Texas executes more people than any other state, but recently, state lawmakers were able to halt one at the last minute, saving Robert Roberson's life, at least for now. Michelle Lyons, a former public information officer for the Texas Department of Criminal Justice who witnessed hundreds of executions and wrote a memoir about it, joins host Nikki DaVaughn to break down Roberson's case, what life has been like for him on death row these past 20 years, and what might happen next. Research Nerd Alert! This is an overview of our current understanding of what central sensitisation (aka nociplastic pain) is and how it impacts pelvic pain syndromes and of course...what we are goibg to do about it! Some great research around this topic has emerged over the past couple of years, with a growing understanding of why just focusing on the issues in the tissues is not enough and how we can incorporate lifestyle strategies such as sleep, nutrition and physical activity into our anti-inflammatory treatment toolbox So although this review is very evidence based for the first part, I do explore why this means when we are working with real life people - why and how to screen for CS and what steps we can take to get better outcomes - ultimately to help women with persistent pelvic pain live well! Welcome to this week's episode, where we're starting to look at preparing for the upcoming festive season - full of stress, different eating, sleeping and travel planning - the perfect recipe for stress, unregulated nervous systems, digestive systems and pelvic floor dysfunction! All this month, we're going to look at the evidence for multimodal pelvic rehab & lifestyle strategies for pain & digestive distress. Let's start the conversation with Inflammatory Bowel Disease - including Crohn's Disease and Ulcerative Colitis - how does it impact female pelvic health, from sexual health, contraception choices, fertility, pregnancy & postpartum recovery? There's a significant (and growing!) body of evidence to support the role of a biopsychosocial pelvic rehab approach to helping women with IBD live well Did you know: that 61.4% of women with IBD have some degree of sexual dysfunction? that there's an increased risk of gestational diabetes? that one of the best way to improve the fatigue associated with IBD is exercise? that the research looking at pelvic floor dysfunction, whether we're talking about pelvic pain, bowel frequency/urgency, sexual pain - all points to the efficacy of a multimodal pelvic rehab approach The problem might be that gastroenterologists don't know you can help! All the research, especially around sexual health and pelvic floor dysfunction emphasises the need for a multimodal pelvic rehab approach as part of an inter-disciplinary approach to improving quality of life for women with IBD Want to take an even deeper dive into Bowel Health during the perinatal year? Hello & welcome to this week's episode of the podcast!Today is World Osteoporosis Day and it falls right in the middle of both Menopause Awareness and Breast Cancer Awareness month (probably not a coincidence as bone health is a major issue for both of these often overlapping groups)In last week's episode, I talked about musculoskeletal health at menopause, and everything I said then is applicable for women going through breast cancer treatment, especially if they are on endocrine treatment, but there are a few extra provisos to consider when we talk about bone health during and after breast cancer treatmentWe know adjuvant endocrine treatments for hormone receptor positive breast cancers (tamoxifen and aromatase inhibitor) affect bone, muscle and joint health, albeit in different waysBut we also know that the evidence supports a comprehensive bone health strategy, including education, exercise, nutrition and other lifestyle strategy modifications can really have a positive impact on bone healthStudies that extrapolate from menopausal research and studies looking exclusively at women during and after breast cancer both emphasise the importance of exercise, especially resistance, impact and balance training.Using the mantra, Strong, Steady & Straight, and allowing for decreased energy availability during and after cancer treatment, balance compromises due to chemo, especially taxane drugs and of course, being mindful of the pelvic health challenges she may be facing, from GSM to prolapse to pain, we are in the perfect position of having the evidence from the literature and the whole person approach to give the breast cancer survivor the empowering tools she needs to take charge of her bone health and her overall recovery.This podcast episode was inspired by the new module I've created for the online Breast Cancer Rehab which will be available by the end of the month - it will feature sections on musculoskeletal health, bone health, a breast cancer related lymphoedema research update and of course, pelvic health after breast cancer, with some delicious movement videos from my friend & colleague, movement expert Laura Feeney.I'll announce on instagram/facebook when the updates are available - if you're already enrolled, no need to do anything it will be automatically accessible...and if you'd like to join us, click here for the Breast Cancer Rehab courseThanks for being here - I'd love it if you can subscribe, rate & review the podcast and of course, you can follow my continuing adventures in women's health on Instagram @michellelyons_muliebrity and all of my online course information is here at CelebrateMuliebrity.comUntil next time, Onwards & Upwards, Mx
October is awareness month for both Menopause & Breast Cancer, and one area of overlap between these two issues is that of musculoskeletal health Sometimes, when we think about musculoskeletal health at menopause, we might immediately think of osteoporosis. Although that's a really important issues, and one I'll be addressing in upcoming episodes, for today I want to talk about muscle & joint pain at menopause Musculoskeletal dysfunction affects between 70 and 80% of women going through peri to post menopause, with 25% of women having severe symptoms, including rotator cuff dysfunction or gluteal tendinopathy (which affects 1 in 4 women in their 50's and is often misdiagnosed as hip arthritis) In today's episode, I'll discuss the latest research around why this happens (spoilers: oestrogen, inflammation & sleep disruption) and of course the evidence based strategies that we can use to help women live well before, during & after menopause, and (of course!) where pelvic health at menopause fits into the discussion! Hello and welcome to this week's episode where I'm sharing my top 5 non-hormonal approaches for pelvic health at Menopause. GSM or Geniturinary Syndrome of Menopause covers the pelvic health changes we see as oestrogen levels decline and includes such issues as overactive bladder, continence, VV dryness/ irritation and pelvic floor muscle dysfunction. Although topical hormonal therapies such as oestrogen or dhea can be great - not every woman wants to or can use them - I wanted to share the latest evidence to support the evidence for non-hormonal approaches that I cam across as I was updating the Menopause module. All of the options I discuss can be used alongside hormonal therapy or instead of it - I'm not a prescriber of pharmaceuticals so while I am a big fan of hormonal therapies, at the right time for the right person at the correct dosage - I feel that these non-hormonal strategies are effective & evidence based too. This is part 2 of my Menstrual Health mini-series (pt1 covered the basics of the menstrual cycle and pt 3 will cover The Missing Menstrual Cycle - stay tuned!) In this episode, we'll cover dysmenorrhea (from the Greek, meaning ‘painful monthly bleeding' Did you know that menstrual pain affects 90% of those who menstruate, with 9% of those dealing with severe pain? That endometriosis affects at least 10% of women worldwide? That although meds and surgery are often offered as first line approaches, there's good evidence to support other self management strategies including a whole person pelvic rehab biopsychosocial approach that includes lifestyle, movement, manual therapy to calm the nervous system and the pelvic floor, tens and pelvic pain education including strategies for bladder, bowel & sexual health. If we remember that lots of things go into the biopsychosocial bucket, including how we move, how we manage stress, how we eat, how we sleep …as well as tissue disorders, then we can provide, in the words of Mardon et al ‘validation, reassurance, education and empowerment'. The conversation around the menopause is finely gathering pace. It's a hot topic in the press and there are many great campaigns, more books and resources that can help women manage the menopause and seek the treatment they want and deserve. The menopause can be difficult to navigate and is still shrouded in mystery. Grainne and Emma had a great conversation about the menstrual cycle with Michelle Lyons in season 2 where we touched on menopause. In this episode Orla debunks myths and provides some sound advice for any woman who is peri or post menopausal. What do you do when you have a patient who doesn't believe you can help?Many times these are the patients we end up thinking about when we go home at night. Or over our lunch break. Or even during our next patient.They can make us feel inadequate, sap our energy or cause us to replay conversations in our heads."What do I say in order to convince them I can help?"Many of these patients have very valid reasons to be skeptical. Their emotions can range from anger to depression to "yeah, but...ing" So what do we do?Take our ego out of it. It's not about us, and we don't have to be right and win.Stop talking. There's nothing you can say to change someone's mind.Show them a change. Help them to see a change. Focus on 1-2 things. Demonstrate a change in the session and help them to see it. He was packed in dry ice and wrapped in blankets. He was left there, so his super-elite covert team could recover his body. When the LAPD got there, they discovered hundreds of thousands of dollars in cash, 1200 firearms, a bullet proof car, and six tons of ammunition. Quite simply, the dead Jeffrey Alan Lash was a spy. Or a conman. Or a cult leader. Or an alien hybrid. You listen, you decide. Lash was born in Westchester and had a normal upbringing by all accounts. He dropped out of UCLA, and in the late 1980s, he started a 20 year romance with a woman named Michelle Lyons. The only problem was, when she bought a condo, he secured a room that she could never enter. And he gave her a false name. And he told her he was a spy. She said he was “the one” and stayed with him, even as he amassed an arsenal. Eventually, he told her a big operation was happening, and he had to move in with a another woman. And so it went. Eventually, Jeffrey Alan Lash moved in with Catherine Nebron (and her husband?), and befriended Nebron’s personal assistant Dawn Marie VadBunker. After only one meeting with Lash, Dawn Marie started eating raw meat and told her mother that Lash was an alien hybrid bent on saving the world. Like you do. So after years of living under the alias Bob Smith, amassing weapons and cash, and being paranoid, Lash finally succumbed to some sort of cancer. As per his instructions, they packed his body on ice, so his mercenary group could dispose of it, and VadBunker and Nebron bugged out to Oregon for a time. But, no group ever took his body, so there it rotted until the LAPD was tipped off. So, how did this Oregon move lead to a missing person’s report? Where did VadBunker end up, and how does it connect to a mystical healer code-named Tara? Why did VadBunker cut off all ties to her mother and daughters? Why did Jeffrey Alan Lash eat a diet of raw grass fed meat? How did nobody manage to get arrested by the LAPD when they found this two week old corpse rotting in a car? At age 22, Lyons started reporting for The Huntsville Item, which gave her the opportunity to see her first lethal injection. In 2001, she became the Director of Public Information for the Texas Department of Criminal Justice (TDCJ), a spokesperson for 110 prisons across the state.
