Here are some highlights of this special episode: Barbara thinks she should have been a social worker but her mother talked her into nursing. She and her fellow hospice nurses learned how to provide their own self care without really knowing what it was. Barbara's aversion to causing patients any pain or suffering made patient care difficult for her. She believes it's important for hospice leadership to truly understand end of life work, and to have a comprehensive knowledge of hospice care. OTs are underutilized in end-of-life care, but they're an essential part of the hospice interdisciplinary team. There's a lot of support OTs can offer to make caregiving tasks easier, practical solutions to everyday issues for activities of daily living. Occupational Therapy interventions can include managing equipment, setting up meals, and transfer techniques. It's about problem-solving to make caregiving tasks easier and promote independence for the patient. Using her unique gifts, Erin and the Compassionate Coaching theater troop employ “playback theater” to develop engaging programming that centers around giving and receiving empathy. They work with end of life teams to mitigate burnout, enabling the teams to continue providing care for those who are dying. Erin understands grief, loss, and the importance of caring for ourselves as we care for others. Erin lived her whole life in a state of anticipatory grief for the loss of her older sister, Gina. Pharmacist Ellen Fulp is passionate about using medications appropriately to care for patients who are at their most vulnerable. As a part of the hospice interdisciplinary team, Ellen collaborates with the hospice director and nurse to ensure quality of life is supported, and a patient's suffering is reduced. Caregivers and patients often misunderstand how medication is used in hospice care. Ellen believes in symptom resolution, keeping the patient's goals of care in mind. In hospice care, pharmacists do more than just review medications for side effects or adverse effects. They help the clinical team find the best medication options to keep the patient comfortable. That includes finding the right route, the most convenient and safe times for administering the medications, and ensuring the medications are utilized correctly by the patient. When medication diversion (meds being stolen or used by someone else) is a concern, pharmacists coordinate with the hospice team to furnish the meds in a safe, legally compliant way. Pharmacists might not be part of the “core team”, but they're a vital part of the care of a hospice patient. Hospice nurse Eileen Spillane of Befriending Death has learned to be comfortable with the uncomfortable. Eileen is the Founder of Befriending Death and has been a Registered Nurse for over thirty years, specializing in Oncology, Critical Care, and Obstetrics. She is a certified Life Coach from New Ventures West, a former Zen Hospice Project Volunteer, a Meditation Teacher, and an Active Travel Guide. She has been at the bedside of many as they transition through the dying process and cares very deeply about this topic. If we prepare and share our wishes, we can give our loved ones the gift of clarity and direction so they feel confident they are following our wishes and have the opportunity to grieve without further unnecessary stress”. Today's guest Dr. Qwynn Galloway-Salazar is a Veteran, death doula, compassionate educator, and the founder of In Their Honor, a national initiative raising awareness and addressing Veterans' unique challenges as they approach the end of life. Qwynn is also the spouse of a combat veteran and understands the role of veteran caregivers. 1 in 4 Medicare hospice patients is a U.S. military veteran - a quarter of the Medicare hospice population. Those veterans have special needs when it comes to the end of their lives. Qwynn recognizes the importance of identifying moral injury, a history of combat service, PTSD, and events of military sexual trauma (for both female and male veterans). Discussion of suicidal ideation and depression should also be a part of their end of life care. Often veterans have terminal illnesses that are related to their military service, including Agent Orange and burn pit exposure. The needs of veteran caregivers are unique as well. End of Life Doula Trina Wacasey works to ensure that clients and families have the kind of personalized death care they want. Using family-guided care customized to fit their beliefs, needs, and wants, Trina guides caregivers through deaths, post-mortem care, and rituals after death. After working in the corporate world, Trina was drawn to the work of an end of life doula. Dr. Kenneth Doka and Alex Lamorie of the Hospice Foundation of America's Autism and Grief Project are advocates for people with autism. Dr. Doka is a prolific author, editor, and lecturer, past president of the Association for Death Education and Counseling, and a member and past chair of the International Work Group on Death, Dying, and Bereavement. Alex Lamorie is an undergraduate student at the University of Maryland Global Campus and an autism advocate. He serves as an advisor on the Hospice Foundation of America's Autism & Grief Project. Alex's expressive grief artwork was featured in 2021 in the textbook Superhero Grief: The Transformative Power of Loss. For any professional who works with grieving adults, having awareness of the needs of persons on the autism spectrum is an important part of their education. The Remarkable Story of Humanist Chaplain D.S. Moss

Devin (D.S.) Moss brings a sense of reverence for life and a curiosity about death to his work as a chaplain. During a conversation with a Zen Master about Buddhist beliefs about the afterlife, the suggestion was made to him that he would make a good chaplain. Devin didn't think he could do the work of spiritual care because of his humanist beliefs. He doesn't believe in God. Turns out the Zen Master didn't believe in God, either. Despite a religious upbringing, Devin embraces nontheistic, or humanist, beliefs. His sense of exploration led him to attend seminary and become a chaplain. He has found great fullness and human connection in the work of a chaplain. After completing his CPE (Clinical Pastoral Education) internship, Devin accepted a position to provide spiritual care for an inmate on death row in Oklahoma. He spent most of 2023 supporting convicted murderer Phillip Hancock. Their relationship culminated in Hancock's death by execution in November of that year. D.S. is also the host of the Memento Mori podcast, a show about spirituality and death he started before he became a chaplain. An Informative Discussion on How to Serve Military Veterans on Hospice

1 in 4 Medicare hospice patients is a U.S. military veteran. End of life workers need to know how to take care of their special needs and enhance their quality of life. It's important for EOL teams to know the signs of PTSD and how to listen when these patients (and their caregivers!) are communicating what they need. Many military veterans experience trauma, guilt, and anger, causing what is identified as “soul injury”. Having a soul injury or PTSD creates a need for specialized care. Opus Peace has a great inventory tool for assessing whether someone has experienced a soul injury. The Soul Inventory is available for free on the Opus Peace website. You can access the Soul Injury Inventory here. Every discipline on the hospice team, including the End of Life (or Death) Doula, brings skills that seriously ill and terminally ill veterans need. The caregivers providing support for those veterans need support as well. Those caregivers are advocates, coordinators, and spokespersons for the veterans they care about. Award-winning End of Life Educator Barbara Karnes takes a deep dive into the unique challenges of caring for someone with dementia at end of life. Caregivers often find their responsibilities accumulating over time as the disease trajectory continues. They're forced to live in the present, to see their loved one as who they are today. The goal is to bring joy and quality of life to a beloved today, in this moment. Hospice care for a person with end stage dementia can be tricky. The timing of the admission is often difficult to determine, and patients are often discharged from hospice if their health decline is not evident, which means the patient no longer meets hospice criteria. When a patient needs to be discharged, it's vital that the hospice team support the caregiver with robust discharge planning. That caregiver needs all the resources and support the team can provide before the hospice care ends. Discharge from hospice can feel like abandonment to a caregiver. Where will the medical equipment, medications, and personal care assistance come from? The hospice team is responsible for helping caregivers set up a system of support that can be there when hospice workers are gone. Isolation is also a big concern for caregivers providing care for someone with dementia. Today, my guest is Barbara Karnes, an award-winning nurse and end-of-life educator. Barbara has put her 40 years of experience regarding education, care, and support of dying people and their loved ones into numerous books and resources. Barbara Karnes is a pioneer of the modern hospice movement in the United States. She got her start as a hospice nurse when medical professionals didn't know how to pronounce the word hospice, let alone understand what care it could provide their patients. This is part two of Anthony Facello's difficult story. The value of having a POA (power of attorney) is of utmost in any family with a loved one. And if that loved one is suffering from dementia and is a mother of an only child, it's even more imperative. What makes Anthony's story even more tragic is the abuse his mother received from her second husband.  Making things worse, the cocktail of meds that were given to Anthony's mother, sent her to a tragic end, when indeed it didn't have to be that way. Sadly, it's not unusual in the realm of care for those with dementia. His mother wasn't afraid to die, and thankfully Anthony knew that his mother was dying. That was obvious. It was the goodbye that was the worst part. Thankfully, host Mary Anne Oglesby-Sutherly walked alongside Anthony with counsel and guidance during his most desperate of times. Medicare's hospice regulations are not working as intended for many people with dementia, says many experts of end-of-life care. The "long goodbye", the phrase Barbara Karnes describes about hospice patients with dementia, is, for the medical profession, a difficult diagnosis. About 5 percent are pulled from hospice when their condition seems to have stabilized. Plus, the number of Americans over 65 with dementia is projected to more than double to 13.8 million by 2060. Many will need hospice care. Today's conversation with Barbara Karnes is to announce her new book, "By Your Side," (A Guide for Caring for the Dying at Home) and the discussion of how vital End of Life Doula and Dementia Doula work is becoming in our society. Her new book is one of the best for family caregivers and since Barbara has been a family caregiver herself, her knowledge of Hospice and the experience in that field is exceptional.  Barbara Karnes, is an RN Award-Winning End of Life Educator, an Award-Winning Nurse, an NHPCO Hospice Innovator Award Winner 2018 and 2015 International Humanitarian Woman of the Year.  While at the bedside of hundreds of people during the dying process, this Hospice Pioneer noticed that each death followed a near-identical script. End-of-life educator Barbara Karnes is back to talk about death in the home, what it looks like, and how caregivers can manage a dying loved one at home.  The reality of a death at home is that it's a challenge for caregivers, and can put a heavy burden on the caregiver.  Before death became a medical event, it was a psychosocial event that was managed at home, and caregivers were supported by the community.  These days people frequently die in the hospital or in a nursing facility.  Family often is not present when a death happens.  A good death can happen at home; however, with family present.  Hospice support can be an important part of that care.  End-of-life workers can provide education, support, and encouragement that help caregivers manage the death experience.  Barbara describes the dying process as labor, very similar to labor for the birth of a baby.  Advance care planning (ACP) can help define how a person wants to die at home, identifying a medical decision-maker (MPOA or healthcare proxy).  Seriously ill adults who are living alone need a caregiving system.  Hospice social workers and chaplains can help solo agers identify an MPOA and build a support system for their caregiving.   Visit the BK Books website at bkbooks.com.
