Body Banking: Why It's Better to Keep It w/ Dr. Douglas Steinbrech

Play Episode Listen Later May 20, 2025 42:18 Transcription Available

Dr. Douglas Steinbrech joins Dr. Bass to discuss his game-changing concept of body banking. Instead of tossing out fat after liposuction, body banking preserves it and strategically reinjects it into areas like the chest, shoulders, arms, or buttocks to enhance your shape and keep your body in balance. It's not just about taking fat away. It's about sculpting, reshaping, and maximizing what you already have. This technique can also reduce the risk of that frustrating post-lipo “rebound fat,” where your body tries to make up for what was lost by storing fat in all the wrong places. Hear why fat transfer to the breasts doesn't always go as planned, and why fat tends to “stick” better in some areas than others. Drs. Bass and Steinbrech also discuss how body banking may help prevent visceral fat buildup and related health problems, the role age plays in fat graft success, and options for people who don't have much fat to begin with but still want a sculpted, athletic look. About Dr. Douglas Steinbrech Dr. Douglas Steinbrech specializes in minimally invasive aesthetics, blending this approach into both surgical and non-surgical techniques. Named one of America's Top Plastic Surgeons by the Consumers' Research Council of America, he's known as the go-to surgeon for men. Drs. Bass and Edinger break down what a facelift can and can't do. Facelifts are great for tightening loose skin, lifting jowls, and restoring volume in the face and neck—helping you look more refreshed and youthful. But they don't address everything. Wrinkles, sun damage, and changes around the eyes and brows usually need other treatments. There's a big difference between how facelifts are described online and how surgeons talk about them. The doctors break down that language gap so you know what those buzzwords actually mean. Bottom line: A facelift is the gold standard for sagging skin, but knowing its limits is key to getting natural-looking results. Find out who's a good candidate for a facelift, how long the results typically last, and whether combining it with other procedures might give you the outcome you're really looking for. When it comes to beauty, some people approach it with excitement and a flurry of activity, while others take a calm, methodical approach. When it comes to plastic surgery, boring is actually better. Dr. Bass explains his concept of “boring beauty”—why slow, thoughtful decision-making leads to the best results. Rushing into procedures may feel exciting, but taking your time ensures you make the right choices. Of course, achieving your beauty goals should be fun and fulfilling, but on the execution side, a steady, careful approach by your plastic surgeon is key to success. In preparation for bathing suit season, we present a discussion of the current status of hair removal treatments.  Traditional hair removal methods like shaving, waxing, sugaring, and threading work temporarily, but medical-grade, in-office treatments are more powerful. Electrolysis is the gold standard—it uses a tiny needle and an electric current to kill the hair follicle. It's great for small areas, but not practical for large ones. Because the neck tends to reveal your age more clearly—and age faster—than your face, it is a key area to address when talking about facial aging. Some people catch the early signs, like skin texture changes, and horizontal lines across the neck. For these concerns, skin treatments, like chemical peels, energy treatments for skin and dermal stimulating filler treatments can help.  For minimal sagging or excess fat is approached with liposuction, or muscle tightening through small incisions. As skin laxity worsens, energy-based lifting treatments come into play. But once you reach the point where there's noticeable sagging or that “turkey neck” look, surgery is needed for real improvement. The neck is just as visible as the face, so it's crucial to include it in any rejuvenation plan. Don't focus only on the face and neglect the neck—it's on display every day, too. Stackable Beauty Treatments w/ Dr. Julius Few

In the past, looking younger usually meant surgery. But plastic surgeon Dr. Julius Few believes that dramatic results are possible with less invasive methods. He uses a “treatment ladder” to combine nonsurgical procedures to improve skin texture, volume loss, and sagging skin all at once. Stacking nonsurgical treatments, namely giving multiple treatments in a single sitting, used to be seen as too risky. Now, doctors see that combining them can often be done safely, delivering more complete rejuvenation or occasionally  better outcomes than doing each treatment separately. This approach is gaining popularity for those who are not yet at a stage of aging that mandates surgical correction in order to obtain significant results.    Dr. Few joins Dr. Bass to discuss his inspiration for the treatment ladder, which treatments are typically included, and the advantages of this approach. About Dr. Julius Few Dr. Julius Few is a Chicago plastic surgeon with a very busy and successful practice, The Few Institute. How a plastic surgeon treats a patient's mind is just as important as how they treat their physical appearance. Beverly Hills facial plastic surgeon Dr. Jonathan Sykes joins Dr. Bass to discuss how both plastic surgeons and patients can succeed at seeing eye-to-eye with each other before, during, and after a procedure. Sometimes there's a mismatch between what patients explain they want to look like and what plastic surgeons envision based on this description. To prevent an unhappy patient, it's best to spot align early on by listening intently and communicating thoroughly. Listening is key for plastic surgeons both before and after a procedure. This helps the surgeon know how best to prepare patients psychologically to go through the procedure and recovery.  As a patient, you want to pick a surgeon not just for their technical expertise, but also for their listening abilities. Drs. Bass and Sykes share their insights on how plastic surgeons should approach different types of patients, how to work towards satisfying results, and why it's a bad idea to try to chase unrealistic expectations. For people in their 70's, Dr. Bass shares the surgical and non-surgical treatments you may want to consider, highlighting how beauty plans have changed in the last few decades. Many people do a major facial “reset” during their late 50's or early 60's. By the 70's, it may be the last chance to do something major to improve facial appearance. Sometimes this is a first facelift and other times a secondary or repeat facelift if enough aging has taken place since the first procedure.  Surgery may not be right for everyone in their 70's depending on health, and current aesthetic needs.  Surgical and non-surgical approaches can yield big results, but there may be some limitations due to loss of skin elasticity. Maintenance, prevention and restoration don't stop at this stage. Non-surgical treatments can still make a difference in your 70's, but you'll need to invest more time and effort than you did in your 50's or 60's. In this sixth episode of our mini series “Decades of Face,” find out the top cosmetic surgeries, either for the first time or as a second time around, for people in their 70's. The eyebrow plays a vital role in how we express our emotions, so it's no surprise that many people seek solutions when their brows start to droop, flatten, and lose their shape. The horror stories you may have heard about brow lift are nothing to worry about today, as techniques have greatly improved over the years. Longevity is difficult when it comes to the brows as you're constantly battling gravity. Luckily today, injectables and energy treatments postpone the need for surgical intervention and can in turn help maintain surgical results much longer. Brow lift is a highly customizable treatment. Brow position and shape for men and women are much different, so specific techniques are used to preserve gender identity. Depending on how low the hooding is, a brow lift may be done in combination with a blepharoplasty.  Facial plastic surgeon Dr. Jason Bloom returns to the podcast with Dr. Bass to discuss how brow lift surgery has improved over the years, the most common techniques they use today, and how to decide if a brow lift, blepharoplasty, or both is right for you.   About Jason Bloom, MD Located in Bryn Mawr, Pennsylvania, Dr. Jason Bloom is a double board certified facial plastic and reconstructive surgeon.  He is an Adjunct Assistant Professor of Otorhinolaryngology – Head & Neck Surgery at the University of Pennsylvania and Clinical Assistant Professor (Adjunct) of Dermatology at the Temple University School of Medicine. Dr. Aston often pairs these bipolar radiofrequency treatments with face and neck lifts to address what surgery alone cannot, such as wrinkles or localized areas of laxity. While FaceTite and AccuTite are in a large class of devices using radiofrequency energy to heat and remodel tissue, they stand out because they deliver far more energy to the underlying tissue and can be more precisely customized to each patient's needs. A wide range of people can benefit from these treatments, from young patients with some skin laxity who aren't ready for surgery to post-facelift patients who want to touch up their results.  Patients appear visibly tighter and more contoured almost immediately after FaceTite or AccuTite, but the results don't stop there. Improvements in tightness and overall skin quality continue over the following months as new collagen continues to build. As our bodies age, our cells are aging with us, and the messages they produce are decreasing. While growth factors have been used to take the message of youthful cells and use them to stimulate the skin, exosomes offer a more complete, natural, and protected version of those messages. Scientific researcher Dr. Jordan Plews, CEO and co-founder of ELEVAI Labs, joins Dr. Bass to discuss what's happening in the space of exosomes, stem cells, and regenerative medicine as a whole. Through experiments with stem cells, Dr. Plews and his colleagues discovered that the cells provided some benefit, but they weren't sticking around or fixing problems such as wounds. This led to the discovery of exosomes, nano-packages that the body uses to send signals to cells. When searching for the best exosomes to separate, Dr. Plews and his team found that by taking exosomes from age zero stem cells, they can safely be put back into the body to “remind it how to be young again.” Drawing on his two decades of research in the study of stem cells and regenerative medicine, Dr. Plews explains what exosomes are, how they were discovered, the potential they hold, and how we may be using them in medicine going forward. About Dr. Jordan Plews Dr. Jordan Plews is the co-founder and CEO of ELEVAI Labs, Inc. He has dedicated nearly 20 years to the study of stem cells and regenerative medicine. Following many years investigating the use of various types of stem cells on injury and degenerative diseases, he has gone on to build and lead teams, setting up stem cell culture labs and developing stem cell-based regenerative medicine solutions before pivoting into aesthetics. Dr. Gregory Mueller is the innovator of MyEllevate, a minimally invasive technology for lifting skin and muscle in the neck. He joins Dr. Bass In this second episode of our mini series “Lifting Without Cutting.”  Using a light-guided delivery to lift the skin and muscle together as a composite, MyEllevate offers a research-based, data-driven approach with obvious improvement, minimal recovery time, and long-lasting durability. Early in his career, Dr. Mueller recognized many people needed attention in the neck area, but weren't ready for a big face and neck lift. He found a way to lift the underlying ligaments without making any big incisions or moving the muscles and shares the full story of how he developed MyEllevate. MyEllevate can not only tighten up the neck, but also sharpen the chin and jawline. It can also be used as a tool to refresh face and neck lift results by retightening the neck and smoothing vertical muscle bands. Hear directly from the inventor and find out who the ideal candidate is for this unique and much-needed treatment. About Dr. Gregory Mueller Dr. Gregory Mueller is a board-certified plastic surgeon and innovator who has practiced in Beverly Hills, California since 1998. Dr. Mueller invented the medical device called the ICLED Surgical System used to construct the suture matrix called MyEllevate. The use of Ozempic/Wegovy and similar medications such as Mounjaro for weight management is rapidly evolving. As they continue to be studied further and receive FDA approvals, including the approval of Zepbound this month, the question becomes whether they can be a meaningful substitute for bariatric surgery. Ozempic has been used to treat type 2 diabetes since 2017. When used for weight loss, semaglutide is used at higher doses and marketed as Wegovy.  Newer medications such as Mounjaro (tirzepatide) have entered the market for and will be marketed under the name Zepbound for  the sole purpose of weight loss. When combined with healthy lifestyle choices, people can lose 15-20% of body weight over a six month course on semaglutide and up to 25% on tirzepatide. Dr. Bass answers everyone's biggest questions about the use of these medications for weight loss, including: Does insurance help cover the cost? Who is a candidate? Skin Science: Fresh Again w/ Dr. Julius Few

Chicago plastic surgeon Dr. Julius Few joins Dr. Bass to discuss why skin care is not a one size fits all industry and debunk common myths surrounding skin care. Good skin care is a necessary adjunct to plastic surgery and beauty as a whole. There are thousands of product options out there, but it's about finding a routine that's easy to follow and works for you.  According to Dr. Few, the secret to great skin is good cleansing (but not over-cleansing), good hydration with protection of that hydration, and something that facilitates cell turnover and elimination—not stripping, but enhancing that process. Back in the 1970's and 1980's, products and treatments were universally used on people of all skin colors and types, sometimes doing more harm than good. Choosing the wrong topical agent for skin care can lead to significant inflammation, or in the worst case scenario, a reactive dermatitis. Today, we have options developed for a wide range of skin types.  With the goal of developing medical grade options for people with hyperpigmentation, dyspigmentation, and dyschromia that does not require physician oversight, Dr. Few created a skin care line consisting of topical ingredients in a melanocyte friendly solution. Drs. Bass and Few discuss: Why you should consider adjusting your skin care routine based on the season Why you should avoid washing your face with cleanser too often if you have dry skin How to adjust your routine as your skin ages When the best time of day is to apply restorative treatments How long it takes to judge whether a skin care regimen works for you   About Dr. Julius Few Dr. Julius Few is a Chicago plastic surgeon with a very busy and successful practice, The Few Institute. Facelift surgery has come a long way since the 1980's, amplifying the degree of correction and preserving a natural look. Dr. Sherrell Aston, a leader in facelifting, joins Dr. Bass to discuss the evolution of facelift techniques and how they are customized to each patient. In the last 40 years, the focus has moved from lifting the skin to altering the underlying foundation, removing excess skin, and redraping over a changed foundation in the face, cheeks, neck, and jawline.  Today, the facelift is often combined with other treatments for well-rounded, dramatic, long-lasting results. It can be complemented with fat transfer into areas of the face with lost volume, or radio frequency technology such as FaceTite or AccuTite or Morpheus8 to improve the quality of skin. While the facelift/necklift is the gold standard for facial rejuvenation, there's not a single best technique for how that procedure is performed.  The approach is customized to every patient not cookie cutter. Hear about current controversies in facelift technique from the experts. Learn more about the various facelift techniques Drs. Bass and Aston use regularly, in what circumstances they find a mini facelift or neck lift alone to deliver meaningful results, and why they advise against getting continuous filler treatments when it's time for surgery.   About Dr. Sherrell Aston Dr. Sherrell Aston is a professor of plastic surgery at New York University, a past president of the American Society for Aesthetic Plastic Surgery, and was the chair of plastic surgery at Manhattan Eye, Ear and Throat Hospital for 23 years. Lifting Without Cutting #1: The Biology of Non-surgical Lifting

Lifting the face and neck without surgery is the holy grail of aesthetic medicine. While there's no meaningful substitute currently available that can give the degree of laxity correction in the face, neck and jawline as the facelift, there are good non-surgical skin lifting and tightening treatments for mild to moderate signs of aging. Non-surgical skin treatments started with various smoothing treatments such as chemical peels. As doctors noticed the more aggressive use of these treatments created some tightening as well, the idea of developing a standalone skin tightening treatment in and of itself piqued their interest. In the 1990's, there were attempts to use lasers and other infrared light sources to tighten skin, but it wasn't until the early 2000's with the introduction of Thermage and fractional laser treatments that the modern era of non-invasive skin treatments emerged. Through trial and error, doctors discovered that it's safer and more effective to treat with these energy devices over time rather than do one really aggressive treatment. More recently, the FDA has cleared ultrasound treatments Ultherapy and Sofwave for lifting and tightening the skin. Today, energy can be applied beneath the skin, too, with treatments like FaceTite, which delivers radiofrequency energy. About Dr. Lawrence Bass Innovator. Industry veteran. Although relatively small, the eyes are the first area of the face to show changes, and have a big impact on one's appearance.  When considering which procedure to have for the eyes, first identify your specific concerns: is it volume loss, crepiness underneath the eyes, crow's feet, low brow position, or fat bulging causing bags beneath the eyes? There's no meaningful substitute for eyelid surgery, but non-surgical treatments are useful for certain concerns around the eyes. Botox and fillers, for example, can raise the brow and treat crow's feet and frown lines, and chemical peels can treat crepiness around the eyes. But when you notice the upper eyelid skin hooding, a lower brow position, or bulging fat pads, it's time for a surgical approach. Dermatologist Dr. Vic Ross characterizes this approach as being like “napalm in the forest,”   Then, Harvard researcher and dermatologist Dr. Rox Anderson recognized these treatments would be much more selective if they were delivered in short pulses and tailored to each condition.  From there, researchers in laser medicine began investigating and experimenting, starting with pulsed dye lasers for redness and frequencies targeting red and brown spots.  A whole host of laser hair removal devices for different skin types were developed and improved upon over time to be more effective and more controlled.  Today we also have devices for birthmarks, discoloration, acne, sun damage, lupus lesions, skin tightening and body contouring, and the possibilities are infinite.  Dermatologist Dr. Vic Ross and Dr. Bass discuss the history and research behind today's laser devices, an overview of laser-tissue interaction, and what conditions each type of device can treat and correct.   About Dr. Vic Ross Dermatologist Dr. Vic Ross specializes in laser treatments and is an active researcher on skin rejuvenation approaches. With any surgery, it's normal to be concerned about the scars you'll be left with and whether they heal properly. In this episode, Dr. Bass covers the different types of scars and what to do if you find yourself with an unpredictable scar during your recovery.  Two conditions which sometimes occur with scar healing are hypertrophic scars and keloid scars. A hypertrophic scar is red and thick, caused by inflammation combined with your wound healing biology and how your skin behaves. Keloid scars are the result of specific genes, growing thick, raised, and wider than the original incision or wound. With scar issues of redness and thickening, the scar will be targeted through gel bandages, compression therapy, or lasers and energy based devices. Fractional lasers have revolutionized the treatment of scars, both traumatic and surgical.  For widened scars, surgical revision is typically needed, trimming out the old scar and resewing the skin edges no sooner than 6 months after surgery.  Good scars start with good surgeries. Be cautious that you're teaming up with a surgeon who knows how to place incisions well and treat problem scars if you're one of the rare people whose skin reacts differently. This versatility allows them to be used in plastic surgery, where benefits include improving blood supply, making new blood supply, reducing inflammation, and repairing tissue, to name a few. In the future, stem cells might be used for such applications as rehabilitating the tissue during breast reconstruction before putting a breast implant in, healing or reversing radiated tissue in head and neck cancer patients, or in combination with laser treatments for the best possible skin resurfacing results.  Dr. Cohen and Dr. Bass share how stem cells can be incorporated today to get ideal results and how they might help reduce the complexity of procedures and increase longevity of results in the future.   About Steve Cohen, MD Dr. Steve Cohen is a San Diego-based and internationally recognized plastic surgeon, inventor, author, artist, and founder of FACES+.
