Is the Medical Aid in Dying Movement Opening Up a Bigger Conversation? As Medical Aid in Dying (MAID) legislation continues to expand across the United States, a deeper question is emerging: What is really driving the conversation? In this episode, Suzanne B. O'Brien, RN, Founder and CEO of Doulagivers Institute and author of The Good Death, explores the growing movement around Medical Aid in Dying and asks us to look beneath the headlines, politics, and debates to uncover what many people are truly seeking. Rabbi Richard F. Address, D.Min, is the Founder and Director of Jewish Sacred Aging. Rabbi Address served for over three decades on staff of the Union for Reform Judaism; first as a Regional Director and then, beginning in 1997, as Founder and Director of the URJ's Department of Jewish Family Concerns. Rabbi Address was ordained from Hebrew Union College-Jewish Institute of Religion in 1972 and began his rabbinic career in Los Angeles congregations. He also served as a part time rabbi for Beth Hillel in Carmel, NJ while regional director and, after his URJ tenure, served as senior rabbi of Congregation M'kor Shalom in Cherry Hill, NJ from 2011-2014. A major part of Address's work has been in the development and implementation of the project on Sacred Aging. This project has been responsible for creating awareness and resources for congregations on the implication of the emerging longevity revolution with growing emphasis on the aging of the baby boom generation. This aging revolution has begun to impact all aspects of Jewish communal and congregational life. Rabbi Address received a Certificate in Pastoral Counseling from the Post Graduate Center for Mental Health in 1998 and his Doctor of Ministry from HUC-JIR in 1999. He also received his honorary Doctorate from HUC-JIR in 1997. Rabbi Address has authored numerous articles, book chapters and books related to the issue of aging. I had the privilege of learning from fellow Greenwall Faculty Scholar Lisa Harris about a term she termed, "dangertalk." As an ob/gyn and abortion provider, Lisa found the debate around the legality of abortion so polarizing that it created a false dichotomy: you're either for or against. Any talk about misgivings, uncertainty, ambiguity, or ambivalence was silenced. Talking about these issues in the face of polarization was deemed dangerous and undermining to one side or another. "How could you?" For Lisa's work in finding common ground and embracing nuance she was awarded the 2023 Bernard Lo Award for forging connections across divisions. In today's podcast we focus on the equivalent experience of moral uncertainty, distress, and residue among prescribers of medical aid in dying. We are joined by Carly Zapata and Dani Chammas, prescribers of medical aid in dying in California. We discuss: Their journey prescribing medical aid in dying, and reasons for choosing to prescribe The legality of prescribing in California. We compare California to Canada, as we have previously on this podcast. We discuss new limited survey data suggesting that legal barriers may not explain the remarkable 20 fold differences in use of medical aid in dying between California and Canada; rather, Canada has 6x the number of providers per capita as California, and much greater awareness of the legality of medical aid in dying. We talk about cases that are not as clear - e.g. people who have voluntarily stopped eating and drinking. Moral issues, including ambiguity and ambivalence, distress and residue. For example the moral distress created when a patient requests medical aid in dying due to what is clearly a systems failure (see this Atlantic article for clear examples from Canada). We ask if they sometimes feel frustrated that more people who are in favor of medical aid in dying are not prescribing, instead leaving prescribing responsibility to a relatively small group of clinicians. How core ethical ideas might lead to very different conclusions about medical aid in dying, and ways Dani teaches ethics to trainees. Psychological models that can help navigate this complex terrain with patients and families, including formulations and countertransference. My son Renn plays guitar on the audio only version. -Alex Smith Additionally, some take home points, sent by Dani after recording: (1) Holding the dialectic: On one hand, people deserve the highest level of attention to their personhood and their suffering—an effort that, at times, can soften or even resolve a desire for hastened death. And on the other hand, some people will authentically experience this as the most values-aligned way of dying, given their circumstances. (2) Learning to accept that while laws create the safety rails, within those boundaries, morality is pluralistic. Both patients and clinicians bring deeply held moral frameworks to these decisions—and those frameworks deserve to be acknowledged and respected. (3) We have to be willing to ask the hard questions—and to show up for one another as we do. Because this work, more than almost any other, has taught us the profound impact of not feeling alone when navigating grey terrain. I view the discussion as an invitation for our field to not necessarily to become more certain, but to be willing to wrestle with the hard questions—while still showing up with rigor and compassion. And to remember that our patients are people before they are cases. If we can stay close enough to truly know them, we're much more likely to respond in ways that honor both their suffering and their dignity—whatever path that ultimately leads to.

From 2021 - Diane Rehm talks about her book "When My Time Comes: Conversations about whether those who are dying should have the right to determine when life should end." Rehm is an advocate for a practice known as M.A.I.D. (Medical Aid in Dying.). Rehm hosted her own radio program that was distributed by National Public Radio until 2016- and hosted her podcast "On My Mind" until 2025.

In December 2025, Governor Kathy Hochul reached an agreement with the New York state legislature to pass the Medical Aid in Dying Act, which would legalize what proponents call "death with dignity" and what critics call physician-assisted suicide. About a dozen other states already permit doctors to prescribe lethal medication to terminally ill patients who request it. The state of Oregon pioneered this practice in 1994 and it has since spread across the Western world. Now, there are people who have an ailing parent or grandparent or, God forbid, a child who is genuinely suffering—suffering in agonizing ways that make the cessation of that suffering seem like the only humane response. It would be inhuman not to acknowledge the enormous emotional, psychological, and physical burdens of that pain, or to minimize it. But the question of physician-assisted suicide ultimately is one about medical ethics as upheld by the physician, the distorting market effects of this practice, and social policy. What happens when the state makes it possible for large numbers of people to receive this option from the very person whose profession calls on him to heal and not harm? What happens to the moral foundations of our culture when assisted death becomes something we learn to abide? The evidence from places like Canada and the Netherlands begins to answer those questions in deeply disturbing ways. What started as a carefully limited option for the terminally ill has expanded dramatically. In Canada, deaths from medically assisted dying rose from 4,480 in 2018 to over 10,000 in 2021—and by 2022 accounted for 4 percent of all deaths in the country. Patients are now approved for reasons of poverty, loneliness, and mental illness. Veterans seeking PTSD treatment are sometimes offered death instead. The physician and educator Leon Kass warned nearly 30 years ago that once we break the ancient taboo against doctors killing patients, the practice would prove "in principle unregulable." The evidence now seems to vindicate that warning. To discuss this topic, Mosaic's editor Jonathan Silver is joined by Aaron Rothstein, a neurologist at the University of Pennsylvania and a fellow in bioethics and American democracy at the Ethics and Public Policy Center. They discuss the origins of the modern euthanasia movement, and the disturbing reality of how euthanasia functions once legalized. One of the most complex medical, ethical, moral, and religious questions of our era is that of physician-assisted suicide—also known as Medical Aid in Dying, or MAID. Eleven U.S. states and Washington, D.C. have legalized some form of MAID for terminally ill patients. And New York might join them. Over the summer, a Medical Aid in Dying Act passed New York's state legislature. It is now sitting on Governor Kathy Hochul's desk as she decides whether to sign it into law. Under the proposed New York bill, terminally ill adults with a prognosis of six months or less to live would be able to access a prescribed, self-administered life-ending medication. Supporters argue that this is a compassionate option—one that can relieve people of immense pain and suffering, allowing patients to choose when and where they die, and to do so surrounded by loved ones. Opponents see this as a violation of physicians' fundamental oath to do no harm. They also worry that while access may begin narrowly, it could expand over time to include people seeking death for reasons other than terminal illness—such as mental suffering or simply a desire to stop living. Cases like this have already occurred in Belgium, the Netherlands, Canada, and Switzerland. Rafaela Siewert sat down with two experts who see this topic very differently for a heated debate. David Hoffman is a healthcare attorney, clinical ethicist, and professor of bioethics at Columbia University. He argues that hypothetical future abuses of MAID shouldn't outweigh the needs of terminal patients who need this option now. Dr. Lydia Dugdale is a physician, medical ethicist, and professor of medicine at Columbia University. In her view, legalizing this practice of physician-assisted suicide risks undermining the responsibilities of governments, medical systems, and families to care for the mentally ill, the poor, and the physically disabled.
