May is Maternal Mental Health Awareness month so I'm delighted to share this chat I had with Dr Lizzy Deery, who is the author of this paper addressing the psychosocial factors around the return to exercise for postpartum womenThe paper is open access (yay!) and is called 'From insight to action: a qualitative study of the postpartum return to recreational exercise and sport in Ireland and the UK' Deery et al 2025In today's episode:We discussed the challenges of exercising during pregnancy and postpartum, & agreed that there is still a need to educate the general population about the safety and benefits of physical activity during pregnancy. We highlighted the importance of maintaining healthy habits and the need for resources to support women in their transition to motherhood & emphasized the role of exercise in managing mental health and regaining a sense of identity after childbirth. Hello & welcome to this episode all about the intersection of hip health and pelvic health, with one of my favourite researchers/clinicians, Dr Alison GrimaldiIn this episode we discuss the interconnectedness of the pelvic floor, hip muscles, and nerves in the pelvic region, how the pelvic floor works in conjunction with other muscles, particularly the obturator internus, to support pelvic function. We emphasized the importance of considering both pelvic floor and hip issues when treating patients, as weakness in hip muscles can affect pelvic floor function and vice versa. Hello & Welcome to this episode of the podcast, where we're talking about all things prolapse, with advanced practice women's health physio (& pilates teacher!) Leah BryansIn today's conversation we discussed the evolution of our roles as physiotherapists, particularly in pelvic health. Leah shared her journey from not realizing the extent of what physio could do to becoming more confident in managing conditions like prolapse. We talked about the importance of continuous learning, collaboration with other professionals, and understanding the individual needs of patients, and of course I had to hop on my soapbox about the shift from being mechanics to educators, focusing on facilitating patient progress rather than fixing issues. We're both keen to emphasise the need to consider patients' overall health, including their bladder and bowel function, fluid intake, and diet. We also highlighted the significance of assessing patients' pelvic floor movement and identifying risk factors for prolapseWe discussed the potential benefits and challenges of using pessaries for postnatal women, and noted that some women may be hesitant to try pessaries due to psychological barriers, while others may not be seen early enough in primary care or public systems to receive this treatmentWe also discussed the importance of building confidence and gradually increasing strength in patients, particularly those with pelvic health issues. It's important to remember the the need for a low-threat approach to exercise, focusing on breathwork, relaxation, flexibility, and proprioception at first. We also highlighted the benefits of Pilates as a gateway to heavier lifting and osteogenic activities. hello again!I'm still a bit croaky but nevertheless, I persisted!In this week's episode, I want to look at how we can best help women with dyspareunia (and maybe throw a bit of evidence based myth busting into the mix!)In this episode I talk about the different types of sexual pain, the drivers and how we can best come up with clinically reasonable treatment planning, that includes pelvic rehab at the centre of a multi-modal approach.The main focus is on finding the drivers - is it superficial or deep? Is it primary or secondary? Is there a specific diagnosis? Is there always a bio cause (are we including the psychosocial? the nociplastic?) Are we comfortable having the discussions? I think yes, if we know what questions to ask AND what to do with the answersI think the key to good outcomes is to believe the story, take a great history, create a safe space to discuss these issue and having a deep and wide biopsychosocial treatment toolboxLet me know if you'd like a Part 2 on treatment strategies! Research Nerd Alert! This is an overview of our current understanding of what central sensitisation (aka nociplastic pain) is and how it impacts pelvic pain syndromes and of course...what we are goibg to do about it! Some great research around this topic has emerged over the past couple of years, with a growing understanding of why just focusing on the issues in the tissues is not enough and how we can incorporate lifestyle strategies such as sleep, nutrition and physical activity into our anti-inflammatory treatment toolbox So although this review is very evidence based for the first part, I do explore why this means when we are working with real life people - why and how to screen for CS and what steps we can take to get better outcomes - ultimately to help women with persistent pelvic pain live well! Welcome to this week's episode, where we're starting to look at preparing for the upcoming festive season - full of stress, different eating, sleeping and travel planning - the perfect recipe for stress, unregulated nervous systems, digestive systems and pelvic floor dysfunction! All this month, we're going to look at the evidence for multimodal pelvic rehab & lifestyle strategies for pain & digestive distress. Let's start the conversation with Inflammatory Bowel Disease - including Crohn's Disease and Ulcerative Colitis - how does it impact female pelvic health, from sexual health, contraception choices, fertility, pregnancy & postpartum recovery? There's a significant (and growing!) body of evidence to support the role of a biopsychosocial pelvic rehab approach to helping women with IBD live well Did you know: that 61.4% of women with IBD have some degree of sexual dysfunction? that there's an increased risk of gestational diabetes? that one of the best way to improve the fatigue associated with IBD is exercise? that the research looking at pelvic floor dysfunction, whether we're talking about pelvic pain, bowel frequency/urgency, sexual pain - all points to the efficacy of a multimodal pelvic rehab approach The problem might be that gastroenterologists don't know you can help! All the research, especially around sexual health and pelvic floor dysfunction emphasises the need for a multimodal pelvic rehab approach as part of an inter-disciplinary approach to improving quality of life for women with IBD Want to take an even deeper dive into Bowel Health during the perinatal year? October is awareness month for both Menopause & Breast Cancer, and one area of overlap between these two issues is that of musculoskeletal health Sometimes, when we think about musculoskeletal health at menopause, we might immediately think of osteoporosis. Although that's a really important issues, and one I'll be addressing in upcoming episodes, for today I want to talk about muscle & joint pain at menopause Musculoskeletal dysfunction affects between 70 and 80% of women going through peri to post menopause, with 25% of women having severe symptoms, including rotator cuff dysfunction or gluteal tendinopathy (which affects 1 in 4 women in their 50's and is often misdiagnosed as hip arthritis) In today's episode, I'll discuss the latest research around why this happens (spoilers: oestrogen, inflammation & sleep disruption) and of course the evidence based strategies that we can use to help women live well before, during & after menopause, and (of course!) where pelvic health at menopause fits into the discussion! Hello and welcome to this week's episode where I'm sharing my top 5 non-hormonal approaches for pelvic health at Menopause. GSM or Geniturinary Syndrome of Menopause covers the pelvic health changes we see as oestrogen levels decline and includes such issues as overactive bladder, continence, VV dryness/ irritation and pelvic floor muscle dysfunction. Although topical hormonal therapies such as oestrogen or dhea can be great - not every woman wants to or can use them - I wanted to share the latest evidence to support the evidence for non-hormonal approaches that I cam across as I was updating the Menopause module. All of the options I discuss can be used alongside hormonal therapy or instead of it - I'm not a prescriber of pharmaceuticals so while I am a big fan of hormonal therapies, at the right time for the right person at the correct dosage - I feel that these non-hormonal strategies are effective & evidence based too. In the intro to the episode, I talk about some new research that I recently came across from Briand et al (2024) looking at changes in body composition and how women lose lean muscle mass starting at the age of 31!!! Vulvodynia - is it a mucosal issue, a pelvic floor issue, a psychological issue...or all of the above? That was the question I asked on my Instagram feed recently, paraphrasing the research into this topic by Chisari et al. So in this week's episode, I'm delighted to welcome Dr Jessica Reale back to the podcast, this time, to discuss Vulvodynia. It can be an intimidating diagnosis for both patient and provider but we explore what it takes to be an effective clinician working with people on this journey. This is part 2 of my Menstrual Health mini-series (pt1 covered the basics of the menstrual cycle and pt 3 will cover The Missing Menstrual Cycle - stay tuned!) In this episode, we'll cover dysmenorrhea (from the Greek, meaning ‘painful monthly bleeding' Did you know that menstrual pain affects 90% of those who menstruate, with 9% of those dealing with severe pain? That endometriosis affects at least 10% of women worldwide? That although meds and surgery are often offered as first line approaches, there's good evidence to support other self management strategies including a whole person pelvic rehab biopsychosocial approach that includes lifestyle, movement, manual therapy to calm the nervous system and the pelvic floor, tens and pelvic pain education including strategies for bladder, bowel & sexual health. If we remember that lots of things go into the biopsychosocial bucket, including how we move, how we manage stress, how we eat, how we sleep …as well as tissue disorders, then we can provide, in the words of Mardon et al ‘validation, reassurance, education and empowerment'. What is good communication and why does it matter when we are working with people dealing with chronic pain? There are a couple of papers that have caught my attention on this - Kays (2021) paper looking at the lack of empathy and its effects when we work with people who have chronic pelvic pain, but especially Kays (2022) paper 'Effects of Provider Interactions on diagnosis and care of people with chronic pelvic pain'. These papers discuss the consequences of negative patient/ provider interactions, how it can increase the emotional toll and decrease trust and communication...essentially impeding proper treatment And so, who better to discuss this with than Nicola Robertson, of Diamond Physiotherapy in Canada, who gave a brilliant presentation on this topic at CSM in Boston earlier this year. We discussed: her journey to working in a position combining women's health in sports medicine which is the big difference - is it sex or is it gender? nurturing physical activity and sports in girls and keeping women in sport the importance of understanding how women's health is more than pelvic health - we talked about REDs, ACL injuries, the importance of menstrual literacy, concussion and postnatal recovery the importance of more female-centric research, body image..and dark shorts! drug free strategies for tens/endo - important for athletes who can't use certain medications/drug testing the importance of external skills for pelvic health and so many more considerations!
