In this fourth and final episode of our special series in partnership with Stanford Healthcare, our guest Susan Evans shares the ins and outs of caregiving, and talks about her work to support other caregivers dealing with the challenges of Alzheimer's Disease. As a caregiver, Susan worked to ensure her dad was safe and well cared for. She and her brother incorporated humor into their caregiving journey, and now she works to ensure other caregivers have the resources they need. Here are a few highlights from our conversation: Susan knows the power of story sharing, and how caregivers can learn from each other. When her dad began experiencing symptoms of Alzheimer's Disease, he took himself to his doctor at Stanford Healthcare. Susan recommends taking advantage of the present time caregivers have, to be in the moment instead of trying to remind your person of things they can't recall. Patty Webster of The Conversation Project is talking with me about the swoopers and seagulls that are sometimes part of the caregiving experience. As part of her caregiving journey with her dad and siblings, Patty realized that she had been a “swooper”, offering her opinions about her dad's care during an intermittent visit. With courage and vulnerability, Sheralin Samuels shares her experience as a caregiver for her mom, and their journey through Early Onset Alzheimer's disease. Here are a few highlights from this episode, created in partnership with Stanford Health Care: Sheralin's mom Carlys began showing symptoms in 2012, and a definitive diagnosis of Alzheimer's disease wasn't found until about 10 years later. Carlys spent a year in hospice care before she was discharged. As a sandwich caregiver, Sheralin cares for her two kids while working a full time job and ensuring her mom's needs are being met. Being a caregiver has impacted Sheralin's family in numerous ways, affecting family dynamics, relationships, and finances. Sheralin knows there's strength in self care. Her self care includes getting outside, enjoying nature, and hiking. By creating YouTube videos to help other caregivers, Sheralin is creating meaning from her experience. Dr. Fred Mirarchi is working to create personalized, specific advance care plans for patients, accommodating every stage of life and end of life wish. After seeing clinicians in the hospital struggle to determine exactly what patients' wishes were, Fred realized documentation of advance directives didn't truly communicate clearly. My guest Gabby Jimenez and I are busting the hospice myths to get to the truth about end of life care. Because there are so many misconceptions about hospice care, many people don't choose to have care from a hospice team or they delay hospice care until very close to death. It's important to know the truth about all the aspects of hospice care - how medicines are managed, how morphine is really used for someone who's dying, when to start hospice, and Do Not Resuscitate orders work. Gabby Jimenez is a hospice nurse, end-of-life doula, death and dying educator, and author. Dr. Robert Klitzman has learned the truth about the value of spiritual care through his professional practice and his own lived experience. Welcome to Season 9 of The Heart of Hospice Podcast! Occupational Therapists Emilia Bourland and Dr. Brandy Archie are sharing how OTs can improve the quality of life for patients and caregivers on hospice care. OTs are underutilized in end-of-life care, but they're an essential part of the hospice interdisciplinary team. There's a lot of support OTs can offer to make caregiving tasks easier, practical solutions to everyday issues for activities of daily living. Occupational Therapy interventions can include managing equipment, setting up meals, and transfer techniques. It's about problem-solving to make caregiving tasks easier and promote independence for the patient. Caregivers have a big job, so reducing the caregiving burden goes a long way to improving the end-of-life experience for both the person at end of life and the ones who are providing their care. Erin Whalen, founder of Compassionate Coaching, is working to help end of life and grief professionals love their work again. Using her unique gifts, Erin and the Compassionate Coaching theater troop employ “playback theater” to develop engaging programming that centers around giving and receiving empathy. They work with end of life teams to mitigate burnout, enabling the teams to continue providing care for those who are dying. Erin understands grief, loss, and the importance of caring for ourselves as we care for others. Erin lived her whole life in a state of anticipatory grief for the loss of her older sister, Gina. Gina was born with severe cerebral palsy, and died in 2016. Grief milestones are very familiar to Erin. Pharmacist Ellen Fulp is passionate about using medications appropriately to care for patients who are at their most vulnerable. As a part of the hospice interdisciplinary team, Ellen collaborates with the hospice director and nurse to ensure quality of life is supported, and a patient's suffering is reduced. Caregivers and patients often misunderstand how medication is used in hospice care. Ellen believes in symptom resolution, keeping the patient's goals of care in mind. In hospice care, pharmacists do more than just review medications for side effects or adverse effects. They help the clinical team find the best medication options to keep the patient comfortable. That includes finding the right route, the most convenient and safe times for administering the medications, and ensuring the medications are utilized correctly by the patient. When medication diversion (meds being stolen or used by someone else) is a concern, pharmacists coordinate with the hospice team to furnish the meds in a safe, legally compliant way. Pharmacists might not be part of the “core team”, but they're a vital part of the care of a hospice patient. Hospice nurse Eileen Spillane of Befriending Death has learned to be comfortable with the uncomfortable. Eileen is the Founder of Befriending Death and has been a Registered Nurse for over thirty years, specializing in Oncology, Critical Care, and Obstetrics. She is a certified Life Coach from New Ventures West, a former Zen Hospice Project Volunteer, a Meditation Teacher, and an Active Travel Guide. She has been at the bedside of many as they transition through the dying process and cares very deeply about this topic. On the Befriending Death website, Eileen writes that “if we have conversations about it (death), we might loosen up the fear and anxiety around it. If we prepare and share our wishes, we can give our loved ones the gift of clarity and direction so they feel confident they are following our wishes and have the opportunity to grieve without further unnecessary stress”. Eileen's courses can benefit everyone, from people with life-changing illnesses to elder law attorneys and caregivers for someone with a chronic illness. The courses include smaller “buddy groups” that create a deeper connection between participants. Today's guest Dr. Qwynn Galloway-Salazar is a Veteran, death doula, compassionate educator, and the founder of In Their Honor, a national initiative raising awareness and addressing Veterans' unique challenges as they approach the end of life. Qwynn is also the spouse of a combat veteran and understands the role of veteran caregivers. 1 in 4 Medicare hospice patients is a U.S. military veteran - a quarter of the Medicare hospice population. Those veterans have special needs when it comes to the end of their lives. Qwynn recognizes the importance of identifying moral injury, a history of combat service, PTSD, and events of military sexual trauma (for both female and male veterans). Discussion of suicidal ideation and depression should also be a part of their end of life care. Often veterans have terminal illnesses that are related to their military service, including Agent Orange and burn pit exposure. The needs of veteran caregivers are unique as well. End of Life Doula Trina Wacasey works to ensure that clients and families have the kind of personalized death care they want. Using family-guided care customized to fit their beliefs, needs, and wants, Trina guides caregivers through deaths, post-mortem care, and rituals after death. After working in the corporate world, Trina was drawn to the work of an end of life doula. She volunteered at a hospice agency with their bereavement program, and completed training at the Conscious Dying Institute. The deep and meaningful conversations had great meaning for her. She was fascinated by how individual people feel about death and advance care planning. As part of her Doula services, Trina offers families what she calls A Natural Goodbye. It's an alternative to the traditional funeral. Here's how she describes it: “I offer my clients a unique opportunity to say a Natural Goodbye following the death of a loved one as an alternative to a traditional funeral or formal wake or visitation. Dr. Kenneth Doka and Alex Lamorie of the Hospice Foundation of America's Autism and Grief Project are advocates for people with autism. Dr. Doka is a prolific author, editor, and lecturer, past president of the Association for Death Education and Counseling, and a member and past chair of the International Work Group on Death, Dying, and Bereavement. Alex Lamorie is an undergraduate student at the University of Maryland Global Campus and an autism advocate. He serves as an advisor on the Hospice Foundation of America's Autism & Grief Project. Alex's expressive grief artwork was featured in 2021 in the textbook Superhero Grief: The Transformative Power of Loss. For any professional who works with grieving adults, having awareness of the needs of persons on the autism spectrum is an important part of their education. Adults with autism can be overlooked by bereavement and grief workers because their grief expression might look maladaptive, perceived as inappropriate, or not present at all. Alex, along with other autistic adults, contributes a vital perspective in his role as an advisor for the Autism and Grief Project. Devin (D.S.) Moss brings a sense of reverence for life and a curiosity about death to his work as a chaplain. During a conversation with a Zen Master about Buddhist beliefs about the afterlife, the suggestion was made to him that he would make a good chaplain. Devin didn't think he could do the work of spiritual care because of his humanist beliefs. He doesn't believe in God. Turns out the Zen Master didn't believe in God, either. Despite a religious upbringing, Devin embraces nontheistic, or humanist, beliefs. His sense of exploration led him to attend seminary and become a chaplain. He has found great fullness and human connection in the work of a chaplain. After completing his CPE (Clinical Pastoral Education) internship, Devin accepted a position to provide spiritual care for an inmate on death row in Oklahoma. He spent most of 2023 supporting convicted murderer Phillip Hancock. Their relationship culminated in Hancock's death by execution in November of that year. D.S. is also the host of the Memento Mori podcast, a show about spirituality and death he started before he became a chaplain. 1 in 4 Medicare hospice patients is a U.S. military veteran. End of life workers need to know how to take care of their special needs and enhance their quality of life. It's important for EOL teams to know the signs of PTSD and how to listen when these patients (and their caregivers!) are communicating what they need. Many military veterans experience trauma, guilt, and anger, causing what is identified as “soul injury”. Having a soul injury or PTSD creates a need for specialized care. Opus Peace has a great inventory tool for assessing whether someone has experienced a soul injury. The Soul Inventory is available for free on the Opus Peace website. You can access the Soul Injury Inventory here. Every discipline on the hospice team, including the End of Life (or Death) Doula, brings skills that seriously ill and terminally ill veterans need. The caregivers providing support for those veterans need support as well. Those caregivers are advocates, coordinators, and spokespersons for the veterans they care about. Christina Keys can tell you the realities of being a caregiver for someone with a serious illness - the good, the bad, and the ugly. As the sole caregiver for her mother following a catastrophic stroke, Christina experienced financial, emotional, mental, physical, and relational impacts. The relationship between the two women was complicated, following a history of childhood abuse. When her mom needed end of life care, Christina was juggling a full-time job as well as all of her mom's care. The caregiving needs became much more intense, often requiring two people. End of life conversations were frequent so Christina knew exactly what her mother's wishes were. One specific request was a casket from Costco! The hospice care got off to a rough start at first, but Christina found an agency that was a better fit for her mom's care. Knowing she would need the additional support, Christina also engaged an End of Life Doula. Christina has taken the memories of her caregiving experience and all the lessons she's learned to support other caregivers.
