Dr Jon Kamler, EM Doc at Cornell who is first author on a paper to be published in JSOM, discusses our literature review and relevant findings to pushing for change in MASCAL response. Triage accuracy, plethora of systems, actual use of MASCAL systems in training and real world events and more.
Dr. Rhee is Board Certified in General Surgery and surgical critical care and is a Fellow of the American College of Surgeons. He served in the Navy for 26 years in many important roles in the operational and training environments. In the episode, he describes what it's like to be a physician on a nuclear aircraft carrier and perform surgeries while underway at sea. He also relates his experiences helping design small forward surgical units that he would later deploy with to Southwest Asia. He talks about lessons learned on the battlefield caring for casualties which led him to be involved in setting up the Navy Trauma Training Center in Los Angeles which sustains skills and prepares teams for downrange requirements. Dr. Rhee recalls his experience with Mass Casualty incidents of more than 200 patients and provides a behind-the-scenes look into the challenges encountered by medical providers when needs overwhelm resources and how tough decisions have to be made. CAPT Rhee explains how the lessons he learned during his training and during operational assignments led him to be a founding member on the Tactical Combat Casualty Care Committee Following retirement, Dr. Rhee help prepare the trauma system to deal with a significant MASCAL situation in Tucson, AZ in 2014 during a mass shooting event at a political event. We aim to start a conversation here with the hope that it continues with your Medical Director, PA, Surgeon and fellow Medics before you are ever faced with this difficult situation out on your own. Often prolonged field care involves treating the most critically sick or injured patients longer than you expect to. Inevitably some of these “sickest-of-the-sick” will not make it to see definitive care and you will be left to ease the suffering during end of life care alone. While you may have to deliver end of life care by yourself, you may not have to make all the decisions alone. In this episode Dennis and Doc Powell discuss how to treat expectant patients. This could be as part of a multi-patient MASCAL or a happen to a single patient who is critically ill or injured. If it happens during a MASCAL, once you are done treating your urgent patients, what do you do when you go back to your expectant patients? It's common to skip over discussing and training on losing patients… Taboo even. The fact is that it will eventually happen to some of us; No matter how good of a medic we are, patients will die. Doc Powell has spent innumerable hours in Intensive Care Units with the best and brightest medical teams a patient could hope for. Even in this setting the top notch care, medicines and interventions are not enough and patients code and die. This is part of medicine whether we talk about it openly or not and while many of these situations will be complicated and stressful we hope to give you a few tools to help manage the situation in a more professional manner. How do you decide if your critically ill patients are expectant, when alone in a tactical or resource strained environment? After that decision is made, what can we still do? How is telemedicine different for curative vs. palliative cases, if at all?

In this interview, MAJ Andrew Fisher gives us a great review of pre-hospital whole blood in the military; where we are now and how we got there. He reminds us that, "everything old is new again." The first whole blood transfusion research was done by the military in 1940 and was used extensively in WWII and the Korean War. Supply limitations during MASCAL events in the early 2000's led to authorization to use walking blood banks at fixed facilities, and the results were promising. Further evaluation led to codification of walking blood banks in deployed theatres when demand outstripped supply, driving the creation and implementation of a prehospital blood program that continues to be the gold standard.
