In the episode, I divide QVAR and beclomethasone into syllables, tell you which syllables to emphasize, and share my sources. We don't cover pharmacology in this series. Just pronunciations. Today's drug name was chosen from Davis's Drug Guide for Nurses, Seventh Edition.    Written pronunciations are helpful! They are below and in the show notes on https://www.thepharmacistsvoice.com/podcast.   oxcarbazepine = OX-kar-BAZ-e-peen The first two syllables sound like “box-car.” (Drop the “B.”) Baz, like basketball e, which is a short “e” sound, like the “E” in the middle of the word Elephant Peen, like a ball peen hammer Emphasize OX and BAZ. BAZ gets the most emphasis. Did you know that the United States Adopted Names (USAN) Council advises against using the letter "K" in generic drug names? Some languages struggle with pronouncing “K.” In fact, four letters are generally avoided in naming drugs: H, J, K, and W. While the USAN guideline applies to generic drug names, brand-name drugs also tend to avoid these letters.   A Fun Thought Experiment If you've ever been told not to do something and immediately wanted to do it, you're not alone! Just for fun, I combined all four of the "forbidden letters" into a made-up drug name: Jawkherol Sounds like a treatment for TMJ, doesn't it?   Common Mispronunciations to Avoid Some sources—including YouTube videos with thousands of views—incorrectly pronounce ketamine. Olentangy Local School District, argued before the en banc U.S. Court of Appeals for the Sixth Circuit on March 19, 2025. Argued by Cameron Norris (on behalf of Parents Defending Education); Elliott Gaiser, Solicitor General of Ohio (on behalf of Ohio and 22 other states as amici curiae); and Jaime Santos (on behalf of the Olentangy Local School District Board of Education, et al.).Background of the case, from the Institute for Free Speech's second amicus brief (in support of reversal): While students may freely identify as having genders that do not correspond to their biological sex, other students enjoy the same right to credit their own perceptions of reality—and to speak their minds when addressing their classmates. Students cannot be compelled to speak in a manner that confesses, accommodates, and conforms to an ideology they reject—even if that ideology's adherents are offended by any refusal to agree with them or endorse their viewpoint. Yet that is what the Olentangy school district's speech code does.“Pronouns are political.” Dennis Baron, What's Your Pronoun? 39 (2020). History shows that people have long used pronouns to express messages about society and its structure—often in rebellion against the prevailing ideology. And the same is true today. Choosing to use “preferred” or “non-preferred” pronouns often “advance[s] a viewpoint on gender identity.” Meriwether v. Hartop, 992 F.3d 492, 509 (6th Cir. 2021). So mandating that students use “preferred” pronouns or none at all elevates one viewpoint while silencing the other. It compels students to adopt the district's ideology on gender identity while at school, and in doing so, “invades the sphere of intellect and spirit which it is the purpose of the First Amendment to our Constitution to reserve from all official control.” W. Va. Bd. of Educ. v. Barnette, 319 U.S. 624, 642 (1943).Statement of the Issues, from the Brief of Appellant Parents Defending Education:The use of gender-specific pronouns is a “hot issue” that “has produced a passionate political and social debate” across the country. Meriwether v. Hartop, 992 F.3d 492, 508-09 (6th Cir. 2021). One side believes that gender is subjective and so people should use others' “preferred pronouns”; the other side believes that sex is immutable and so people should use pronouns that correspond with biological sex. Id. at 498. Like the general public, students have varying views on this important subject, and the Supreme Court has long recognized that students don't “shed their constitutional rights to freedom of speech or expression at the schoolhouse gate.” Tinker v. Des Moines Indep. Cmty. Sch. Dist., 393 U.S. 503, 506 (1969). Yet the Olentangy Local School District has adopted policies that punish speech expressed by one side of the debate—the use of pronouns that are contrary to another student's identity. In this episode, we review the new 2025 ACC/AHA Acute Coronary Syndrome (ACS) guidelines, with a particular focus on guideline recommendations for analgesics, P2Y12 inhibitors, parenteral anticoagulation, and lipid management. Key Concepts Nitrates and opioids are recommended for symptomatic relief of chest pain. Some patients may not be appropriate for nitrates (e.g. recent PDE-5 inhibitor use, hypotension, or right ventricular infarction). Opioids are used for nitrate-refractory angina but have a theoretical risk of delaying the effect of oral antiplatelet medications. Prasugrel and ticagrelor are preferred P2Y12 inhibitors over clopidogrel in most patients. Patient-specific factors, including the use of PCI, play a role in P2Y12 inhibitor selection. Anticoagulation with heparin is recommended in nearly all acute coronary syndrome (ACS) scenarios. Alternative anticoagulants may be used depending on whether PCI/CABG is planned and whether the anticoagulant is used prior to PCI/CABG (“upstream”) or during the PCI procedure itself. LDL goals after ACS have changed again. All ACS patients should have an LDL goal < 70 with a consideration of an LDL goal of 55-69. A variety of non-statin therapies may be added to a high intensity statin regimen if LDL is not at goal. Nitroglycerine is vasodilator that affects peripheral blood vessels and coronary arteries. Because of its widespread dilation effects on blood vessels, nitro can quickly lower a patient's blood pressure, sometimes to the point of making a patient hypotensive. Assessment of vital signs prior to administering nitro is necessary to ensure patient safety. Indications for use of nitroglycerine. Nitroglycerine's contraindications & considerations for use. Cyclosporine is a calcineurin inhibitor that suppresses the immune system. This is useful in patients who have had an organ transplant. Insulin glargine is a long-acting insulin product that can be used in type 1 and type 2 diabetes. I discuss adverse effects, dosing, and much more. Tadalafil is a PDE-5 inhibitor that can be used to treat sexual dysfunction. The nitrate drug interaction is one of the most highly tested drug interactions in school. Amitriptyline is a TCA. It is highly anticholinergic and because of this is not a great medication to utilize in elderly patients. Gemfibrozil is primarily used to lower triglyceride levels. This medication can significantly increase the risk for rhabdomyolysis in patients taking statin medications.

MONA is the acronym sometimes used to help us remember the interventions to consider for patients with Acute Coronary Syndrome or ACS. Morphine's use in the Acute Coronary Syndrome (ACS) algorithm. Why Morphine is helpful for patients with ACS. Contraindications and considerations for the safe administration of Morphine. Morphine as an alternative to nitro for patients with chest pain that take PDE inhibitors. Common dosing & administration of Morphine. Monitoring of the patient's level of consciousness, pain, blood pressure, and respirations after administration. Possible side effects of Morphine administration. Nitroglycerine is vasodilator that affects peripheral blood vessels and coronary arteries. Because of its widespread dilation effects on blood vessels, nitro can quickly lower a patient's blood pressure, sometimes to the point of making a patient hypotensive. Assessment of vital signs prior to administering nitro is necessary to ensure patient safety. Indications for use of nitroglycerine. Nitroglycerine's contraindications & considerations for use. Effects of nitro on patients taking PDE inhibitors. Administration of nitroglycerine to patients with ischemic chest pain. Considerations for patients that took their home nitroglycerine. It is often combined with numerous other medications so patients need to pay attention Valium (diazepam) is a benzodiazepine used to treat anxiety or seizures. Falls, dizziness, confusion, and sedation are common adverse effects. Viagra (sildenafil) makes the list at number 73. This medication is a PDE-5 inhibitor that can be used for erectile dysfunction. Bactroban (mupirocin) is a topical antibiotic that can be used for superficial skin infections. Januvia (Sitagliptin) is a DPP4 inhibitor used for treating hyperglycemia in type 2 diabetes. It doesn't reduce A1C quite as much as other diabetes medications like SGLT2 inhibitors and GLP-1 agonists.
