When someone becomes a victim of a crime, a long and often overwhelming process towards justice begins. It can be extremely difficult and taxing for anyone but when people with disabilities, especially those with intellectual/developmental disabilities (IDD), enter this process, barriers and systemic failures might mean that they never get to see the justice they deserve. This documentary exposes these failures through the lens of advocacy and victim support providers.

Jake Haro Case: Castration, Competency Tests & Child Safety: Should Society Go Further? What does it mean to have the capacity to parent? In this gripping segment, Tony Brueski and psychotherapist Shavaun Scott wrestle with the uncomfortable truth: not everyone can or should be raising children. The Haro case sparks a brutally honest conversation about America's obsession with “positive thinking” — the idea that anyone can change if they just try. But when it comes to protecting kids, blind faith is a death sentence. Jake Haro attended parenting classes, checked the required boxes, and was rated “low risk.” Weeks later, his baby was dead. Tony and Shavaun explore the difference between positive thinking and magical thinking, questioning why courts continue to assume abusers can be rehabilitated with a weekend class or compliance checklist. The discussion veers into dystopian territory — competency tests for reproduction, mandatory castration for repeat abusers — but the underlying point is serious: we are giving violent, unfit parents chance after chance, while children pay the price. This segment is provocative, raw, and absolutely necessary. It challenges viewers to confront uncomfortable questions about parental rights, societal responsibility, and whether protecting children should come before protecting the freedom of abusers. In this episode, we speak to Dr Diarmuid Quinlan, a GP and MD candidate based at the Department of General Practice at University College Cork.Title of paper: Competencies and clinical guidelines for managing acne with isotretinoin in general practice: a scoping reviewAvailable at: https://doi.org/10.3399/BJGP.2025.0135There is evidence of inequitable access to the most effective treatment for severe acne, isotretinoin. This scoping review identified the clinical competencies to safely manage acne using isotretinoin. No global consensus exists among clinical practice guidelines (CGPs) on whether GPs are appropriate prescribers of isotretinoin. Appropriately resourced and CPG-guided patient access to isotretinoin in primary care may promote safe, timely, and equitable acne management for patients and improve antimicrobial stewardship.Transcript:This transcript was generated using AI and has not been reviewed for accuracy. Please be aware it may contain errors or omissions.Speaker A00:00:01.440 - 00:01:07.850Hello and welcome to BJGP Interviews. My name is Nada Khan and I'm one of the associate editors of the bjgp. And welcome to our autumn edition of the BJGP podcast. We're kicking off with a new set of interviews for the next few months. So thanks again for joining us.Today we're speaking to Dr. Dermod Quinlan, who is a practicing GP in Cork and is also an MD candidate at University College Cork in Ireland. We're here today to discuss his paper, recently published in the BJGP titled Competency and Clinical Guidelines for Managing Acne with Isotretinoin in General Practice. A Scoping Review. So thanks very much, Dermid, for joining me here today to talk about this paper. But yeah, I guess I just wanted to start by saying that this is a really interesting paper and I think it covers a very common condition that we see in general practice and covers treatment, which can be quite difficult as well for acne. But I wonder if you could just start by telling us a little bit about why you wanted to do this research and just a bit about the treatment of it and why you focused down on this topic, really.Speaker B00:01:09.610 - 00:02:59.510So lovely to meet you, Nada. I'm first and foremost a GP and I see patients three days a week, 20 hours a week. And I did a diploma in dermatology over a decade ago and I still do some online tutoring. So I have a long standing interest in dermatology and have an extended role in dermatology.I work in an urban practice with lots of young teenagers and young people in it.Acne is a common chronic disorder and I would see a lot of young people with acne of all grades of severity, mild, moderate and severe, and very severe. And as a clinician, very clearly recognize that behind acne is a patient very commonly suffering profound distress. And we know that the morbidity associated with acne and particularly severe acne, is very extensive. There's the emotional morbidity, there's psychological morbidity, it impacts people's employment opportunities, their education achievements, and then more widely, because treating acne is resource intensive, it has an impact on the healthcare workforce. And then there are concerns about the very prolonged use of antibiotics in acne, raising real antimicrobial stewardship concerns. So I have an interest in this. And then we decided that we would do research into it because we don't know the clinical competencies for safe use of isotretinoin. So I was particularly interested in severe acne and the management of severe acne, and also it didn't clearly...

Business knowledge is what changes.»In this episode of MetaDAMA, we dive into the data strategy of Statens Vegvesen with Bjørn Broum. With 30 years of experience in the data field, Bjørn shares valuable insights.Bjørn takes us on the journey from Statens Vegvesen's first data strategy to today's revised version and explains why a strategic approach to data is essential for success. The conversation also touches on critical questions around privacy, skills development, and organizational challenges. Bjørn shares his most important lesson: keep technical complexity low in the early stages, focus on demonstrating value, and remember that decisions are made with or without data, so make data available when it's needed! Here are some key takeaways:We need a mindset shift in organizations. Think about the following:You need to keep focus on operational success, that is what you are measured by. You need to understand how you can impact your architecture, before the actual impact happens.New services needs to be build around data. Data cannot be an afterthought,You need to work actively with your executive management to ensure that data becomes a stable, not something that is hype-driven.Your data infrastructure is what you need consistent focus and buy-in on. This is the foundation that can support the «hype» on top of it.Statens VeivesenLong standing tradition dating back to 1864, tasked to build and maintain road infrastructure in Norway.Maintaining 10.500 km of road infrastructure, including about 6000 bridges and 600 tunnels.Utilization of sensors to predict risks and impact of e.g. rockfall, snowdrift, avalanches that all could impact traffic and road security.Research on how Norway can prepare for autonomous vehicles and traffic safety.Modern cars can be used as sensors. You can e.g. get information on how many people are in a car. There is a focus on ensuring personal data protection while maximizing insights from data.Ensure that the organization develops in a direction that can address and handle new challenges (data-driven).Remember that decisions are made with or without data. If you want data-informed decision data needs to be available for decision making, when needed.Data StrategyIt's a living strategy, that needs to stay updated.Data Strategy is always designed to accompany for a need, and that need can change over time.Data Strategy needs to hold a big-picture that people can relate their work to, that is recognizable.Digital value chain needs to be understood also towards the cross-functional needs across the organization.Conways law needs to be recognized. The value of data is realized in business. Therefore Data Strategy is build on business objectives.Be deliberate with your strategy: what do you need to focus on? For veivesenet this was e.g. to establish a common language and vocabulary.Statens Veivesen also makes data accessible for research. This needs to be reflected.AI is a topic as well, including data readiness for AI.Competency and skills are an important part of data strategy.Don't focus on dat roles, but rather upskilling in your organization.Understanding boundaries, goals, and strategic priorities is key when working with data strategy.Scaling and dimensions need to be strategically understood.Focus on the value of data and the business value it potentially generates, not technology.Bjørns' blog

Send us a textIn this powerful conversation, Hannah Elmore, Senior Clinical Outreach Coordinator for the IAFF Center of Excellence, reveals the critical role of culturally competent care in supporting firefighter mental health. Drawing from her extensive background in clinical social work and deep immersion in fire service culture, Hannah illuminates the often-overlooked nuances of how mental health challenges uniquely manifest in first responders. The discussion takes us behind the curtain of the IAFF Center of Excellence, a specialized treatment facility created specifically for firefighters seeking help with behavioral health challenges. Unlike traditional hospital settings, this 15-acre facility offers various levels of care in an environment where firefighters can heal among peers who truly understand their experiences. Hannah paints a vivid picture of the Center's approach, from clinical programming to the nightly gatherings around the fire pit—a space intentionally kept free from clinical staff where some of the deepest healing occurs.Perhaps most compelling is the honest examination of barriers that prevent firefighters from seeking help. Hannah addresses head-on the fears about confidentiality and career impact that keep many suffering in silence. "If you're sweeping things under the rug," she cautions, "eventually the rug is going to get a really big mountain that you're going to trip over." This metaphor perfectly captures the cumulative weight of unaddressed trauma and stress.Steve and Hannah also discuss the frustrating reality of therapists who lack the necessary understanding of first responder culture. They share stories of firefighters who've had negative experiences with providers who were overwhelmed by their trauma narratives or misinterpreted symptoms due to cultural ignorance. Their passion for connecting first responders with appropriate, vetted resources shines through as they emphasize their collaborative approach to resource sharing.Whether you're a firefighter struggling with your mental health, a peer support member looking to better assist your colleagues, or a clinician wanting to better serve the first responder community, this episode offers invaluable insights into creating pathways to healing. We previously released this episode on December 18, 2024. We're excited to revisit some key ideas before we dive into fresh, exciting topics soon. In this episode, Ethan Nash introduces the Challenge-Competency Continuum, a framework for understanding how stress impacts employee productivity. Picture an upside-down “U” with productivity on the y-axis and stress on the x-axis. Too little stress leaves employees unmotivated; too much stress overwhelms them. Peak productivity? It's right in the middle. The key to finding that balance, Nash says, ask your employees. Ethan dives into how understanding where someone lands on the continuum—how “challenged” and “competent” they feel—can help managers make smarter decisions. With the right insights, leaders can fine-tune workloads, reduce unnecessary stress, and unlock their team's full potential. To get our Effective and Enjoyable Meetings Guidebook, send a note to contact@nashconsulting.com. Text the word “LEADING” to 66866 to be added to Nash Consulting's monthly newsletter. What does it really mean to grow as a disciple of Jesus and are we starting in the right place? In this episode, Chris, Jacob, and Jessie Cruickshank unpack the “Five Cs” of discipleship: Christ, Community, Character, Calling, and Competency. Together, they explore why starting with a skills assessment or plugging people into ministry roles based on talent can short-circuit the deeper work of identity formation in Christ.Jessie draws from neuroscience, her experience training church leaders, and even jellyfish biology (yes, really!) to help us reimagine discipleship as a relational, community-driven journey of becoming. Character is shaped in community, calling is discerned in relationship, and competency should be the last step, not the first.ORDER Jessie's newest book, Ordinary Discipleship: How God Wires Us for the Adventure of Transformation → https://a.co/d/51j86DGFor more great stuff, check out: Ordinary Discipleship by Whoology: https://whoology.coFollow us on social media:https://instagram.com/ordinarydiscipleshiphttps://facebook.com/ordinarydiscipleshipFollow Jessie on social media:Instagram: https://instagram.com/yourbrainbyjessFacebook: https://www.facebook.com/jessica.s.cruickshank/Twitter: https://twitter.com/yourbrainbyjessJessie Cruickshank is a disciple-maker, wilderness guide, and ordained minister. With technical education under pressure to deliver skilled talent faster, Lakeshore College is flipping the model—measuring learning by competency, meeting students where they are (even in high school), and using grants + national advocacy to scale the impact. In this episode of The TechEd Podcast, host Matt Kirchner talks with Dr. Paul Carlsen, President of Lakeshore College, about the different methods to rapidly upskill a technical workforce. Like Lakeshore's competency-based education (CBE) approach, which has now been adopted by half the college's programs and earned national attention and a White House conversation on scaling CBE.In addition to CBE, we discuss Rocket Academy's early manufacturing pathway for high school students, why industry certifications (SACA) matter (because employers say they matter), and how the college has leveraged $32M+ in competitive grants to fund innovation. Carlsen also explains his work on the AACC Board—including the coming impact of Workforce Pell on short-term training.Listen to learn:Why competency-based education is reshaping how we recognize skillsHow high schoolers are earning real college credentials before graduationWhat makes an industry certification valuable—according to employersThe mindset shift that helped Lakeshore win $32M+ in competitive grantsHow new federal policies could redefine workforce training programs3 Big Takeaways from this Episode:1. Competency-based education measures student success by skill mastery, not seat time. Lakeshore's shift to CBE began with an employer who couldn't wait 16 weeks for students to complete a course. Today, students demonstrate mastery through real-world assessments, faculty build flexible learning pathways, and program delivery adapts to the individual needs of working adults, veterans, and learners balancing multiple responsibilities.2. Through innovative partnerships, high school students are graduating with a college degree and workforce-ready credentials. Rocket Academy gives high school students access to a full integrated manufacturing pathway, often leading to a technical diploma before they even receive their high school diploma. The curriculum is designed to fit directly into the school day, removing scheduling barriers and building true college transcripts—not just college “experience.”3. Grant funding follows big ideas and execution capacity. Lakeshore has secured over $32 million in competitive grants, including major awards from the National Science Foundation. Success comes from proposing bold, high-impact projects with measurable outcomes, and from having the internal systems and cross-functional teams in place to deliver those outcomes on time and at scale.Resources in this Episode:To learn more about Lakeshore College, visit: lakeshore.eduConnect with Paul Carlsen on LinkedInOther resources:Rocket AcademyWhat is competency-based education (We want to hear from you! I'll never forget Bruce's story about his car—check engine light on, a mechanic insisted it needed a $1,500 catalytic converter. Bruce knew better and fixed it by simply tightening the gas cap. That story isn't just about auto repair; it perfectly illustrates why questions a good financial advisor should ask matter. Without probing, you might be sold something you don't need. Competency—not just good intentions—matters. https://www.youtube.com/live/oyEbgdU1MGI It's not about distrust—it's about asking the right questions so you're not blindly following advice. And that principle applies fully when choosing a financial advisor, especially when your spouse might need to take over the reins someday. Why “Questions a Good Financial Advisor Should Ask” Are Essential1. The Big Picture: Comprehensive Financial Planning2. Spouse Financial Preparedness: Including Both of You3. Risk and Protection: Insurance, Deductibles, and Peace of Mind4. Tax Strategy and Social Security Planning5. Legacy Planning: Aligning Values and Wealth Transfer6. Financial Alignment Between SpousesWhy You Need These QuestionsReady to Empower Yourself With Questions a Good Financial Advisor Should Ask?Book A Strategy Call Why “Questions a Good Financial Advisor Should Ask” Are Essential Bruce makes a powerful point: finance isn't limited to investment products. Just like a mechanic or doctor examines the whole system, a skilled advisor should ask questions that uncover your entire financial ecosystem. Without comprehensive inquiry, blind spots linger—insurance gaps, overlooked risks, or hidden fees can derail your legacy. Are you unknowingly trusting a financial advisor without knowing enough about your overall financial picture? In today's complex financial world—from taxes and Social Security to estate planning, insurance, and cash flow—a narrow focus on one product is risky.Questions a good financial advisor should ask aren't optional—they're essential. They give you clarity, align planning with your goals, and ensure your spouse is equipped to manage your shared financial future. 1. The Big Picture: Comprehensive Financial Planning Bruce sums it up: “You cannot make financial decisions in a vacuum.” Advisors who focus only on investments or insurance miss how those decisions affect cash flow, taxes, estate planning, and more. Ask: What are your current net worth and cash flow statements? How do your investments, insurance, and debts interrelate? Why it matters:Like a doctor who reviews your medical history before prescribing treatment, a competent advisor will want to see your full financial picture before making recommendations. 2. Spouse Financial Preparedness: Including Both of You Too often, one spouse is left out of discussions and can feel lost if the other dies.Key questions include: Who are your trusted advisors (financial, legal, tax)? Does your spouse know how to access online accounts, passwords, and digital assets? What's your “Alternative Income Plan” for the surviving spouse? How comfortable is your spouse with the household financial framework? Bruce and Rachel discuss this as part of the LIFE framework: Liquid assets—money accessible within 15 minutes Income plan—monthly income goals Flexible investments—capital that can be reallocated Estate plan—how wealth transfers to future generations Both spouses should discuss and agree on how these pieces look today and tomorrow. 3. Risk and Protection: Insurance, Deductibles, and Peace of Mind Bruce shared his own experience with PNC: they asked about deductible choices and emotional tolerance for risk during the house fire recovery process.Essential questions a good financial advisor should ask include: What insurance do you have—life, disability, health, auto, home? Are deductibles appropriate to your cash reserves and risk tolerance? Are beneficiary designations updated and aligned with estate go...

As a board-certified anesthesiologist with decades of experience, Dr. Marn reveals the critical safety gaps in dental practices and how his journey from academic medicine to founding Blue Pacific Medical Simulation led him to address this overlooked public health issue.Dr. Marn introduces us to the fascinating "beehive versus lung" concept – explaining how hospital teams function collaboratively like beehives during emergencies, while dental practices often rely on the dentist alone (the "lung") to handle everything. This fundamental difference in approach creates dangerous vulnerabilities when seconds matter most. The heart of the episode explores Dr. Marn's innovative Four Pillars of Sedation Emergency Readiness: Infrastructure, Competency, Culture, and Habit. Beyond just having the right equipment or knowing CPR, these pillars address psychological safety, team dynamics, and creating systems where everyone knows their role in a crisis. Through compelling examples and practical insights, Dr. Marn demonstrates how dental practices can move from simply checking regulatory boxes to building genuinely effective emergency response capabilities.Perhaps most valuable are the actionable steps dental teams can implement immediately – from five-minute huddles that create psychological safety to using Dr. Marn's free online assessment tool (emergencyscorapp.com) to identify specific practice vulnerabilities. His approach acknowledges the realities of busy dental practices while providing a clear framework for meaningful safety improvements.Whether you're a dental professional looking to enhance your practice's safety protocols or simply someone curious about what happens when things go wrong in healthcare settings, this episode offers profound insights into saving lives when every second counts. Ultimately, by focusing on both competency and consistency, an organization is aiming for a culture of continuous improvements that can be both measured and felt.Culture is how we do things around here which implies both competency and consistency in those “chosen things.”Competency relates to the quality of practice.Consistency is about the frequency of practice.A starting point is defining what it means to be both competent (how) and consistent (how often) in each required practice or standard.Understanding the 'why' behind practices enhances efforts to achieve both competency and consistency.Training, mentoring, and coaching are key to elevating competency.Accountability tactics are key to gaining and maintaining consistency. The long game in culture change requires sustained efforts focused on both competency and consistency of “how we do things around here.”Need help improving the culture, performance, and results of your healthcare organization? A New Approach to Competency to Stand Trial | OAS Episode 236

A core tenet of the U.S. legal system is that an accused person must be competent to stand trial, which means a defendant can understand the charges against them and assist their lawyer in preparing a defense. In practice, defendants who need to be evaluated for competency often spend long periods in confinement because of a shortage of behavioral health services. When they are finally released, it's usually without ongoing care. On this episode, we spoke with three people who are involved in efforts to reform the process used to determine a defendant's mental competency: Indiana Supreme Court Chief Justice Loretta Rush and two Colorado legislators, Senator Judy Amabile, a Democrat, and Senator Byron Pelton, a Republican.Chief Justice Rush explained the dimensions of the problem and discussed how a national task force on the issue developed. Policy recommendations. Senator Amabile, who has sponsored legislation on the issue talked about how Colorado ended up with a significant backlog of cases. Senator Pelton explained some of the unique challenges people in rural America face when dealing with behavioral health issues. Resources“Legislative Primer Series on Front-End Justice: Competency to Stand Trial,” NCSL“Juvenile Justice: States with Juvenile Competency Laws,” NCSL

L&D has long been focused on behaviour change—but what if that’s the wrong goal? In this episode, Carl Binder challenges conventional wisdom and makes the case for shifting our focus from behaviours to work outputs—the measurable accomplishments that drive business results. Carl explains why this approach is more effective, how L&D teams can correctly identify and measure accomplishments, and what it takes to implement this thinking at scale. He also shares practical strategies for redesigning onboarding, leveraging non-training interventions, and aligning managers to focus on performance outcomes rather than just competency models. For L&D professionals looking to elevate their impact, this conversation offers a roadmap to making learning more business-critical, measurable, and effective. Take your L&D to the next level Take advantage of thousands of hours of analysis. Hundreds of conversations with industry innovators and 25+ years of hands-on global L&D leadership. It's all distilled into one framework to help you level up L&D. Access the L&D Maturity Model here - https://360learning.com/maturity-model KEY TAKEAWAYS When you focus on results, L&D delivers. There is no incompatibility with focusing on work outputs and changing behaviours, one leads to the other. Competency models are too subjective. The relationship between organizational results, outputs and behaviour is dynamic. When managers focus on what needs to be produced, providing feedback to change behaviours is easier. Each individual should have a performance map. Ask yourself if this training were successful what is it that I expect the participants to be able to produce? Stop asking what skills they will learn. This simple change in language is powerful. Fully map your processes. Use the Six Boxes® model. BEST MOMENTS “Figure out the purpose before you define the behaviour.” “Don´t just spray and pray.” “Behaviour might change but accomplishments are typically more stable.” “There is a “the emperor has no clothes” problem here.” Carl Binder Bio Carl Binder is a leading expert in performance improvement and the founder of The Performance Thinking Network. With a background in behavioural psychology and decades of experience in organisational learning, he has helped businesses worldwide shift their focus from behaviour change to measurable work accomplishments. Originally a student of B.F. We talk a lot about differentiating instruction, measuring growth, and preparing students for the real world, but how do you actually do that in a system still driven by grades? Maybe you need a new model altogether. In this episode, we're exploring an approach to school called competency-based learning. I'm joined by three educators — Susie Bell, Heather Messer, and Beth Blankenship — who show us what this model actually looks like in classrooms. They share real teaching examples, how they assess growth over time, and how the Mastery Transcript Consortium supports this work. Thanks to Zearn and EVERFI for sponsoring this episode.
