This “MOGcast” edition of the “Ask the Expert” podcast series is a collaborative episode titled, “MOGcast 2: Understanding Cortical Encephalitis.” Dr. Eoin Flanagan and Dr. Cristina Valencia Sanchez joined Julia Lefelar of The MOG Project and Dr. GG deFiebre of SRNA to discuss cortical encephalitis, its symptoms, and the connection to MOG antibody disease (MOGAD) [00:04:21]. Audience members asked about the distinction between ADEM and cerebral cortical encephalitis, their treatments, diagnostic methods, and long-term impacts [00:35:34]. Dr. Flanagan and Dr. Sanchez agreed that the preventive treatment approach remains similar regardless of the MOGAD phenotype [00:40:36]. The discussion touched on recent studies on the diagnostic utility of MOG antibody testing in cerebrospinal fluid, and ongoing research on treatments, including clinical trials for developing FDA-approved medications for MOGAD [00:43:05]. Dr. Flanagan and Dr. Sanchez addressed community questions on fulminant cortical involvement cases [00:50:00], the long-term effects of Rituximab treatment [00:51:23], anxiety attacks and mood swings in ADEM [00:53:34], and treatment decisions based on antibody levels [00:54:49]. Eoin Flanagan, MB, BCh is a Professor of Neurology and Consultant in the departments of Neurology and Laboratory Medicine and Pathology at the Mayo Clinic (Rochester, MN). He completed his medical school training at University College Dublin in Ireland in 2005. He did a medical residency in Ireland and then completed neurology residency, fellowships in neuroimmunology and a masters in clinical and translational science at Mayo Clinic (Rochester, MN). He works in the Autoimmune Neurology and Multiple Sclerosis Clinics and the Neuroimmunology Laboratory at the Mayo Clinic. His clinical expertise and research are focused on inflammatory myelopathies and their imaging patterns, myelin oligodendrocyte glycoprotein (MOG) antibody associated disorder, neuromyelitis optica spectrum disorders, autoimmune encephalitis, paraneoplastic neurologic disorders, and multiple sclerosis. He is principal investigator on an NIH RO1 grant studying MOG antibody associated disorder. Cristina Valencia Sanchez, MD, PhD is an Assistant Professor of Neurology and Senior Associate Consultant in the Department of Neurology at the Mayo Clinic (Phoenix, AZ). She completed her medical school training and PhD in Neuroscience at the Universidad Complutense de Madrid. She did a Neurology residency in the Hospital Universitario Clinico San Carlos and then completed Neurology residency and fellowships in ARZ Multiple Sclerosis and RST Autoimmune Neurology at the Mayo School of Graduate Medical Education, Mayo Clinic College of Medicine, in Arizona and Minnesota. The research interests of Dr. Valencia Sanchez focus on autoimmune disorders involving the central nervous system. These include neuromyelitis optica spectrum disorders, myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), autoimmune encephalitis, and paraneoplastic neurological syndromes. She is particularly interested in the neurological complications of immune checkpoint inhibitor cancer immunotherapy. The clinical trials that she leads at Mayo Clinic in Arizona are among the first studies that may lead to approval of new targeted therapies for MOGAD and autoimmune encephalitis. Additionally, Dr. Valencia Sanchez's clinical research allows for increased recognition of autoimmune neurological disorders. Also, her work is helping to avoid misdiagnosing autoimmune encephalitis in the clinical setting. Her research leads to earlier diagnosis and appropriate treatment to ultimately improve patient outcomes.

In this episode of the Postural Restoration Podcast I am joined by Joan Hanson who is a Physical Therapist located in Sioux Falls, SD. Joan has been practicing physical therapy for 30 years and for most of those years has done so with Sanford Health Systems. As we navigate her career and how PRI has shaped and directed it, it is clear to see why Joan was awarded the PRI Directors Dedication Award this past April for her ongoing commitment to this science. In 1992, she earned her Master's degree in Physical Therapy from the Mayo School of Health-Related Sciences in Rochester, MN. Joan started her private practice, Physical Therapy Solutions, in 1999 after several years in various outpatient settings.Later that year, Joan was introduced to Protonics  and encouraged to take a course. Because of a prior knee injury, on the right side, Joan was selected as a model in this course and sensed what a left hamstring could do for her right sided knee pain for the first time. Joan discusses how this first encounter with asymmetry and using both sides of the body to treat it, shaped her practice from the beginning. In 2001 Joan took her first Myokinematic Restoration course and began applying the science with her patients. Over the following two decades Joan would go on to complete over 30 PRI courses, and discusses how this journey has directly shaped her clinical practice.In 2008 Joan sold her practice to Sanford Health. This partnership has allowed her and her clinical staff to continue to use PRI  over the years. We discuss both the challenges and opportunities this partnership has provided as well as some of the people that were involved along the way. As she continued to run her practice under the lens of PRI based treatment, Joan has mentored and collaborated with many clinicians introducing asymmetrical concepts, breathing and positional sense along the way.Although she had this community around her, the science was in its early years and the resources that exist today were not all always available. As a way to get more involved with the PRI community and other clinicians apart of it, Joan completed her PRC certification in 2005 as part of the second class. Joan goes on to discuss how this and other opportunities allowed her to stay connected to the science. Throughout the years Joan has taken advantage of nearly every opportunity available to ask questions, stay connected, and continue to learn through not only course offerings but also the many other resources that have since been created. We discuss how these resources, such as the PRIVY platform, PRI Google group, and many trips to Lincoln have helped Joan answer her questions over the years. Early on Joan recalls taking advantage of one-on-one clinical time spent alongside Ron Hruska, seeing first hand how each repositioning activity was meant to be performed and who they were meant for.As Joan continued to incorporate PRI activities with her patients, she discusses the importance of relying on the tests to confirm ones ability to alternate, breathe and sense both sides of their bodies. These basic concepts are often the start point of her treatment and we discuss the importance of “the process” and how often times these concepts can be overlooked by trying to progress through integration of systems too quickly, or with patients that may not need integration of multiple systems. Throughout the years Joan has not only desired to learn and grow with this science individually, but has always attempted to share and help those around her learn and utilize it as well.  This has increased her interest in things like dance, sleep, audiology, boxing, and many others as she integrates PRI into her everyday life.

Did you hear about the study of 116 professional cyclists to see who had back pain? 58% had experienced lower back pain in the previous 12 months. 41% had sought medical attention for back pain. Unfortunately, even if you are not a professional cyclist sitting on your bicycle for 20 or more hours a week, you ARE sitting in a chair and/or a bike for 50, 60 or more hours a week, and year after year. After so many years of sitting, our bodies have adapted to the sitting position, making us older athletes susceptible to injury and pain, and making us weaker on the bike and in life. There is no time like the present to get the wheels of progress working in the right direction. A cyclist with a healthy back is a strong cyclists. I am joined today by Doug Hanna, President of Physical Therapy of Boulder (ptofboulder.com), the largest private practice in Boulder County. Doug brings over 30 years of clinical experience to his clients as well as a deep education in the field. Doug received a Bachelor of Science degree from Nebraska Wesleyan University and a Graduate Certificate in Physical Therapy from the Mayo School and Health Related Sciences in Rochester, Minnesota. Doug later completed Certification in Competency in Manual Therapy from the Institute of Graduate Health Sciences and a post-professional Masters of Science degree from the University of St. Augustine in Saint Augustine, Florida. Doug can relate to the masters athlete because he is one. Doug rides and competes in gravel cycling, MTB, and bike pack races. Doug knows well the subject of back pain for cyclists. A Co Mayo primary school which closed over Covid-19 concerns will reopen today following instructions from the Department of Education.  Claremorris Boys National School told parents it would close its doors after a dozen pupils  tested positive for Covid-19 since the school reopened after the mid-term break. Now many of the parents in Claremorris are reluctant to send their children into the school today and are choosing to keep them at home. One of those parents is Catherine Walsh, who joined us on Breakfast followed by the schools principal, Mark Loftus. ABRAMOWITZ BIOGRAPHY Dr. Abramowitz joined the Clinical Psychology faculty at UNC-Chapel Hill in 2006. He is currently Professor and Associate Chair of Psychology, as well as Director of the UNC  Anxiety and Stress Disorders Clinic. His previous positions include Associate Professor at the Mayo Clinic and Mayo School of Medicine, and Instructor at the University of Pennsylvania. His research interests focus on anxiety disorders (mainly obsessive-compulsive disorder [OCD]). He teaches graduate and undergraduate courses at UNC and also trains clinical psychology graduate students in how to implement state-of-the-art assessment and treatment techniques for anxiety/OCD. He has published over 150 research articles, book chapters, and encyclopedia entries, along with 12 books on anxiety, OCD, and related topics. He is Editor-in-Chief of the Journal of Obsessive-Compulsive and Related Disorders, and Associate Editor of Behaviour Research and Therapy. He is a member of the International OCD Foundation Scientific and Clinical Advisory Board.
