#297 From OR To ICU: How Checklists And Clean Hands Save Lives

Anesthesia Patient Safety Podcast

Transfers don't have to feel like controlled chaos. We break down how to move a critically ill patient from the OR to the ICU with confidence by pairing structured handoffs with disciplined infection prevention—so information moves seamlessly while pathogens hit a dead end.We start by revisiting the ICU's influence on anesthesia practice through the story of ARDS and lung-protective ventilation. The shift to 6 ml/kg ideal body weight didn't just save lungs in the unit; it reshaped intraoperative strategy to reduce ventilator-induced injury for surgical patients. From there, we zoom into the human factors of handoffs: why complex, time-sensitive details—hemodynamics, antimicrobials, ventilator settings, imaging, and goals of care—so often fall through the cracks, and how IPASS, OR-to-ICU structured handoffs, and explicit role assignments align teams. Then we tackle pathogen transmission where it thrives: device-rich environments and high-touch surfaces. We unpack how environmental reservoirs and biofilms turn bed rails and anesthesia machine into unseen vectors, and why consistent, high-frequency hand hygiene is the most powerful countermeasure. Clear targets make habits stick: at least four sanitizer uses per hour in the ICU and eight per hour in the OR, coupled with strict isolation adherence and diligent decontamination. By the end, you'll have a tight, transferable playbook: adopt lung-protective settings across care areas, script handoffs with shared tools and timed calls, measure sanitizer touches, and treat the environment as a clinical variable. If this conversation helps your team cut errors or infections, share it with a colleague, subscribe for future episodes, and leave a review with one change you'll make this week. For show notes & transcript, visit our episode page at apsf.org: https://www.apsf.org/podcast/297-from-or-to-icu-how-checklists-and-clean-hands-save-lives/© 2026, The Anesthesia Patient Safety Foundation

This episode offers a comprehensive deep dive into the foundational principles of property transmission, focusing on the law of gratuitous transfers, including trusts, estates, and the legal doctrines that govern inheritance and gift law. Perfect for law students and legal practitioners preparing for exams or practicing estate planning. Most people think estate planning is just about writing a will, but beneath the surface lies a complex legal battle between personal freedom and societal safeguards. In this episode, we dissect the core legal conflict: how the law balances your right to control your property with the need to prevent fraud, undue influence, and dead hand control. You'll uncover how American law uniquely prioritizes donor autonomy—allowing you to disinherit family, leave assets to pets, or give to fringe causes—unless public policy steps in to impose limits. We explore the intricate frameworks that govern property transfers—probate versus non-probate—and why understanding these pathways is crucial. Discover how donative intent must be clear and objective, matching the transfer's timing: immediate for lifetime gifts, and at death for wills. You'll learn about the rigorous formalities required for valid wills—signatures, witnesses, and the evidentiary purpose they serve—versus the flexible world of trust agreements and contract-style transfers, like life insurance, POD accounts, and revocable trusts, which bypass probate and streamline estate management. The episode also delves into the infamous "dead hand" control—how a testator's stipulations can influence heirs decades after their death. Case studies like Shapira reveal the fine line courts walk in allowing partial restraints on marriage to promote social policy without outright banning personal autonomy. We detail how incentive trusts—conditional gifts for education or behavior—are generally valid, but conditions that encourage illegal acts or family disintegration cross the line into voidness. Finally, we tackle exam pitfalls—common errors like conflating rules applicable to wills and gifts, neglecting the objective proof standard for intent, or overestimating fairness arguments—which can cost you points. Learn how to analyze legal issues step-by-step: identify transfer type, apply the relevant doctrine, and support your reasoning with policy insights. This episode arms you with the conceptual map to navigate the tangled maze of estate law confidently, transforming a confusing jumble into clear, exam-ready mastery.Whether you're preparing for the bar or just want to understand how the law manages death, inheritance, and legacy, this episode is essential listening. It's not just about rules—it's about the profound questions of control, family, and society's role in shaping our final wishes.Key TopicsThe core principles of property transmission and inheritance lawThe balance between personal autonomy and public policy in estate lawThe legal requirements for valid gifts and willsThe distinction between probate and non-probate transfersThe role of donative intent and formalities in estate planningtrusts, estates, inheritance law, gratuitous transfers, donative intent, probate, non-probate, formalities, dead hand control, estate planning

Former Alabama national championship quarterback Jake Coker joins Mick Gillespie for a wide-ranging conversation on the current state of Alabama football, the impact of the transfer portal and NIL, and whether Alabama is investing enough to stay on top in today's college sports landscape. The discussion opens with a look ahead to Alabama spring practice, including the players Coker is most excited to watch. He talks about both quarterbacks, highlights Jase McClellan as one of the most dynamic athletes on the offense, and explains why McClellan's loyalty to Alabama stands out in the NIL era. They also break down the biggest concern for the Tide heading into the season: the offensive line, where a lack of continuity could be a major issue. Mick and Jake also share memories from some of their favorite Alabama football games, including the LSU national championship win, the Rocky Stop against Tennessee, Alabama's title run with Coker, the 2009 SEC Championship Game, and the 1996 Iron Bowl. The conversation also revisits legendary Alabama defensive back George Teague and two of his most iconic football moments. Later, the show takes a strong turn into the changing economics of college athletics. Mick and Jake discuss how NIL, the transfer portal, and overall program spending are reshaping the future of college football. They debate whether Alabama must focus more of its financial resources on revenue-producing sports to avoid slipping behind the national elite. They also dive into Alabama basketball, with praise for Nate Oats and discussion about whether this team can make another deep postseason run. Plus, they touch on roster questions, March expectations, and how this year's group compares to Alabama's Final Four squad. #976 | A listener mailbag episode with Wayne and Ed, recorded during a gap in the men's fixture list (though United Women notched a seventh straight win). Wayne's been deep in writing a book on an under-explored era of United history, with fascinating cross-era player comparisons. Listener questions drive wide-ranging discussion: the ongoing burden of Glazer debt and whether Ratcliffe's involvement changes anything meaningful; the stadium regeneration plan; and pointed criticism of Ratcliffe's public comments, staff cuts, ticket price hikes, and chaotic decision-making - even as the org chart and finances have arguably improved. On football: the Wilcox-Amorim dynamic and where "oversight" crosses into interference; whether Michael Carrick's audition is going well; and why Ole's tenure deserves more respect than it gets. On the squad: Bruno Fernandes is irreplaceable, midfield needs a serious rebuild if Casemiro and Ugarte leave, the striker situation remains an area for more depth, and youth prospects offer some hope. 00:00 Intro 01:29 Wayne's Writing Projects 07:06 Ownership & Stadium Questions 13:06 Jim Ratcliffe's Comments 20:10 Jason Wilcox & Ruben Amorim 31:40 Michael Carrick Discussion 38:38 Bruno Fernandes Questions 43:20 Midfield & Transfer Needs 47:24 Striker Situation 51:34 Center Back & Youth Prospects 54:49 Most Damaging Post-Fergie Match 58:30 Fun Questions & Outro If you are interested in supporting the show and accessing a weekly exclusive bonus episode, check out our Patreon page or subscribe on Apple Podcasts. Send a textMike Romance has spent nearly two decades operating at the intersection of manufacturing engineering, automation, validation, and operations leadership within the life-sciences ecosystem. His career spans startups and established organizations alike, with hands-on experience taking products from early development through GMP-ready, high-volume production. Across roles in process development, automation, quality systems, and manufacturing strategy, Mike has built a reputation for combining technical rigor with pragmatic execution. Most recently at Quantum-Si, Mike played a central role in scaling operations to support the commercialization of the Platinum protein sequencing platform while laying the groundwork for next-generation technologies like the Proteus platform. Working within a lean and highly agile leadership team, he helped establish scalable manufacturing foundations spanning CM-managed instrument supply, internal reagent kit production, and advanced silicon-based consumables—while navigating the realities of fast-moving product roadmaps and constrained resources.Earlier in his career, Mike held engineering and leadership roles at organizations including Illumina, Dexcom, GenMark Diagnostics, Truvian, and Encodia. Along the way, he's led pilot-line development, automation strategy, equipment qualification, validation programs, and process controls—often in environments where the path forward wasn't clearly defined. What sets Mike apart is not just his command of acronyms—GAMP, CQV, QbD, DFSS, FMEA—but his philosophy that systems only work when people do. He actively practices emotionally intelligent leadership, prioritizing trust, clarity, and psychological safety while still holding teams to high technical and operational standards. As Mike explores his next chapter, this conversation focuses on the lessons he's learned building resilient manufacturing systems—and the kind of organizations where he believes he can make the biggest impact next.LINKS:Guest LinkedIn: https://www.linkedin.com/in/mikeromance/Aaron Moncur, host The Wave is  a place for engineers to actively learn, share ideas, and engage with people doing similar work.
