She received her Doctorate in Physical Therapy and Master of Science in Clinical Investigation from Washington University in St. Louis. Dr. Weyrauch has served as a consultant for a multi-billion dollar company to develop a workplace injury prevention program, which resulted in improved health outcomes, OSHA recordables, and decreased healthcare costs for the company's workforce. She has served on multiple national task forces for the American Physical Therapy Association (APTA)—most recently, she was elected to the APTA Nominating Committee—and actively lobbies for healthcare policy issues at the local, state, and national levels of government. She currently serves as Vice President of the American Physical Therapy Association Connecticut Chapter and is a member of the American Congress for Rehabilitation Medicine. Dr. Weyrauch has performed scientific research through grants from the National Institutes of Health and National Science Foundation at institutions including Stanford University and Washington University in St. Louis. Her research examining movement patterns and outcomes in people with and without low back pain has led to numerous local, regional, and national presentations and a peer-reviewed publication in Archives of Physical Medicine and Rehabilitation, a top journal in rehabilitation. More about Dr Krystyna Holland Krystyna Holland, DPT (she/her) is the founder and CEO of Inclusive Care, a physical therapy office in Denver, Colorado specializing in the provision of trauma informed pelvic floor care to individuals across the gender identity spectrum. Krystyna's journey as a provider started as a patient. Traumatic experiences in her own medical treatment inspired her to open Inclusive Care. In addition to helping folks feel confident in their ability to live without leaking and have intimacy without fear, Krystyna aims to change the fundamental patient-provider relationship from one that centers the provider as a problem solver to one that focuses on collaboration between the patient and the provider. She is a well-known Instagram educator (@Krystyna.Holland), an instructor of trauma-informed care trainings, and a healthcare consultant. More about Dr Jennifer Hyer Jennifer Hyer, MD joined the Obstetrics and Gynecology faculty at Denver Health in 2007. She is an Associate Professor of Clinical Practice in Obstetrics and Gynecology at the University of Colorado School of Medicine. She completed medical school at the University of Tennessee Health Sciences Center and completed her Obstetrics and Gynecology residency at the University of Colorado. She is a Fellow of the American College of Obstetrics and Gynecology and a Diplomate of the American Board of Obstetrics and Gynecology. She is a member of the 2017 class of the Association of Professors in Gynecology and Obstetrics (APGO) Academic Scholars and Leaders Program. In 2022, she joined the Denver Health Medical Staff Executive Committee. In 2021, she was honored by her peers with the Denver Health Outstanding Clinician Award. Her clinical activities include full scope practice of Obstetrics and Gynecology at Denver Health Medical Center as an attending physician. She has been providing surgical management for transmasculine patients since 2016. In 2018, she expanded her surgical skill set under the direction of Dr. Marci Bowers and Dr. Chris Carey to include vaginoplasty for transfeminine patients. She continues to provide these surgical services and is the only transfem bottom surgeon at Denver Health. She has presented Denver Health gender affirming surgical data at both national and international conferences. She hopes to expand the Denver Health Gender Affirming Surgical Program and participate in research opportunities to improve care of all LGBTQ+ patients. She also has a research interest focused on maternal mental health, and she has presented on this topic at both the national and international level. She has received grant funding from the Denver Health Foundation and Zoma Foundation to support integration of behavioral health into obstetrics clinics. Additionally, she has partnered with colleagues at academic institutions within Denver to collaborate on projects to improve maternal mental health and access to support services. What do you do when you have a patient who doesn't believe you can help?Many times these are the patients we end up thinking about when we go home at night. Or over our lunch break. Or even during our next patient.They can make us feel inadequate, sap our energy or cause us to replay conversations in our heads."What do I say in order to convince them I can help?"Many of these patients have very valid reasons to be skeptical. Their emotions can range from anger to depression to "yeah, but...ing" So what do we do?Take our ego out of it. It's not about us, and we don't have to be right and win.Stop talking. There's nothing you can say to change someone's mind.Show them a change. Help them to see a change. Focus on 1-2 things. Demonstrate a change in the session and help them to see it. Trauma informed care has many definitions, including some fantastic work done by the Substance Abuse and Mental Health Services Administration. Krystyna Holland joined us for an episode (and we can't wait to have her back on) and we talked about everything from pelvic pain to pregnancy exercise recommendations to leaking while coughing, jumping, and sneezing, low back pain and the relationship it has to pelvic floor dysfunction, and how athletes can improve their performance by optimizing their core stability (your pelvic floor plays a huge role, btw).The take home message from the episode was that although pelvic dysfunction may be common, it is not inevitable. In this episode, we continue with part 2 of the SOAP series: the Objective Exam. Most of us learned explicitly and implicitly from our training that the purpose of the exam is for us (the provider) to learn about the patient's body in order to create an appropriate plan of care. However, we've realized that the patient is actually learning about their body during the exam as well! Everything that we do during the exam process is teaching the patient something about their body. They are internalizing our language and reactions - for better or worse! So in this episode, we focus on tips to help us center the patient during the exam, to tailor each exam to the individual, and empower them with the exam process. In this episode, we explore the concept of grounding in pelvic health. Grounding is “directing attention toward what is happening in the present moment.” It has many benefits for patients AND providers including improved bodily awareness, improved focus, and better problem-solving. In our experience, grounding can also reduce sympathetic nervous system activation and help regulate emotional reactions. Oftentimes people in pelvic PT feel betrayed by their body or try to avoid bodily signals as a way of adapting to pain. When people are disconnected from their bodies it can affect their progress. Helping people feel safe is a prerequisite to many treatments, such as dilator training and pelvic floor muscle training. Grounding through breathwork, movement, and cognitive exercises can help patients re-orient to the present moment. Providers may also need to practice this skill throughout sessions for many of the same reasons as patients. When providers are grounded we can hold space for patients and better teach this essential skill. We end by sharing our favorite techniques to use for ourselves and with patients. We are so excited to bring you this interview with Dr. Krystyna Holland, DPT!! She specializes in trauma-informed pelvic health practice for people across the gender identity spectrum. Her practice philosophy centers around intentionally minimizing our risk of perpetuating harm. We went live @ CSM in Denver with some new and some familiar voices. On the show for the first time is Jason Steer with his former student Matt Tuttle who's a friend of the show. Jason currently serves as a physical therapist for a special operations unit in the U.S. Army. He has been a member of a multidisciplinary team for most of his career which began in the athletic training room at the University of South Dakota in 1995. After receiving his Master’s degree in Physical Therapy in 2000, he was able to spend 3 years with the Los Angeles Dodgers and 4 years with the Seattle Mariners working side by side with esteemed physicians, physical therapists, athletic trainers, and strength and conditioning professionals. Pelvic floor physical therapy often has the singular goal of decreasing pain with penetration, usually done with a specific approach of manual therapy, what people perceive to be trigger point release, and graded exposure to stimulus. For people who are having painful intercourse, their experience is made up of much more than their muscular tone – how can we bridge the gap from painfree to pleasurable professionally? What being a patient taught me about the medical system and the way that providers mess it up We talk about patient behaviors such as anxious tendencies and pain catastrophization like they are inherent characteristics of the individual, not as coping mechanisms shaped by life experience. How can we increase patient engagement and improve outcomes for them by understanding (and explaining to them!) their behaviors and experiences as evolutionarily beneficial protective mechanisms in overdrive? Who is to blame and what is there to do when your patient isn’t responding as you expect? Providing affirming care – tips and trips for treating transgender, intersex and gender nonbinary clients There are huge healthcare discrepancies in care for members of the LGBTQIA+ community, especially the latter half – transgender, intersex, and gender non-conforming individuals. What assumptions are you (unintentionally!) bringing to the encounter, and how can you sidestep them to provide affirming care without doing harm? Pelvic Floor PT should be about more than decreasing pain – it should be about increasing pleasure; How to talk to patients about sex in a professional way – Sex is an ADL and we have nationally crappy education regarding what is “normal” and how to talk about it professionally. What being a patient taught me about the medical system and the way that providers mess it up – Providers typically got into the field to care for patients and help people. They often end up frustrated when they feel unable to assist and can’t put their finger on why. The expression of this frustration and, often, subsequent ambivalence, has long reaching impacts on the patient. Providing affirming care – tips and trips for treating transgender, intersex and gender nonbinary clients – Transgender healthcare has been shown in the research to be incredibly lacking; in providers that feel confident in providing care, in patients trusting providers to provide care, and in patients’ access to care. Normalizing and providing providers with tools to feel confident in unfamiliar encounters may be able to decrease the perpetuation of medical trauma in a population that is, statistically, already highly traumatized. People are trusting the medical system to help them and experiencing medical trauma instead. It’s happening to new moms, individuals with pelvic pain, people of color, LGBTQIA+ individuals, and other marginalized groups. We have to do what we can to stop it. It’s important not to assume that just because someone is your patient that they owe you anything – access to their body, following your ideal treatment plan as prescribed without their input, or intimate details of their life. Even if you want all of those things to help them, engaging people in a way that makes them feel in control and safe will: improve their outcomes, decrease their likelihood of experiencing medical trauma, and make your job easier. It’s important to trust people when they tell you about themselves – about their gender, about who and how they love, about what they need from you.
