Brigitte Brisson, DMV, DVSc, DACVS from the Ontario Veterinary College at the University of Guelph joined us on OsteoBites to introduce Magnetic Resonance guided High Intensity Focused Ultrasound as a treatment modality for osteosarcoma and describe preliminary results in canine patients.High Intensity focused ultrasound (HIFU) is a non-invasive percutaneous thermal ablation technique that allows deep tissue treatment with precise control over the shape and location of energy deposition. It causes minimal collateral damage and can be used to treat primary and metastatic tumours. In addition to its role in tumour ablation, HIFU has attracted attention for its potential to stimulate the immune system and possibly mount a response against metastasis. In humans, HIFU has been used for ablation of a variety of soft tissue tumours and most recently, it has been used in bone applications, specifically for the treatment of osteoid osteoma (OO) lesions in pediatric patients. Other reported bone applications include bone metastasis and osteosarcoma.Dr. Brigitte Brisson is a professor of soft tissue surgery in the Department of Clinical Studies at the Ontario Veterinary College (OVC). She graduated from the Faculté de Médecine Vétérinaire at the University of Montreal in 1996. She performed a small animal rotating internship at the OVC followed by a Surgery residency with concurrent Doctor of Veterinary Science (DVSc) in small animal surgery. She became board certified in small animal surgery (ACVS) in 2001 and has since been on faculty at the Ontario Veterinary College. She is an ACVS Founding Fellow in Minimally Invasive Surgery and a Founding Member of Veterinary Neurosurgical Society.

Trials discussed include: i) HIFI, a non-inferiority, prospective, non-randomised, nationwide study performed in 46 French centres comparing HIFU vs RP; ii) The Next Generation Trial, a phase II prospective validating paired-cohort trial assessing 18F-PSMA1007 PET/CT and mpMRI for locoregional staging of prostate cancer, with final histopathology as the gold standard comparator; iii) IN APA-RP: a multicentre, open-label, single-arm, Phase 2 study conducted at 27 US urologic practices, focusing on pts with high risk localised PCa who had undergone RP were treated with apalutamide (240 mg; once daily) and ADT for 12 cycles (1 cycle [28 days]). Murray Bocian, 76, was prone to spilling things: His hands were so shaky that he had to begin using a child's sippy cup. Murray was battling a condition called essential tremor, and it was impacting many aspects of his daily life. A nervous system disorder, essential tremor starts in the brain and triggers shakiness in the extremities, most often the hands. For most of the 10 million Americans living with essential tremor, lifestyle changes and medications can help manage symptoms. But when the condition interferes with daily activities as it was Murray, more targeted therapies are needed. For decades, the solution has been to open the skull and operate on the brain. Now there's a new incision-less procedure called high-intensity focused ultrasound — HIFU — which can be done with an MRI scanner in a single two-hour outpatient session. And the effects are immediate. They discuss its dynamic nature, allowing for a wide range of applications. --- SHOW NOTES Dr. Bhavya Shah is a neuroradiologist at UT Southwestern in Dallas, TX and the director of their transcranial-focused ultrasound lab. While in residency at Boston MIT, he studied the radiology applications for nerve regeneration and expanded his scope of practice during his fellowship at Stanford. Dr. Bhavya Shah explains the use of low intensity focused ultrasound (LIFU) and high intensity focused ultrasound (HIFU), particularly in the context of movement disorders including essential tremor and Parkinson's disease. LIFU is used to identify the appropriate targets in the brain in relation to the disease and may be used to alter how neurons behave. In contrast, high intensity focused ultrasound (HIFU) is utilized to ablate and destroy tissues typically after the localization of the intended treatment area. Dr. Shah developed a way to identify targets in the brain for treatment with focused ultrasound with the use of four-tract tractography in cadavers. Using this technology, the brain can be thinly sliced into sections which could then be registered off an MRI back to the path using block face photography, allowing the identification of white matter tracts that enter and leave the thalamus. With these tracts identified, neuroradiologists can first stimulate the localized area with LIFU to confirm the location, then ablate using HIFU. The procedure lasts approximately 30-45 minutes as the patient remains awake. Remarkably, patients with essential tremor usually experience benefit immediately following the procedure as patients with Parkinson's have symptom improvement within days to weeks. After two hours of observation, patients are discharged assuming no side effects. Side effects are uncommon but can include numbness and tingling around the mouth or fingertips as well as muscle weakness. Beyond its use for movement disorders, the adaptable nature of focused ultrasound technology shows promise for a broad range of applications, particularly for the use of neuropsychiatric conditions. Dr. Shah offers the potential for the use of HIFU as a wearable device that delivers constant stimulation modulated by biofeedback, potentially eliminating the need for MRI for the procedure. The Virginia-Maryland College of Veterinary Medicine is one of the few locations in the world studying histotripsy, also known as high-intensity focused ultrasound or HIFU. This non-invasive therapy uses powerful sound waves to, well, basically disintegrate tumors. It may sound crazy, but it's a real thing, and there are even clinical trials in humans ongoing in Spain. Dr. Joanne Tuohy explains the method and the current studies that she is involved with. Current work is focused on osteosarcoma (bone cancer) and soft tissue sarcoma, but they are hoping to expand to other solid tumors in the future. While we don't yet know all of the answers and HIFU is far from being considered standard of care for dog cancer, dogs with bone cancer or soft tissue sarcoma may be eligible to participate in clinical trials and receive this therapy.
