzheimer's Disease, and certain cancers, what they all have in common is insulin resistance. Experts should have discussions politely and respectfully and acknowledge that there is room for disagreement. There ought to be a degree of skepticism that we apply to every authority. We should challenge that authority to some degree. There are people with Ph.D. degrees who we shouldn't trust. Overall, we should always be on our own journey to understand what is accurate and true.  A Basic Understanding of Ketosis  Ketosis is the state where blood ketones are elevated, but pH is normal. Every human has some amount of ketones, even if it's below the detection level on commonly used devices. When you're in ketosis, you will have a detectable level of ketones on a moderately used device and yet pH is not changing. Ketones have the potential to make the body more acidic. When someone's in ketosis, the amount of hydrogens coming off the ketone is relatively modest. Within the blood, there are numerous buffering mechanisms. That way, your body will defend its pH range. Overall, ketones are fat-burning products; when insulin is low, the body will burn fat at a high rate.  Ketones Are Energetic Molecules; Plus, Ketones Are Signaling Molecules Ketones are energetic molecules roughly similar to the calories you get from glucose. Plus, ketones are signaling molecules. If a ketone is being used as a fuel, it will come to a cell. Then the ketone will get shuttled into the cell through an MCT (monocarboxylate transporter). When the mitochondria have a lot of pressure to be burning molecules, they get the signal that they need more mitochondria. Fatty acids can have the same effect. When a cell is burning a lot of fatty acids for fuel, it will start stimulating mitochondrial biogenesis.  Why The Laws of Thermodynamics Are Misleading Regarding Weight Loss People are producing ketones and breathing out ketones, and urinating them out. Remember, ketones have a caloric value. Calories matter, but don't worry about them. Let the calories take care of themselves. Invoking the laws of thermodynamics in nutrition and human metabolism was one of the greatest tragedies in the history of human-relevant science. It absolutely misled us and made us demonize fat and embrace carbohydrates. Hormones matter at least as much as calories because hormones tell the body's cells what to do with the available calories.  The Problems With Metabolic Syndrome and Insulin Resistance  Metabolic syndrome used to be called insulin resistance syndrome. Insulin resistance is the single most common problem in the US and abroad. Plus, insulin resistance is relevant to virtually every chronic disease. Covid has only made metabolic problems worse. Luckily, keto and intermittent fasting can help lower insulin. Remember, there is no insulin resistance without elevated insulin. Lowering insulin is a perfect way to help the body become more sensitive to it. If no nutrients are coming into the blood, insulin starts running out of nutrients in the blood that it can store away. So, insulin comes down when nutrients aren't coming into the blood. We're talking about tools. We were talking about skills. Had an interesting experience where I was building a box but it wasn't any kind of box. It was actually a strongman pinata for a friend of mine and so I hung up this box in a tree. And had a sledgehammer that he had to go find as part of this scavenger hunt and when a box a wood box is not wedged against the ground if it's just hanging. 00:42.54 mikebledsoe Um. 01:52.60 Max Shank You can hit that thing pretty much as hard as you want if it's wellmade with a sledgehammer and it's really hard because the forest dissipates throughout anyway, it was a fun ah thing for my friend's birthday shout out Anders. You're the man did a great scavenger hunt. But it made me realize how little I know about the principles of woodworking construction in general and it also made me start thinking about the tools required to do certain jobs and I was talking Mike I was talking with you just a second ago. About how I breezed right past using the simple tools. Well and I just in None fell swoop I bought a friend's full set of tools when I didn't know anything I had basically just purchased a home. And I had a drill from before but I didn't really have any any tools so I bought this whole ah batch of tools and half of them I didn't even know what they were a quarter of them I knew what they were but I didn't know when to use them and the other quarter. Actually have learned how to use and it makes me wish that I had gone back and just started with a drill and a hammer and a saw or a chisel or something like that and I think um, the same thing is true for all types of education. 04:01.32 mikebledsoe Oh. 05:06.28 Max Shank I Think we fly right through so quick to ah, some end result before really understanding the principles involved. Ah I found myself having to use a little math for these projects which was very exciting but what I realized is so. Ah, cool is when you build something the the math is important but all of the geometry and stuff is very incomplete because I remember in math Class. You usually don't account for the thickness of a material so there are all these. Minor adjustments that you have to make so I think ah yeah I would ah I am doing it now I'm going back in time and relearning the the simple principles of using a saw and a drill and fastening. Pieces of wood together and cutting different types of angles and slots and it's It's been.. It's been really fun. 06:36.70 mikebledsoe Yeah, yeah, that that journey really resonates with me you were you were talking about the tools and ah everything that was happening over the weekend and and how you all this shit. You don't need and I had a very similar experience. Yeah I grew up I grew up in Construction. My dad had his own construction company and um, you know I witnessed him use a lot of tools that I didn't use that I never use because they were advanced So like I'd seen them and then I got a toolbox when I was a kid. I was like in my mid teens and that's that was my Christmas gift the most disappointing Christmas gift I ever received was a tool empty Toolbox I could put tools in it I would greatly appreciate that now at some it. Um, you know I think he was He did a good job of instilling the things that are important even if it wasn't the thing that was gonna make me the happiest in the moment. That were like they're they're good for vehicles like they're everything you would need in a vehicle to is something were to go wrong. You know, just ah, a socket set and and a few wrenches. Nothing. 11:10.40 Max Shank E. 11:21.20 Max Shank Would you know what to do I I would not know what to do ah with the perfect speaking of skills if you gave me the perfect car tool set and a car that wasn't working forget it I would have no idea how to apply that tool set. 11:09.44 mikebledsoe Yeah, well I was fortunate because my dad in his all of his foresight. He made sure I had classic cars growing up and so he made me pay for them but he really encouraged me to get classic cars because they are. 12:11.74 Max Shank Oh. 11:47.54 mikebledsoe Much easier to work on you know? Yeah, so if you anything before 1973 or 4 Yeah, everything is a hundred percent mechanical so right yeah the the electronics go as far as like the radio. 12:34.48 Max Shank They're simpler machines. 12:49.78 Max Shank No No electronics. The dash or something. Yeah. 12:26.62 mikebledsoe The yeah the dash but like you don't you don't have fuel injection. There's nothing happening in the engine that's run electronically. Um, yeah, there's a lot of similarities. They wanted vehicles to become much more fuel efficient because the cost of oil was going up because of some opec shit that was going on so that's where fuel injectors came in now before that was mechanically driven fuel injection through Carburetors. So The the cool thing is is. My dad I don't know if he did this on purpose or not but with ah with a older vehicle. It is Simpler So It's easier to learn how to work on the other thing is is I'm confident now because I I rebuilt everything on cars I'm confident that if you gave me a carburet like a car that had a carburetor I could figure it out. 16:24.78 Max Shank Qua. 15:43.32 mikebledsoe And so um I'm not saying I would like figure it out like Macgyver in 10 seconds but I could I would eventually figure it out. It's been a long time but you start throwing all these electronics in and yeah, now you have to plug a computer into it just to know what's going on. It's a bit of an ego check too to go back and relearn the simple mechanics of things right? How how exact like how fluid passes through. Ah, educated in it. But you realize just what a tremendous amount of the world Around. You is only functioning because all of these parts are working exactly as they should and as soon as they would stop. You. At least I would at the time have no clue what's wrong or even how to ah track. Ah what might be wrong is wild. 18:28.24 mikebledsoe Yeah I I find myself I want to get back to the tools but I like this track we're on because it makes me think about None thing I notice out in the world is um and again I think a lot of this has to do with. Ah. What a good job. My dad did raising me is I understand I didn't know that I understood how things worked better than most people and what I witness in the world now is people I watched them doing something I'm like don't do it like that you're going to fuck that thing up. 20:21.78 Max Shank And it seems obvious to you it. It seems like I knew this when I was 8 But that's how I would be if I went to a farm the 10 year old kid would watch me doing something be like what are you? an idiot. My dad did a good job in teaching me like end to end how it worked and why it worked like he was very big on the principles of like. 20:59.28 Max Shank Dumb ass. 20:50.50 mikebledsoe Yeah, when you do this at least this this is the end result we're looking for and so I think a lot of times people are so focused on the end result that they don't They don't take the time to understand the process of how it gets there and they don't appreciate it either. Ah technology because I understand enough about the ins and outs of how this thing works that um that it's I appreciate more and I can fix it quickly if I needed if needed. There's no such thing as a thing because it's. Only describable in relation to other things. So if you have all of these end results and you have no idea how to track the energy flow of what's going On. It's like you're just in an ocean of darkness where you have no Clue. Ah, my most recent experience of this is um, for 14 years of entrepreneurship I never did my books I always had a bookkeeper I I outsourced it heavily and one of the reasons. 24:06.12 Max Shank By these forces. 24:35.52 Max Shank Me too still. I stopped doing that recently one of the reasons I outsourced it heavily is because my none business partner ever what he had a ah master's in finance so he didn't even. He didn't even share I didn't even bother to look and he didn't share the profit loss the balance sheet like I really had no idea he would just tell me a thing. The drawback was was when I no longer had that person and I found another I found another Cfo who also did an amazing job. It required me to understand learn things and I felt like and and this was accurate I had to go back and learn some really fundamental things about bookkeeping about accounting about ah cash flow cause really it comes down to cash flow and Cash Flow. Management and so I mean if you look at Accounting. It's I mean it's a big scam because the whole point of accounting is so you know how much to pay in taxes at the end of the day. But if you that's what that's what modern day accounting is yeah the majority of. 28:06.26 Max Shank True True true. Yeah. 27:40.34 mikebledsoe You know I wouldn't have to spend thousands of dollars a year on accounting if cut. Yeah, exactly yeah go. They're like go cut a tree branch so I can beat you with it and that's what accounting is like. 28:11.22 mikebledsoe Yeah, go go pick out. Go pick out your switch. Yeah, ah so ah, so the way I look at it now is look I gotta count I handle the tax ship but my job is cash flow management and the way I see it. 29:14.22 Max Shank That's the. 29:30.28 Max Shank Which is energy management right? which comes back to relationships you got to be able to track the energy flow and the relationship from 1 point to the next just the same way you got to be able to track the gasoline from the fuel tank. 28:51.16 mikebledsoe Is if energy management and if I. 29:11.92 mikebledsoe Yeah, yeah. 30:09.76 Max Shank All the way to the exhaust can you track that gasoline all the way through you're probably going to have a decent idea of how this system is working together and actually cash is really good symbol for fuel or vice versa actually like fuel and cash. 29:59.16 mikebledsoe I Think it's a great. Ah. 30:49.80 Max Shank And a motor is like ah you you could also think of it like a cell instead of something man-made. But actually you know an organization can be looked at as a motor or as a cell or a creature or as an ecosystem I Think the motor analogy is a little bit more. Ah, tool-based and cold and the cell analogy or like a tree or something like that. But you're still tracking the flow of energy just the same Way. You could track the flow of gasoline through a car you could track the flow of Photon Photosynthesis. Ah, through a tree and you can understand the water and how the carbon dioxide from the air is converted into sugar through this process like you can understand the energy flow and transformation and cash flow is kind of the same thing you're understanding. How energy is transformed through that organism or organization which is your business. 32:10.50 mikebledsoe Yeah, and and um, you're spot on I mean I like the the classic car mechanical analogy because it is so much simpler and so that's a great place to start if you start with the complexity of a cell. You know, now there's ah it exactly? Yeah, not. That's that's advanced I mean I think that is a great analogy but in the beginning if you don't understand the cash flow which is. 33:38.98 Max Shank Our sales team is the Mitochondria and it's like wait wait. Can you look at your bash your balance sheet and your cash flow reporting like if you can't look at those and those tell you a story that make that they go Oh I know now how to make it doesn't It won't inform you on how to make all the decisions in your business. It's not gonna. 34:44.80 Max Shank Right. 34:06.34 mikebledsoe Gonna tell you who to hire. But it's gonna tell you what position to hire for right? And so um and people at the highest levels are making most of the decisions based on the numbers the higher up you get in your in your thinking and your business is that now here's the thing about I've learned about money. Is that I am um I'm in charge of the cash flow of my own cash flow management and the better I can manage what I currently have the more I'll be able to manage in the future If You think that. Adding more Cash. Ah I think most people walk around and I used to I used to walk around thinking like this if I just had more money then I'd be able to save and invest and I'd be I'd be a better steward of the money I had if there were more pete. They don't understand how it flows because they're afraid to look at it. There's an emotional response to being present with the money. And it's basically 1 plus 1 makes 3 because it's you and I and now there's this ah Harmony or resonance between us and it could be more dissonant or it could be more resonant and harmonious. But it's this third, it's this. Third thing that is the relationship between those entities and if you say something I like or you just say nice things about me. Um, how fucking stupid am I that I can't make a square I can't do it I can draw a square but the idea of manufacture. So what I ended up doing is I took a bunch of tiny planks and I learned about the relationship of pieces of wood. By thickness width and length and I made these boards that have a one ah for thickness to None to width and None to length and I learned how they fit together not thinking of the dimensions as um. This week we're talking about why you are exhausted - Three reasons why you are tired, fatigued, knackered, whatever word that you want to use. I'll share more about how energy is actually made in the body and why it is that you feel exhausted and therefore what you can do about it. This isn't just some fluffy ‘Ooh, drink a coffee' We are looking at what the system of energy production is and what can go wrong with it. When you know this, there's so much that you can do about your energy levels. If you had more energy, maybe you would start growing some vegetables, maybe you'd get that second dog that you want, maybe you would go back and retrain for a totally different career, maybe you would start advocating for certain things, maybe you'd volunteer, maybe you would do up your house.  Energy is such a foundational thing in our lives, yet for some reason, most of us are willing to accept crap energy. We think of energy as like this outward thing like ‘I hope I get energy' but energy is a system in your body, in your biology that you create. When you know the system, you can create energy. - It's not a limitless amount of energy, but you can really optimise the creation. There is a reason why you are tired, and there's something you can do about it. What is energy? How do we make it? To get more energy we need to understand the system of energy. Energy is cultivated, it's created and it is the result of a very finely honed system within the body.  To put it simply, we create energy in most of the cells in our bodies, apart from our red blood cells. You are made up of billions of cells and within each of these cells is something called mitochondria. Mitochondria is like your power plant - where you create energy.  When we're talking about energy within the body, we're talking about something called ATP (Adenosine triphosphate) There is a whole set of reactions that take place all the time to produce this thing that is energy. In order to make ATP, it actually costs us ATP - The process of making energy means that you need to spend some energy. So, if you are exhausted, it means one or both of the following: You are struggling to make optimal ATP - I say optimal ATP because if you are alive you are obviously making ATP. You are wasting or overspending the energy you produce so you struggling to create more ATP. I find with clients, it's probably a mix of both but for some people it is more one than the other, depending on their health, history and symptoms. Maybe you have brilliant ATP - Production is optimal, but you are spending left, right and centre. It's a bit like money. You're really struggling to make the currency, so you're not even necessarily wasting it, but you've just not got it. So, energy is a bit like money - I think about it like a currency. - It's something that we make. It's not a limitless resource and that your body just has tons of it. It costs you to run it. It costs energy to make money just like it often costs you money to get to work. If you're exhausted all the time, it's kind of like the equivalent of you being in debt. (I call it energetic debt) You don't have enough to pay for what you're doing. You don't have enough energy to pay for what you're doing. Now, this is metaphorical. It's not necessarily the exact biochemistry, but it's a really helpful way of thinking about it. What we need to do is manage our energy, just like we have to manage our money. You need to manage your energy because it isn't limitless. We need to prioritise where our energy goes. This is a part of the conversation but for some of us, there's also some real, biology wonkiness happening that we need to address as well. Reason 1: Struggling to produce energy So, if we think about that part, your cell is making ATP that can be affected by mycotoxins or mould, things like heavy metals inflammation, certain medications etc, fertility issues, fibromyalgia, cognitive decline, etc. Mitochondrial dysfunction can look like lots of different things. What we really need to do is look at the actual energy production cycle. Reason 2: Overspending: Now, another reason why you could be exhausted is because you are overspending. One of the ways that I see people overspending is by what I call the mental load. They are carrying too many things, juggling too many things - There is too much stuff going on in their brain. It doesn't necessarily mean doing everything. It's about having too much going on in your head all at once. This is especially the case with women, because women tend to do most of the unseen, unpaid, let's be honest, unappreciated work in our society still. Fortunately it's changing, but it's generally the case. This can be huge - and that doesn't just mean you doing the stuff.  Maybe you're in a partnership and your partner is really proactive but you're still the delegator. You are managing your household potentially and if that is a massive load to carry. Another way to look at this is that you really struggle to switch your brain off. You might be thinking that you aren't doing too much, but you are struggling to switch your brain off in the evening. Ask yourself ‘what is my mental load? Am I carrying everything? Where can I get help? Where can I offload things?' Being able to switch into a parasympathetic state, which can be really hard if you're somebody that just struggles to switch off and you're always prepared to be on the go and then just fall asleep. That is not healthy for your biology, for your energy production, for your brain. It will lead to burnout or you will just slowly get more exhausted. So, being able to switch off is really important. Reason 3: Inflammation Leaks: When I say inflammation leaks, I'm talking about food intolerances, mould, heavy metals, high sugar diets, or just a bad diet for your body, poor sleep quality, chronic stress or too much/too little fat for your body's ideal nutrient depletions. There might be: Auto-immune conditions going on. Immune system dysregulation happens when there's poor digestion. Skin issues because of poor detox pathways. Drinking too much alcohol Taking a lot of medications that just have known side effects - that doesn't mean you have to change that - It's just knowing and being aware that that's maybe a plug or a hole in your bucket. Being chronically stressed. Poor sleep. Being slightly underweight. He lost about thirty-five pounds in the span of a month; he couldn't eat because the back of his throat was so swollen.  Ari was left with chronic fatigue; it shifted his entire life.  So, Ari started to focus on the world of energy optimization; he became obsessed with energy science. Conventional medical doctors had no information to offer Ari.  After doing research, Ari found that science actually does not support the idea that adrenal fatigue is a significant cause of chronic fatigue. Well, Ari realized that no one truly understands what is causing fatigue.  [08:45] The Key To Overcoming Fatigue Is Growing More Mitochondria  Finding ways to grow more mitochondria and strengthen your mitochondria is a massive key to the puzzle of overcoming fatigue.  The mitochondria are the powerhouse of the cell.  We are taught to think of mitochondria as these mindless energy generators. Something you may not know is the mitochondria have a dual role beyond energy production, which is just as important as their role in energy production.  The mitochondria also help with energy regulation.  Your energy levels are a function of the degree to which your mitochondria decide if you're safe or if you're under attack.  [17:50] Hormetic Stressors Will Make Your Mitochondria Bigger and Stronger  Hormetic stress is how a transient metabolic stressor works to stimulate beneficial adaptations that ultimately increase resilience and resistance in the face of a broad range of other stressors.  Basically, what doesn't kill you makes you stronger.  By exposing yourself to moderate stressors within your body's capacity, you make your body more robust and more resistant.  Hormetic stressors are vital for challenging our mitochondria and stimulating them in a way that makes them bigger and stronger. [27:30] Is Your Body Under Attack? Your Brain Will Create Sickness Behavior When cells are under attack, they shift all their resources to defense mode.  Cells will go into defense mode to protect themselves. They also go into defense mode as an evolutionary adaptive mechanism to help the body's energy and resources be redirected towards defending against the threat. When we have lots of immune activation and elevated levels of inflammatory cytokines, the brain responds by also creating sickness behavior, which includes fatigue. When your brain creates sickness behavior, it protects you from over-expending energy and depleting resources.  [42:40] The Importance of Melatonin: What You Need To Know You get 50 to 70 percent suppression of melatonin secretion by the brain in the evenings due to simply being in a typical home under standard indoor home lighting.  Melatonin is a vital hormone for protecting your mitochondria and protecting your brain's neurons.  If you're chronically suppressing melatonin at the cellular level, you will have chronic melatonin deficiency.  Sadly, melatonin doesn't get the respect that it deserves.  [1:05:15] Ari Is Hypersensitive To Melatonin Supplements - Are You?  Ari is hypersensitive to melatonin if he takes it in supplement form. After polling his audience, about 25% of people are sensitive to melatonin.  People who take melatonin for a significant time will sleep poorly after coming off of it.
