Dr. Saima Khan is a physician with over 27 years of experience and a deeply personal story of resilience and transformation. Diagnosed with narcolepsy in 2016 after stepping away from medicine due to chronic fatigue and excessive sleepiness, she found herself navigating the healthcare system not as a doctor but as a patient. When conventional treatments failed, she took her health into her own hands, uncovering and addressing multiple causes of brain inflammation including mold illness, Lyme disease and coinfections, chronic SIBO, mast cell activation, autoimmunity and much more. Dr. Saima holds a Doctor of Medicine from Aga Khan University, known as the "Harvard of Pakistan." Today I'm welcoming back Dr. Michelle Leary. Dr. Leary is a Functional Medicine Physician and the Director of Functional Medicine at Vida Integrated Health.Her specialties include primary care, sexual and hormonal health, fertility, and anti-aging medicine for both men and women. Dr. Leary began her early career in cardiac rehab prior to receiving her doctorate from Bastyr University. In 2016 she was the first naturopathic physician ever to be selected for a fellowship in multiple sclerosis management through the National MS Society. She has also completed training at the Institute of Women's Health, American Academy of Anti-Aging Medicine (A4M), and the Institute for Functional Medicine (IFM) in areas of sexology, hormone management, anti-aging medicine, and metabolic weight-loss. So, lie back & keep reading.   Preventive Cardiology vs. Cardiac Surgery  When talking about cardiovascular diseases, the first image that comes to one’s mind is of a person clutching their chest during a heart attack, followed by taking the person to a cardiac surgeon for a stenting procedure.  While this is a common notion, it’s certainly not true. Procedural cardiology (such as surgery or stenting) is the end-stage management of cardiovascular diseases; just the tip of the iceberg. This notion has given the preventive aspect of cardiology a low priority.  On the other hand, preventive cardiology is the study & analysis, followed by the treatment of chronic heart diseases; to effectively prevent or fix them. Most of the curing of heart diseases isn’t always as dramatic as putting in a stent or a defibrillator. Dr. Druz explains this through an exceptional analogy of forest fires; where heart diseases are uncontrollable fire in someone’s vascular system. “Through the normalization of these acute treatments like surgeries & stenting, what we’re doing here is putting out the fires, but not preventing the forest from getting dry.”  — Dr. Regina Druz, Board Certified Cardiologist and Functional Medicine Physician   So, how do we prevent these diseases from a global standpoint? We need to stop adding fuel: By paying attention to apparent risk factors such as hypertension, diabetes, high cholesterol, or obesity, and not normalizing them. We need to take care of the forest so that it doesn’t become too dry and susceptible to catching fire by taking care of the foundations of health—eating the right food, managing stress, etc.    Cardiovascular Diseases   All the heart conditions concerning damaged or diseased parts of the human heart are called cardiovascular diseases, as simple as that. However, people still fail to realize that there are multiple other forms of heart disease other than heart attacks, cardiac arrest, or coronary artery disease (CAD).   Structural diseases Heart failure  Blood Clots Deterioration in the function of the heart muscle Diastolic dysfunction Arrhythmia Vascular diseases   This understanding will help clear the misconception that all of the cardiology is concerned with just bypass surgeries & stent placements.   Traditional Cardiology vs. Functional Medical Approach When a person goes to a traditional doctor, they will put the person on medications related to what their situation is. Then, the doctor might send them back after stenting that one dangerous vessel & ask them to follow up for elective stenting. Unlike the traditional approach to cardiovascular assessment, functional medical cardiologists go to the root cause of the problem. The functional medical perspective asks more “why” questions to reveal the history behind the risk factors, depending from patient to patient. They delve deep into the root cause of hypertension, or obesity, or stress, or high cholesterol, etc. Practitioners assess the impact of the environment, genetics, diet & even pollution on the person's condition. Thus, functional medicine provides the foundation that traditional cardiology doesn’t.     Risk Factor #1— Hypertension Most people tend to exaggerate the numerical measurements when it comes to high blood pressure. The functional medical cardiologists approach such situations by helping the patient realize that the blood pressure measurement is more than just a separate measurement at a specific time; there’s so much more to it. Such involvement in the whole journey helps them understand the gravity of their reactions to numbers and even calms them down. Root Causes  Before looking for the root causes, the doctor considers the context & the timeline of the condition and cross-checks any secondary causes. After this, the doctor confirms if the patient is hypertensive or not. Whether the patient has labile blood pressure or established hypertension, there are two causes: Insulin resistance: Insulin resistance is one of the most overlooked causes of hypertension, ignoring which you may/may not increase your chances of gradually developing type 2 diabetes. Sympathetic Activation: While most people are not aware of their sympathetic reactions, being in this state of fight or flight causes consistent changes in the vascular tone, changing the vessels without the person even realizing it. Being in this zone of an increased heart rate contributes to hypertension.   Risk Factor #2—High cholesterol   Cholesterol alleviation is a stress reaction or an indicator of imbalance. In functional medicine, cardiologists strive to find the reason behind the high cholesterol levels instead of just prescribing cholesterol decreasing medications.   Misperceptions: No need for statin drugs if there’s an optimal level of cholesterol. Wrong. The patient still needs to be put on statin drugs for LDL because of the residual risks of cardiovascular problems. Having high levels of HDL or the “good” cholesterol protects you from cardiovascular diseases. Not always. At times people have HDL cholesterol levels that are just too good to be true. In such cases, patients should check:   the amount of LDL & HDL particles the size of LDL & HDL particles   Having extremely high levels of HDL can be a sign of zygosity that can cause dementia.   Medication—Statin Drug Statin, a cholesterol-lowering drug, increases the coronary arterial calcification causing the more unstable plaques to come to their final healing stage and get calcified, which is a good thing. The likelihood of memory-related issues only increases when the cholesterol level goes to less than 150.   The decision to take Statin drug depends on: Total cholesterol production Cardiac risk Cognitive risk Genetic risk (direct or polygenic) Lipoprotein (a) levels   A Heart-friendly Diet Cardiologist-recommended diet for a healthy heart has clean eating, hydration, timely eating & elimination of sugars as the foundation.   Do’s Follow the Mediterranean-style diet Incorporate healthy fats Avoid processed foods Cut off all saturated & trans fats Follow time-restricted eating patterns   Don’ts Consumption of simple sugars Opt for a Ketogenic diet as a long-term solution Over-consumption of fish oils (Omega 3 & Omega 6)   Supplements Optifibre, by Xymogen Alpha Lipoic Acid (ALA) (for patients with insulin resistance) BergaCor Plus, by Xymogen Metamucil This blog covered just the highlights from our latest podcast by Dr. Nisha Chellam, Board Certified Internist and Founder of Holistic ICON, and Dr. Regina Druz, Board Certified Cardiologist, Functional Medicine Physician, FACC, and FASNC. It’s time for some fun in this not your daughter’s workout Not your mother’s workout episode. We spent an hour comparing and contrasting Millennials & Gen X/Boomers (50+). So get your long hike shoes on and be ready for the ride. There are a lot of pearls inside. Moms round up your daughters, in-laws and let’s do this. Are you working out with your daughter? Giving her tips so she can start exercise or get better results? I’m here with my good friend and the Millenial health expert Dr. Debbie Bright to discuss what’s same and what’s different about your workouts. My Guest Weight loss and gut-health expert Dr. Debbie Bright is an internationally recognized and board-certified Functional Medicine Physician, Exercise Physiologist and Nutritionist. As the Founder and CEO of the International Millennial Health™ Movement, Dr. Debbie helps millennials discover their hidden food sensitivities so they can heal their face (skin), fat and fatigue all within 21 days or less. While weight loss is what Dr. Debbie does—restoring self-worth is who she is. Not Your Daughter’s Workout Not Your Mother’s Workout, Or Is It? Both generations know they need to exercise/move more. When it comes to weight loss the first thing they think is, “I need to get into the gym.” They both falsely believe in the old science of eat less, move more — they think body composition is all about calorie restriction and cardio. So, they start eating more fruit, skipping meals, feeling like crap and jogging. The results they do get are not sustainable and they  rebound or yo-yo diet. I think it’s very common for both generations to always be on a diet in some way shape or form. They still falsely think they should be grazing too or eating their 100-calorie snack packs! Gen X & Boomers go for the treadmill (or the elliptical or walking) and Millennials go for the spin bike. Very cardio-driven mindset. We all need to move more. For Millennials it’s a very sedentary lifestyle due to long hours sitting at the desk and scrolling social media. For Gen X & Boomers it’s being planted in front of the computer too, and then the TV, and caring for aging parents. There’s a little social media time there too.  What Happened? BOTH: What used to work before is no longer working anymore (caloric restriction). Depending on age of Millennial (24-38), they’re also starting to experience hormonal shifts. Gen X & Boomers 50-75 experiencing after-effects of no weight lifting while working and no emphasis on it while they were young adult women. Stress Levels? Millennial women are career-driven, typically have a lot of student loan debt, depending on where they’re at in the age-range they are moms…very busy lifestyle and challenging to keep up with… Dads are getting the dad bods and I don’t see them recovering from it. Women 50+ are still working, juggling home, or now home and juggling more at home, more college kids not at college, young adults displaced due to COVID, aging parents whether very close or very far – both cause stress. Male boomers are losing muscle (and hair) gaining belly fat and slowing down…especially if alcohol is still a big part of social life. For men and women - Still working, don’t want to be working, stopping working, identifying with the career, or worrying about not having the income from working and will it be enough. What’s driving the Millennial generation to choose their workouts? Social interaction and a sense of community, which is why they’re skipping out on the regular do-it-yourself gym memberships and aiming for boutique-style group classes. What’s driving 50+ Women to choose workouts? “Don’t Hurt Me!” and fat loss, belly fat loss promises. So, they’re vulnerable. They can fall for the snake-oil salesman and a savvy marketer. Look, even a beautiful 50 something trainer with flat abs doesn’t guarantee she’s using a hormone balancing exercise prescription that is based on YOU and individual need. What are the top 5 most popular fitness activities among the Millennial generation? Yoga Spin like SoulCycle HIIT training or bootcamps Pilates CrossFit Women 50+ exercise favorites Walking Pilates Yoga, Barre Zumba Elliptical, rowing, “toning classes" What do you see as beneficial or not with these workouts? Let’s be real…spin is NOT a full-body workout! Yoga keeps you soft. Lots of injury with CrossFit and bootcamps. Moving is great, but only focusing on any one of these and skipping weight training does you no favors at all! There are (7) functional human movements include: pushing, pulling, squatting, lunging, hinging, rotation/anti-rotation  gait We never drop into pushups on a bike or lift your baby with your feet pedaling beneath you — not functional. Dr. Debbie Tip: When I’m on a spin bike and they go to weights, I crank my resistance up so high that I can now use the pedal as a ground reactive force so I can engage my core and I don’t blow out my shoulders with 2 pound weights. I understand grip strength and how that protects the shoulder… I frown on this style of false "full-body" workout. Debra Tip: I leave.  I’ve got less time on this earth for nonsense than you. Professional and Olympic athletes don’t even do crazy stunt workouts, so why would you? There is exercise (full of creativity and variety for variety sake) and then there’s training. ME: Let’s clarify that statement. We don’t want necessarily to train like professional and Olympic athletes … they train twice a day, they nap and treat nutrition like it’s their job between, it’s their life. But … the point… that you train with specificity. What do you want to be better at? What aren’t you good at now? What hurts? What doesn’t feel good? What do you want to be able to do? Not Your Daughter’s Workout Not Your Mother’s Workout on Social Media True for BOTH: Too many Instagram stars doing the most ridiculous workouts! Oh, god, thank you for that! And too many who have clearly had surgery – be it boob, belly or butt – that viewers are believing they’ll get that look if only they repeat. True for BOTH: Pink dumbbells outside of rehab are just embarrassing. Skipping weights is a huge disservice to longevity. Me: and it’s asking for disability. End stages of perimenopause, early stages of post menopause, muscle and bone losses are accelerated. So offsetting those with strength training and protein is mandatory unless you just want to say… okay… just say bring it on aging. But during COVID if you felt more stress, isolation, flirted with depression or anxiety and you’re taking medications, those can also compound body composition changes. This minute we’re in right now is lasting longer than anyone wants it to…and we need to decide right here and now we take back control in 2021. Your choices during the pandemic especially if you’re in menopause will determine your next 10, 20, and 30 years. Spin Bunnies? Spin bunnies and boot campers are NOT functional trainers. They don’t even know human anatomy or those basic 7 human movements if you asked them!! It’s embarrassing!! Me: And they literally just “spank” participants. As if the worse you feel, the more you can barely walk leaving or tomorrow, the more “it was a great workout.” In 2020 that kind of mentality costs you immunity. That kind of muscle breakdown – catabolism – on top of emotional, financial, stressor… Example: Pull-ups or chin-ups work the core. Put in a tampon and do them and tell me your core and pelvic floor aren’t activated enough to push that thing out of place. Me: True! And that I hope ever Flipping 50 listener has at least one tampon lying around to go try this. Where Not Your Daughter’s Workout Not Your Mother’s Workout Collide Weights do not make you bulky, weights make you toned, sexy, lean…you need to be strong as you age. Me: Truth. Let’s get to the bottom of "bulk" when doing weights. A sign you are retaining water, puffy, and a hormone imbalance A sign you are not eating enough protein but have justified just eating more, and making it carbs – of the not so resistant starches kind A sign you’re not adhering to a smart schedule of training and recovery. What drives Millennial women crazy if you say/advise them ______________ Being confronted with the idea that what they’re doing isn’t really serving them—cardio, caloric restriction. They don’t know how to train functionally. They don’t understand they need to rest! They think it’s go harder, stronger,  longer and they’re heading towards burnout with the poor food choices and excessive exercise.  Frequency Reality Dr Debbie: I only train 2x a week coming out of burnout myself, but people look at me and claim I must train 2x a day, 7 days a week.  And to dive deeper into rest. After about 40 the need for recovery time increases. I always like to emphasize there isn’t a reduced need for stimulus, meaning you need to lift, and you need to lift to fatigue. But you will enjoy greater strength and fitness gains if you recover fully and that is more often 72 hours than the 48 between strength training sessions you used to do. You can workout once with your daughter and then the rest of the week you’ve got unique needs for recovery. Me: Yes, A 67-year old client of mine recently crossed the 100 lb weight loss threshold, and she recently had a PR doing a virtual 5k (she does many). A member of our community asked in a post whether she runs every day? She runs just twice a week at most, one is a short hill interval workout and another is a low to moderate mileage easy jog – always with a purpose. That might be she does 2.5 miles and I ask her to go out easy and then have a negative split – making her second half of the run significantly faster. We don’t just log miles. Dr. Maita got into Functional Medicine out of Necessity. After 9 month of being on antibiotics, she didn't feel like herself anymore and found herself needing help. She took herself on a journey and began healing herself with an elimination and detoxification diet. Over the years, she has dealt with so many different concerns for her clients, but has also help through the pandemics of AIDS, SARS, Avian Flu as well as Bioterrorism and Now COVID-19. Her ultimate goal was to help people through prevention. In the beginning top executive women would come to her, begging for help as they were experiencing symptoms of perimenopause. And at Pfizer, men sought her out wanting Viagra. She became known as the “Hormone Guru”. Dr. Maita truly began giving people their life back. Dr. Maita shares that there are 3 kinds of Estrogen and the Estrogen in Menopause can be very inflammatory. When assessing patients I take a 3D look: She tests the blood, the Urine and the Saliva. Ultimately estrogen is the hormone for Energy and Growth. But when it is not balanced out by Progesterone, women can develop symptoms such as anxiety, irritability, impatience, insomnia, weight gain, bleeding, spotting, fibroids as well as painful cysts. It is really as easy fix. First, she states, we need to decrease the Estrogen in the body by first decreasing the chemicals they are exposed to that act like Estrogen. Next look at possibly adding Progesterone if needed and life can be beautiful again. And of course regular follow ups are necessary. For most people starting off just looking at lifestyle changes and a detoxification diet, to calm the immune system in key. We all have about 200 chemicals in our bodies everyday. So to begin: Start with clean filtered water, organic foods, and eliminate the chemicals in your personal care and household. Toxins disrupt the immune system, hormone system and nervous system. How Dr. Engelman has extensive experience managing autoimmune conditions Testing, diet, and lifestyle recommendations from Dr. Engelman Some highlights of this episode on Fatigue & Autoimmunity with Functional Medicine Physician Mark R. Engelman MD, FSCM: Dr. Engelman came from a medical family in Ohio. He was going to follow in his father's footsteps, but moved to Arizona to get out of the cold weather. He was an emergency medical physician for 23 years. He was the President of the Heart Association,  and he was a mainstream medicine guy. [02:09] He then started a bilingual hospital in Mexico and then worked in China. He learned a lot about Eastern medicine, and then became mesmerized by stem cells. He found a stem cell fellowship. The stem cell medicine got him interested in cellular biology. Integrated medicine is the pathway to how things work. If you know this, then you can figure out what is going wrong when someone gets sick. [03:39] The End of Alzheimer's prior to the 2000s the word was what? Now the new word is why? Why am I sick and how can I do something about it. Integrative medicine is about the why. [06:34] How people see Dr. Engelman after other doctors not being able to diagnose them. How patients have a series of symptoms and sign with a label put on them. Dr. Engelman asks the question of why are they experiencing these symptoms. Autoimmune diseases are always triggered by something. [10:23] The importance of finding a doctor who focuses in on the history of the patient before testing. Regular doctors get a blood panel. Chemistry panel and electrolytes. Get a general idea of how your organs are working. For fatigue, looking at thyroid is an option. These tests can be inaccurate with the spread rates. Functional medicine doctors can hone in better. [13:23] Finding the trigger for autoimmune thyroid problems. First look at the gluten panel on Cyrex. How autoimmune goes after one thing and then goes after your body too. How autoimmune disease is autoimmune activity or a bunch of things happening. [19:18] My immune system has decided to start attacking me. This is about immunology and how we manufacture energy. We have millions of immune cells that feel their way around our bloodstreams and tissue. If our immune system doesn't recognize something from being different from us. Main triggers are hidden allergens, environmental toxins, inflammatory diets, trauma, stress, and pathogens. [22:51] How we have control over our treatments and seeing and finding a functional medicine doctor to manage the disease. Many of these autoimmune diseases are reversible. Finding out what the issues are can help to reverse this stuff. How the barrier gets breached and then our immune systems go after the foreign things and then it makes mistakes. [32:17] The importance of quality sleep and having a healthy gut. Testing for gluten and cross reactivity. Antibodies to new foods introduced to the body.
