A whole month (and sixteen failed attempts) later, I finally found some time to record the follow up to our birth story episode and answer all your questions. Thank you so much to everyone who sent lovely DMs and feedback after the first episode hosted by the wonderful Sophie of Australian Birth Stories and for submitting such interesting follow up questions that give me the loveliest excuse to reflect on things in a way I probably wouldn't otherwise make time for. It's never lost on me what a privilege it is for this yayborhood to be so engaged and excited about each season of our life (and to be so patient with a very inconsistent schedule for releasing episodes!) I can't promise my answers are any less incoherent than they were in the last episode - the postpartum haze is still going strong - but I did my very best and can't actually believe we got through it before the little bear demanded my full attention back. My only disclaimer is that parenthood is like no other area in life for the diversity of opinions, volume of conflicting information, intensity of judgment (both from ourselves and others) and depth of emotion. If I have one piece of advice, it's that NO advice will suit every parent or every baby so please take everything I say as the personal anecdote it is - I'm not a medical expert and am still very early in our journey, so my answers are purely from our experience so far. Having said that, I've tried to be as helpful as I can (amidst the rambling and tangents of this new Mum brain) and the questions are listed below so you can skip forward to what's relevant to you. There are some links where relevant too and, as always, if you have any other questions feel free to DM any time. DELIVERY Did you feel any warning going into labour? In this episode of HealthiHer, host Dr. Amy Brenner delves into the world of alternative fibroid treatment with the distinguished expert Dr. David Toub. Together, they unravel the complexities surrounding fibroids, shedding light on who is affected by them and the symptoms they entail with a focus on a minimally invasive option for fibroid treatment. Fibroids are non-cancerous growths that develop in the uterus, affecting many women during their childbearing years. Symptoms can vary widely, from heavy menstrual bleeding to pelvic pain, impacting quality of life and fertility. Understanding the significance of alternative treatments is crucial, particularly for those seeking options beyond traditional surgeries, such as hysterectomies. The spotlight of the discussion falls on the Sonata Uterine Fibroid Minimally Invasive Procedure, a groundbreaking approach to addressing fibroids with precision and minimal invasiveness. Dr. Toub guides listeners through the intricacies of the Sonata procedure, explaining how advanced ultrasound technology is leveraged to precisely target fibroids. By using real-time imaging guidance, radiofrequency energy is applied to shrink and destroy fibroids while preserving healthy surrounding tissue. Delving into the history of this innovative treatment, Dr. Toub highlights its emergence as a patient-friendly alternative to traditional surgeries like hysterectomy. This procedure offers a conservative approach, allowing individuals to retain their uterus while effectively treating fibroids. It addresses the limitations and challenges associated with invasive surgeries, offering fewer complications and a quicker return to normal activities. Tune in to this enlightening discussion as Dr. Brenner and Dr. Toub navigate the landscape of fibroid treatment, offering valuable insights into the transformative potential of the Sonata Uterine Fibroid Minimally Invasive Procedure. Dr. Astrid Blom is a professor Civil Engineering & Geosciences at Delft University of Technology in the Netherlands is perhaps best known for her recent reach and rive scale work, modeling hundreds of kilometers, sometimes for hundreds or thousands of years.  These models explore the long-term equilibrium state of river responses to human modifications and the alternate potential futures associated with different climate change scenarios and management practices. Most of her recent work and, as you'll here in our conversation - the work she is most passionate about – is that actionable – morphological modeling of the Dutch reach of the Rhine that can influence wise and sustainable management. And I've been interested in Dr. Blom's work on the Rhine for a while, partially because of its similarity to the Missouri in the US – which is a river of comparable size with comparable human modifications, which is also incising.  But I first started following Dr. Blom's research over 15 years ago – with work she did at the particle scale – with detailed laboratory work on the vertical mixing processes in bedforms composed of a wide range of grain sizes. So we talked about both of these scales.   We mostly talked about the Rhine, because the river's natural template, the long history of human modification, the reach scale incision, and contemporary management efforts on that system are so interesting.   And we covered some fundamental processes at that scale, including how gravel-sand transitions evolve on engineering and geologic time horizons on a river that size, the impacts of incision on a large, multi-use, waterway, and some of the management practices targeted to mitigate these impacts. But we also downscaled a little to talk about her early lab work…because it really has affected the way I look at bimodal rivers and gravel-sand transitions.Links to Dr. Blom's papers are on the podcast website (link below). This series was funded by the Regional Sediment Management (RSM) program.Stanford Gibson (HEC Sediment Specialist) hosts.Mike Loretto edited the episode and wrote and performed the music.Video shorts and other bonus content are available at the podcast website (which was temporarily down but is back up now):https://www.hec.usace.army.mil/confluence/rasdocs/rastraining/latest/the-rsm-river-mechanics-podcast...but most of the supplementary videos are available on the HEC Sediment YouTube channel:https://www.youtube.com/user/stanfordgibsonIf you have guest recommendations or feedback you can reach out to me on LinkedIn or ResearchGate or fill out this recommendation and feedback form: https://forms.gle/wWJLVSEYe7S8Cd248

Arun Menawat, CEO of Profound Medical, discusses the challenges of treating prostate cancer and how traditional treatments like prostate removal or radiation often result in side effects and complications. While historically, a PSA blood test has been used for diagnosis, an MRI provides a more precise and efficient diagnostic tool. In addition, Profound is using real-time MRI-guided thermal ultrasound to precisely target and kill cancer cells while maintaining vital functions. Arun explains, "MRI is the best way to really diagnose prostate cancer. And because you can see inside, you can see the cellular structure and that is what's important when physicians review. Our primary message is if you're using the MRI to diagnose, why would you not use that MRI also to treat the patient? Because you can see the urethra where urine is flowing. You can see the ejaculatory duct, where it merges with the urethra. You can get a very good idea of where the nerve bundles are." "Our technology is a combination of using real-time MRI, and we use a very safe way of keeping the urethra at body temperature. Still, we use ultrasound to gently heat the tissue just to kill temperature, typically about 55° Centigrade. So think about this: Men will go into the MR, they will be asleep, they won't feel anything. We insert a catheter right into the center of the prostate. We have water flowing through it, so the urethra that is for urine is always safe. Then we supply some ultrasound through that catheter, and the sound gets absorbed by the prostate tissue, which gently heats. The MRI that we use for imaging, also in real-time, gives us the temperature of the prostate." #ProfoundMedical #ProstateCancer #BPH #Urology #MensHealth #MRI #ScienceandTechnology #Radiology #TULSApro profoundmedical.com Download the transcript here

Thomas loves working with people and his job as a casino supervisor kept him on his feet his entire eight hour shift. But peripheral artery disease, which is plaque buildup in mainly the leg arteries, was slowing him down and putting his job in jeopardy. So, he trusted his vascular surgeons to perform whatever procedure was needed to keep him on his feet. But he didn't know the advances in treatment for PAD and that minimally invasive options were available until complications occurred following an invasive bypass procedure. He reached out to a PAD advocacy organization called, The Way To My Heart, and they informed him of innovative, minimally invasive options. But it was too late for those as the complication at that point from the previous invasive bypass procedure led to an infection of the bypass graft and incision site. His incision site blossomed like a flower and needed immediate care. Dr. David Allie, a cardio thoracic surgeon, who specializes in advanced PAD care, and has advanced wound care experience with his involvement with various breaking studies, offered new options to save his leg from amputation. Dr. Allie and Thomas share this incredible journey to get him back on his feet for good.Thomas loves working with people and his job as a casino supervisor kept him on his feet his entire eight hour shift.
