Reaching your 80's is a special time in life. This decade is different for different folks: some are preoccupied with health issues and spend most of their time at home, while others are still very healthy, active, and social. When you think about breast reconstruction do you only think about breast implants? If so, it is because many people are still choosing this option for reconstructing their breast after mastectomy. However, there are options. Using your own tissue is an option and the tissue, along with a blood source, can come from various parts of the body. Fat grafting to shape and form the breast is also an option many ask about but not all know about. This is our topic on this episode of the DiepCJourney podcast. My guest, Dr. Eric Wimmers, is a board-certified plastic and reconstructive surgeon practicing in Hopewell, New Jersey at the Plastic Surgery Center. He is part of the Institute for Advanced Reconstruction (IFAR) network of surgeons across the U.S. Dr. Wimmers has unique training in microsurgery and immunology. Although he operates on the entire body, his special interests are breast reconstruction, abdominal wall reconstruction, and peripheral nerve repair. His pathway to his current practice as a microsurgeon developed over time. Dr. Wimmers shares his personal journey with us from beginning as an emergency room physician to a surgeon. Once he had exposure to the scope of plastic surgery, restoring form and function to the human body affected by trauma and disease he never looked back. Dr. Wimmers shares the value of his surgical practice being part of the IFAR network. He points out that they are grouping up with other like-minded microsurgeons across the U.S. to tackle problems and issues patients face when they have been told there is no hope or fix. Combined resources and talent within the IFAR network are providing greater access to care for patients. We continue our discussion with an explanation of the types of breast reconstruction available. Dr. Wimmers utilizes the tummy tissue most often to perform DIEP flap breast reconstruction. However, he points out not all patients have the excess tissue in their own body in the right places to do autologous reconstruction, meaning using the patients own tissue. He does get inquiries about fat grafting only. Patients are interested in it for a variety of reasons. It might be that they have already had a tummy tuck. It may be that their fat distribution is in other places other than the abdomen. Most of his patients requesting fat grafting only tend to be on the smaller side and prefer a smaller size breast. He explains the fat grafting only procedure in detail in our interview. Dr. Wimmers tells us fat grafting only is a series of procedures done over time. Fat grafting is a very technique driven procedure. Achieving symmetry and understanding fat grafting in breast reconstruction are topics patients frequently ask about. I reached out to an expert in the field. My guest on this episode of the DiepCJourney podcast is Dr. Tim Matatov of Southwest Breast and Aesthetics in Phoenix, Arizona. Dr. Matatov is a board-certified microsurgeon who performs procedures such as implant breast reconstruction, several autologous reconstruction procedures, autologous meaning using the patient's own tissue to reconstruct the breast, nipple reconstruction, and a variety of other breast reconstruction procedures. He works with a team of two other surgeons, Dr. Joseph Zakhary and Dr. Matthew J. Mino. We discuss mastopexy, or breast lift, in the setting of breast reconstruction. Achieving symmetry in the contralateral breast for a unilateral DIEP flap is important to the patient. It serves a functional purpose as well when wearing wardrobe items like a bra. He points out the psychological benefits to the patient since this becomes a quality of life and self-image concern for women. Dr. Matatov outlines the various names and details of mastopexy procedures and why one might be chosen over the other for optimal patient outcomes. Dr. Matatov shares how important it is to have an open discussion with patients about managing reasonable expectations. Dr. Matatov continues the discussion with fat grafting. He points out that a lot of the techniques and percentages, and how it all works is for a majority of cases, theoretical. He states there is not any hard proof that one technique or device works better than the other. There are studies that are peer-reviewed but not randomized and controlled to a degree which backs up his statement on fat grafting methods. However, Dr. Matatov feels it is a very effective part of the breast reconstruction process. He points out again that managing patients' expectations is very important in this step of breast reconstruction. It often takes more than one fat grafting procedure. Changing breast implants for breast fat grafting is possible with a fat transfer surgery. This all-natural breast enhancement surgery increases the breasts by about one cup size without implants. My Results with Fat Transfer = Fat Necrosis & Fibrocystic Breasts “There are a few disadvantages of breast augmentation fat grafting, such as the survival rate of the breasts is not stable, patients are prone to breast cancer, breast calcification, etc. Despite being a disadvantage, the survival rate of breast can actually be increased through the technology of stem cells. It must be known that the risk of getting breast cancer does not have anything to do with autologous fat grafting. This can be scientifically proven. Hence, the other major problems that patients should keep an eye on is the post-operation calcification and the formation of lumps. According to medical journals, the success rate of autologous fat grafting is largely related to the process of injection. Lumps that are smaller than 2cm in diameter can be taken care with fomentation and massage. Consequently, the lump will decrease in size and disappear. For lumps that are greater than 2cm in diameter, the lumps won’t go away if nothing is done. Even though this is not breast cancer and it does not increase the risk of breast cancer, to those who are facing the problem, the surgeons who had carried out the operation should take up the responsibility to resolve the problem instead of treating it with negligence. Prioritize rest, CHI tenants. Before you detox, Open pathways FIRST (colon, kidneys, etc) Then begin to work on the Lymphatics and getting that pumped Calm the CNS, vagal tone, lymphatic efficiency (coats the GI tract). Heavy Metal detox after alongside Gut Healing I wouldn’t have gone through insurance. I’m STILL battling them on who owes what. Think about it as 3 separate surgeries, it’s so confusing. I would have slowed down. Don't miss the message. I have clients that remove them and just keep working full time or bury themselves more. Still hiding from the story of not enough and covering it up with perfectionist prison just in another form. I would get a looser bra and compression garment and try them on/ wear them for a full day or 2 before using them for surgery I would do a SonoCine rather than the MRI, ultrasound, and Thermogram after being constantly harassed to do a mammogram. I would avoid Fluoroquinolones, a class of antibiotics. STEPS I WOULD ADVISE YOU ON NOW: Get my book. Leanne has had a bit of a journey when it comes to the procedures she needed after massive weight loss and had her first surgery in May 2018 where she had a breast lift and inner thigh lift. Then in August had a lower back lift and outer thighs, followed by an abdominoplasty with muscle repair and fat grafting to her breasts and under her eyes in November, all in the same year It wasn't over though, because Leanne had a revision surgery on her back lift earlier this year! Plastic surgeons have sent out a warning that the chance of dying with the Brazilian Butt Lift (BBL) is 1 in 3000. You may not think that is high, but it is the most dangerous procedure in plastic surgery. In the BBL, many ounces of fat are injected into the buttocks to create a chubby buttock. (Interestingly, even 10 years ago, I was asked to REMOVE fat from the buttocks via liposuction....) Fat can enter the veins and travel through the heart into the lungs. The advisory did not tell plastic surgeons not to perform the procedure, but, reading between the lines, my feeling is that this procedure should simply not be performed. Period.
