In this episode, Stuart Haines, JACCP Editor-in-Chief, interviews Dr. Andrea Sikora, Dr. Brian Murray, and Dr. Susan Smith about their recently published consensus recommendations for integrating critical care pharmacists into intensive care unit (ICU) teams which was endorsed by ACCP as well as the American Association of Critical-Care Nurses, American Society of Health-System Pharmacists, Institute of Safe Medication Practices, and the Society of Critical Care Medicine. The paper was published in the September 2025 issue of JACCP and is available open access. The document addresses the "unmet need" of operationalizing the pharmacist's role, as evidence suggests that many ICUs still lack a critical care pharmacist. The recommendations go beyond simply advocating for pharmacists; they provide a blueprint for hospitals to optimize pharmacist services, including direct and indirect patient care activities. The paper and its supplementary materials, which include an institutional assessment checklist, can be used to improve the quality of patient care.

Dr Kavi (Mufti) Haji arrived in Australia with her toddler son and husband in 1995 due to the difficult situation in Iraq during the Gulf War. She and her husband had both studied medicine and worked in rural areas in Iraq. When they arrived in Melbourne they had to study and pass exams in order to continue in their fields. Dr Haji has achieved a lot since arriving here despite the obstacles she faced. Dr Haji is now a staff consultant intensivist, and supervisor of training at Peninsula Health, Mornington Peninsula Victoria, Australia. She is an academic physician and an adjunct senior lecturer at Monash University, Faculty of Medicine, Nursing and Health Sciences. She has special interest in echocardiography and ultrasound. She has a PhD on the role of examination-assisted ultrasound in the Intensive Care Unit. Her other interest is teaching. Brett has been an RN since 2007 and became an Acute Care Nurse Practitioner in 2011. Both degrees are from Texas Tech University Health Sciences Center. Most of his career has been in Critical Care, covering Intensive Care Units, both in person and by telemedicine. Over time, Brett noticed his patients, largely, were in a managed decline by their providers and their laundry lists of medication. He began to question the legitimacy of his chosen specialty. The ICU Liberation Campaign from the Society of Critical Care Medicine (SCCM) has transformed critical care, but the COVID-19 pandemic and subsequent staffing challenges have posed major obstacles to maintaining progress. In this episode of the SCCM Podcast, host Ludwig H. Lin, MD, speaks with Juliana Barr, MD, FCCM, a key architect of the ICU Liberation Campaign. Dr. Barr was a lead author of the 2013 “Clinical Practice Guidelines for the Management of Pain, Agitation, and Delirium in Adult Patients in the Intensive Care Unit,” known as the PAD guidelines, an original cornerstone of the ICU Liberation Campaign (Barr J, et al. Crit Care Med. 2013;41:263-306). The guidelines' recent 2025 update also addressed immobility and sleep disruption (Lewis K, et al. Crit Care Med. 2025;53:e711-e727). Dr. Barr shares her personal journey from traditional ICU practices of heavy sedation and immobility to leading efforts that prioritize patient recovery, well-being, and post-ICU quality of life. She emphasizes how ICU Liberation reintroduced low-tech, high-impact interventions such as minimizing sedation, promoting early mobility, and engaging families—leading to better outcomes at lower costs. She cites the 2017 international survey by Morandi et al that demonstrated uneven but steady improvements in global ICU Liberation practices before the pandemic (Morandi A, et al. Crit Care Med. 2017;45:e1111-e1122). Dr. Barr details the need for reeducation, multidisciplinary team engagement, and reworking electronic health record (EHR) systems to better support ICU Liberation goals. Looking forward, Dr. Barr offers a "burning platform" approach, stressing that delaying ICU Liberation practices risks poorer patient outcomes. She advocates for cultural change, leadership engagement, real-time metrics visibility, and hospital-wide investment—including IT support to surface buried ICU Liberation Bundle data within EHRs. By reframing ICU Liberation as a "team sport" and making best practices part of daily ICU culture, Dr. Barr believes institutions can reestablish the bundle's momentum and reconnect healthcare teams to their core mission—helping patients return to meaningful lives after critical illness. This conversation offers energizing, practical strategies for ICU teams at every stage of ICU Liberation implementation or reinvigoration.

Dr. Brown has worked in several capacities as physician. He worked as Escort Doctor for MERCI on boats repatriating Sierra Leonean Refugees back to Sierra Leone. He then worked as the County Health Officer for Grand Bassa County and Medical Director for the Buchanan Government Hospital from 2006 to 2008 after working as a Volunteer Physician at the ELWA Hospital and General Practitioner from 2004 to 2006. In March 2014 he was employed as Medical Director and General Surgeon at the ELWA Hospital a position he held until February 2018 when he was appointed by the President of Liberia to serve as the Chief Medical Officer of the John F. Kennedy Medical Center, the premier teaching and referral hospital. During those years at ELWA, he worked tirelessly performing varieties of surgeries in this low resource setting. He became Clinical Supervisor and Clinician at the ELWA II Ebola Treatment Unit from July 2014 to June 2015. Under his leadership and guidance this unit produced the highest number of Ebola survivors changing the survival rate from ten percent to seventy percent of Ebola Patients at his Center. From October 2014 to December 2016, he served as Principal Investigator on two research projects with the Clinical Research Management on convalescent plasma and the sequelae of Ebola in survivors. In 2018, he was appointed by the President of Liberia as the Chief Executive Officer of the John F. Kennedy Medical Center, the premier referral hospital in Liberia, a position held until January 30, 2024, due to the political transition of power. While at JFKMC, he established the only active functional Intensive Care Unit in country with support from partners such as Project Cure International and NOCAL. Under his leadership JFKMC, obtained accreditation for training specialists in the areas of pediatrics, internal medicine, general surgery, ophthalmology and psychiatric. He also established the only histopathology unit; a state of the art executive private ward; a dialysis center among others. On May 23, 2019, he was elected Civilian Representative and Advisor to APORA. He also serves as Acting Faculty Head, Department of Surgery, A. M. Dogliotti College of Medicine for two years, and is currently a Part-Time Faculty member, in the Department of Surgery, Liberia College of Physician and Surgeon. In March 2020, he was appointed by the president of Liberia to serve as the Head of the National Case Management Pillar of COVID-19. He coordinated the management of COVID-19 patients across the country and the care of patients with COVID-19 vaccine related complications. He supervised the drafting of Liberia COVID-19 Clinical Guidelines. Danny Perez wrote a great book titled Intensive Care Units. In it, he talks about how important it is for us as Christians to grow into Christ together. We are created for connection, and the only way to become mature in Christ is to do it together. And if we think we're becoming mature in Christ, but our love for people isn't growing, we are not becoming mature in the Jesus of the Bible. Danny and his wife, Kathryn, have been serving and loving military members and their families with Cadence International since 1984. Their lives are filled with people: loving, encouraging, mentoring, caring for and coming alongside them as they grow towards maturity. They offer the body of Christ a healthy dose of transparency and laughter while pursuing the lost and building up the believer. Danny has a Masters in Biblical Counseling from Colorado Christian University. They have been married since 1988 and are the parents of four grown men and grandparents of two adorable grandchildren. Barend was run over by a freight truck and killed. His body was flattened. While in the Intensive Care Unit, bleeding out and organs crushed, he experienced several spiritual manifestations. One of these was an encounter with God. But others presented a more ominous story. (Adam@TheWorldview.com) By Jonathan Clark Eight Christians attacked in India by radical Hindus A radical Hindu group recently attacked eight Christians in India's central state of Chhattisgarh. The group beat the Christians in the street as they returned from their weekly worship service. The injured Christians had to go to the Intensive Care Unit, including their pastor. He suffered the worst injuries. And it's not the first time he faced an attack like this. Yet, he has continued to serve his church for years. One local Christian told International Christian Concern, “The persecution has increased after the new Government formed in the state. The Christian community has been going through psychological trauma, as both radical Hindu nationalists and the successful administration of the state target the Christian minorities.” British Parliamentarians opposed a ban on so-called “conversion therapy” Earlier this month, the House of Lords in the British Parliament debated a bill that would ban so-called “conversion therapy.” Christians warn the bill could be used to punish parents, therapists, and pastors who want to help individuals overcome unwanted sexually perverted desires and identities. Remarkably, parliamentarians opposed the bill. The U.K.-based Christian Concern shared, “This was the first time in British history that more politicians in Parliament opposed a conversion therapy ban than supported one. This was also an international first, as this has never previously happened elsewhere either.” COVID vaccine recipients had neurological, blood, and heart-related issues The Global Vaccine Data Network released the largest global study on COVID-19 shots this month. The study looked at nearly 100 million people who got the shot. It covered the countries of Argentina, Australia, Canada, Denmark, Finland, France, New Zealand, and Scotland. The research found people who got the shot from Pfizer, Moderna, or AstraZeneca had increased risk for certain adverse effects. Adverse events included neurological, blood, and heart-related issues. Trump hinted at possible VP pick On Tuesday, former President Donald Trump spoke at a Fox News town hall event hosted by Laura Ingraham. Trump confirmed several names that are on his vice presidential shortlist. The names include entrepreneur Vivek Ramaswamy, Florida Governor Ron DeSantis, South Carolina Senator Tim Scott, Florida Rep. Byron Donalds, South Dakota Governor Kristi Noem, and former Hawaii Rep. Tulsi Gabbard. INGRAHAM: “Are they all on your shortlist?” TRUMP: “They are.” INGRAHAM: “And when can we expect that you will announce your choice.” TRUMP: “Honestly, all of those people are good. They're all good. They're all solid. And I always say I want people with common sense because there's so many things happening in this country that don't make sense. Who wants an open border? Who wants high interest rates? Who wants all electric vehicles?” Capital One is buying Discover Capital One Financial agreed to acquire Discover Financial Services on Monday for $35.3 billion. The deal would merge two of America's largest credit card issuers. If completed, the acquisition would create the sixth largest bank in the U.S. and the largest credit card issuer. The deal would give Capital One ownership of one of the four major credit card networks. The other networks are Visa, Mastercard, and American Express. George Barna: "America needs spiritual renewal more than political majority” And finally, Dr. George Barna released his latest report, titled “America Needs Spiritual Renewal More than a Political Majority.” With the upcoming presidential election, many hope a political leader can unite the country. However, the report noted Americans are very fragmented based on their worldview beliefs: “A majority of adults possess the same points of view in relation to less than one-third of the 53 unique worldview indicators in the Inventory.” Dr. Barna wrote, “Placing the burden of uniting this divided a country on the shoulders of a president or political party is unrealistic. Only a spiritual awakening can usher in the required tidal wave of spiritual wisdom and commitment, and that kind of renewal does not happen overnight or randomly.
