Social Determinants of Health, or SDOH, are a buzzword these days—but often little more. We acknowledge their importance, but actual progress on capture (and subsequent patient support) has been slow. To be fair real barriers including technological limitations and limited financial incentives stand in the way. But my current guest understands better than most the major role SDOH play in patient health. He and his organization have developed a digital tool to facilitate capture and improve the lives of patients in their community. They're making a difference. In this episode learn about how the Generational Health Program at Sharp Memorial Hospital in San Diego was developed and expanded to create an extensive care program that provides specialised care for older adults. What started as a pilot program in 2019 is now a multidisciplinary age-friendly collaboration that begins right at admission. Host Christina Shenvi is joined by 3 experts from Sharp Memorial Hospital who provide insight into how this program operates to provide the highest level of care for older patients. Diane Wintz, MD, a trauma surgeon who was heavily involved in creating the Generational Health Program. Her goal was to change how traditional health care models approach elderly patients presenting to the ED. Kelly Wright, RN, trained as an oncology nurse and now serves as manager of the program, and Stacy Nilsen, PhD RN, who's focus is on the feasibility, implementation, education and data assessment of the program. Together they discuss the creation and expansion of the program and highlight the most important aspects of what makes it a success.

Dr. Jason Giles shares his own story of addiction as a catalyst to helping others in their recovery journey. A word from Dr. Giles: The founding of Addiction Doctors stems from a deeply personal experience that changed the trajectory of my life. Twenty-five years ago, after realizing my dream of becoming an anesthesiologist, I became addicted to fentanyl—a drug that was then only available in hospitals. My habit escalated quickly, spinning my life completely out of control. Unable to stop on my own, in despair I sought help through California's Physician's Health Program, a specialized treatment plan aimed at supporting medical professionals with substance use disorders. In a humbling twist of fate, I left the OR and a job I loved to check into rehab. The doctor became a patient. It's no exaggeration to admit that program was my saving grace, guiding me toward recovery and inspiring me to devote my career to helping others free themselves from addiction, but it wasn't all rosy. Rehab afforded me valuable insight into some of the ways we doctors get it wrong when it comes to treating addiction. As I set out on a new professional path, I saw the need for a fresh approach, one that replaced the outmoded methods so prevalent in the field with a mix of cutting-edge scientific research and the kind of compassion and empathy that mostly characterized my experience in the Physician's Health Program. Drawing on my own journey, I developed an approach that assumes by the time someone seeks help for addiction, they have suffered enough. My method uses every science-backed tool in the box to ease the discomforts of detox because I've been there, and I know how frightening and unpleasant it can be. This fall, the Meadowlark Day Program held an official ribbon cutting ceremony, to celebrate the unique mental health program for Salem-Keizer students in need. Trillium Community Health Plan and the school district had an existing partnership to provide lower levels of mental health care in some schools. But for some kids, their challenges from depression, anxiety, PTSD and other disorders are so steep as to make it impossible for them to be able to get any educational benefit in school. Meadowlark is a 10-12 week program that gives kids intense treatment as well as instructional support every day, so they don’t fall further behind. We get the details from Chris Moore, the director of mental health and social-emotional learning for the district, and from Chiharu Blatt, Trillium’s vice president of community services in the Willamette Valley and Central Oregon.
