Das Erythema nodosum wird als allergische Überreaktion (Typ III) der Haut angesehen, die im Zusammenhang mit Sarkoidosen, Infektionen, entzündlichen Darmerkrankungen, rheumatischem Fieber oder Arzneimitteln auftritt.

#018 - Wynke Haanstra's health journey has involved many gut and digestive issues, including Crohn's disease, colitis, chronic constipation and inflammatory bowel disease (IBD) from a fairly young age. And while it was problematic, it only got worse as she got older. As her health continued to deteriorate, she was plagued by a variety of other symptoms, such as extreme acne, Erythema Nodosum - which is a condition where someone develops lumps on their lower legs, and a misdiagnosed fistula as a result of an infection. Erythema nodosum is a symptom complex characterized by tender erythematous nodules that typically appear on the extensor surfaces of the lower legs (but can appear on the arms, face, and trunk). The lesions should range in color from pink to red and be between 1-10 centimeters in diameter. The malady usually lasts for six weeks and can be associated with pregnancy, oral contraceptive use, primary coccidioidomycosis, streptococcosis, tuberculosis, syphilis, diverticulitis, Y enterocolitica, Yersinia pseudotuberculosis, and other deep fungal infections. Erythema nodosum can also be accompanied by inflammatory bowel disease, Behcet disease, or sarcoidosis. The tender erythematous nodules can be preceded by arthralgia, malaise, and fever. The practitioner should evaluate the patient's medication exposure history, and conduct a physical examination for a chest radiograph, a PPD, two consecutive ASO/DNAse B titers at 2 to 4 week intervals, diarrheal illness, symptoms of any deep fungal infection specific to the area, and prior upper respiratory infection. In its late stages, erythema nodosum must be distinguished from simple contusions and bruises. The patient can be treated with NSAIDs which typically takes the form of potassium iodide (5-15 drops three times daily), corticosteroid therapy, dapsone, colchicine, or hydroxychloroquine. Complete bed rest should be ordered if the lesions are painful. The reader should note that erythema nodosum differs from other forms of panniculitis in that it does not ulcerate. Tim Francis is an entrepreneur from Edmonton, Alberta, Canada. He is the owner of Tim Francis Marketing and the founder of Profit Factory. Tim has a BPE degree from the University of Alberta, and a Certified Scrum Product Owner. Tim is a sought out consultant and speaker in entrepreneurship, marketing, and systems. Tim has been an entrepreneur for over 10 years now. His first business was in 2001 when he was a franchisee of College Pro Painters. In 2009 Tim was awarded one of Edmonton's coveted “Sizzling 20 Under 30” awards as one of his city's brightest up-and-coming business leaders. In March 2015, Tim was a guest lecturer at NYU in New York City. In July 2015, Tim spoke atop a mountain in Utah at the exclusive, invitation-only Baby Bathwater mastermind. A perfect storm of stressful events were major contributors to Tim falling dramatically ill in 2011 with a case of Erythema Nodosum. He was unable to walk or work for three full months. Amidst searing frustration, bitter disappointment and creeping self-doubt, Tim was forced to scrap everything and start from scratch. The soul-searching of those three months, and the businesses birthed from that crippling time have led to a business renaissance for Tim, enjoying greater success than ever before. Away from business, Tim's been a multi-award-winning athlete in both High School (ESSMY in St. Albert) and at Grant MacEwan College in Edmonton. Tim was selected to the prestigious House of Commons Page Program (Ottawa) in 2000. Tim obtained a Health Promotion degree from the University of Alberta in 2006. I am writing to you because my mother in law has just been diagnosed with Huntington’s Disease, her symptoms are not very severe yet, some cognitive issues, OCD tendencies etc and she physically can’t stay still. She lives by herself and struggles to maintain motivation to keep a balanced diet. Could you please let me know what results your clinic has been able achieve in treating this condition? We would like to keep realistic expectations.Could you also please let us know what tests are recommended for her to take in order of importance, as my mother in law has not been able to work for many years and is on a very tight budget we would really appreciate your efforts to give us the most cost effective, value for money plan of attack. I have done fertility treatments for the last 3 years and feel this has impacted my body negatively. I use to have high testosterone and now have low testosterone low estrogen and low progesterone. I know Dr. Cabral says supplements should be taken as you need them and not on a continuous basis. Cara: I've been told by my gastroenterologist during a recent colonoscopy exam that my colon is very twisty and that is why I experience constipation. I never had constipation growing up and I only recently have had constipation in the past couple years so how did my colon get twisted all of a sudden and if it was always that way, then why didn't I experience constipation my whole life? I am a lean 50 year old female who exercises almost every day. I had methane dominant sibo in the past and still do a low-moderate FODMAP diet. I eat plenty of vegetables daily and do a celery juice every morning. What other suggestions do you have and will any untwist my colon and what caused it to get twisted in the first place? Thank you - love the podcast as I am a licensed nutritionist myself. Jennifer: I don’t know where to begin. I listen to your pod cast from time to time and I’ve got your book- the rain barrel effect. I’ve done your detox- I was able to do 2 weeks this past July. I’m hoping you can help give me some advice. I was born with lots of musculoskeletal abnormalities. Hip dysplasia, cleft pallet, all my joints are messed up, barrel chest and scoliosis. I’ve made a good life for myself. I’m married- have 2 kids. However in February 2016, I woke up one day and couldn’t walk because I had Vertigo so bad. I was given some medicines by my primary Doctor and was told it would go away within a few days. It didn’t- my condition got worse. Within 2 weeks I also started to lose my hearing. Within 2 months I’d lost 100% of my hearing in my right ear and about 90% in my left. I also have tinnitus start with all that and the vertigo turned to a constant daily unsteadiness- weather I’m sitting, standing or laying. My vision twitched with my pulse too sometimes. I’ve seen probably 20-30 doctors over the last 2.5 years. I’ve had numerous MRI, CAT scans, eye exams, blood work, Allergy tests, food sensitivity, tested for Lyme, and everything else under the sun. Everything came back negative or fine. The thing with this hearing loss is it fluctuates, but the vertigo and tinnitus are constant. My hearing can be affected by weather- if it’s raining or snowing. Thank God I’m in Las Vegas, where we don’t get much of either. I have good days and bad days. Some days I can hear water running and other days not. Sometimes I wake up in the morning and hear great – only to have it fade a few hours later. At one point I was told it was allergies and given 5-6 different meds to take. After 4 weeks there wasn’t any change and I got off all that. In years past I’d only have seasonal allergies- more like hay fever just for 2 weeks in the spring. I know stress affects it I can tell. If I’m dehydrated my vertigo and tinnitus get worse. Also when I drive my dizziness is horrible unless my head is against the headrest of the car seat- then it’s completely manageable and I can drive fine. My main ENT Doctor (I’ve seen 6) at 1st said he thought it was autoimmune related because I also have ulcerative colitis- I have for 20 years. He sent me to a rheumatologist- who said for lack of anything else that it was autoimmune inner ear disease. I was put on high doses of steroids and chemo drugs. When I’d do my research & think maybe I had Addison’s, or something else- she’d order the tests for me. I asked her about diet such as the paleo plan. She had no clue what that was. But during that time I switched myself to an autoimmune paleo eating plan. I did that religiously for 6 months and then switched to a more paleo plan. I also used the treadmill every day during the time since I could barely walk unassisted, but with the treadmill I could hold on and walk. My brain has adjusted where I can walk unassisted even with the constant unsteadiness/dizziness. I also weaned myself off all the toxic harmful drugs after 10-12 months. My rheumatologist was good at indulging me. Anyway, only problem though after about 4 months off I had a bad ulcerative flare. I also had Erythema Nodosum on my legs. My body got so use being on steroids and chemo drugs. So I had to go back on a biologic- Simponi in May of 2017. I injected at 1st every 4 weeks… I’m able to space the injections out to about 8 weeks now. I’m weaning myself off these too. I’m probably 80-85% paleo now. But I’m not perfect. I don’t do caffeine, or soda. I avoid processed foods as much as I can. I’ve also done so many holistic treatments through the last 2 years such as acupuncture, cranial sacral massage, essential oils, meditation, CBD oil, I even recently I tried Kambo. We are currently building a house now. I’ve made sure to get options such as water softener and a reverse osmosis, advanced air filters too in an effort to improve my environment. Deep in my heart I KNOW I will figure this out, and I WILL get my hearing and balance back. I’m just trying to figure out what else to try. Have you ever had a patient it similar situation? Do you have any suggestions what I can try or what you think might be the root cause? The only thing different before all this began is I was having work related stress, and we got a puppy (we already had cats & I’ve had dogs before). I’ve done allergy testing- blood and skin pricks, 2 different times each 1 year apart. They all came back that I wasn’t allergic to anything including dogs/dander. And my workplace stress resolved itself. I’m truly at a loss for what else to try. Any suggestions would be appreciated. Thank you for your time. Maureen: Im a 24yo female with major GI issues due to chronic UTI's and the consequential antibiotics taken for them. Im working with a FMD and did the organic acids test and a GI MAP stool test. We found bacterial overgrowth and candida overgrowth (no parasites), extremely low Vitamin C/Asc Acid (1.0 was the result) and high gut inflammation (calprotectin is at 217 ug/g.) My question is, my hair has been falling out tremendously over the last few months, and I'm wondering what you think the cause may be? To note: my ferritin is low at 21ng/ml. Do you think it's because of the low ferritin? Low Ascorbic Acid? Gut Issues? or just all of the above? It's very scary - I've lost at least 60% of my hair. I'm starting a protocol now to fix my gut and everything else, but I would love your take on the hair loss ordeal. Today, Nicholas interviews Tim Francis, who is an award-winning entrepreneur from Canada, and the founder of Profit Factory and Great Assistant. Tim is a graduate of the University of Alberta, and a Certified Scrum Product Owner. In 2010, Tim was blindsided by a rare illness called Erythema Nodosum, leaving him crippled for three months. After nearly going bankrupt, Tim decided that he was never going to a burned-out entrepreneur again, which lead to the creation of his successful businesses. If it’s something that you’re doing constantly, you dislike it, and it takes time away from actually profiting in your business, you can hand it off to someone else. Dramatically expand the amount of potential assistants that you’re looking at. Don’t hire the first person you meet. Show your assistant how you want things done instead of telling them what to do. Show them examples. Here are today's Q&A topics: Larissa: Hi, I've already tested my DNA through 23andMe. I was wondering if you or your clinic would interpret the raw data that I already have and offer diet/lifestyle recommendations from that. I carry one APOE-ε3 allele and one APOE-ε4 allele. I was wondering if that means a Ketogenic diet (high in healthy fats) would be beneficial or maybe not at all. I've already experimented with Paleo (but I think I was consuming too much meat over all) and that, (in addition to a high stress lifestyle) led to leaky gut, parasites, and bacterial imbalances (IBS symptoms). I experimented with Gluten Free Veganism and consumed a lot of high carb veggies, potatoes, beans, and rice. After doing a parasite cleanse, I did an Organix Acid test and it said I wasn't processing all the carbs. So I think I was over doing it on the carbs and maybe should live a more middle road approach. I'm on the mends from healing my gut finally but was wondering what the best diet would be for me moving forward once I'm healed. Thank you! I really enjoy listening to your podcasts! :) Catherine: Hi Dr. Cabral, I recently graduated as a certified Integrative Health Coach from IIN. One of my first clients is a mysterious case. He says when he was born he came out all black and blue and from that moment till about 20 he was prescribed massive amounts of antibiotics. He was recently diagnosed with Squamous cell carcinoma and had a huge cyst removed from his back. He now says they diagnosed Cellulitis for many gaping wounds on his legs. Now here's the kicker...he works at a health food store and has gone through every protocol to kill this bacteria that is causing severe inflammation. He says he has taken all the probiotics like Syntol, CBD oil, silver, oil of oregano etc. You name it he's done it. He is a hard core Paleo follower only eating organic grass fed beef, lots of fruit and veggies and no processed food. I think he should lean more plant based, and address his Primary Food, like the lack of support system and finances and spirituality. Maybe hypnosis or visualization. I know you had major issues with rebuilding your body after all those antibiotics and I told him you could be a real asset in finding the solution to his underlying fungus issues. My question is regarding my boyfriend, we´ve been dating for four years now and I have to say that sex WAS amazing for the first three years and a frustration over the last year since we moved together, he has very low libido so we have sex once a month and sometimes not even once!! He is 32 years old and lifts weights 3-4 times a week, he participated in body building competitions when he was 16-18 years old and did very strict diets back then, his brother had thyroid nodules and I´m wondering if his low libido has something to do with something physical, maybe adrenal fatigue or thyroid issues (I listened a podcast of yours regarding this). One more thing, he told me it´s been a pattern since he started having sex, his libido is totally fine for no more than two years, after that time it disappears and his old girlfriends with it. Kellie: Hello Dr. Cabral- I started listening to your podcasts about 4-5 months ago and I have to say you really turn much of what we know on its head. I could be asking one of a million questions pertaining to myself or my immediate family members but have finally decided what I want to ask. My 20 y/o daughter has had a very unhealthy year. Lots of upper respiratory infections that didn't quit,cystic ovaries( discovered from painful periods that sidelined her for at least the first day) UTI's; strep; Bronchitis; general malaise- culminating with a Bells Palsy in April and now Erythema Nodosum( which is extremely painful). She has taken on a Vegan diet about 2 years ago- however I have to caveat- she is an extremely picky eater. Potatoes, salad, broccoli, cucumber rolls , pizza with no cheese and Pho comprise the bulk of her diet. Occassionally a fruit smoothie. Fats/ oils actually make her feel physically sick. Lots of OCD like tendencies specific to expiration dates on foods. If we are approaching a date- 3-4 days out; cannot eat. She has seen a neurologist for the Bells - which she had a pretty full recovery. Once in a great while you can see her eyes blink at a slightly different rate. Her GYN prescribes birth control to regulate her hormonal imbalance- evidently she was way out of wack and her periods are 100% improved. She saw a functional doc who did a blood work up and discovered she had extremely low D, iron and B12- she is taking supplements for those. In listening to your podcasts I believe she is a prime example of what you call epi-genetics. I believe something I. Her environment maybe her poor diet flipped a switch. She is seeing a rheumatologist next week to try and figure out why this Erythema Nodosum has come about. Researching that is definitely mind boggling. Could be nothing could be precursor to something very serious. Lastly she is also prone to high anxiety. Because she is young I am hoping everything can be fixed but am more than concerned these things are happening. I think she might have been a classic adrenal fatigue candidate. The functional labs included Cortisol and it was 3.6. Last summer she did work 2 jobs about 60+ hrs a week on her feet. Went straight to second year of college and well- we all know that lifestyle is not always the most conducive to good health. By March she had nothing left - zero energy, lack of desire to try. Than came BP and it's a miracle she didn't drop out of school. What are you thinking? Where does she even begin? I know there is no magic bullet but truly at a loss. Ps- lots of labs over the year. Tested negative for Lyme, HIV, mono, Epstein Barr etc. thank you for all you do. Can't wait for your book to come out. TIA.
